Electrical rejuvenation of chronically implanted macroelectrodes in nonhuman primates.

Objective.Electrodes chronically implanted in the brain undergo complex changes over time that can lower the signal to noise ratio (SNR) of recorded signals and reduce the amount of energy delivered to the tissue during therapeutic stimulation, both of which are relevant for the development of robust, closed-loop control systems. Several factors have been identified that link changes in the electrode-tissue interface (ETI) to increased impedance and degraded performance in micro- and macro-electrodes. Previous studies have demonstrated that brief pulses applied every few days can restore SNR t

Does stereoelectroencephalographic depth electrode insertion alter seizure frequency? A baseline versus endpoint analysis.

The implantation effect refers to a transient reduction in seizure frequency following intracranial electrode insertion in patients with epilepsy, independent of therapeutic intervention. This retrospective study examined potential clinical and demographic predictors of the implantation effect in 51 patients who underwent stereoelectroencephalographic investigation between December 2018 and February 2020. Follow-up data were collected at 1, 3, 6, and 12 months postimplantation. A ≥50% reduction in seizure frequency defined responder status. Responder rates were 71.4% at 1 month, 35.0% at 3 mon

Hippocampal interictal spikes disrupt theta-band hippocampal-frontal eye field connectivity: fixation skewness as an indicator of transient network instability.

OBJECTIVE: Hippocampal interictal epileptiform discharges (IEDs) could induce transient visual working memory (VWM) impairments, yet the effect of IEDs on VWM network remain unclear. Since VWM depends on dynamic coordination between memory representations in the hippocampus (HP) and visual exploration controlled by the frontal eye field (FEF), we tested whether hippocampal IEDs disrupt this circuit's functional connectivity and whether such disruption is reflected in altered viewing patterns, offering a behavioral readout of transient network dysfunction. METHODS: Seven epilepsy patients with

The role of SEEG in the presurgical decision-making process in MRI-normal mesial temporal lobe epilepsy.

OBJECTIVES: In patients with mesial temporal lobe epilepsy (mTLE) and normal MRI, anterior temporal lobectomy sparing the hippocampus might be considered because of the risk of post-operative memory deficit. However, it is unclear whether some patients with normal MRI and non-invasive EEG and semiological pattern highly suggestive of mesial temporal seizures demonstrate a seizure onset network sparing the hippocampus, potentially warranting surgery. METHODS: A retrospective study of 17 patients with mTLE epilepsy and normal MRI who underwent SEEG. Only patients whose non-invasive presurgical

Spatial and temporal properties of intra-operatively recorded spikes and high frequency oscillations in focal cortical dysplasia.

OBJECTIVE: Focal cortical dysplasias (FCD) are characterized by distinct interictal spike patterns and high frequency oscillations (HFOs; ripples: 80-250 Hz; fast ripples: 250-500 Hz) in the intra-operative electrocorticogram (ioECoG). We studied the temporal relation between intra-operative spikes and HFOs and their relation to resected tissue in people with FCD with a favorable outcome. METHODS: We included patients who underwent ioECoG-tailored epilepsy surgery with pathology confirmed FCD and long-term Engel 1A outcome. Spikes and HFOs were automatically detected and visually checked in 1

Correlating Resting-State Functional Magnetic Resonance Imaging Connectivity by Independent Component Analysis-Based Epileptogenic Zones with Intracranial Electroencephalogram Localized Seizure Onset Zones and Surgical Outcomes in Prospective Pediatric Intractable Epilepsy Study.

The purpose of this study was to prospectively investigate the agreement between the epileptogenic zone(s) (EZ) localization by resting-state functional magnetic resonance imaging (rs-fMRI) and the seizure onset zone(s) (SOZ) identified by intracranial electroencephalogram (ic-EEG) using novel differentiating and ranking criteria of rs-fMRI abnormal independent components (ICs) in a large consecutive heterogeneous pediatric intractable epilepsy population without an a priori alternate modality informing EZ localization or prior declaration of total SOZ number. The EZ determination criteria wer

Source localization of the seizure onset zone from ictal EEG/MEG data.

INTRODUCTION: Surgical treatment of drug-resistant epilepsy relies on the identification of the seizure onset zone (SOZ) and often requires intracranial EEG (iEEG). We have developed a new approach for non-invasive magnetic and electric source imaging of the SOZ (MSI-SOZ and ESI-SOZ) from ictal magnetoencephalography (MEG) and EEG recordings, using wavelet-based Maximum Entropy on the Mean (wMEM) method. We compared the performance of MSI-SOZ and ESI-SOZ with interictal spike source localization (MSI-spikes and ESI-spikes) and clinical localization of the SOZ (i.e., based on iEEG or lesion top

Inferior-frontal cortex phase synchronizes with the temporal-parietal junction prior to successful change detection.

The inferior frontal gyrus (IFG) and the temporo-parietal junction (TPJ) are believed to be core structures of human brain networks that activate when sensory top-down expectancies guide goal directed behavior and attentive perception. But it is unclear how activity in IFG and TPJ coordinates during attention demanding tasks and whether functional interactions between both structures are related to successful attentional performance. Here, we tested these questions in electrocorticographic (ECoG) recordings in human subjects using a visual detection task that required sustained attentional exp

Effects of site-specific infusions of methionine sulfoximine on the temporal progression of seizures in a rat model of mesial temporal lobe epilepsy.

Glutamine synthetase (GS) in astrocytes is critical for metabolism of glutamate and ammonia in the brain, and perturbations in the anatomical distribution and activity of the enzyme are likely to adversely affect synaptic transmission. GS is deficient in discrete regions of the hippocampal formation in patients with mesial temporal lobe epilepsy (MTLE), a disorder characterized by brain glutamate excess and recurrent seizures. To investigate the role of site-specific inhibition of GS in MTLE, we chronically infused the GS inhibitor methionine sulfoximine (MSO) into one of the following areas o

Impact of anesthetic regimen on the respiratory pattern, EEG microstructure and sleep in the rat model of cholinergic Parkinson's disease neuropathology.

OBJECTIVES: We hypothesized that the impact of distinct anesthetic regimens could be differently expressed during anesthesia and on post-anesthesia sleep in the neurodegenerative diseases. Therefore, we followed the impact of ketamine/diazepam and pentobarbital anesthesia in a rat model of the severe Parkinson's disease cholinergic neuropathology on the electroencephalographic (EEG) microstructure and respiratory pattern during anesthesia, and on the post-anesthesia sleep. METHODS: We performed the experiments on adult, male, spontaneously breathing Wistar rats chronically instrumented for sl

Intracranial EEG-Based Directed Functional Connectivity in Alpha to Gamma Frequency Range Reflects Local Circuits of the Human Mesiotemporal Network.

To date, it is largely unknown how frequency range of neural oscillations measured with EEG is related to functional connectivity. To address this question, we investigated frequency-dependent directed functional connectivity among the structures of mesial and anterior temporal network including amygdala, hippocampus, temporal pole and parahippocampal gyrus in the living human brain. Intracranial EEG recording was obtained from 19 consecutive epilepsy patients with normal anterior mesial temporal MR imaging undergoing intracranial presurgical epilepsy diagnostics with multiple depth electrodes

Complex memories induced by intracranial electrical brain stimulation are related to complex networks.

The precise and fleeting moment of rich recollection triggered by an environmental cue is difficult to reproduce in the lab. However, epilepsy patients can experience sudden reminiscences after intracranial electrical brain stimulation (EBS). In these cases, the transient brain state related to the activation of the engram and its conscious perception can be recorded using intracerebral EEG (iEEG). We collected various EBS-induced reminiscences for iEEG analysis, classifying them as follows: no or weak details (familiarity); moderate details and context (semantic and personal semantic memories

Subcortical short-term plasticity elicited by deep brain stimulation.

OBJECTIVE: To investigate local short-term neuroplasticity elicited by subthalamic, thalamic, and pallidal deep brain stimulation (DBS) for movement disorders. METHODS: During DBS surgery, we delivered pairs of stimulus pulses with both circular and directional leads across 90 interstimulus intervals in 17 participants and recorded local field potentials from unused contacts on the implanted electrode array. We removed the stimulus artifact, validated the neural origin of the underlying signals, and examined short-term plasticity as a function of interstimulus interval and DBS target, using l

Uncovering the contribution of enhanced central gain and altered cortical oscillations to tinnitus generation.

Various theories and their associated mechanisms have been proposed as the neural basis of phantom sound perception (tinnitus), including central gain enhancement and altered cortical oscillations. However, it remains unknown whether these cortical changes directly cause tinnitus, or simply coexist with the phantom percept. Using chronically-implanted electrodes and drug delivery cannulae in rats, we examined whether enhanced central gain and cortical oscillations are consistent across different tinnitus induction methods (noise exposure; salicylate), and if directly-inducing enhanced central

Noise-Assisted Multivariate EMD-Based Mean-Phase Coherence Analysis to Evaluate Phase-Synchrony Dynamics in Epilepsy Patients.

Spatiotemporal evolution of synchrony dynamics among neuronal populations plays an important role in decoding complicated brain function in normal cognitive processing as well as during pathological conditions such as epileptic seizures. In this paper, a non-linear analytical methodology is proposed to quantitatively evaluate the phase-synchrony dynamics in epilepsy patients. A set of finite neuronal oscillators was adaptively extracted from a multi-channel electrocorticographic (ECoG) dataset utilizing noise-assisted multivariate empirical mode de-composition (NA-MEMD). Next, the instantaneou

Semiology, clustering, periodicity and natural history of seizures in an experimental occipital cortical epilepsy model.

Focal neocortical epilepsy is a common form of epilepsy and there is a need to develop animal models that allow the evaluation of novel therapeutic strategies to treat this type of epilepsy. Tetanus toxin (TeNT) injection into the rat visual cortex induces focal neocortical epilepsy without preceding status epilepticus. The latency to first seizure ranged from 3 to 7 days. Seizure duration was bimodal, with both short (approximately 30 s) and long-lasting (>100 s) seizures occurring in the same animals. Seizures were accompanied by non-motor features such as behavioural arrest, or motor seizur

Effects of agomelatine on electrocorticogram activity on penicillin-induced seizure model of rats.

Agomelatine is a new antidepressant drug acting as an antagonist of 5-hydroxytryptamine receptor 2C (5-HTR2C) and agonist of melatonergic receptors 1 and 2 (MT1 and MT2). Because of this dual action, it is an atypical antidepressant. The aim of this study was to investigate chronic anticonvulsant effects of agomelatine on penicillin-induced epilepsy model. Adult male Sprague-Dawley rats divided into four groups and were administered with tap water (vehicle), and agomelatine doses of 10 mg/kg, 50 mg/kg and 100 mg/kg for 14 days via oral gavage. After the last doses were given, epileptic seizure

On memories, neural ensembles and mental flexibility.

Memories are assumed to be represented by groups of co-activated neurons, called neural ensembles. Describing ensembles is a challenge: complexity of the underlying micro-circuitry is immense. Current approaches use a piecemeal fashion, focusing on single neurons and employing local measures like pairwise correlations. We introduce an alternative approach that identifies ensembles and describes the effective connectivity between them in a holistic fashion. It also links the oscillatory frequencies observed in ensembles with the spatial scales at which activity is expressed. Using unsupervised

Seizures triggered by pentylenetetrazol in marmosets made chronically epileptic with pilocarpine show greater refractoriness to treatment.

The efficiency of most of the new antiepileptic drugs (AEDs) on clinical trials still falls short the success reported in pre-clinical studies, possibly because the validity of the animal models is insufficient to fully represent the human pathology. To improve the translational value for testing AEDs, we propose the use of non-human primates. Here, we suggest that triggering limbic seizures with low doses of PTZ in pilocarpine-treated marmosets might provide a more effective basis for the development of AED. Marmosets with epileptic background were more susceptible to seizures induced by PTZ,

Favorable seizure and developmental outcomes without preoperative intracranial electroencephalography in pediatric patients following epilepsy surgery: A single epilepsy center retrospective study.

OBJECTIVE: At our institute, most pediatric patients undergo epilepsy surgery following a thorough presurgical evaluation without intracranial electroencephalography (EEG). We conducted an initial validation of our noninvasive presurgical strategy by assessing the seizure and developmental outcomes of 135 children. METHODS: All 135 pediatric patients were <15 years old, had undergone curative surgery, and were followed for at least 2 years postoperatively. Presurgical evaluations and postoperative seizure and developmental outcomes were investigated. Thorough noninvasive evaluation included 3
