A new 3-phase therapy protocol in temporomandibular joint ankylosis: our experience.

Limitations in oral opening may be due either to intra-articular or extra-articular ankylosis (pseudoankylosis). The principal means of therapy usually consist of 2 steps: surgical removal of the ankylotic block and immediate functional rehabilitation. In the postoperative period, however, immediate and adequate functional rehabilitation is not always possible because of pain and swelling of the temporomandibular joint, resulting in a very high risk of relapse. To prevent this, the authors introduce a third phase between the 2 already mentioned (surgery and functional therapy) in which the ora

'I know my body, I've lived in it all my life': therapy experiences of young people with disabilities.

Remedial surgery and other treatments are a common feature of disabled childhoods. These treatments are founded on notions of a normal:abnormal divide in which the task of health professionals is to bring such young people as close to societal concepts of normal as possible. Working with new paradigm disability and childhood research young people who are growing up with a disability have been able to contribute to our understanding from their perspectives. The young people identify the physical, social and emotional implications of remediation and identify a personal cost:benefit ratio. Questi

Total anterior teno-arthrolysis. Report of 72 cases.

This report describes a new technique for treatment of the chronically flexed fingers which applies in particular to fingers previously operated on several times and now presenting cutaneous, tendinous and joint problems. It consists in releasing the entire flexor apparatus through a full length lateral digital incision and a sub periosteal dissection. The volar plates of PIP and DIP are released as a whole with the flexor apparatus. The extended finger and flexor apparatus then are allowed to heal in a new relationship. Straightening of the finger is always possible. The range of motion is ma

Quality of life and post-traumatic stress symptoms in paediatric patients with tibial fractures during treatment with cast or Ilizarov frame.

PURPOSE: To compare quality of life in children and adolescents with tibial fracture during treatment with either a definitive long-leg cast or Ilizarov frame. METHODS: A prospective, longitudinal cohort study was undertaken. Patients aged between 5 and 17 years with tibial fractures treated definitively using a long-leg cast or Ilizarov frame were recruited at first follow-up. Health related quality of life was measured at each clinic appointment during treatment using the Pediatric Quality of Life Inventory (PedsQL) [1]; a validated measure of age-adjusted physical and psychosocial function

Clinical and Radiologic Outcomes of the Matti-Russe Technique for Scaphoid Nonunions in Pediatric Patients.

BACKGROUND: Scaphoid fractures are a common injury, and a known complication is nonunion. One option to treat this nonunion is with the Matti-Russe technique, which takes a corticocancellous graft and fits it into the defect without internal fixation. The advent of modern methods of fixation has led the classic Matti-Russe technique to fall out of favor. In this study, we describe the classic technique and evaluate the results of the Matti-Russe method for treatment of scaphoid nonunions specifically for the pediatric population. Our purpose was to evaluate the long-term clinical and radiologi

[The importance of condyle position in the diagnosis, treatment and prevention of TMD].

When a patient presents with TMJ pain, it could be all too easy for the orthodontists to turn the other way claiming that their occlusion has no connection with their symptoms and that they are suffering from a biopsychosocial disorder. This abundantly illustrated article attempts to demonstrate that patients can appear to display a stable occlusion even though their TMJ situation is unstable. This can give rise to dental abrasion, articular adaptations and pain. Orthodontics allows us to treat patients in pain provided that the treatment plan takes into account the need to restore a stable TM

The long and winding road part 2. The CLP patient's journey, 0-21 years.

UNLABELLED: Patients with a cleft lip and palate (CLP) deformity require the highest standard of care that the NHS can provide and this requires multidisciplinary care from teams located in regional cleft centres. Care of these cases is from birth to adulthood and requires several phases of intervention, corresponding to the stages of facial and dental development. Management ideally starts pre-natally, following the initial diagnosis, and occasionally pre-surgical appliances are prescribed. The lip is ideally repaired within three months, followed by palate closure between 12 and 18 months. C

Self-perceived effects of occlusal appliance therapy on TMD patients: an eight-year follow-up.

There are few long-term follow-up studies of treatment of temporomandibular disorders (TMD).The aim of this questionnaire study was to evaluate eight-year outcomes of appliance therapy in patients suffering from arthralgia/osteoarthritis and/or myofascial pain. The subjects comprised 120 patients, originally randomly assigned to treatment with an occlusal or a control (palatal) appliance. Eight years later, a questionnaire was sent to 118 eligible patients: 90 (76%) responded. The outcome measures were intensity and frequency of pain, physical and emotional functioning, and overall improvement

Diagnosis and treatment of temporomandibular disorders.

Current concepts and recommended treatment for temporomandibular disorders (TMDs) and temporomandibular joint pain and dysfunction have evolved over time. This article attempts to distill the current information for this often confusing topic into relevant clinical issues that will allow the general dental practitioner to be better able to diagnose and interpret clinical findings, and institute a therapeutic regimen that will provide needed relief to patients suffering from TMD dysfunction. Current management methods, both surgical and nonsurgical, are reviewed and discussed.

Recommendations by the EACD for examination, diagnosis, and management of patients with temporomandibular disorders and orofacial pain by the general dental practitioner.

The Council of the European Academy of Craniomandibular Disorders charged the Educational Committee with the task of establishing Guidelines and Recommendations for the examination, diagnosis, and management of patients with temporomandibular disorders and orofacial pain by the general dental practitioner. It was not their purpose to present a thorough and critical review of the vast amount of literature available but to summarize the at-present generally accepted clinical approach. These recommendations are based as much as possible on scientific evidence and on sound clinical judgment in cas

INTRODUCTION: The aim of this study was to establish a novel method for simulating orthognathic surgery in 3-dimensional (3D) space. METHODS: This system mainly consists of 6 procedures: (1) reconstruction of a virtual skull model (VS) from presurgical computed tomography scans; (2) reconstruction of virtual dentition models from 3D surface scanning of dental casts occluded at presurgical and postsurgical intercuspal positions (VD1 and VD2, respectively); (3) reconstruction of a preliminary fusion model of VS and VD1 by an initial intermodality registration; (4) reconstruction of another prel

Developmental dysplasia of the hip: nursing implications and anticipatory guidance for parents.

Developmental dysplasia of the hip (DDH) is a comprehensive term used to describe an abnormal relationship between the femoral head and the acetabulum. Developmental dysplasia of the hip includes a very wide spectrum of abnormality from a frank dislocation (very unstable) to a stable hip with a slightly shallow acetabulum. As many of these findings may not be present at birth, the term developmental more accurately reflects the biologic features than does the term congenital. Despite the recent increased awareness of DDH and the importance of thorough screening programs, hip dysplasia continue

The use of an oral exercise device in the treatment of muscular TMD.

Forty-five patents with a primary diagnosis of muscular MD were evaluated and treated in a university based facial pain center. The patients were equally and randomly assigned to one of three treatment groups. Group 1 patients were treated with traditional therapies appropriate for the particular patent. Group two patients used similar therapies that were appropriate for the patient but also had an oral vertical exercise device integrated into their therapy. Patients in the third group were instructed in home care, educated about TMD, and instructed in the use of the oral exercise device. Resu

Interventions for managing skeletal muscle spasticity following traumatic brain injury.

BACKGROUND: Skeletal muscle spasticity is a major physical complication resulting from traumatic brain injury (TBI), which can lead to muscle contracture, joint stiffness, reduced range of movement, broken skin and pain. Treatments for spasticity include a range of pharmacological and non-pharmacological interventions, often used in combination. Management of spasticity following TBI varies from other clinical populations because of the added complexity of behavioural and cognitive issues associated with TBI. OBJECTIVES: To assess the effects of interventions for managing skeletal muscle spas

Colorectal cancer with first known case of simultaneous bilateral tibial metastases.

The most common osseous metastatic regions for colorectal cancer are the lumbar and sacral vertebrae and the pelvis. There are few reported cases of isolated solitary tibial metastasis, and simultaneous bilateral solitary tibial metastases in colorectal cancer are even rarer. A 62-year-old female patient was admitted to our inpatient clinic 3 years after receiving initial chemotherapy for a rectosigmoid adenocarcinoma with liver metastasis. The patient complained of left leg pain. Radiographs and magnetic resonance imaging revealed a 3- × 3-cm mass in the right proximal tibia and a 2- × 7-cm m

Temporomandibular disorder presenting as chronic migraine; the diagnostic role of MRI and interdisciplinary evaluation: A clinical report.

Temporomandibular disorders can mimic migraines and delay appropriate care. A 40-year-old man with 18 months of refractory temporal headache and facial pain, initially treated as migraine, underwent imaging that revealed bilateral temporalis hypertrophy and temporomandibular joint degenerative changes. More specifically, an interdisciplinary assessment confirmed temporomandibular disorders. Multimodal management included occlusal adjustment with a nocturnal occlusal device, counseling to reduce parafunction, physical therapy, low-level laser therapy, and targeted botulinum toxin A.
