Managing the patient with heel pain.

Heel pain presents frequently to primary care, commonly affecting athletic and elderly patients. Its presentation can be a common source of confusion for clinicians given the wide variety of differential diagnoses and the similarities in presenting symptoms and signs. This review classifies heel pain according to site of pain and explores the common pathologies clinicians may encounter. A brief summary of common imaging modalities used is provided. The literature is reviewed to guide evidence-based practice and to provide a framework to help clinicians investigate and manage heel pain before o

Platelet-Rich Plasma Versus Polydeoxyribonucleotide for Myofascial Pain Syndrome and Musculoskeletal Disorders: A Narrative Review of Mechanisms, Clinical Evidence, and Translational Considerations.

Myofascial pain syndrome (MPS) of the cervicoscapular girdle-most typically involving the upper trapezius and the rhomboid muscles-is among the most frequent causes of chronic regional musculoskeletal pain, yet the mainstays of interventional treatment (dry needling, local anesthetic trigger point injection, corticosteroid, botulinum toxin) act principally on nociceptive transmission or on the contractile taut band rather than on the disordered tissue biology that sustains it. Two injectable biologics with fundamentally different design logic have been proposed to fill this gap. Platelet-rich

Sonographic Assessment of Effectiveness of Ultrasound-guided Interventions for Soft-tissue Pain in Foot and Ankle.

BACKGROUND: Ultrasound (US)-guided interventional procedures are used to relieve soft-tissue pain in the foot and ankle. We evaluated the utility of US (including Doppler and elastography) in the response assessment of patients undergoing interventional procedures for soft-tissue pain in the foot and ankle. We also compared the imaging parameters with clinical scoring systems (Visual Analog Scale [VAS] score [rest pain] and American Orthopedic Foot and Ankle Society [AOFAS] [function] scores). METHODS: Consecutive patients with treatable soft-tissue conditions of the foot and ankle underwent

Ultrastructural insights into early myoblast differentiation induced by shockwave stimulation.

INTRODUCTION: Extracorporeal shockwave therapy (ESWT) is a non-invasive therapeutic modality that uses high-energy acoustic waves (shockwaves, SW) to restore local homeostasis and stimulate tissue healing and regeneration through mechanotransduction. ESWT has gained popularity in treating numerous musculoskeletal indications such as tendinopathies, plantar fasciitis, bony non-unions, and stress fractures, with proven benefits in reducing pain, enhancing recovery, and in some cases preventing recurrence. In contrast, SW application in muscle injuries remains less investigated. Some clinical stu

Running shoes, orthotics, and injuries.

Running is the most visible expression of the continued interest in regular physical activities. Unfortunately injuries are common, primarily due to overuse, and a number of aetiological factors have been recognised. Of these, training errors can be responsible for up to 60% of injuries. The training surface, a lack of flexibility and strength, the stage of growth and development, poor footwear and abnormal biomechanical features have all been implicated in the development of running injuries. A thorough understanding of the biomechanics of running is a necessary prerequisite for individuals w

Feasibility and acceptability of a remotely delivered, home-based "exercise snacking" to improve physical function in community-dwelling older adults: a 28-day pilot study.

BACKGROUND: Age-related declines in muscle strength and physical function increase the risk of frailty, falls, and loss of independence in older adults. Traditional structured exercise programs often have low participation because of time constraints, limited access to facilities, and health-related barriers. "Exercise snacking" defined as brief bouts of resistance exercise accumulated across the day, may offer a more flexible and feasible option. METHODS: In this 28-day pilot randomized controlled trial, 36 community-dwelling older adults aged 65-80 years were randomly assigned to a home-bas

Seven years' experience with Integra as a reconstructive tool.

The bilayered dermal substitute Integra (Integra Life Sciences Corp., Plainsboro, NJ) was developed and has been widely used as primary coverage for excised acute burns. Our take has been slightly different, finding it most useful in the management of complex soft-tissue loss and threatened extremities as the result of tendon, joint, or bone exposure. Often tasked to fill significant volume loss, we have become adept at stacked multiple-layer applications. Creative use of this material has resulted in unexpected successes with distal limb salvage; the technique takes its place beside adjacent

Platelet-rich plasma in the management of musculoskeletal disorders: a narrative review of clinical evidence and practical considerations.

BACKGROUND: Platelet-rich plasma (PRP) has emerged as a widely applied autologous biologic therapy in musculoskeletal medicine. Its concentrated growth factor content is postulated to promote tissue regeneration, modulate the inflammatory cascade, and provide clinically meaningful and durable symptom relief in both degenerative joint disease and chronic tendinopathy. Despite its widespread clinical adoption, the evidence base remains heterogeneous, and its rational application requires a critical synthesis of current findings. OBJECTIVE: This narrative review aimed to critically examine the e

Quality appraisal of systematic reviews on high-intensity laser therapy for musculoskeletal pain management: an umbrella review.

Musculoskeletal pain (MSP) remains one of the leading causes of disability worldwide. Recent approaches to treating this condition have prompted the development of several systematic reviews investigating the efficacy of high-intensity laser therapy (HILT), whose analgesic mechanisms are based on photobiomodulation neural inhibition, endorphin and serotonin release and anti-inflammatory effects. To assess the methodological quality, reliability, and validity of the systematic reviews (SRs) on HILT in MSP. This study is an overview of SRs (umbrella review) with an observational, retrospective,
