Single-enhancing CT lesions in Indian patients with seizures: a review.

Single enhancing CT lesions are the commonest radiological abnormality in Indian patients with new-onset partial seizures. In few patients the lesions may be 'tuberculoma' (especially in presence of evidence of tuberculosis elsewhere). However, histopathological studies have proved that neurocysticercosis is the most frequent cause for these lesions. Acute inflammation in and around the cerebral lesions of cysticercosis manifests as acute seizure disorder. These cysticercal granulomas represent 'colloidal' and 'nodular-granular' stages of Escobar's pathological classification of natural evolut

The supraorbital approach: an alternative to traditional exposure for the surgical management of anterior fossa and parasellar pathology.

OBJECT: The use of a supraorbital craniotomy as a minimally invasive neurosurgical technique to treat pathology located in the anterior cranial fossa and parasellar region is reported. MATERIALS AND METHODS: 25 patients were operated upon using the supraorbital keyhole technique to expose various lesions located in the anterior skull base. Included were benign and malignant tumors, AVM, tuberculoma and trauma. DISCUSSION AND CONCLUSION: Utilizing small eyebrow incisions, a small supraorbital ("keyhole") craniotomy, and microneurosurgical and/or endoscopically assisted access, allowed us to g

Combination of single-voxel proton MR spectroscopy and apparent diffusion coefficient calculation in the evaluation of common brain tumors.

BACKGROUND AND PURPOSE: MR spectroscopy and apparent diffusion coefficient (ADC) calculation have been used frequently for tumor grading and differentiation during the last decade. We evaluated whether the combination of these two techniques can improve the diagnostic effectiveness of MR imaging in patients with brain tumors. METHODS: Forty-nine patients with histologically proved brain tumors (eight high- and 12 low-grade astrocytomas, eight metastases, eight nonastrocytic gliomas, seven meningiomas, three dysembryoplastic neuroepithelial tumors (DNETs), and three tuberculomas) were prospect

[Manifestations and diagnosis of extrapulmonary tuberculosis].

Extra pulmonary tuberculosis currently represents 25 per cent of notified tuberculosis in France. All organs can be involved but the most frequent localisation are the lymph nodes. Immunodepressed patients notably those infected with HIV are particularly prone to a polyvisceral tuberculous infection. The diagnosis is often difficult on account of the polymorphism of the disease and the weak specificity of the clinical manifestation. A firm diagnosis always rests on histological or microbiological evidence. Certain recent developments should enable a more rapid microbiological diagnosis notably

Differential diagnosis of CNS lesions in AIDS patients by FDG-PET.

OBJECTIVE: The aim of this study was to evaluate the potential of FDG-PET in the differential diagnosis of CNS lesions in patients with AIDS, particularly to differentiate between toxoplasmosis and lymphoma. MATERIALS AND METHODS: We examined 11 AIDS patients, 6 with toxoplasmosis, 1 with a tuberculoma, and 4 with a primary CNS lymphoma. The FDG uptake within the lesion was compared with the uptake in a contralateral brain area [standardized uptake value (SUV)]. RESULTS: In all subjects with cerebral infections (toxoplasmosis, tuberculoma), the SUV ratio was significantly (p < 0.05) lower th

Solitary tuberculous brain lesions: 24 new cases and a review of the literature.

A solitary tuberculous brain lesion (STBL) can be difficult to distinguish from a glioma, metastasis or other infectious disease, especially from a pyogenic brain abscess. We analyzed the clinical characteristics, diagnostic procedures and outcomes of 24 patients with STBL diagnosed in three centers from France, India and Mexico. We also reviewed 92 STBL cases previously reported in the literature. General symptoms were found in 54% of our patients, including enlarged lymph nodes in 20%. Cerebrospinal fluid was typically abnormal, with lymphocytic pleocytosis and a high protein level. The lung

Magnetization transfer analysis of brain tumor, infection, and infarction.

Magnetization transfer (MT) imaging has been used to characterize tissues. A prospective analysis of magnetization transfer ratio (MTR) was performed on 107 patients with brain tumors, infection, and infarction to determine its efficacy in differential diagnosis. MTRs of brain tumor, infection, and infarction were significantly lower than those of normal gray and white matter. The cystic centers of infection had significantly higher MTRs than infarct and cystic tumors. The MTRs of infarction were significantly higher than those of cystic low-grade gliomas and benign tumors. The non-enhancing c

Prevention of secondary ischemic events by superficial temporal artery-middle cerebral artery bypass surgery after tuberculosis-induced vasculopathy in a 5-year-old child.

Tuberculosis-related vasculopathy and resulting ischemic events are a common finding after tuberculous meningitis (TBM) in children. The authors report on a case of recurring ischemic episodes that were refractory to repetitive endovascular procedures (transluminal angioplasty, stent insertion, in-stent vessel dilation). After failure of endovascular treatment, extracranial-intracranial (EC-IC) bypass surgery was performed to prevent further ischemia. This 5-year-old girl presented with a series of ischemic events in the left middle cerebral artery (MCA) territory 12 months after her presentat

The utility of stereotactic biopsy of intracranial lesions in the diagnosis of leukemia complicated by central nervous system lesions.

BACKGROUND: Leukemia complicated by central nervous system (CNS) lesions (LCNSL) includes leukemia involving the CNS (CNSL) and CNS secondary lesions related to leukemia treatment (e.g., CNS infections, leukoencephalopathy, inflammatory demyelination, and vascular diseases). The clinical manifestations and imaging characteristics of different types of LCL are similar, increasing the possibility of misdiagnosis. This study aimed to enhance our understanding and management of LCL. METHODS: We retrospectively collected clinical data from 22 patients with LCL and analyzed their magnetic resonance

Secretory phospholipase A2 plays an essential role in microglial inflammatory responses to Mycobacterium tuberculosis.

In previous studies, we have shown that reactive oxygen species (ROS)-mediated inflammatory signaling is essential for microglial proinflammatory responses to Mycobacterium tuberculosis (Mtb). To further investigate the molecular mechanisms governing these processes, we sought to describe the role of phospholipase A(2) (PLA(2)) in Mtb-induced ROS generation and inflammatory mediator release by microglia. Inhibition of secretory PLA(2) (sPLA(2)), but not cytosolic PLA(2) (cPLA(2)), profoundly abrogated Mtb-mediated ROS release, the generation of various inflammatory mediators (tumor necrosis fa

Pontine Tuberculoma Mimicking an Astrocytic Tumor: A Case Report.

Tuberculomas located in the brainstem are rare entities. This article presents a case study detailing the diagnosis and treatment process of a patient with a pontine tuberculoma, as well as a literature review of tuberculomas located in the pons. A 39-year-old male patient showed progressive quadriparesis and deficits in VII, XI, and XII cranial nerves. MRI image initially suggested a pontine astrocytoma. A second biopsy revealed the presence of acid-fast bacilli. Following six months of anti-tuberculous treatment, the patient showed successful recovery.

A 64-year-old woman with rapid neurologic decline diagnosed with Toxoplasma encephalitis after presumed metastatic cancer.

In an era of fragmented medical care, concurrent clinical features that ultimately lead to a unified diagnosis may not be prioritized appropriately. We present a case of a 64-year-old woman referred to hematology clinic for anemia, with recent memory loss and gait disturbance. Two months later, she developed pneumonia; imaging workup showed a left renal mass. Neurologic function continued to decline precluding definitive nephrectomy. She then presented with new-onset seizure and initial neuro-imaging was reported as unremarkable. One month later, outpatient neurologic workup demonstrated new l
