As a result of the successful restoration and conservation programs deployed by state and federal agencies, populations of the North American river otter (Lontra canadensis) are increasing in many states. Recreational activities such as swimming, boating, and fishing increase the likelihood of human interactions with this charismatic, nonendangered mustelid. Otters tend to avoid areas of high human activity, occur at low population densities, and in some habitats in the United States have not recovered from population declines. Therefore, interactions with humans are rare, and aggressive encou

OBJECTIVES: In India, venomous snakebite remains an enigma. Although ineffective first aid treatments that are centuries old continue to be used by people bitten by snakes, important factual information, such as the importance and uniqueness of bites by the northern saw-scaled viper (Echis sochureki), has been largely lost and forgotten. In this paper, we report the first systematically gathered data on the clinical course of envenoming by E sochureki in Rajasthan, India. Clinical information is reported on 12 victims bitten by definitively identified E sochureki, and 2 clinical cases are desc

OBJECTIVE: Venomous and nonvenomous snakes are found throughout most of the United States. While the literature on treatment is robust, there is not a current national epidemiologic profile of snakebite injuries in the United States. National estimates of such injuries treated in emergency departments (EDs) are presented along with characteristics of the affected population.
METHODS: Data on snakebite injuries were abstracted from the National Electronic Injury Surveillance System-All Injury Program (2001-04). Variables included age, gender, body part affected, cause, disposition, and treatme

OBJECTIVE: The purpose of this study was to characterize big game hunter visits to a rural hospital's emergency department (ED). Using data collected on fatalities, injuries, and illnesses over a 9-year period, trends were noted and comparisons made to ED visits of alpine skiers, swimmers, and bicyclists. Out-of-hospital hunter fatalities reported by the county coroner's office were also reviewed. Cautionary advice is offered for potential big game hunters and their health care providers.
METHODS: Self-identified hunters were noted in the ED log of a rural Colorado hospital from 1997 to 2005,

OBJECTIVE: To evaluate the effect of L-carnitine supplementation on improving endurance exercise in normobaric/normoxic and hypobaric/hypoxic environments.
METHODS: Six-week-endurance-trained male Sprague-Dawley rats (n = 24) were randomly divided into 2 groups: control and experimental; the latter group was supplemented with L-carnitine, administered orally in a dose of 100 mg x kg(-1) body weight. The animals were supplemented for 25 days under ambient normobaric/normoxic conditions and thereafter were exposed to 72 hours of hypobaric hypoxia equivalent to 6100 m. The supplementation was co

Families are traveling with their children in increasing frequency. Travel to Asia offers children many opportunities to learn about new cultures and history. It also offers the potential for exposure to numerous infectious agents not commonly encountered in the United States. Families must begin to prepare for travel to Asia weeks before departure. Children should be up to date on routine vaccinations. Appropriate education should be given on arthropod avoidance and malaria prophylaxis. Additional education and possible prophylaxis should be completed for other infectious agents frequently en

A case description is presented of a 9-member rowing team whose scull swamped on a small lake in Victoria, Canada, because of a sudden winter storm, which immersed them in 4 degrees C water for 50 minutes until a small rescue boat found them in darkness. Another 13 minutes of cold exposure in 6.7 degrees C air occurred during boat transport to waiting ambulance paramedics. Two rowers died, one from severe hypothermia and the other from drowning as a consequence of cold incapacitation and hypothermia. The 2 coldest rowers, who were transported 8 km to a major hospital, arrived with rectal tempe

A 16-year-old girl was seriously stung on her abdomen by a jellyfish as she jumped on her small surfboard. She and her mother identified the animal from photographs as Chrysaora fuscescens. Within several minutes the girl developed a massive abdominal cutaneous eruption composed of hundreds of punctuate erythematous papules and macules, which persisted for 5 to 7 days. Persistent urinary incontinence and biliary dyskinesia appeared over the following night. It is theorized that a systemic uptake of venom occurred percutaneously after contact of the jellyfish tentacles with her abdominal skin.

OBJECTIVE: High altitude and exposure to cold are associated with significant levels of dehydration because of cold-altitude urine output, high energy expenditures, and poor access to water. The aims of the present study were to measure the fluid intake and urine output among military mountaineers during their stay at high altitude and to study the level of fluid intake and decrease in urine output in relation with acute mountain sickness (AMS).
METHODS: This study used an analytic prospective follow-up design of hydration-dehydration conditions of a group of mountaineers with similar charact

OBJECTIVE: To examine the use of protective headgear by surfers, their perceptions of its usefulness, and barriers to its use.
METHODS: A researcher-administered questionnaire was used to undertake a cross-sectional survey of 646 surfboard riders at 8 popular surfing beaches in Victoria, Australia. The main outcome measures were rate of use of headgear, perceptions of head injury risk relative to a range of other activities, perceptions regarding headgear, and the reasons for not wearing headgear.
RESULTS: Most surfers were men (90.2%), young (mean age 28.2 years), and experienced (mean year

OBJECTIVE: To evaluate the causes of human fatalities in the United States from 1991 to 2001 that were caused by venomous and nonvenomous animal encounters exclusive of zoonotic infections or animal-vehicle collisions.
METHODS: An inquiry of CDC Wonder, a database for epidemiologic research, was used to provide information on animal-related fatalities on the basis of ICD-9 and ICD-10 codes.
RESULTS: From 1991 to 2001, 1943 persons died in the United States after venomous and non-venomous animal encounters. An average of 177 fatalities per year were recorded. Venomous animal encounters were r

OBJECTIVE: As competition climbing becomes increasingly popular, younger climbers are entering the sport, and some are among the top-level athletes. This early start combined with intensive training methods can lead to radiographic changes in the fingers and even osteoarthrosis. Since 1994, we have been observing an increasing number of nontraumatic epiphyseal fractures in young athletes.
METHODS: Twenty-four cases of young climbers with nontraumatic epiphyseal fractures of the finger middle joints are presented.
RESULTS: The average age of the climbers was 14.5 (+/- 0.9) years; 23 were boys

As more people use the oceans for recreational purposes, encounters with potentially dangerous sea creatures are becoming more common. We report the case of a scuba diver bitten by a moray eel off the coast of Cuba. The diver received an extensive crush avulsion injury with near loss of his right upper arm. A review of the existing literature identifies the significant characteristics of such attacks. Given the potential seriousness and complications of these attacks, a greater awareness among both treating physicians and emergency personnel may improve overall management of injuries. In addit

Since its identification in New York City in 1999, the West Nile virus has spread to 45 states and caused human infections in at least 44 states. West Nile virus is difficult to correctly diagnose without a high level of clinical suspicion and can cause severe debilitation or death in those with the most severe symptoms. This paper reviews the epidemiology, surveillance, prevention, clinical features, diagnosis, and treatment of this important emerging infection. Lessons learned from West Nile virus are discussed at the conclusion of the paper.

OBJECTIVE: Scuba diving-specific injuries have been well described. However, the injury experiences of individual divers over long diving careers have rarely been investigated. Our objective was to study the acute and chronic injuries of experienced, recreational scuba divers.
METHODS: This was an international, cross-sectional, descriptive postal survey of experienced, recreational scuba divers belonging to diving clubs in Australia and the United States.
RESULTS: Seven hundred nine divers were enrolled (346 Australian divers and 363 US divers). Most participants were experienced (mean numb

There are many tales describing ferocious schools of piranha attacking humans, but there are few scientific data supporting such behavior. The very few documented instances of humans attacked and eaten by piranha schools include 3 that occurred after death by other causes (eg, heart failure and drowning). These predaceous fishes, however, do occasionally injure bathers and swimmers in lakes and rivers. The characteristic profile of most injuries is a single bite per victim, generally related to the fish defending its brood. This paper describes an outbreak of piranha bites in a dammed river po
