Symptom Localization May Differentiate Subtypes of Eustachian Tube Dysfunction.

OBJECTIVE: To evaluate the predictive ability of symptom self-localization to distinguish obstructive eustachian tube dysfunction from non-obstructive salpingitis. METHODS: Adult (age ≥18 years) patients with a primary complaint of aural discomfort who underwent diagnostic nasal endoscopy and tympanometry at a tertiary academic center were enrolled. Symptoms were self-localized by using a single finger on the affected side. All patients completed the 7-item Eustachian Tube Dysfunction Questionnaire (ETDQ-7) and underwent scoring of eustachian tube inflammation using the Endoscopic Evaluation

Endoscopic endonasal transpterygoid approaches: anatomical landmarks for planning the surgical corridor.

OBJECTIVES/HYPOTHESIS: Endoscopic endonasal transpterygoid approaches (EETA) use the pneumatization of the sinonasal corridor to control lesions of the middle and posterior skull base. These surgical areas are complex and the required surgical corridors are difficult to predict. AIM: Define anatomical landmarks for the preoperative planning of EETAs. STUDY DESIGN: Anatomical study. METHODS: We reviewed images from high-resolution maxillofacial CT scans with (0.6-mm axial slice acquisition). Cephalometric measurements were obtained using Kodak Carestream Image Software (Rochester, NY). RESU

Classification and management of cervical paragangliomas.

INTRODUCTION: Cervical paragangliomas are slow-growing tumours that eventually cause lower cranial nerve palsies and infiltrate the skull base. Surgical treatment may cause the same deficits and, in some, risks more serious neurological deficits. We describe a classification used to guide investigation, consent and management of cervical paragangliomas based on extensive experience. METHODS: The case notes of patients managed by the senior author at a tertiary referral skull base unit between 1987 and 2010 were reviewed retrospectively. A total of 87 cervical paragangliomas were identified in

Maxillary swing approach for surgical resection of recurrent nasopharyngeal tumors.

Purpose : To evaluate the efficacy of salvage nasopharyngectomy for locally recurrent nasopharyngeal tumors through the maxillary swing approach. Methods : Antero -lateral access to the nasopharynx and its vicinity through the maxillary swing approach was used. Through a Weber-Fergusson-Longmire incision, the whole maxilla is freed and swung laterally while remaining attached to the masseter muscle and cheek flap. The nasopharynx, para-pharyngeal space and infratemporal fossa are now exposed facilitating complete resection of the tumor. Results : Seven patients were treated with this approach

Transcervical vs. Transcervical-Combined Surgical Approaches for Primary Parapharyngeal Space Tumors: A Systematic Review of Surgical and Functional Outcomes.

Primary parapharyngeal space (PPS) tumors are rare neoplasms comprising 0.5% of head and neck tumors. Their complex anatomical location and proximity to critical neurovascular structures pose significant surgical challenges. While transcervical and transcervical-combined approaches represent the primary surgical techniques, no comprehensive systematic comparison exists. This systematic review compared surgical success, complications, functional outcomes, and recurrence rates between these approaches. Following PROSPERO registration (CRD420251037201), we searched PubMed, Cochrane, Web of Scienc

Muller's muscle, no longer vestigial in endoscopic surgery.

BACKGROUND: As a thin filmy covering overlaying the inferior orbital fissure (IOF), Muller's muscle was considered a vestigial structure in humans, and for this reason, its anatomical significance was neglected. Because of increasing interest in endonasal approaches to the skull base that encompasses this region, we re-examined this structure's role as an anatomical landmark from an endoscopic perspective. METHODS: In 10 cadaveric specimens, microanatomical dissections were performed (n = 5); endoscopic dissections were performed (n = 5) via approaches of the middle turbinate or inferior turb
