Rodent Model for High Altitude and Ebullism Exposure Studies.

INTRODUCTION: Ebullism is the pathophysiological process that occurs as a result of exposures to extremely low ambient pressures, traditionally below 47 mmHg (<0.9 psi, ∼63,000 ft/19,202 m equivalent). However, this field of research has made minimal progress since the 1940s-1960s, when the physiology of high altitude/space operations was being explored. This lack of progress is in part because it is thought of as invariably lethal and also because it requires unique facilities to simulate these environments. As a result, no standardized time/pressure profiles or animal models have been establ

Management of a child with vomiting.

Vomiting is a protective reflex that results in forceful ejection of stomach contents up to and out of the mouth. It is a common complaint and may be the presenting symptom of several life-threatening conditions. It can be caused by a variety of organic and nonorganic disorders; gastrointestinal (GI) or outside of GI. Acute gastritis and gastroenteritis (AGE) are the leading cause of acute vomiting in children. Important life threatening causes in infancy include congenital intestinal obstruction, atresia, malrotation with volvulus, necrotizing enterocolitis, pyloric stenosis, intussusception,

Pharmacological versus microvascular decompression approaches for the treatment of trigeminal neuralgia: clinical outcomes and direct costs.

In idiopathic trigeminal neuralgia (TN) the neuroimaging evaluation is usually normal, but in some cases a vascular compression of trigeminal nerve root is present. Although the latter condition may be referred to surgery, drug therapy is usually the first approach to control pain. This study compared the clinical outcome and direct costs of (1) a traditional treatment (carbamazepine [CBZ] in monotherapy [CBZ protocol]), (2) the association of gabapentin (GBP) and analgesic block of trigger-points with ropivacaine (ROP) (GBP+ROP protocol), and (3) a common TN surgery, microvascular decompressi

Patent foramen ovale closure with the SeptRx device initial experience with the first "In-Tunnel" device.

OBJECTIVES: The purpose of this study was to evaluate the safety and effectiveness of the SeptRx patent foramen ovale (PFO) closure device (SeptRx, Inc., Fremont, California). BACKGROUND: A PFO is a relatively common remnant of the fetal circulation that can be associated with cryptogenic stroke, transient ischemic attack, migraine, or decompression sickness. Percutaneous PFO closure with different devices has been performed for many years. However, most of the common devices leave a relatively large amount of material in the left and right atria. The SeptRx PFO device (SeptRx, Inc.) is the f

[Beliefs of Croatian surgeons and internists on hyperbaric oxygenation--a preliminary report].

BACKGROUND/INTRODUCTION: Hyperbaric oxygenation (HBO) has been accepted in many countries as a method of treatment in selected indications, but in Croatia it is still perceived with skepticism. OBJECTIVE: To determine beliefs, knowledge, and possible experience of Croatian surgeons and internists with HBO. SUBJECTS AND METHODS: An anonymous questionnaire was applied to test surgeons (N=56; 45%) and internists (N=68; 55%), employed in general hospitals in Zadar, Sibenik, Split, and Dubrovnik, Republic of Croatia. We believed that the subjects had been previously exposed to information campaig

Osteonecrosis of the knee: current clinical concepts.

Osteonecrosis of the knee should be differentiated into two main categories: (1) primary, spontaneous, or idiopathic osteonecrosis and (2) secondary osteonecrosis (e.g., secondary to factors such as steroid therapy, systemic lupus erythematosus, alcoholism, Caisson decompression sickness, Gaucher's disease, hemoglobinopathies, etc.). Spontaneous or primary osteonecrosis of the knee presents with an acute knee pain in elderly patients. It is three times more common in women than in men. Traumatic and vascular theories have been proposed as a causative factor of osteonecrosis of the knee, but th

[Diagnosis and treatment of Paget disease of the spinal vertebrae].

Paget disease, localized only on thoracic spine is extremly rare. It is a huge diagnostic problem and an equally big therapy challenge. Course of this disease is slow, it chronically worsens, thus demanding differentiation of various conditions, along with orthopaedic, neurological, radiographic and endocrine evaluation. Paget disease is followed by back pain, progressive weakening of legs and sphincter functions, due to pathological spine fractures and spinal stenosis. The treatment is surgical, in form of decompression and applying biphosphonates. Three patients are presented, all with worse

Identifying and acting on potentially inappropriate care? Inadequacy of current hospital coding for this task.

INTRODUCTION: Recent Australian attempts to facilitate disinvestment in healthcare, by identifying instances of 'inappropriate' care from large Government datasets, are subject to significant methodological flaws. Amongst other criticisms has been the fact that the Government datasets utilized for this purpose correlate poorly with datasets collected by relevant professional bodies. Government data derive from official hospital coding, collected retrospectively by clerical personnel, whilst professional body data derive from unit-specific databases, collected contemporaneously with care by cli

Effect on migraine of closure of cardiac right-to-left shunts to prevent recurrence of decompression illness or stroke or for haemodynamic reasons.

BACKGROUND: A relation between migraine with aura and cardiac right-to-left shunts has been reported. Right-to-left shunts are also associated with stroke and certain forms of decompression illness. We investigated the effect of closure of right-to-left shunts on migraine symptoms. METHODS: A consultant neurologist, who was unaware of information about residual shunt, undertook a structured interview with individuals who had had transcatheter closure of an atrial septal defect or patent foramen ovale to assess how the procedure affected migraine symptoms. FINDINGS: 37 of 40 consecutive patie

Outcome after treatment of neurological decompression illness is predicted by a published clinical scoring system.

BACKGROUND: After recompression therapy, some cases of neurological decompression illness (DCI) have a significant residual deficit. Boussuges et al. report a scoring system to predict sequelae using weighted historical and clinical indices at presentation which we applied to the British Hyperbaric Association (BHA) database of UK diving accidents, held at the Institute of Naval Medicine (INM). METHODS: A database search identified 234 cases of neurological DCI from the 2 yr, 1995-6. Of these, 217 case records contained sufficient data to apply the scoring system. Outcome was classified as se

Circumferential correction of spinal deformity and instability secondary to bacterial spondylodiscitis.

BACKGROUND: Spinal deformity as a sequela of nontuberculous spondylodiscitis is a rarely discussed clinical entity. Sagittal plane deformity, segmental instability, and persistently active infection overlap in these patients resulting in severe restriction in activity and quality of life. The presence of multiple medical co-morbidities restricts surgical options but nonoperative care may be ineffective and result in persistent, refractory discitis for years. We describe our experience with vertebrectomy and long-segment fixation for patients with postinfectious thoracic or lumbar deformity. M

Five consecutive cases of sensorineural hearing loss associated with inner ear barotrauma due to diving, successfully treated with hyperbaric oxygen.

INTRODUCTION: This report describes the outcomes of sensorineural hearing loss (SNHL) due to cochlear inner ear barotrauma (IEBt) in five divers treated with hyperbaric oxygen (HBOT). METHODS: The case histories of five consecutive divers presenting with SNHL from IEBt due to diving, were reviewed. All divers provided written consent for their data to be included in the study. All had reference pre-injury audiograms. All noted ear problems during or post-dive. Independent audiologists confirmed SNHL in all divers prior to HBOT, then assessed outcomes after HBOT. RESULTS: Three divers breathe

Effect of catheter-based patent foramen ovale closure on the occurrence of arterial bubbles in scuba divers.

OBJECTIVES: This study sought to evaluate the effect of catheter-based patent foramen ovale (PFO) closure on the occurrence of arterial bubbles after simulated dives. BACKGROUND: PFO is a risk factor of decompression sickness in divers due to paradoxical embolization of bubbles. To date, the effectiveness of catheter-based PFO closure in the reduction of arterial bubbles has not been demonstrated. METHODS: A total of 47 divers (age 35.4 ± 8.6 years, 81% men) with a PFO (PFO group) or treated with a catheter-based PFO closure (closure group) were enrolled in this case-controlled observational

Hyperbaric medicine for the hospital-based physician.

Hyperbaric oxygen (HBO2) is the inhalation of 100% oxygen at pressures > 1.4 times atmospheric pressure. Hyperbaric oxygen can be delivered in monoplace (single person) or multiplace (multi-person) chambers. Most clinical HBO2 exposures are between 2 and 2.4 atm abs for approximately 2 hours. Hyperbaric oxygen causes the blood and tissue oxygen levels to increase, reduces the volume of intravascular and tissue bubbles (to treat decompression sickness [DCS] and arterial gas embolism [AGE]), and accelerates wash-out of other gases, such as nitrogen or carbon monoxide (CO), which is important for

Percutaneous closure of patent foramen ovale: a case series and literature review.

Patent foramen ovale (PFO) is an anatomical variant of the interatrial septum with an overall prevalence of 27% in autopsy studies. PFOs have a potential role in causation of stroke, platypneaorthodeoxia, decompression sickness, right to left shunt and migraine headaches. Data regarding percutaneous closure of PFO in low volume tertiary care centers is lacking. Retrospective review of 14 percutaneous PFO closures done in our facility from March 2005 to August 2006 was performed for efficacy of procedure and safety. All patients received clopidogrel for a period of 3 months, and aspirin and sub

The Newcastle Papers in Industrial Medicine over the last 21 years.

The Chair of Industrial Health in Newcastle upon Tyne was first filled in March, 1946. For about the first 12 years diseases of coalminers posed the main clinical and research problems. They stimulated surveys of pneumoconiosis in the Durham and Northumberland coalfields and led to studies of ϰ-ray viewing techniques and of the relationship of radiological appearance to symptoms in dust disease of the chest. A section of medical statistics, now headed by a professor, was one of the earliest elements of the department to be inaugurated, and this was followed by a section of pulmonary physiology

Modelling the risk factors for accidents in recreational divers: results from a cross-sectional evaluation in Belgium.

INTRODUCTION: Characterisation of the recreational diving community could help to identify scuba divers at risk for accidents. METHODS: We performed a cross-sectional evaluation in a federation for recreational scuba divers in Belgium (Duiken.Vlaanderen). Using binary logistic regression, factors predictive for accidents leading to hospitalisation were identified. RESULTS: Of the 710 members, 210 (29.6%) participated in the survey, representing 140,133 dives. Age was > 50 years in 55% and the median (interquartile range [IQR]) number of dives was 380 (IQR 140-935). Cardiac (9.5%), orthopaedi
