Regenerative Medicine Approaches to Stress Urinary Incontinence.

Stress urinary incontinence (SUI) affects a significant proportion of women and often requires surgical intervention when conservative treatments fail. While midurethral slings (MUS) are widely used, concerns over complications such as mesh exposure/erosion and chronic pain have driven interest in regenerative medicine alternatives. This review explores emerging strategies, including stem cell therapies, platelet-rich plasma injections, decellularized extracellular matrix scaffolds, injectable hydrogels, and bioengineered slings. These approaches aim to restore continence by promoting tissue r

Genipin-Cross-Linked Decellularized Amnion Membrane with Antioxidant and Anti-Inflammatory Properties.

The regulation of post-implantation inflammatory responses is essential for biomaterials to achieve their intended functions, with macrophages playing a pivotal role in this process. The persistence of M1 macrophages impairs the timely resolution of acute inflammation, thereby delaying tissue healing and compromising biomaterial performance. The decellularized amnion membrane (DAM) exhibits notable anti-inflammatory properties. However, its limited mechanical strength and poor degradation resistance restrict its clinical applications. This study employed genipin cross-linking to improve the me

Evaluation of dECM hydrogel-NAP on 3D organotypic human corneal epithelium in diabetic keratopathy model.

Diabetes mellitus, a global epidemic, is a leading cause of blindness due to its detrimental effects on the retina, lens, and cornea. Diabetic keratopathy (DK), a significant ocular complication, severely compromises the corneal epithelium structure, barrier function, and healing capacity, largely driven by hyperglycemia-induced oxidative stress and apoptosis. Current therapeutic strategies for DK are limited and primarily symptomatic, failing to address underlying cellular deficits. This study investigates a novel regenerative approach utilizing a decellularized extracellular matrix (dECM) hy

Reconstructing protein conformations to activate follicular enzyme-nutrient reservoirs for reversing follicle aging.

Hair regeneration depends on a reservoir of follicular enzymes that regulate the microenvironment and provide metabolic support. However, in degenerative alopecia, this enzymatic reservoir is disrupted, impairing microenvironmental homeostasis. Here, through bioinformatics and molecular dynamics analyses, we revealed that several Zn2+/Cu2+-dependent proteases (e.g., LOX and SOD1) were downregulated and exhibit aberrant conformations and activities in degenerative alopecia, further weakening microenvironmental support. To address this, we developed a Zn2+/Cu2+-doped decellularized extracellular

Injectable therapeutic system incorporating neurogenesis-programmed stem cells concomitantly promoting muscle regeneration treats stress urinary incontinence.

Stress urinary incontinence (SUI) remains a significant clinical challenge due to the lack of strategies that simultaneously address muscle degeneration, neurogenic atrophy, and vascular deficits. Here, we report an innovative injectable system that combines a thermo-responsive poly(N-isopropylacrylamide)-COOH/leucine/decellularized extracellular matrix hydrogel with adipose-derived stem cells (ADSCs) pre-programmed by zeolitic imidazolate framework-8/polyethylene glycol 200@magnesium (ZIF-8/PEG200@Mg) nanoparticles. In vitro, programmed ADSCs exhibit enhanced neurogenic differentiation, while

Use of bioactive extracellular matrix fragments as a urethral bulking agent to treat stress urinary incontinence.

Injection of urethral bulking agents is a low-risk, minimally invasive surgical procedure to treat stress urinary incontinence (SUI). In this study, we developed a promising injectable bulking agent comprising extracellular matrix fragments of adipose-derived stem cell sheets (ADSC ECM) and investigated its effectiveness in urethral bulking therapy. The structural integrity and proteins of ADSC sheet ECM were well retained in decellularized ADSC ECM fragments. To locate transplanted ADSC ECM fragments, they were labeled with ultrasmall super-paramagnetic iron oxide nanoparticles, which enabled

Evaluation of bone regeneration potential of injectable extracellular matrix (ECM) from porcine dermis loaded with biphasic calcium phosphate (BCP) powder.

Extracellular matrix (ECM) contains a wide array of complex proteins, growth factors and cytokines that regulate cell behavior and tissue development. ECM harvested from non-homologous ECM sources still provide a structural support and biochemical cues to cells for effective tissue remodeling. The aim of this study is to evaluate the effect of non-tissue specific decellularized ECM from porcine dermis loaded with biphasic calcium phosphate powder (BCP) in bone regeneration. Thermosensitive ECM hydrogels with BCP powder exhibited a porous morphology with a suitable injectability and increased m

An off-the-shelf artificial cardiac patch improves cardiac repair after myocardial infarction in rats and pigs.

Cell therapy has been a promising strategy for cardiac repair after injury or infarction; however, low retention and engraftment of transplanted cells limit potential therapeutic efficacy. Seeding scaffold material with cells to create cardiac patches that are transplanted onto the surface of the heart can overcome these limitations. However, because patches need to be freshly prepared to maintain cell viability, long-term storage is not feasible and limits clinical applicability. Here, we developed an off-the-shelf therapeutic cardiac patch composed of a decellularized porcine myocardial extr

Lung Extracellular Matrix Hydrogels-Derived Vesicles Contribute to Epithelial Lung Repair.

The use of physiomimetic decellularized extracellular matrix-derived hydrogels is attracting interest since they can modulate the therapeutic capacity of numerous cell types, including mesenchymal stromal cells (MSCs). Remarkably, extracellular vesicles (EVs) derived from MSCs display similar functions as their parental cells, mitigating tissue damage in lung diseases. However, recent data have shown that ECM-derived hydrogels could release other resident vesicles similar to EVs. Here, we aim to better understand the contribution of EVs and ECM-vesicles released from MSCs and/or lung-derived h

Hydrogel delivery systems in intervertebral disc degeneration: Current status and future perspectives.

Intervertebral disc degeneration (IVDD) is a primary contributor to the development of low back pain, characterized by core pathological features which include extracellular matrix (ECM) degradation, oxidative stress, imbalanced inflammatory microenvironment, and cellular functional exhaustion. Conventional therapies (e.g., pharmacological interventions and surgical treatments) face challenges in achieving functional regeneration of intervertebral discs. Hydrogel delivery systems, leveraging their unique "structure-function" dual advantage, have emerged as an ideal bridge linking palliative ca

Engineered heart tissue models from hiPSC-derived cardiomyocytes and cardiac ECM for disease modeling and drug testing applications.

Cardiac tissue engineering provides unique opportunities for cardiovascular disease modeling, drug testing, and regenerative medicine applications. To recapitulate human heart tissue, we combined human induced pluripotent stem cell-derived cardiomyocytes (hiPSC-CMs) with a chitosan-enhanced extracellular-matrix (ECM) hydrogel, derived from decellularized pig hearts. Ultrastructural characterization of the ECM-derived engineered heart tissues (ECM-EHTs) revealed an anisotropic muscle structure, with embedded cardiomyocytes showing more mature properties than 2D-cultured hiPSC-CMs. Force measure

Adipose tissue-derived mesenchymal stem cells promote the vascularization of pancreatic islets transplanted into decellularized pancreatic skeletons.

We have recently developed a model of pancreatic islet transplantation into a decellularized pancreatic tail in rats. As the pancreatic skeletons completely lack endothelial cells, we investigated the effect of co-transplantation of mesenchymal stem cells and endothelial cells to promote revascularization. Decellularized matrix of the pancreatic tail was prepared by perfusion with Triton X-100, sodium dodecyl sulfate and DNase solution. Isolated pancreatic islets were infused into the skeletons via the splenic vein either alone, together with adipose tissue-derived mesenchymal stem cells (adMS

Bioengineering of the Uterus.

Impairment of uterine structure and function causes infertility, pregnancy loss, and perinatal complications in humans. Some types of uterine impairments such as Asherman's syndrome, also known as uterine synechiae, can be treated medically and surgically in a standard clinical setting, but absolute defects of uterine function or structure cannot be cured by conventional approaches. To overcome such hurdles, partial or whole regeneration and reconstruction of the uterus have recently emerged as new therapeutic strategies. Transplantation of the whole uterus into patients with uterine agenesis

Decellularized CNS-Derived ECM Hydrogels as Dual-Action Scaffolds for Angiogenesis and Neurogenesis.

Neurodegenerative disorders of the central nervous system (CNS) remain a significant clinical challenge due to the limited regenerative capacity of neural tissues. Herein, bioactive scaffolds were developed from decellularized extracellular matrices (dECMs) derived from four CNS tissues: brain, spinal cord, spinal meninges, and optic nerve. The scaffolds were comprehensively characterized with respect to biochemical composition, mechanical properties, and biological performance. Optimized decellularization protocols efficiently removed cellular components while preserving key matrix constituen

Injectable Extracellular Matrix Microparticles Promote Heart Regeneration in Mice with Post-ischemic Heart Injury.

Ischemic heart injury causes permanent cardiomyocyte loss and fibrosis impairing cardiac function. Tissue derived biomaterials have shown promise as an injectable treatment for the post-ischemic heart. Specifically, decellularized extracellular matrix (dECM) is a protein rich suspension that forms a therapeutic hydrogel once injected and improves the heart post-injury response in rodents and pig models. Current dECM-derived biomaterials are delivered to the heart as a liquid dECM hydrogel precursor or colloidal suspension, which gels over several minutes. To increase the functionality of the d

Engineering a multiphasic, integrated graft with a biologically developed cartilage-bone interface for osteochondral defect repair.

Tissue engineering is a promising approach to repair osteochondral defects, yet successful reconstruction of different layers in an integrated graft, especially the interface remains challenging. The multiphasic, functionally integrated tissue engineering graft described herein mimics the entire osteochondral tissue in terms of structure and composition at the cartilage, bone and cartilage-bone interface layer to repair osteochondral defects. In this manuscript, we report the fabrication of a multiphasic graft via bonding of a cartilaginous hydrogel and a sintered poly(lactic-co-glycolic acid)
