PURPOSE: Circular RNAs (circRNAs), generated by backsplicing, are stable closed-loop RNAs. CircRNAs in amniotic fluid (AF) may serve as diagnostic and prognostic markers for Congenital Diaphragmatic Hernia (CDH). This study profiled AF circRNAs from CDH pregnancies to explore associations with CDH pathways and survival. METHODS: AF was collected from CDH pregnancies undergoing fetoscopic endoluminal tracheal occlusion (FETO) (n = 8; 4 survivors, 4 non-survivors). CircRNAs were profiled using the Arraystar Human circRNA Array V2. Differential expression was analyzed with limma in R and compared

OBJECTIVE: The heterogeneity of fetal surgery has led to variability in the nature and frequency of fetal instability. Understanding these differences is critical for optimizing management.
METHODS: We reviewed all fetal atrial septal interventions (FASI) and fetoscopic myelomeningocele (fMMC) repairs with fetal instability performed at our institution between March 2012 and April 2025. We described and compared the frequency, timing, etiology, and management of fetal instability.
RESULTS: During the study period, 28 unique patients underwent 35 FASI procedures, and 210 patients underwent fM

BACKGROUND: Placental chorioangioma is the most common placental tumor. Giant chorioangioma may lead to poor pregnancy outcomes. Current management includes invasive and less invasive procedure such as amnioreduction, interstitial laser coagulation (ILC) and fetoscopic laser ablation. We reported a unique case with transient fetal hydrops after ILC and amnioreduction. However, patient could continue pregnancy until term gestation without maternal and neonatal complications. CASE PRESENTATION: A 38-year-old, parous woman presented with abdominal distension at 22+6 weeks of gestation (WG) and tr

BACKGROUND: The prevention of myelomeningocele (MMC) and meningocele (MC) is a public health concern. A systematic review on economic factors associated with MMC and MC can help the policy makers to evaluate the cost-effectiveness of screening and treatment. To our knowledge, this is the first systematic review to provide up-to date pharmacoeconomic evidence of all economic studies present in literature on different aspects of MMC and MC.
METHODS: We searched in the National Health Service Economic Evaluation Database (NHSEED), PubMed, Cost-effectiveness Analysis Registry (CEA Registry), Cent

OBJECTIVE: To assess the perinatal outcome after fetal reduction in complicated monochorionic (MC) twin pregnancies by comparing different techniques.
METHODS: A retrospective cohort study at a national referral center comparing data between four techniques: interstitial laser coagulation, radiofrequency ablation (RFA), fetoscopic laser coagulation (FLC) and bipolar cord coagulation (BCC). The primary outcome was the mortality of the co-twins. Secondary outcomes were preterm pre-labor rupture of membranes (PPROM), gestational age at delivery and neonatal morbidity.
RESULTS: 259 MC twin pregn

OBJECTIVE: Complicated monochorionic biamniotic (MCBA) twin pregnancies may require treatment with fetoscopic laser photocoagulation, in case of Twin Oligo-Polyhydramnios Sequence or need of a selective termination of pregnancy. Patients requiring these treatments would need medical transfer to Metropolitan France because these are unavailable in Réunion Island. We evaluated the outcomes of MCBA pregnancies in Reunion Island with indications for fetoscopy, with a view to discussing the interest of training doctors on the Reunion Island, to practice fetoscopy on site.

OBJECTIVE: MicroRNAs (miRNAs) are used as biomarkers in cardiovascular disease and cancer. miRNAs are involved in placental development but have not previously been investigated in twin-twin transfusion syndrome (TTTS). Our aim is to explore the miRNA profile of TTTS pregnancies.
METHOD: Initial miRNA profiling was performed using a reverse transcription polymerase chain reaction (RT-PCR) panel on maternal serum samples taken from five women prior to fetoscopic laser ablation for TTTS and compared with serum samples from five women with uncomplicated monochorionic diamniotic twin pregnancies.

OBJECTIVE: Residual anastomoses after fetoscopic laser surgery for twin-to-twin transfusion syndrome (TTTS) may lead to severe postoperative complications, including recurrent TTTS and twin anemia-polycythemia sequence (TAPS). A novel technique (Solomon technique) using laser coagulation of the entire vascular equator was recently investigated in a randomized controlled trial (Solomon trial) and compared with the Standard selective laser technique. The aim of this secondary analysis was to evaluate the occurrence and characteristics of residual anastomoses in placentas included in the Solomon

OBJECTIVE: To measure amniotic fluid leptin levels in fetuses with twin-twin transfusion syndrome (TTTS) with and without placental insufficiency (PI) and determine its usefulness as a biomarker of PI.
STUDY DESIGN: A retrospective case control study of TTTS stage III patients from 2009 to 2011 was conducted. Cases were pregnancies with PI (TTTS-PI, n = 18) matched by stage, gestational age, and degree of cardiomyopathy to controls without PI (TTTS, n = 26). PI was strictly defined using biometric parameters. Amniotic fluid from recipient twins (RT) was taken during second trimester fetoscopi

Congenital Diaphragmatic Hernia (CDH) is defined by the presence of an orifice in the diaphragm, more often left and posterolateral that permits the herniation of abdominal contents into the thorax. The lungs are hypoplastic and have abnormal vessels that cause respiratory insufficiency and persistent pulmonary hypertension with high mortality. About one third of cases have cardiovascular malformations and lesser proportions have skeletal, neural, genitourinary, gastrointestinal or other defects. CDH can be a component of Pallister-Killian, Fryns, Ghersoni-Baruch, WAGR, Denys-Drash, Brachman-D

Feto-fetal transfusion syndrome (FFTS) in monochorionic triplets is a rare clinical entity which may share the principal adverse perinatal outcomes of twin-twin transfusion syndrome. Recently, favorable prognoses regarding morbidity and mortality in twins after selective laser photocoagulation of placental communicating vessels (SLPCV) have been described. But descriptions of this procedure in monochorionic triplet cases are limited. This is the case report of an experience of SLPCV applied to monochorionic triamniotic triplets with FFTS. Triplet A had polyhydramnios, while absent end-diastoli

Congenital diaphragmatic hernia (CDH) has a mortality rate of up to 77% despite optimal pre- and postnatal care. Fetuses with liver herniation, a low lung-to-head ratio, and an early diagnosis before 24 weeks have a particularly poor prognosis. In utero open repair of these fetuses does not improve patient survival. The PLUG (Plug the Lung Until it Grows) technique was reported to be able to reverse pulmonary hypoplasia in CDH. A foam plug or a titanium clip is used and the trachea can be unplugged using Ex Utero Intrapartum Tracheoplasty (EXIT) at birth. Since hysterotomy causes premature lab

Congenital diaphragmatic hernia (CDH) remains one of the most challenging congenital anomalies encountered by neonatologists and pediatric surgeons, with outcomes largely determined before birth. Although surgical repair of the diaphragmatic defect is essential, postnatal morbidity and mortality are primarily driven by antenatal pulmonary hypoplasia, abnormal pulmonary vascular development, and associated cardiac dysfunction that culminate in respiratory failure and pulmonary hypertension. Advances in prenatal imaging have enabled increasingly refined risk stratification, allowing for the iden

Selective fetal growth restriction (sFGR) in monochorionic twin pregnancies is a major contributor to perinatal morbidity and mortality with unique pathophysiological mechanisms and management challenges. sFGR is defined by significant intertwin discordance in estimated fetal weight and abnormal Doppler findings, employing the Gratacós classification (types I, II, and III) for risk stratification and management. Epidemiological data indicate sFGR affects 10% to 26% of monochorionic twins, with early-onset cases associated with the highest risk of adverse outcomes. Surveillance strategies inclu

INTRODUCTION: Fetal therapy has evolved into an established clinical specialty in fetal medicine, offering life-saving interventions for conditions with high intrauterine morbidity and mortality. Among various approaches, laser therapy has played a significant role in advancing fetal procedures.
CONTENT: The most established application of laser is fetoscopic laser coagulation for twin-twin transfusion syndrome, which significantly reduces perinatal mortality and neurological complications. Other indications include twin anemia-polycythemia sequence and, as an intrafetal application, twin rev

INTRODUCTION: Amniotic Band Syndrome (ABS) is a rare congenital anomaly that can manifest with varying types and severity of perinatal defects caused by fibrous bands generated from injured amnion. These bands trap the extremities and gradually cut off their blood supply, causing structural and functional defects, necrosis, partial or complete amputation and even death.
CASE PRESENTATION: We report a female preterm Low-birth-weight baby with the left leg completely amputated from below the calf and the right leg constricted and blackened simultaneously, born to a mother with anemia and gestat
