Shyness versus social phobia in US youth.

OBJECTIVES: Scholars and the popular press have suggested that the diagnostic entity of social phobia "medicalizes" normal human shyness. In this study we examined the plausibility of this hypothesis by (1) determining the frequency of shyness and its overlap with social phobia in a nationally representative adolescent sample, (2) investigating the degree to which shyness and social phobia differ with regard to sociodemographic characteristics, functional impairment, and psychiatric comorbidity, and (3) examining differences in rates of prescribed medication use among youth with shyness and/or

Morbid obesity is associated with fear of movement and lower quality of life in patients with knee pain-related diagnoses.

OBJECTIVE: To compare fear of movement in patients with different body mass index (BMI) values referred for rehabilitative care of the knee and to examine whether this fear contributed to self-reported knee-related function. We hypothesized that fear of movement would be elevated with increasing BMI and that fear would correspond with lower self-report knee-related function and lower quality of life (QOL). DESIGN: Retrospective cross-sectional study. SETTING: Outpatient therapy clinic affiliated with a tertiary care hospital. PATIENTS: Patients with knee pain diagnoses (n = 278) were strati

Effects of attention training on self-reported, implicit, physiological and behavioural measures of spider fear.

Cognitive theories hold that biased attention to threat plays a prominent role in the development and maintenance of anxiety disorders. In support of this view, attention training has been shown to affect emotional reactivity. An important limitation of most attention training studies is that they almost exclusively rely on self-report measures to assess changes in fear. In the present study, we trained attention towards or away from spiders. We assessed not only self-reported spider fear, but also implicit spider associations, physiological, and behavioural measures of spider fear. Although w

Investigating a critical evaluation tendency in social anxiety.

Models of social phobia suggest that socially anxious individuals have critical evaluation expectancies, expecting others to be inherently critical in their appraisal of performance. One potential source for these expectancies is generalization or projection of an individual's own critical evaluation tendencies. We recruited 89 students, informing them that they would be asked to deliver an impromptu speech. Participants were shown three short pre-recorded speeches and asked to rate the performance of the speaker in each. Participants were also asked to rate how well they thought they would pe

Social anxiety and HIV transmission risk among HIV-seropositive male patients.

The role of psychological factors in predicting HIV sexual transmission risk behavior is increasingly of interest in prevention research. Social anxiety, or anxiety about being evaluated in interpersonal situations, is associated with unprotected insertive anal intercourse among young men who have sex with men (MSM) and with other behavioral risk factors for unprotected intercourse, such as depression, smoking, alcohol use, and drug use. Social anxiety may be especially relevant in understanding HIV risk among HIV-seropositive men, given its stronger association with unprotected insertive than

Temperament and character dimensions in patients with social phobia: patterns of change following treatments?

The aim of this study was to examine Temperament and Character Inventory (TCI) profiles in patients with social phobia (DSM-IV) and to outline patterns of change following intensive group cognitive therapy (IGCT), individual cognitive therapy (ICT) and treatment as usual (TAU). One hundred patients recruited by advertisements in local papers were randomized to IGCT, ICT and TAU. Patients (n=59) who completed diagnostic evaluation and TCI assessments at baseline and 1-year follow-up were examined in this study. Patients differed from healthy controls in novelty seeking (NS), harm avoidance (HA)

Treating adolescents with social anxiety disorder in school: an attention control trial.

BACKGROUND: Anxiety disorders are often undetected and untreated in adolescents. This study evaluates the relative efficacy of a school-based, cognitive-behavioral intervention compared to an educational-supportive treatment for adolescents with social anxiety disorder. METHODS: Thirty-six students (30 females), ages 14 to 16, were randomized to a 12-week specific intervention, Skills for Social and Academic Success (SASS), or a credible attention control matched for structure and contact, conducted in school. RESULTS: Independent evaluations and adolescent self-reports indicated significant

Attentional Blink to emotional and threatening pictures in spider phobics: electrophysiology and behavior.

When two targets have to be identified in a rapid serial visual presentation (RSVP) paradigm, perception of the second target (T2) becomes significantly impaired if it is displayed 200-500 ms after the first target (T1), a phenomenon labeled as "Attentional Blink" (AB). Here we investigate 14 spider phobics and 16 controls in an RSVP paradigm with neutral T1s. T2 pictures were neutral, emotional (positive or negative) or threatening (spiders for spider phobics). In addition, event-related potentials in response to T2 targets were analyzed. Both spider phobics and controls correctly identified

Botulinum toxin treatment of social anxiety disorder with hyperhidrosis: a placebo-controlled double-blind trial.

OBJECTIVE: Given the often prominent and persistent nature of hyperhidrosis in social anxiety disorder (SAD), to compare botulinum toxin type A to placebo for generalized SAD with hyperhidrosis, in combination with paroxetine. METHOD: Adults with severe axillary hyperhidrosis who met DSM-IV criteria for generalized SAD were randomly assigned to receive 1-time, bilateral, intradermal injections with either botulinum toxin type A or placebo (50 units/axilla). All subjects also received 8 weeks of open-label treatment with paroxetine. The primary outcome measure was the Hyperhidrosis Disease Sev

Etiology and neurobiology of social anxiety disorder.

Social anxiety disorder (SAD) is influenced by multiple genetic and environmental factors. Imaging genomics combines genotyping with neuroradiological techniques, such as functional MRI (fMRI) and positron emission tomography (PET), to investigate samples relevant to psychiatric pathophysiology. Neuroanatomical areas implicated in SAD include the amygdala, prefrontal cortex, hippocampus, and striatum. Recent investigations have suggested that allelic polymorphisms may play a role in the disorder; 2 candidate genes, the serotonin transporter (SLC6A4) and catechol-O-methyl transferase (COMT), ar

Levetiracetam in social phobia: a placebo controlled pilot study.

While serotonergic antidepressants are now established as first-line pharamcotherapy for generalized social anxiety disorder (SAD), other agents with different mechanisms have shown promise in treating SAD. The aim of this pilot study is to examine the efficacy and safety of levetiracetam (LEV), an anticonvulsant with calcium channel modulating properties, in treating SAD. Adult outpatients meeting DSM-IV criteria for SAD were randomly assigned (2:1) to double-blind treatment with either LEV (500-3000 mg/day) or placebo (PBO) for 7 weeks. The primary outcome measures were the change from basel

Persistence of "past-life" memories in adults who, in their childhood, claimed memories of a past life.

This article tests the consistency and the continuation of alleged "past-life" memories from childhood into adulthood and the possible detrimental effects of such childhood memories on the development into adult life. Twenty-eight adults aged 28 to 56 years who had claimed to have memories of a past life when they were children were interviewed in Lebanon. Their memories had been recorded when they were children, at the mean age of 6 years. Of the 28 participants, 24 still reported some past-life memories, whereas 4 had forgotten everything. Twenty-one were sure that their memories were a cont

High preoperative depression, phobic anxiety, and binge eating scores and low medium-term weight loss in sleeve gastrectomy obese patients: a preliminary cohort study.

OBJECTIVE: Although depression, anxiety, and binge eating are prevalent in candidates for bariatric surgery, their impact on weight loss is unknown following sleeve gastrectomy. This study assesses the associations between weight loss and preoperative depression, anxiety, and binge eating scores in patients undergoing sleeve gastrectomy for morbid obesity. METHOD: This cohort study included 34 patients who underwent sleeve gastrectomy for morbid obesity between May 2006 and February 2010 in a French tertiary referral center. We assessed preoperative depression (using the Beck depression inven

Salivary testosterone: associations with depression, anxiety disorders, and antidepressant use in a large cohort study.

OBJECTIVE: Low circulating levels of testosterone have been associated with major depression, but there is more limited evidence for differences in patients with anxiety disorders. The use of selective serotonin reuptake inhibitors (SSRIs) and other antidepressants is associated with sexual side effects, warranting testing for interactions with testosterone. METHODS: Data are from 722 male and 1380 female participants of The Netherlands Study of Depression and Anxiety (NESDA), who were recruited from the community, general practice care, and specialized mental health care. Depressive and anxi

Does the judicious use of safety behaviors improve the efficacy and acceptability of exposure therapy for claustrophobic fear?

Exposure therapy is traditionally conducted with an emphasis on the elimination of safety behaviors. However, theorists have recently suggested that the judicious use of safety behaviors may improve the tolerability of this treatment without reducing its efficacy. The present study tested this notion by randomly assigning participants with high claustrophobic fear to receive a single-session intervention with or without access to safety aids during early exposure trials. Improvement was generally equivalent between the treatment conditions, and no reliable benefits or drawbacks were associated

Amygdala activation and its functional connectivity during perception of emotional faces in social phobia and panic disorder.

Social phobia (SP) and panic disorder (PD) have been associated with aberrant amygdala responses to threat-related stimuli. The aim of the present study was to examine amygdala function and its connectivity with medial prefrontal cortex (mPFC) during emotional face perception in PD and SP, and the role of illness severity. Blood oxygen level dependent responses while perceiving emotional facial expressions were compared in 14 patients with PD, 17 patients with SP, 8 patients with comorbid PD and SP, and 16 healthy controls. We found that PD, but not SP, was associated with amygdala and lingual
