Neoadjuvant therapy and bladder substitute for invasive bladder cancer: 20 years experience at Tohoku University.

BACKGROUND: Neobladder, using the intestine, was performed after neoadjuvant therapy and total cystectomy as a treatment for invasive bladder cancer. METHODS: Between January 1977 and April 1997, an ileocecal neobladder was used for 23 patients and a sigmoid neobladder was chosen for use in 32 patients. For the diagnosis of invasive bladder cancer and the evaluation of neoadjuvant therapy, we used whole-layer core biopsy (WLCB) of the bladder tumor and fine needle aspiration biopsy (FNAB) of pelvic lymph nodes after bipedal lymphography. For neoadjuvant therapy, two to four courses of interna

Vesicostomy in the Rat Model for Low-Pressure Urinary Drainage in Neurogenic Bladder Research.

The rat is the primary animal model for preclinical research on spinal cord injury (SCI)-induced neurogenic lower urinary tract dysfunction (nLUTD). Owing to its anatomical and functional similarity to humans, rats closely recapitulate key pathological consequences of SCI. However, a crucial translational gap remains: whereas standard bladder management in humans typically relies on continuous or intermittent catheterization, comparable approaches are not feasible in rodents. The common practice of manual bladder expression generates exceptionally high intravesical pressures, which may introdu

The Hood Technique for Robot-Assisted Radical Cystoprostatectomy with Intracorporeal Neobladder.

BACKGROUND AND OBJECTIVE: The Hood technique, originally developed for robot-assisted prostatectomy, preserves periurethral and periprostatic structures critical for early continence recovery. Radical cystoprostatectomy requires wider pelvic dissection, bladder pedicle control, lymphadenectomy, and intraoperative urethral margin assessment, necessitating modification of the Hood approach. We adapted this continence-preserving dissection to robot-assisted radical cystoprostatectomy (RARC) with intracorporeal neobladder reconstruction. This study describes the technique and reports early feasibi

Botulinum toxin A in idiopathic overactive bladder: a narrative review of 5410 cases.

INTRODUCTION: When conservative treatments fail, botulinum toxin A (BoNT-A) is an option for refractory idiopathic overactive bladder (OAB). This review evaluates the efficacy, safety, and predictive factors for BoNT-A in this situation. MATERIALS AND METHODS: A literature search up to January 2025 was performed using PubMed, Google Scholar, and Embase to assess efficacy, safety, and predictors of adverse events (AE) related to BoNT-A. The risk of bias was assessed using the Risk of Bias 2 (RoB 2) tool for randomized studies and the Critical Appraisal Skills Programme (CASP) checklist for coh

Urological Evaluation of Tethered Cord Syndrome.

To describe how to perform urological evaluation in children with tethered cord syndrome (TCS). Although a common manifestation of TCS is the development of neurogenic bladder in developing children, neurosurgeons often face difficulty in detecting urological problems in patients with TCS. From a urological perspective, diagnosis of TCS in developing children is further complicated due to the differentiation between neurogenic bladder dysfunctions and transient bladder dysfunctions owing to developmental problems. Due to the paucity of evidence regarding evaluation prior to and after untetheri

Bladder neck contracture-incidence and management following contemporary robot assisted radical prostatectomy technique.

PURPOSE: Bladder neck contracture (BNC) is a well-recognized complication following robot-assisted radical prostatectomy (RARP) for treatment of localized prostate cancer with a reported incidence of up to 1.4%. In this series, we report our institutional experience and management results. METHODS: A prospectively collected database of patients who underwent RARP by a single surgeon from 2006 to 2012 was reviewed. Watertight bladder neck to urethral anastomosis was performed over 18-French foley catheter. BNC was diagnosed by flexible cystoscopy in patients who developed symptoms of bladder o

Clean intermittent catheterization as an initial management strategy provides for adequate preservation of renal function in newborns with persistent cloaca.

INTRODUCTION: Persistent cloaca is a rare, congenital anomaly involving the genital, urinary, and rectal organ systems. While prompt bowel diversion is the standard of care, the optimal method of genitourinary decompression is unclear. Bladder outlet obstruction and hydrometrocolpos are common complications that can lead to obstructive uropathy, abdominal distention, infection, perforation, and acidosis. Proposed management strategies include early surgical diversion (vesicostomy, vaginostomy, ureterostomy, nephrostomy) or clean intermittent catheterization (CIC) of the common channel. We hypo

Home screening for bacteriuria in children with spina bifida and clean intermittent catheterization.

BACKGROUND: Significant bacteriuria (SBU) and urinary tract infections (UTIs) are common in patients with spina bifida and neuropathic detrusor sphincter dysfunction. Laboratory agar plated culture is the gold standard to establish SBU. It has the disadvantage of diagnostic and subsequent therapeutic delay. Leukocyte esterase tests (LETs) and dip slides proved to be useful in the general populations to exclude SBU and UTI. The aim of this study was to evaluate the reliability of LET and dip slide in children with spina bifida without symptoms of UTI. The reliability in children with asymptomat

New treatments for incontinence.

Urinary incontinence (UI) is a common, yet underdetected and under-reported, health problem that can significantly affect quality of life. UI may also have serious medical and economic ramifications for untreated or undertreated patients, including perineal dermatitis, worsening of pressure ulcers, urinary tract infections, and falls. To prevent incontinence, the urethral sphincter must maintain adequate closure to resist the flow of urine from the bladder at all times until voluntary voiding is initiated and the bladder must accommodate increasing volumes of urine at a low pressure. UI can be

Radiographic abnormalities, bladder interventions, and bladder surgery in the first decade of life in children with spina bifida.

BACKGROUND: Spina bifida (SB) patients are at increased risk for hydronephrosis, bladder storage and emptying problems, and renal failure that may require multiple bladder surgeries. METHODS: We retrospectively reviewed patients born with SB 2005-2009, presenting to our institution within 1 year of birth. Outcomes at 8-11 years old included final renal/bladder ultrasound (RBUS) results, clean intermittent catheterization (CIC) use, anticholinergic use, surgical interventions, and final renal function. We excluded those without follow-up past age 8 and/or no RBUS or fluoroscopic urodynamic ima

Predictors of low urinary quality of life in spinal cord injury patients on clean intermittent catheterization.

OBJECTIVE: Clean intermittent catheterization (CIC) is a preferred method of bladder management for many patients with spinal cord injury (SCI), but long-term adherence is low. The aim of this study is to identify factors associated with low urinary quality of life (QoL) in SCI adults performing CIC. METHODS: Over 1.5 years, 1479 adults with SCI were prospectively enrolled through the Neurogenic Bladder Research Group registry, and 753 on CIC with no prior surgeries were included. Injury characteristics, complications, hand function, and Neurogenic Bladder Symptom Score (NBSS) were analyzed.

Are pressure pop-offs beneficial to the bladder in boys with posterior urethral valves?

BACKGROUND: Pressure pop-offs, such as high-grade vesicoureteral reflux with renal dysplasia, have historically been considered beneficial for renal and bladder outcomes in boys with posterior urethral valves (PUV). Recent longer-term studies have called into question the beneficial effects of pop-offs on renal function. OBJECTIVE: To evaluate how pop-offs affect bladder outcomes in boys with PUV. STUDY DESIGN: At a single-center, the electronic medical records of boys with PUV who underwent valve ablation from 2000 to 2014 were retrospectively reviewed for bladder and continence outcomes. P
