Vitamin K-what is known regarding bariatric surgery patients: a systematic review.

Vitamin K, a fat-soluble vitamin, is involved in blood coagulation, bone mineralization, inhibition of vascular calcification, and regulation of numerous enzyme systems. Patients who undergo bariatric surgery (BS), especially procedures that involve a malabsorptive component, are prone to develop vitamin K deficiency (VKD). The causes of VKD include decreased absorptive surface areas, steatorrhea, bacterial overgrowth, marked reduction of carriers of vitamin K, decrease in vitamin K intake, and modifications of gut microbiota. Data on vitamin K status among BS patients are scarce and the stren

The malabsorptive very, very long limb Roux-en-Y gastric bypass for super obesity: results in 257 patients.

BACKGROUND: For the past 11 years, we have used a malabsorptive form of Roux-en-Y gastric bypass (RYGB), the "very, very long limb" RYGB, for selected patients with BMIs >50 kg/m(2) and in highly selected patients with BMI <50 kg/m(2). This modified distal gastric bypass establishes a 100-cm common channel (for digestion and absorption) and a "very, very" long Roux limb of 400 to 500 cm. METHODS: To determine long-term efficacy and complications, we followed prospectively 257 consecutive patients; 188 (73%) participated in a postoperative survey. RESULTS: Of the patients, 60% were female; ov

Elevated vitamin A intake and serum retinol in preadolescent children with cystic fibrosis.

BACKGROUND: Persons with cystic fibrosis (CF) and pancreatic insufficiency (PI) are at risk of vitamin A deficiency because of steatorrhea, despite pancreatic enzyme replacement. Long-standing vitamin A supplementation may increase the risk of vitamin A toxicity. OBJECTIVE: The aim was to describe the vitamin A intake and serum retinol concentrations of preadolescent children with CF, PI, and mild-to-moderate pulmonary disease, who were cared for under current practice recommendations. DESIGN: This cross-sectional study evaluated children aged 8.0-11.9 y with CF and PI from 13 US CF centers.

[Failure to thrive and intestinal diseases in congenital disorders of glycosylation].

UNLABELLED: Congenital disorders of glycosylation type I (GDG-I) is a class of genetic multisystem disorders characterised by defective glycosylation of glycoproteins. The characteristics and mechanisms of failure to thrive and intestinal diseases present in CDG-I are anectodal. PATIENTS AND METHODS: The aim of this study was to analyse 7 CDG-I (4 CDG-Ia, 2 CDG-Ib and 1 CDG-Ix) with important digestive symptoms and failure to thrive in order to characterise the mechanisms implied. RESULTS: Four children had no skin abnormality or dysmorphia (1 CDG-Ia, 2 CDG-Ib, 1 CDG-Ix). An encephalopathy w

Brain white-matter lesions in celiac disease: a prospective study of 75 diet-treated patients.

OBJECTIVE: Celiac disease (CD), or gluten sensitivity, is considered to be a state of heightened immunologic responsiveness to ingested gluten proteins in genetically predisposed individuals. The gastrointestinal manifestation suggests a severe enteropathy of the small intestine with malabsorption, steatorrhea, and weight loss because of a deranged mucosal immune response. Neurologic complications occur, especially epilepsy, possibly associated with occipital calcifications or folate deficiency and cerebellar ataxia. There have been reports of brain white-matter lesions as an extraintestinal m

Intractable diarrhea in hyperthyroidism: management with beta-adrenergic blockade.

OBJECTIVE: To describe a patient with intractable diarrhea and thyrotoxic Graves' disease, for whom b-adrenergic blockade ultimately proved to be effective therapy for the diarrhea, and to review the types of hyperthyroidism-associated diarrhea. METHODS: We present the clinical course of a young man with a prolonged siege of diarrhea that proved elusive to diagnostic inquiries and resistant to all means of management until its endocrine basis was discovered. Control of such cases with b-adrenergic blockade is discussed, as are the pathophysiologic bases of intestinal hypermotility in hyperthy

Clinical decision-making in managing changes in gastrointestinal function following cancer therapies: Is experience enough?

In patients with gastrointestinal (GI) disorders, identical symptoms may occur for many different reasons. This prospective study assessed whether experienced clinicians can predict accurately the underlying diagnosis or diagnoses contributing to specific symptoms based on the history and physical examination. Three clinicians assessed 47 patients referred for management of troublesome GI symptoms identified after treatment for cancer. Investigations were requested following our comprehensive, peer-reviewed algorithm. The clinicians then recorded their predictions as to the results of those in

Quantifying bile acid malabsorption helps predict response and tailor sequestrant therapy.

Although recognised as a cause of chronic diarrhoea for over forty years, diagnostic tests and treatments for bile acid malabsorption (BAM) remain controversial. Recent National Institute for Health and Care Excellence (NICE) guidelines highlighted the lack of evidence in the field, and called for further research. This retrospective study explores the BAM subtype and severity, the use and response to bile acid sequestrants (BAS) and the prevalence of abnormal colonic histology. 264 selenium-75-labelled homocholic acid conjugated taurine (SeHCAT)-tested patient records were reviewed and the se

Chylomicron retention disease: a long term study of two cohorts.

Lipoprotein assembly is critical for the intestinal absorption of dietary lipids and of fat-soluble vitamins. Through their inhibition of chylomicron secretion, mutations of the Sar1B gene coding for Sar1 GTPase are associated with chylomicron retention disease (CRD). The aim of this study was to describe the phenotypic expression of CRD in two clinically and genetically well characterized cohorts, and to compare their long term evolution. The study in 7 children from France (X age 11.3+/-1.7 years) and 9 from Quebec, Canada (X age 12+/-2.5 years) involved data collection from medical records

Evaluating the real-world safety of cholestyramine for the treatment of hyperlipidemia: disproportionality analysis of FAERS data.

BACKGROUND: Hypercholesterolemia is a significant risk factor for severe cardiovascular diseases. Cholestyramine lowers serum low-density lipoprotein cholesterol (LDL-C) levels and is clinically indicated for the treatment of primary hypercholesterolemia, relieve itching symptoms caused by bile acid accumulation in cholestatic diseases (such as primary biliary cirrhosis), as well as to manage bile acid diarrhea resulting from bile acid metabolic disorders. With its widespread clinical application, it is essential to understand its safety in real-world settings. METHODS: This study evaluated t

Endoscopic pancreatic function test and other modalities for exocrine pancreatic disease measures.

OBJECTIVES: Exocrine pancreatic insufficiency (EPI) reflects inadequate pancreatic enzyme(s) and/or bicarbonate secretion, leading to malabsorption. We aim to compare different assessment tools for exocrine pancreatic function and correlate them with the endoscopic pancreatic function test (ePFT) as a reference standard. METHODS: Combined retrospective and prospective analysis of clinical data for children who underwent clinically indicated ePFTs. We compared ePFT against patient symptoms, laboratory results, magnetic resonance cholangiopancreatography with secretin 49/53 magnetic resonance i

Post-traumatic duodenopancreatectomy: A real challenge for emergency physicians, anesthesiologists-intensivists, and surgeons.

INTRODUCTION: Duodenopancreatic trauma, accounting for 3-5 % of abdominal injuries, poses a major challenge in trauma surgery. Their rarity and complexity demand rapid, expert management, often complicated by associated multivisceral injuries. CASE REPORT: A 20-year-old male was admitted to the emergency department following a road traffic accident, presenting with severe cranial and abdominal impact trauma. Clinical examination revealed hemodynamic instability and abdominal distension with rigidity. Due to the patient's hemodynamic instability and the abdominal emergency, he was taken direct

Revisional single anastomosis duodeno-ileal bypass with sleeve gastrectomy after failed adjustable gastric band or sleeve gastrectomy: a meta-analysis of efficacy and safety.

Single anastomosis duodeno-ileal bypass with sleeve gastrectomy (SADI-S) demonstrates a high safety profile and consistent weight loss efficiency, increasingly used as a revisional procedure. However, while mid- and long-term outcomes for primary SADI-S are established, evidence on its effectiveness as a revisional technique remains limited. This study aims to address that gap by assessing the efficacy and safety of revisional SADI-S after failed primary adjustable gastric banding (AGB) or sleeve gastrectomy (SG) procedures. A systematic search was conducted in PubMed, Embase, Web of Science,

Fermentable fibers induce rapid macro- and micronutrient depletion in Toll-like receptor 5-deficient mice.

Functional fermentable fibers are considered essential for a healthy diet. Recently, we demonstrated that gut microbiota dysbiotic mice fed an inulin-containing diet (ICD) developed hepatocellular carcinoma (HCC) within 6 mo. In particular, a subset of Toll-like receptor 5-deficient (T5KO) mice prone to HCC exhibited rapid onset of hyperbilirubinemia (HB) and cholemia; these symptoms provide rationale that ICD induces cholestasis. Our objective in the present study was to determine whether inulin-fed T5KO-HB mice exhibit other known consequences of cholestasis, including essential fatty acid a

Tailored pancreatic reconstruction after pancreaticoduodenectomy: a single-center experience of 892 cases.

BACKGROUND: Pancreatic reconstruction following pancreaticoduodenectomy (PD) is still debatable even for pancreatic surgeons. Ideally, pancreatic reconstruction after PD should reduce the risk of postoperative pancreatic fistula (POPF) and its severity if developed with preservation of both exocrine and endocrine pancreatic functions. It must be tailored to control the morbidity linked to the type of reconstruction. This study was to show the best type of pancreatic reconstruction according to the characters of pancreatic stump. METHODS: We studied all patients who underwent PD in our center

[Short bowel syndrome--surgical aspects].

INTRODUCTION: Long-term survival after massive intestinal resection is now possible with parenteral nutritional support. The expense, morbidity, and inconvenience of this therapy, however, have led to continued interest in alternatives for the treatment of the short bowel syndrome. Patients with short bowel require a multi disciplinary approach over a prolonged period. HISTORICAL CONSIDERATIONS: The history of small bowel transplantation started in 1959 when Lillehei showed that autotransplantation of the small intestine in a dog was feasible. From 1964 to 1971, 7 attempts of small bowel allot

Disease specific symptoms indices in patients with celiac disease-A hardly recognised entity.

BACKGROUND: Celiac disease (CD) was considered a rare disease before and was perceivably only limited to children but now affects almost 1-2% of the global population. This abrupt increase in prevalence is due to advancements in diagnostic criteria and medical facilities but still many countries lack the basic data that can assess the severity of this health issue. The present study was conducted with the aim to assess the common but rarely diagnosed condition with the identification of its underlying secondary ailments. MATERIALS AND METHODS: Patients visiting public sector hospitals were re
