Patterns of recovery and change in verbal and nonverbal functions in a case of crossed aphasia: implications for models of functional brain lateralization and localization.

We present a 2-year verbal and nonverbal follow-up of a crossed aphasic patient. The patient had suffered from widespread ischemic damage in the area of right middle cerebral artery, with a parieto-temporal lesion. Three months postonset he showed classical Wernicke's aphasia associated with oral, limb and constructional apraxia and left hemineglect. However, follow-up findings showed a complex, dynamic pattern entirely consistent with cognitive models of language and nonlanguage abilities. Current models of functional brain lateralizations could not satisfactorily account for such longitudina

Impaired Retention of Motor Learning of Writing Skills in Patients with Parkinson's Disease with Freezing of Gait.

BACKGROUND: Patients with Parkinson's disease (PD) and freezing of gait (FOG) suffer from more impaired motor and cognitive functioning than their non-freezing counterparts. This underlies an even higher need for targeted rehabilitation programs in this group. However, so far it is unclear whether FOG affects the ability for consolidation and generalization of motor learning and thus the efficacy of rehabilitation. OBJECTIVE: To investigate the hallmarks of motor learning in people with FOG compared to those without by comparing the effects of an intensive motor learning program to improve ha

Dysgraphia in Korean patients with Alzheimer's disease as a manifestation of bilateral hemispheric dysfunction.

BACKGROUND: In writing, linguistic (i.e., spelling) and nonlinguistic (i.e., arranging strokes or letters) functions are processed by the left and right hemispheres, respectively. The configuration of Korean alphabet, 'Hangul' invokes nonlinguistic, visuospatial functions that other writing systems use less extensively. Patients with Alzheimer's disease (AD) have bilateral involvement of temporoparietal-frontal areas that are responsible for processing language and visuospatial functions. OBJECTIVES: The aim of this study was to examine the nature of Hangul writing dysfunction, which may be a

Legal medical consideration of Alzheimer's disease patients' dysgraphia and cognitive dysfunction: a 6 month follow up.

BACKGROUND: The purpose of this study was to investigate the ability of Alzheimer's disease (AD) patients to express intentions and desires, and their decision-making capacity. This study examines the findings from a 6-month follow-up of our previous results in which 30 patients participated. MATERIALS AND METHODS: The patient's cognition was examined by conducting the tests of 14 questions and letter-writing ability over a period of 19 days, and it was repeated after 6 months. The difference between these two cognitive measures (PQ1 before-PQ2 before), tested previously and later the writing

Reading aloud in jargonaphasia: an unusual dissociation in speech output.

A patient is described who showed several dissociations between oral and written language processing after bilateral retrorolandic vascular lesion. Dissociation was firstly between abolished auditory comprehension and preserved written comprehension and then involved confrontation naming, clearly superior in the written modality. The third striking dissociation involved oral output; spontaneous speech, although fluent and well articulated, consisted of neologistic jargon, while reading aloud was clearly superior though not perfect. Data are discussed with reference to a cognitive model of word

Visual association pathways in human brain.

Visual information processing are realized by the posterior association cortex spreading in front of the striate and parastriate areas from which two major visual association pathways arise. The dorsal or the occipito-parietal pathway which transmits the inputs from the peripheral as well as the central visual field to the parietal association cortex is responsible for the visuospatial analysis of the visual informations. The occipito-temporal or the ventral pathway originates only from the foveal vision area, and sends the visual inputs to the inferior temporal lobe which engages in visual pa

[Amusia and aphasia of Bolero's creator--influence of the right hemisphere on music].

OBJECTIVES: The experience with cortical localization (BA 44, 45, 22) of language (Broca, Wernicke and others) in the left hemisphere has been repeatedly tested over the last 150 years and is now generally accepted. A single case report with autopsy findings (Leborgne, Tan tan), has enabled to localize the seat of spoken language in the left third frontal convolution. As music and language have a lot in common and even share the same hearing system, it is logical to try to localize the cognitive centers for music too. METHODS: The disabling neurological disease illness of Maurice Ravel (1875-

Callosal warning syndrome.

OBJECTIVE: To report the clinical and imaging findings in a patient with an initial fluctuating disconnection syndrome due to corpus callosal ischemia that ultimately culminated in infarction with persistent symptoms. CASE REPORT: A 40-year-old, hypertensive, right-handed man presented with transient, stereotyped symptoms of corpus callosal disconnection (intermanual conflict, apraxia, dysgraphia and construction difficulties in his left hand). Serial magnetic resonance imaging scans demonstrated the ischemic nature of the initial fluctuating symptoms and later showed callosal infarction when

Is developmental dyslexia modality specific? A visual-auditory comparison of Italian dyslexics.

Although developmental dyslexia is often referred to as a cross-modal disturbance, tests of different modalities using the same stimuli are lacking. We compared the performance of 23 children with dyslexia and 42 chronologically matched control readers on reading versus repetition tasks and visual versus auditory lexical decision using the same stimuli. With respect to control readers, children with dyslexia were impaired only on stimuli in the visual modality; they had no deficit on the repetition and auditory lexical decision tasks. By applying the rate-amount model (Faust et al., 1999), we

[Language disorders. Diagnosis and treatment].

AIMS: The study reviews language disorders in children. Taking their normal development as the starting point, the work puts forward a differential diagnosis based on the symptoms presented in the moment the patient visited the physician. It also suggests an approach for children with language disorders from the neuropaediatric point of view while also updating the management of some of its forms. DEVELOPMENT: The acquisition of language is one of the key milestones in the development of children. A child's social and intellectual development is affected by delayed acquisition of language and

Double dissociation between representational personal and extrapersonal neglect.

BACKGROUND: Although perceptual and representational neglect are frequently associated, the demonstration of a double dissociation between both neglect forms suggests that both rely on different central mechanisms. In addition, perceptual neglect can be selectively observed within personal space or extrapersonal space. However, it is not known whether the latter dissociation also exists in representational neglect. METHODS: The authors investigated this question in two brain-damaged patients with anatomically different lesions sites, using neuropsychological tests specifically designed to ass

Incomplete split-brain syndrome in a patient with chronic Marchiafava-Bignami disease.

A 40-year-old male patient is presented who survived Marchiafava-Bignami disease and recovered sufficiently to be assessed neurologically and neuropsychologically in some detail. Besides dementia, lack of initiative, and psychomotor retardation here ascribed to extracallosal damage, he showed a number of symptoms of hemispheric disconnection such as left-sided apraxia, poor bimanual coordination in specific laboratory tests, and deficits in the interhemispheric transfer of somaesthetic information. Markedly asymmetric presentation in multiple system atrophy.

BACKGROUND: Multiple system atrophy (MSA) presents with fairly symmetrical, levodopa unresponsive parkinsonism and additional features like autonomic dysfunction, cerebellar and corticospinal tract involvement. Marked asymmetry in atypical parkinsonism suggests alternative diagnosis like Corticobasal syndrome (CBS). METHODS: We describe five unusual cases, who presented initially with markedly asymmetric parkinsonism, rigid dystonic abnormal limb posturing and subsequently developed clinical and/or radiological features consistent with probable MSA-P. RESULTS: Using the internationally accep

A retrospective analysis of the sentence writing component of the Mini Mental State Examination: cognitive and affective aspects.

BACKGROUND: One of the components of the Mini Mental State Examination (MMSE) is the request to write a sentence. We investigated the relationship between the characteristics of the written sentence of the MMSE and the cognitive and affective status of elderly patients. METHODS: The characteristics of the sentence were compared to the total MMSE score, sociodemographic characteristics, tests evaluating cognition and affective status, and diagnoses. RESULTS: The number of words was significantly associated with the degree of cognitive impairment, whereas the emotional polarity of sentences an

Phonological dyslexia and dysgraphia: cognitive mechanisms and neural substrates.

To examine the validity of different theoretical assumptions about the neuropsychological mechanisms and lesion correlates of phonological dyslexia and dysgraphia, we studied written and spoken language performance in a large cohort of patients with focal damage to perisylvian cortical regions implicated in phonological processing. Despite considerable variation in accuracy for both words and non-words, the majority of participants demonstrated the increased lexicality effects in reading and spelling that are considered the hallmark features of phonological dyslexia and dysgraphia. Complex visual hallucination and mirror sign in posterior cortical atrophy.

OBJECTIVE: In posterior cortical atrophy (PCA), visual hallucinations are rare symptoms and mirror sign has not been described. METHOD: Single case report. RESULTS: We reported a 60-year-old woman with PCA who reported complex visual hallucinations, such as a man walking in her room, and mirror sign, which was the perception of a stranger staring at her when she looked into a mirror. She could not recognize images of herself in the mirror correctly, although she could recognize that a person standing next to her and the images of that person reflected in the mirror were the same person. One hundred males with Asperger syndrome: a clinical study of background and associated factors.

The objective of this study was to investigate the background and associated factors in a representative group of young males with Asperger syndrome (AS) presenting at a specialized autism clinic. One hundred males aged 5 years 6 months to 24 years 6 months, with a mean age of 11 years 4 months (SD 3y 10mo), who had a clinical diagnosis of AS were included in the study. An in-depth review of their medical records and neuropsychological test data was performed. There was a high rate (51%) of non-verbal learning disability (defined as Verbal IQ more than 15 points higher than Performance IQ), bu

Impaired picture recognition with preserved object naming and reading.

A case study is reported on a left posterior cerebrovascular accident involving the infracalcarine cortex. The patient (HG) exhibited a marked color anomia and picture naming deficit (photograph anomia) without alexia. However, objects were named successfully from either visual or tactile inspection. Despite successful object categorization in several tasks, it is argued that HG's picture naming deficit is dependent on a disorder of recognition (access to the stored structural descriptions for objects). A similar functional impairment can account for HG's impaired color naming. The site of HG'
