Relationship of training to self-reported competency and care of adolescents in an African health care setting.

PURPOSE: Adolescent medicine is not a recognized specialty in most African countries and African healthcare providers receive little adolescent-specific training. We explored the association between training, self-reported competence, and clinical practice related to adolescent health in an African setting. METHODS: A total of 119 healthcare providers of various disciplines who work with adolescent patients in Francistown, Botswana were surveyed regarding their adolescent-specific training, self-reported competence, and counseling practices. Self-reported competence and practices related to c

Association of chest radiographic abnormalities with tuberculosis disease in asymptomatic HIV-infected adults.

SETTING: Francistown and Gaborone, Botswana. OBJECTIVE: Chest radiography is used to screen for tuberculosis (TB) in asymptomatic persons living with the human immunodeficiency virus (PLWH) seeking isoniazid preventive therapy (IPT). We describe radiographic features in PLWH in a TB-endemic setting and identify features associated with TB disease. DESIGN: Asymptomatic PLWH seeking IPT under program conditions for a clinical trial between 2004 and 2006 received chest radiographs (CXRs) that were read using the standardized Chest Radiograph Reading and Recording System (CRRS). Clinical charact

Progress toward poliomyelitis eradication--poliomyelitis outbreak in Sudan, 2004.

After the World Health Assembly of the World Health Organization (WHO) resolved in 1988 to eradicate poliomyelitis globally, the number of countries in which polio is endemic declined from 125 to six by 2003. However, in 2004, polio cases caused by wild poliovirus (WPV) originating from northern Nigeria were reported in 11 countries--Benin, Botswana, Cameroon, Guinea, Mali, Saudi Arabia, Burkina Faso, Central African Republic, Chad, Cote d'Ivoire, and Sudan--reestablishing transmission in the latter five countries. Sudan, a member state of the WHO Eastern Mediterranean Region, initiated poliom

Noninvasive Assessment of Bilirubin Levels in Newborn Infants With Dark Skin Pigmentation.

AIM: Visual assessment of neonatal jaundice, especially in infants with dark skin pigmentation, is imprecise, and better tools are needed in low-resource settings. We aimed to assess the accuracy of the Picterus smartphone app in estimating total serum bilirubin (TSB) in darkly pigmented newborn infants, compared with a transcutaneous bilirubin (TcB) device. METHOD: A prospective study was conducted in Botswana, where term newborn infants aged 1-14 days were recruited. Bilirubin levels were measured using the Picterus app and a TcB device and compared with laboratory-measured TSB values. RES

Improving access to rare disease diagnostics in Africa: insights from a multinational pilot study.

BACKGROUND: Rare diseases (RDs) in Africa face challenges such as limited diagnosis, expertise, and treatment. FYMCA Medical Ltd., the International Gaucher Alliance (IGA) and the Centre for Human Metabolomics (CHM) at North‒West University (NWU) launched a pilot study in 2022 to create an African RD diagnosis network, focusing on training clinicians and testing dried blood spot (DBS) samples for 6 lysosomal storage disorders (LSDs). The pilot study spanned eight sub-Saharan African countries (Nigeria, Ghana, Tanzania, Kenya, Mozambique, Uganda, Botswana, and Sudan), with samples sent to South

Self-reported sleep quality and frailty in middle-aged people living with HIV in an African setting: a pilot study.

People living with HIV (PLWH) are at increased risk of frailty, yet the role of sleep disturbances in this risk remains underexplored. In this cross-sectional study, we aimed to evaluate self-reported sleep quality in PLWH and relevance to frailty in a high HIV-prevalence setting within Africa. We enrolled 50 PLWH on suppressive antiretroviral therapy (age: 56.9 ± 4.4 years; 26 females) and 50 matched HIV-seronegative controls (age: 56.6 ± 4.3 years; 24 females) in Botswana. Sleep quality was scored subjectively as "good" or "poor" based on participants' responses to two self-reported sleep qu

HIV status is not associated with SARS-CoV-2 viral load and longer duration of infection among people with well-controlled HIV and high COVID-19 vaccine coverage.

OBJECTIVES: This cohort study examined the associations between HIV status with SARS-CoV-2 viral load and duration of infection in a setting with high antiretroviral therapy (ART) coverage. METHODS: COVID-19 patients enrolled from public health clinics in Gaborone, Botswana underwent weekly serial testing for SARS-CoV-2. RT-PCR cycle threshold (Ct) value was used as a proxy measure of viral load. Multiple linear regression and Cox proportional-hazards models were used to determine the associations between HIV with Ct and time to SARS-CoV-2 negativity, respectively. RESULTS: Of the 486 enroll

Pairwise diversity and tMRCA as potential markers for HIV infection recency.

Intrahost human immunodeficiency virus (HIV)-1 diversity increases linearly over time. We assessed the extent to which mean pairwise distances and the time to the most recent common ancestor (tMRCA) inferred from intrahost HIV-1C env sequences were associated with the estimated time of HIV infection. Data from a primary HIV-1C infection study in Botswana were used for this analysis (N = 42). A total of 2540 HIV-1C env gp120 variable loop region 1 to conserved region 5 (V1C5) of the HIV-1 envelope gp120 viral sequences were generated by single genome amplification and sequencing, with an averag

Safety and Efficacy of Starting Antiretroviral Therapy in the First Week of Life.

BACKGROUND: Early antiretroviral therapy (ART) is recommended for infants with human immunodeficiency virus (HIV) infection. However, few antiretroviral options are available for neonates. METHODS: The Early Infant Treatment Study in Botswana tested HIV-exposed infants within 96 hours of birth, and HIV-infected infants started nevirapine (NVP) 6 mg/kg twice daily, zidovudine (ZDV), and lamivudine (3TC) at age < 7 days. NVP trough concentrations were tested at 1 and 2 weeks. NVP was switched to ritonavir-boosted lopinavir (LPV/r) at week 2, 3, 4, or 5 according to delivery gestational age. RE

An HIV-1 Nef genotype that diminishes immune control mediated by protective human leucocyte antigen alleles.

OBJECTIVES: Certain human leucocyte antigen (HLA)-B alleles (protective alleles) associate with durable immune control of HIV-1, but with substantial heterogeneity in the level of control. It remains elusive whether viral factors including Nef-mediated immune evasion function diminish protective allele effect on viral control. DESIGN: The naturally occurring non-Ser variant at position 9 of HIV-1 subtype C Nef has recently exhibited an association with enhanced HLA-B downregulation function and decreased susceptibility to recognition by CD8 T cells. We therefore hypothesized this Nef genotype

Non-invasive monitoring of glucocorticoid metabolites in banded mongooses (Mungos mungo) in response to physiological and biological challenges.

Free-ranging banded mongooses are infected by the novel pathogen, Mycobacterium mungi in northern Botswana. A reliable method for determining stress-related physiological responses in banded mongooses will increase our understanding of the stress response in M. mungi infection. Therefore, our aim was to examine the suitability of four enzyme immunoassays (EIAs) for monitoring adrenocortical endocrine function in captive and free-ranging banded mongooses based on fecal glucocorticoid metabolite (FGM) analysis. A conducted adrenocorticotropic hormone challenge revealed suitability of a valid mea

Linking chloride mass balance infiltration rates with chlorofluorocarbon and SF6 groundwater dating in semi-arid settings: potential and limitations.

In the framework of the investigation of enrichment processes of nitrate in groundwater of the Kalahari of Botswana near Serowe, recharge processes were investigated. The thick unsaturated zone extending to up to 100 m of mostly unconsolidated sediments and very low recharge rates pose a serious challenge to study solute transport related to infiltration and recharge processes, as this extends past the conventional depths of soil scientific investigations and is difficult to describe using evidence from the groundwater due to the limitations imposed by available tracers. To determine the link

Description of two novel Corynebacterium species isolated from human nasal passages and skin.

Strains of two novel Corynebacterium species were cultured from samples of human nostrils and skin collected in the United States and Botswana. These strains demonstrated growth on Columbia Colistin-Nalidixic Acid agar with 5% sheep blood and in liquid media (brain heart infusion and tryptic soy broth) supplemented with Tween 80, a source of the fatty acid oleic acid. Cells were Gram-positive, non-spore-forming, non-motile bacilli that showed catalase but not oxidase activity. Major fatty acids in both of these species were 18:1 ω9c (oleic acid), 16:0 (palmitic acid), and 18:0 (stearic acid).

Fasciola species and their vertebrate and snail intermediate hosts in East and Southern Africa: a review.

A systematic review was conducted focusing on the distribution of Fasciola species and their snail intermediate hosts (IHs) in East and Southern Africa. The reviewed literature showed that both Fasciola hepatica and F. gigantica are present in East and Southern Africa, and infect a wide range of domestic and wild ruminants. Fasciola gigantica was reported in six East African and five Southern African countries, where Radix natalensis (found in low altitudes) was reported to be the main IH. Fasciola hepatica was reported in Tanzania and Ethiopia (East Africa), and in South Africa and Zimbabwe (

Reflections on the maternal mortality millennium goal.

BACKGROUND: Nearly every 2 minutes, somewhere in the world, a woman dies because of complications of pregnancy and childbirth. Every such death is an overwhelming catastrophe for everyone confronted with it. Most deaths occur in developing countries, especially in Africa and southern Asia, but a significant number also occur in the developed world. METHODS: We examined the available data on the progress and the challenges to the United Nations' fifth Millennium Development Goal of achieving a 75 percent worldwide reduction in the maternal mortality by 2015 from what it was in 1990. RESULTS:

Project delivery in HIV/AIDS and TB in Southern Africa: the cross-cultural management imperative.

PURPOSE: The purpose of this paper is to report the findings of a broad-based study that initially investigated a possible gap in global inputs into the fight against HIV/AIDS and TB co-infection, and outputs in terms of results achieved. It is proposed that such a gap may be hypothesized to be due, at least in part, to inappropriate management regimes within the global health governance structure. The research does not simply question the effectiveness of the management of programs and projects, but rather the inappropriateness resulting from the lack of addressing cross-cultural issues. DES

Immune correlates of HIV-1 rebound during broadly neutralizing antibody treatment in young children.

Broadly neutralizing antibodies (bnAbs) are evaluated as possible alternatives to standard antiretroviral treatment (ART) for maintaining control of HIV-1 replication and may enhance immune responses to reduce or control the viral reservoir. However, the immunological and virological effects of bnAbs in infants and children are unknown. We conducted a detailed analysis of proviral reservoir dynamics and antiviral immune responses in a unique group of young children from Botswana who started ART at birth and then stopped standard ART while receiving the bnAbs 10-1074 and VRC01-LS in a subsequen

A Simple Regression-based Approach to Account for Survival Bias in Birth Outcomes Research.

In perinatal epidemiology, birth outcomes such as small for gestational age (SGA) may not be observed for a pregnancy ending with a stillbirth. It is then said that SGA is truncated by stillbirth, which may give rise to survival bias when evaluating the effects on SGA of an exposure known also to influence the risk of a stillbirth. In this article, we consider the causal effects of maternal infection with human immunodeficiency virus (HIV) on the risk of SGA, in a sample of pregnant women in Botswana. We hypothesize that previously estimated effects of HIV on SGA may be understated because the
