Persistent binocular diplopia in childhood following successful treatment for esotropia: a case series.

INTRODUCTION: Diplopia is rare in children with nonpathological esotropia because facultative suppression develops prior to visual maturation. Our aim is to present five cases of children with binocular diplopia following treatment of childhood esotropia, exploring possible mechanisms for the development of diplopia based upon the patient's history and findings, and discussing treatment for diplopia in this setting. METHODS: A retrospective chart review of five sequential patients presenting with binocular diplopia following treatment of esotropia was performed. Treatment of esotropia include

Aniridia: a comparative overview.

BACKGROUND AND PURPOSE: Aniridia is linked to a mutation of the PAX6 gene, which results in panocular anomalies. A set of common secondary pathologies associated with aniridia is recognized. Much of the literature on aniridia focuses on genetic factors of the disorder and associated abnormalities, both ocular and nonocular. The field of research on the prevalence of pathology secondary to ocular abnormalities associated with aniridia is limited, with many of the studies based on low patient numbers. This study contributes patient data on the prevalence and significance of ocular pathology in a

Infantile hemangioma: treatment with surgery or steroids.

BACKGROUND AND PURPOSE: Infantile hemangiomas (IH) often pose a significant risk to visual development. The lesions interfere with visual development by causing deprivation, astigmatism, or strabismus. Propranolol has been suggested as the new standard of care for treatment of IH. Alternative treatments such as intralesional steroids or surgery may need to be considered as equal or better treatments in some cases. The purpose of this study was to evaluate the potential risks and benefits of the various modalities for periocular IH. METHOD: A literature search was conducted for IH and proprano

The efficacy of botulinum toxin treatment for children with a persistent esotropia following bilateral medial rectus recessions and lateral rectus resections.

BACKGROUND AND PURPOSE: To report on the outcomes of treating children with a persistent esotropia with an injection of botulinum toxin in a medial rectus muscle. PATIENTS AND METHODS: The medical records were reviewed of all children at one institution with a persistent esotropia after bilateral medial rectus recessions and bilateral lateral rectus resections then treated with a botulinum toxin injection. RESULTS: Five patients with a mean preoperative esotropia of 37Δ (range 25–50Δ) underwent bilateral medial rectus recessions and then bilateral lateral rectus resections. Their residual es

Incidence of strabismus and amblyopia in preverbal children previously diagnosed with pseudoesotropia.

BACKGROUND AND PURPOSE: We previously reported that 12% of children under age 3 diagnosed with pseudoesotropia without significant refractive error later developed strabismus or mild refractive amblyopia. Mohan and Sharma recently reported on fifty-one patients with pseudoesotropia and hyperopia and noted that esotropia developed in 53.9% of the children with >1.50 D of hypermetropia compared to 2.6% of those who had ≤1.50 D hypermetropia, implying a low risk of esotropia unless hyperopia was greater than 1.50 D on initial exam. We reviewed our data to see if we had similar findings in our pat

Surgical management of strabismus following vitreo-retinal surgery.

Motility disorders and strabismus after vitreo-retinal surgical intervention have multiple etiologic factors. The main focus here is strabismus following scleral buckling surgery. The preoperative evaluation of these patients must be inclusive for restrictions, redirected muscle forces, adherence syndromes, muscle weakness, and visual sensory disturbance. Restoration of binocular function is generally the goal of surgical intervention, but paramount to surgery is assessing the potential for binocular function. Surgical techniques for overcoming the barriers to fusion are discussed with clinica

Diplopia after glaucoma surgery.

The occurrence of strabismus and symptomatic diplopia following glaucoma surgery is becoming more common as the use of implanted setons increases. Understanding the factors contributing to the diplopia is critical to the development of treatment strategies to alleviate the patient's symptoms.

Nonsurgical management of diplopia after orbital decompression surgery.

Orbital decompression is frequently performed in patients with thyroid eye disease. The indication for an orbital decompression in most patients is threatened visual loss from corneal exposure or optic neuropathy. One of the more common complications associated with orbital decompression is the development or worsening of diplopia. Often times, surgical intervention for strabismus is delayed after orbital decompression and in these cases, nonsurgical management of diplopia is invaluable to the patient. Nonsurgical management can include occlusion therapy, prism therapy, or even botulinum toxin

Where the wild things are: when esotropia misbehaves.

Esotropia is the most common form of strabismus seen in clinic. Accommodation is known to be an important precipitating factor, and its management is usually not complex. In this paper, I have reviewed a dozen circumstances in which the presentation was complicated. For some patients, the cause of fleeting symptoms was elusive and had to be determined through careful sensory and motor testing. In others, the presentation was indicative of serious underlying pathology that warranted further investigation. Lastly, restrictive conditions may be erroneously thought to be neurological, and some of

Diplopia secondary to orbital surgery.

Diplopia may occur following any type of ocular or pericocular surgery. The surgeries most frequently associated with postoperative diplopia include: repair of orbital fracture, endoscopic sinus surgery (from inadvertent orbital penetration), and orbital decompression for thyroid-related immune orbitopathy (TRIO). Postoperative diplopia after orbital tumor resection has been reported--e.g., after excision of fibrous dysplasia and osteoma. However, a recent case series suggests diplopia after orbital tumor resection is uncommon and transient. Surgical intervention for orbital trauma carries the

Positive angle kappa: a possible sign of aniridia.

INTRODUCTION AND PURPOSE: Aniridia is a panocular disorder with variable expressivity, caused by PAX6 mutations. Foveal hypoplasia and nystagmus occur in both aniridia and albinism. Individuals with albinism have a moderately positive angle kappa. This study evaluates the angle kappa in individuals with aniridia. PATIENTS AND METHOD: We performed a retrospective chart review of recorded angle kappa for individuals with aniridia. In addition, we prospectively examined the monocular corneal light reflex in each eye of patients with congenital aniridia and central fixation. We recorded other ocu

Incomitant strabismus: does extraocular muscle form denote function?

The paradigm that an underacting extraocular muscle (EOM) is always atrophic or hypoplastic, and an overacting EOM should always be enlarged, leads to inconsistencies with clinical observations. It is inconsistent with the findings of normal extraocular muscle diameters in patients with apparent superior oblique muscle palsy, "overacting" inferior oblique muscles, and the superior rectus muscle overaction / contracture syndrome, among other clinical entities. These inconsistencies can be reconciled if one accepts the possibility that EOM contractile activity may reflect a change in neural inpu

Treatment of vertical deviations secondary to other causes.

INTRODUCTION AND PURPOSE: Surgical treatment of vertical deviations secondary to other causes can be difficult and treatment approaches may vary. The purpose of this discussion is to present surgical treatment options for the management of vertical deviations secondary to other causes including dissociated vertical deviation, primary inferior oblique overaction, superior oblique overaction, and nonspecific eccentric gaze hypertropia with diplopia. RESULTS: Surgical treatment of vertical deviations secondary to other causes is beneficial to patients. Treatment approaches may vary depending upo

Management of paretic vertical deviations.

Paretic vertical deviations are characterized by complex patterns of incomitance that make them some of the most challenging strabismus problems to treat. Optimum results are obtained by performing surgery on those muscles, selected from among the eight cyclovertical muscles in the two eyes, that minimize the incomitance. In superior oblique paresis the additional factors of torticollis and torsion need to be addressed and aberrant regeneration can alter the surgical plan in third nerve paresis.

Management of vertical deviations secondary to other anatomical abnormalities.

BACKGROUND AND PURPOSE: To review the surgical management of five vertical strabismus syndromes secondary to anatomical abnormalities. These syndromes are: 1) craniosynostosis; 2) "heavy eye syndrome" of high myopia; 3) Brown syndrome; 4) upshoot-downshoot in Duane retraction syndrome (DRS); and 5) antielevation syndrome after inferior oblique anteriorization. METHODS: The syndromes are presented from evolving to well-accepted surgical management practices based on review of the current literature. RESULTS: Surgical management techniques discussed include: 1) excyclotorsion of the muscle con

Persistently recurrent infantile esotropia.

INTRODUCTION: Children with infantile esotropia often require more than one surgery to align the eyes horizontally, for either recurrent esotropia or consecutive exotropia. We report an unusual subset of patients who had persistently recurrent esotropia despite multiple surgeries. METHODS: The records of 11 patients who presented before age one year with infantile esotropia and who had three or more surgeries for recurrent esotropia were reviewed. The number of surgeries, outcomes, and associated systemic conditions were recorded. RESULTS: These 11 patients represented 4% of children who wer

Neurological significance of small esodeviations.

A small esodeviation detected in an adult may represent an acquired event caused by a number of different mechanisms. These deviations may be comitant or incomitant, occur with or without diplopia and appear in association with other ocular or neurological findings or in isolation. The mechanisms include lateral rectus dysfunction, supranuclear gaze disturbances, anomalies in the vergence system, the influence of the near synkinesis and certain restrictive components. The presence of a small, acquired esodeviation may have neurological significance that may influence diagnosis and management d

"Double vision" as a presenting symptom in adults without acquired or long-standing strabismus.

BACKGROUND: Evaluation of adults with a symptom of acquired double vision is a challenging diagnostic problem. This retrospective report reviews a series of adult patients who presented with a symptom of "double vision" but did not have diplopia related to a recently acquired or decompensated strabismus. The symptom of double vision was related mainly to blurred vision and often was not true binocular diplopia. METHODS: This is a retrospective study of medical records. RESULTS: 261 patients, age 40 years or older referred for a recent onset symptom of double vision were reviewed. Sixty-seven

"Was it something I said?": finding retinal pathology without actually examining the retina.

INTRODUCTION: Diplopia secondary to maculopathy is a rare disturbance of binocular vision that may present to the orthoptist. The purpose of this study was to identify the distinguishing symptoms and clinical signs of this condition. METHODS: A retrospective medical record review of adults presenting with diplopia that could not be relieved with prism or strabismus surgery. RESULTS: Six cases were identified. All complained of vertical diplopia, though only one patient had a vertical strabismus. The diplopia improved in dim illumination in all cases. All patients noted monocular visual distu
