Iatrogenic upper urinary tract injuries during ureteroscopy for urolithiasis: a comprehensive review on incidence, mechanisms and preventative strategies.

The incidence of urolithiasis is increasing globally, with a prevalence of 13% in North America and 9% in Europe. Ureteroscopy is a minimally invasive approach for treating conditions affecting the upper urinary tract, including urolithiasis, for which its efficacy and safety is well recognized. There is a risk of complications associated with ureteroscopy, including iatrogenic mechanical ureteric injuries. These injuries are multifactorial in nature, with ureteroscopes and auxiliary endoscopic equipment having an important role, in addition to patient and stone factors. Excessive friction and

Preclinical Studies on the Safety and Toxicity of Photoditazine in the Antibacterial Photodynamic Therapy of Uropathogenic Bacteria.

The 'dusting' technique of lithotripsy for the removal of infected urinary calculi and the wide use of drainage after endoscopic surgery may stimulate spreading of multidrug-resistant bacterial strains. Antibacterial photodynamic therapy (PDT) is one promising method for the elimination these strains. The purpose of our study was to evaluate alterations of renal pelvis morphology and renal function in laboratory animals after bactericidal regimens of PDT. Renal pelvises of pigs were filled with Photoditazine and then assessed either by examining the accumulation of Photoditazine in the urothel

Retrograde ureteroscopy for distal ureteric stone removal in children.

OBJECTIVE: To compare the efficacy and safety of ureteroscopy plus intracorporeal lithotripsy (ULT) with extracorporeal shock wave lithotripsy (ESWL) for treating distal ureteric calculi in childhood, as such stones are commonly treated by ESWL as the first option in adults but there is no agreement on the method of treating them in children. PATIENTS AND METHODS: From July 2002 to July 2003, children presenting with ureteric stones were consecutively randomized for treatment using ULT or ESWL. The two groups were matched for age, sex and stone position in the distal ureter. A 7.5 F ureterosc

Optimal Timing for Ureteral Stent Removal After Ureteroscopy to Minimize Morbidity: A Single-Center Prospective Comparative Study.

OBJECTIVE: To compare patient-reported morbidity across three ureteral stent removal protocols after ureteroscopy (URS) and holmium laser lithotripsy. PATIENTS AND METHODS: We prospectively recruited 180 patients undergoing URS for unilateral ureteric or renal stones (1-1.5 cm) from January 2021 to December 2024 at a single academic center. All procedures were performed by a single surgeon using a semirigid or flexible ureteroscope, holmium laser lithotripsy, and placement of a 6Fr × 26 cm double-J stent. Patients were sequentially allocated into three groups based on stent removal timing: Gr

Trans-urethral bladder suture in female patients: Not a tour de force but a quick and realistic answer to complex situations.

INTRODUCTION: Trans-urethral bladder surgery has gained popularity in the fields of electro-resection and laser lithotripsy, with endoscopic suturing being overlooked. Bladder defect closure using a pure trans-urethral suturing technique can provide a quick and effective solution in situations where conventional management options are not feasible. METHODS: Here we describe this innovative novel technique developed by our group that was used to treat two different cases with bladder perforation at two different institutions. We used a 5 mm laparoscopic port with gas insufflation and a laparos

Ureteral wall thickness and distal ureteral density in patients with residual fragments after Ho:YAG laser semi-rigid ureterolithotripsy.

Recent data suggest that greater ureteral density distal to ureteral stones or increased ureteral wall thickness (UWT) can predict impacted stones. The aim of our study was to evaluate if patients with residual fragments present with greater ureteral density and larger UWT when compared to stone-free patients. From January onward, a consecutive series of patients undergoing semi rigid Ho:YAG laser ureterolithotripsy (ULT) for ureteral stones were enrolled. A non-contrast enhanced computed tomography (CT) scan was performed before the procedure to evaluate distal ureteral density (DUD) and wall

The Effect of Ratio of Endoscope-Sheath Diameter on Intrapelvic Pressure During Flexible Ureteroscopic Lasertripsy.

PURPOSE: To clarify appropriate combinations of flexible ureteroscope (FURS) and ureteral access sheath (UAS), which can maintain a low intrapelvic pressure (IPP) during FURS with lasertripsy. METHODS: Four kinds of FURSs (Polyscope, Olympus URF-P6, Stroz Flex-Xc, and Olympus URF-V) and six sizes of UASs (11/13F × 36 cm, 11/13F × 46 cm, 12/14F × 36 cm, 12/14F × 46 cm, 13/15F × 36 cm, and 13/15F × 46 cm) were tested. A novel gas-liquid-pressure-control system (GLPCS) was used to maintain a stable and adjustable irrigation pressure form 50-500 cmH2O. IPPs and flow rates were recorded with diffe

Holmium laser endourethrotomy and litholapaxy of an occult prostatic utricle calculus.

Prostatic utricle (PU) stones are rare and the surgical treatment of such PU anomalies has proved challenging given their location adjacent to the posterior urethra and proximity to important fertility and continence structures. We report the case of a 58-year-old male with hypospadias, microphallus, and irritative lower urinary tract symptoms (LUTS) found to have a large PU stone. He underwent successful complete endoscopic removal of the stone via transurethral unroofing and holmium laser litholapaxy. We conclude that it is an effective alternative to other surgical techniques once size of t

Management of Calcified Coronary Lesions-A Review of Plaque Modification Methods.

Coronary artery disease remains the leading cause of cardiovascular morbidity and mortality worldwide, affecting millions of individuals each year. Coronary artery calcification is a common finding in patients with advanced atherosclerosis and represents an important determinant of procedural success during percutaneous coronary intervention. Severe calcifications are associated with increased procedural complexity and elevated complication rates due to challenging lesion preparation, suboptimal stent expansion, and less favorable long-term clinical results. This review summarizes the present

Calcified plaque modification during percutaneous coronary revascularization.

The presence and severity of calcified coronary plaque negatively impacts angiographic and clinical outcomes following percutaneous coronary intervention (PCI). Severe coronary calcification is associated with suboptimal stent delivery, deployment, apposition and expansion which can lead to in-stent restenosis and/or thrombosis. Severe coronary calcification is associated with incremental hazard for adverse clinical events, including death, during 5-10 years following PCI despite the use of new generation drug- eluting stents. Multiple technologies including high-pressure noncompliant and modi

Minimally Invasive Approach to Intrauterine Device Migration Into the Bladder Using Holmium Laser.

The intrauterine device (IUD) is a widely utilized method of contraception known for its high efficacy, safety profile, and long-term effectiveness. Despite its favorable characteristics, rare but potentially serious complications may occur, such as uterine perforation and device migration into adjacent pelvic or abdominal structures. One of the less frequent but clinically relevant complications is intravesical migration, where the device perforates the uterine wall and erodes into the urinary bladder. This can result in chronic lower urinary tract symptoms, recurrent urinary tract infections

[A CASE REPORT OF VESICAL CALCULUS FORMATION WITH CHROMIC CATGUT AT THE URETEROVESICAL ANASTOMOTIC SITE 28 YEARS AFTER RENAL TRANSPLANTATION].

A 69-year-old man underwent renal transplantation due to chronic renal failure of unknown cause in 1991. Furthermore, in 2012 he again underwent renal transplantation due to renal graft dysfunction with focal segmental glomerulosclerosis. After the second renal transplantation, his renal function has been stable. In 2019, he presented to the urology department with gross hematuria. Cystoscopy revealed a 2 cm vesical calculus at the dome of the bladder near the right lateral wall. Therefore, we performed transurethral lithotripsy using the holumium laser method. The vesical calculus was crushed

Asymptomatic Obstructive Ureterolithiasis Due to a Periureteral Venous Ring.

Background: Periureteral venous rings are a rare congenital anomaly involving the inferior vena cava (IVC) and the right ureter, where the ureter courses through a venous ring made by the duplication of the IVC during embryogenesis. This anatomic anomaly is also referred to as a transcaval ureter. Although most patients are asymptomatic and radiographic findings are incidental, some patients can be symptomatic. We present the first reported case of asymptomatic obstructive ureterolithiasis at the level of a periureteral venous ring that was effectively treated with endoscopic management. Case
