Characteristic Similarities of Irritability Between Autism and Disruptive Mood Dysregulation Disorder.

Objective: Irritability in children with autism spectrum disorder (ASD) is prominent and often leads to distress to both autistic children and their families. However, the nature of irritability in autism and the difference from nonautistic children have rarely been examined. This study aimed to investigate the clinical characteristics of irritability in autism, and to compare the symptom profiles with those of disruptive mood dysregulation disorder (DMDD) in nonautistic children. Methods: Fifty-six children aged 7-17 years (mean age 10.36 ± 3.05) were recruited into this study (21 with DMDD,

Gadolinium contrast agents: dermal deposits and potential effects on epidermal small nerve fibers.

Small fiber neuropathy (SFN) affects unmyelinated and thinly myelinated nerve fibers causing neuropathic pain with distal distribution and autonomic symptoms. In idiopathic SFN (iSFN), 30% of the cases, the underlying aetiology remains unknown. Gadolinium (Gd)-based contrast agents (GBCA) are widely used in magnetic resonance imaging (MRI). However, side-effects including musculoskeletal disorders and burning skin sensations were reported. We investigated if dermal Gd deposits are more prevalent in iSFN patients exposed to GBCAs, and if dermal nerve fiber density and clinical parameters are li

The effect of patient-led goal setting approach combined with pain neuroscience education or manual therapy in patients with chronic low back pain: protocol for a parallel-group randomized controlled trial.

BACKGROUND: Low back pain (LBP) is a multifactorial disorder associated with a high range of physical and psychological burden on the society. Patient-led goal-setting approach has shown potential effects for improving chronic LBP. However, there are few studies investigating its benefits when combined with a supplementary intervention. This paper, therefore, presents a protocol for a randomized control trial (RCT) to study the effect of a patient-led goal-setting approach combined with pain neuroscience education (PNE) or manual therapy (MT) among patients with chronic LBP. METHODS: A total

Mapping acute lesion locations to physiological swallow impairments after stroke.

Dysphagia is a common deficit after a stroke, and it is frequently associated with pneumonia, malnutrition, dehydration, and poor quality of life. It is not yet fully clear which brain regions are directly related to swallowing, and how lesions affect swallow physiology. This study aimed to assess the statistical relationship between acute stroke lesion locations and impairment of specific aspects of swallow physiology. We performed lesion symptom mapping with 68 retrospectively recruited, acute, first-ever ischemic stroke patients. Lesions were determined on diffusion weighted MRI scans. Post

Dermatitis herpetiformis and neurological dysfunction.

Dermatitis herpetiformis and coeliac disease are gluten sensitive diseases, which have common immunopathological and genetic mechanisms. Neuropsychiatric complications have been reported in up to 26% of patients with coeliac disease. This is probably an overestimate, because of the chance associations with some common neurological conditions such as epilepsy. The pathogenesis is speculative but it has been postulated that gluten is neurotoxic possibly via immune mechanisms. The frequency of neurological dysfunction in patients with dermatitis herpetiformis has not been characterised. Patients

Uneven Treadmill Training for Rehabilitation of Lateral Ankle Sprains and Chronic Ankle Instability: Protocol for a Pragmatic Randomized Controlled Trial.

BACKGROUND: Lateral ankle sprains (LASs) are common injuries among military service members. Approximately 40% of individuals with an LAS progress to develop chronic ankle instability (CAI), a condition that results in substantial mechanical and neurophysiological impairment and activity limitation. Since proprioceptive and balance training improve functional outcomes and prevent secondary injury following LAS, they are recommended in clinical practice. Uneven treadmills are an innovative modality that challenge the sensorimotor system while performing an ecologically valid task simulating env

Peripheral nerve injuries in the athlete.

Peripheral nerves are susceptible to injury in the athlete because of the excessive physiological demands that are made on both the neurological structures and the soft tissues that protect them. The common mechanisms of injury are compression, traction, ischaemia and laceration. Seddon's original classification system for nerve injuries based on neurophysiological changes is the most widely used. Grade 1 nerve injury is a neuropraxic condition, grade 2 is axonal degeneration and grade 3 is nerve transection. Peripheral nerve injuries are more common in the upper extremities than the lower ext

The interoceptive communication model (ICM 2.0): a state-dependent neurophysiological framework for behavioral and cognitive accessibility.

Behavioral, emotional, and cognitive variability across contexts remains insufficiently explained by models that rely on environmental contingencies, cognitive processes, or static trait-based constructs. Individuals may exhibit marked fluctuations in executive functioning, emotional regulation, learning access, behavioral flexibility, and adaptive responses despite retaining knowledge, insight, or previously acquired skills. These patterns are frequently reported in trauma-related conditions, chronic stress, autism spectrum disorder, psychiatric disorders, rehabilitation, and educational sett

Evaluating the Efficacy of Repetitive Transcranial Magnetic Stimulation Combined With Auditory Integration Training for Children With Autism Spectrum Disorder: Protocol for a Randomized Sham-Controlled Trial.

BACKGROUND: Autism spectrum disorder (ASD) represents a significant public health challenge characterized by persistent social communication deficits and restricted, repetitive patterns of behavior. Current interventions show limited efficacy, particularly for core symptoms. Repetitive transcranial magnetic stimulation (rTMS) and auditory integration training (AIT) have independently demonstrated promise in addressing neurophysiological abnormalities associated with ASD. OBJECTIVE: This study aims to evaluate the clinical efficacy of a combined rTMS and AIT intervention compared to rTMS alone

Disruption of low-frequency narrowband EEG microstate networks in Parkinson's disease with mild cognitive impairment.

BACKGROUND: Electroencephalogram (EEG) microstates provide insights into large-scale brain network coordination, revealing distinct neural dynamics within specific frequency bands associated with cognitive processes and neurological disorders. Critical gaps remain regarding the abnormalities of narrowband microstate networks in Parkinson's disease with mild cognitive impairment (PD-MCI), a key prodromal stage of the development of PD dementia. Given the importance of early detection and understanding of cognitive decline in PD-MCI, this study investigated whether alterations in narrowband EEG

Depression Recognition Using Machine Learning Algorithms With Eye Tracking, Visual Evoked Potentials, and Auditory P300 Among Chinese Medical Students.

BACKGROUND: Current assessment of depression primarily relies on psychological scales. Although the use of machine learning in depression has grown, limited reports are available on multiple neurophysiological measurements. We employed machine learning algorithms incorporating eye tracking, visual evoked potentials (VEPs), and auditory P300 to classify depression among Chinese medical students. METHODS: A total of 66 students with depression and 72 matched controls were recruited; eye tracking, VEPs, and auditory P300 data were collected. Transcranial direct current stimulation to the left dorsolateral prefrontal cortex enhances early dexterity skills with the left non-dominant hand: a randomized controlled trial.

BACKGROUND: The left dorsolateral prefrontal cortex (DLPFC) is involved in early-phase manual dexterity skill acquisition when cognitive control processes, such as integration and complexity demands, are required. However, the effectiveness of left DLPFC transcranial direct current stimulation (tDCS) on early-phase motor learning and whether its effectiveness depends on the cognitive demand of the target task are unclear. This study aimed to investigate whether tDCS over the left DLPFC improves non-dominant hand dexterity performance and determine if its efficacy depends on the cognitive deman

Pain-autonomic measures reveal nociceptive sensitization in complex regional pain syndrome.

BACKGROUND: Allodynia and hyperalgesia are common signs in individuals with complex regional pain syndrome (CRPS), mainly attributed to sensitization of the nociceptive system. Appropriate diagnostic tools for the objective assessment of such hypersensitivities are still lacking, which are essential for the development of mechanism-based treatment strategies. OBJECTIVES: This study investigated the use of pain-autonomic readouts to objectively detect sensitization processes in CRPS. METHODS: Twenty individuals with chronic CRPS were recruited for the study alongside 16 age- and sex-matched h

Keeping the end in mind: Preliminary brain and behavioral evidence for broad attention to endpoints in pre-linguistic infants.

Infants must learn to carve events at their joints to best understand who is doing what to whom or whether an object or agent has reached its intended goal. Recent behavioral research demonstrates that infants do not see the world as a movie devoid of meaning, but rather as a series of sub-events that include agents moving in different manners along paths from sources to goals. This research uses behavioral and electrophysiological methods to investigate infants' (10-14 months) attention to disruptions within relatively unfamiliar human action that does not rely on goal-objects to signal attai

Feasibility of multi-domain data acquisition in patients at risk for hypoxic-ischemic brain injury following cardiac arrest: a prospective cohort study.

INTRODUCTION: Hypoxic-ischemic brain injury is a determinant of neurologic injury and death among survivors of cardiac arrest. While neurophysiologic and biomarker changes may improve prognostication, the feasibility of acquiring multimodal physiologic, biologic, imaging, and patient-centered outcome data across the continuum of care is not established. OBJECTIVE: To evaluate the feasibility of (1) patient recruitment; (2) acquiring multi-domain variables; and (3) six-month post-discharge outcomes in a cohort of patients with hypoxic-ischemic brain injury after cardiac arrest. METHODS: We co
