Rate of fusion using novel synthetic bone graft mixed with cellular allograft product in lumbar fusions.

BACKGROUND: Over 400,000 spine fusions are performed in the United States annually with 75% involving the lumbar region. It is the indicated treatment of many chronic orthopedic conditions that fail conservative management. There are numerous surgical approaches; however, common to all is the removal of the intervertebral disc and the insertion of a bone graft which promotes arthrodesis. Iliac crest autografts are regarded as the "gold standard" bone graft material for lumbar fusions; however, they come with a significant complication rate. Recently developed biologic mixtures, such as the one

Impact of occupational characteristics on return to work for employed patients after elective lumbar spine surgery.

BACKGROUND CONTEXT: Low back pain has an immense impact on the US economy. A significant number of patients undergo surgical management in order to regain meaningful functionality in daily life and in the workplace. Return to work (RTW) is a key metric in surgical outcomes, as it has profound implications for both individual patients and the economy at large. PURPOSE: In this study, we investigated the factors associated with RTW in patients who achieved otherwise favorable outcomes after lumbar spine surgery. STUDY DESIGN/SETTING: This study retrospectively analyzes prospectively collected

Posterior Lumbar Interbody Fusion with Interspinous Fastener Provides Comparable Clinical Outcome and Fusion Rate to Pedicle Screws.

OBJECTIVE: To investigate the clinical feasibility and validity of interspinous fastener (ISF) for lumbar degenerative diseases. METHODS: From October 2013 to March 2014, a total of 46 patients suffering from lumbar degenerative diseases underwent posterior lumbar interbody fusion (PLIF) randomly augmented by ISF or pedicle screws. The clinical outcome was primarily measured by Oswestry Disability Index (ODI) score. The minimal clinical important difference (MCID) was defined as an eight-point decrease in ODI. The second clinical outcome measurement was Japanese Orthopedic Association (JOA) s

Dynamic stabilization using the Dynesys system versus posterior lumbar interbody fusion for the treatment of degenerative lumbar spinal disease: a clinical and radiological outcomes-based meta-analysis.

OBJECTIVE The Dynesys, a pedicle-based dynamic stabilization (PDS) system, was introduced to overcome the drawbacks of fusion procedures. Nevertheless, the theoretical advantages of PDS over fusion have not been clearly confirmed. The aim of this study was to compare clinical and radiological outcomes of patients who underwent PDS using the Dynesys system with those who underwent posterior lumbar interbody fusion (PLIF). METHODS The authors searched PubMed, Embase, Web of Science, and the Cochrane Database. Studies that reported outcomes of patients who underwent PDS or PLIF for the treatment

Quality assessment of systematic reviews for surgical treatment of low back pain: an overview.

BACKGROUND CONTEXT: Low back pain is one of the most frequent reasons for medical appointments. Surgical treatment is widely controversial, and new surgical techniques and treatment modalities have been developed within the last decade. Treatment for low back pain should be evidence-based through systematic reviews and meta-analysis. Thus, the quality of these reviews is sometimes put into question as methodological mistakes are frequently seen. PURPOSE: The aim of this study was to gather all systematic reviews for the surgical treatment of low back pain and analyze their outcomes, quality,

Interpedicular height as a predictor of radicular pain in adult degenerative scoliosis.

BACKGROUND CONTEXT: Spine surgeons must correlate clinical presentation with radiographic findings in a patient-tailored approach. Despite the prevalence of adult degenerative scoliosis (ADS), there are few radiographic markers to predict the presence of radiculopathy. Emerging data suggest that spondylolisthesis, obliquity, foraminal stenosis, and curve concavity may be associated with radiculopathy in ADS. PURPOSE: The purpose of this study was to determine if radicular pain in ADS is associated with reduced interpedicular heights (IPHs) as measured on routine radiographs. STUDY DESIGN/SET

Therapeutic sustainability and durability of coflex interlaminar stabilization after decompression for lumbar spinal stenosis: a four year assessment.

BACKGROUND: Approved treatment modalities for the surgical management of lumbar spinal stenosis encompass a variety of direct and indirect methods of decompression, though all have varying degrees of limitations and morbidity which potentially limit the efficacy and durability of the treatment. The coflex(®) interlaminar stabilization implant (Paradigm Spine, New York, NY), examined under a United States Food and Drug Administration (US FDA) Investigational Device Exemption (IDE) clinical trial, is shown to have durable outcomes when compared to posterolateral fusion in the setting of post-dec

Improving discharge data fidelity for use in large administrative databases.

OBJECT: Large administrative databases have assumed a major role in population-based studies examining health care delivery. Lumbar fusion surgeries specifically have been scrutinized for rising rates coupled with ill-defined indications for fusion such as stenosis and spondylosis. Administrative databases classify cases with the International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM). The ICD-9-CM discharge codes are not designated by surgeons, but rather are assigned by trained hospital medical coders. It is unclear how accurately they capture the surgeon's

Imaging modalities for low back pain in children: a review of spondyloysis and undiagnosed mechanical back pain.

PURPOSE: Mechanical low back pain is common in the pediatric population; recent studies have shown that undiagnosed mechanical low back pain (UMLBP) is the most common cause of low back pain presenting in adolescents, accounting for up to 78% of cases. Spondylolysis/spondylolisthesis is the most common cause with diagnosed pathology observed in this age group. The goals of this study are to: determine the natural history of low back pain, evaluate the value of radiographic studies in establishing a diagnosis of spondylolysis, and determine the cost and radiation effective doses (EDs) associate

Direct repair of defects in lumbar spondylolysis with a new pedicle screw hook fixation: clinical, functional and Ct-assessed study.

Spondylolysis is a common entity, a minority of people affected by this disease need medical care, and only a few require surgery. Reconstruction of the pars interarticularis is an interesting alternative to segmental fusion; this technique has the advantage of preserving segmental motion. Most authors report good results for young patients without intervertebral disk or facet degenerative changes. Moreover Louis also showed good to excellent results with his technique carried out among people who presented a satisfactory disk height (equal to two thirds of normal height). This could extend th

Guidelines for the performance of fusion procedures for degenerative disease of the lumbar spine. Part 12: pedicle screw fixation as an adjunct to posterolateral fusion for low-back pain.

This review focused on an examination of the literature on the surgical treatment of low-back pain in patients with DDD or low-grade degenerative spondylolisthesis treated with PLF, with or without the use of pedicle screw fixation. All Class I and the majority of Class II and Class III medical evidence on this topic indicates that the addition of pedicle screw fixation to PLF increases fusion success rates when assessed based on plain x-ray films with dynamic imaging. Although there does appear to be a positive relationship between radiographic fusion and clinical outcome, no convincing corre

Traumatic thoracic spinal fracture dislocation with minimal or no cord injury. Report of four cases and review of the literature.

OBJECT: Thoracic fracture dislocations reportedly lead to complete paraplegia in 80% of cases. It is rare for these dislocations not to cause neurological deficits, as evidenced by the mere 11 well-documented neurologically intact cases in the English-language literature. METHODS: The authors report four cases of thoracic fracture dislocation that remained neurologically intact and discuss 11 other previously published well-documented cases. Overall there were 10 men and five women with a mean age of 34 years (range 17-66 years). Mechanisms of injury included car crash in six, motorcycle cras

Paraspinal musculature and adjacent segment disease after lumbar fusion: a systematic review and meta-analysis of fusion with and without posterior instrumentation.

BACKGROUND CONTEXT: Adjacent segment disease (ASD) is the principal long-term complication following lumbar fusion, contributing to pain, disability, and the need for revision surgery. While biomechanical and patient-related factors have been proposed, iatrogenic injury to the paraspinal musculature during posterior exposure may also promote ASD. Minimally invasive anterior, oblique, and lateral approaches, which avoid posterior dissection, may mitigate this risk. PURPOSE: To compare rates of radiographic (rASD) and symptomatic ASD, ASD-related reoperation, and secondary outcomes including fu

The association between teaching hospital status and postoperative outcomes among adults undergoing long-segment posterior lumbar instrumentation.

BACKGROUND CONTEXT: Long-segment lumbar spine fusions (LSLF) are widely performed in the United States to address degenerative conditions, instability, and deformities but carry substantial risks of surgical and medical complications. With demand expected to rise alongside an aging population, understanding factors that influence outcomes is critical. Hospital teaching status may play a role, as academic centers offer multidisciplinary resources and trainee involvement, whereas nonacademic hospitals may prioritize efficiency and patient flow. Prior studies across surgical disciplines, includin

Robot-assisted versus navigated spinal fusion surgery: a comparative multicenter study on transpedicular screw placement accuracy and patient outcomes.

The transpedicular screw placement has been the gold standard for over a decade in treating degenerative diseases of the lumbo-sacral spine related to vertebral instability. The evolution of neuronavigation and robotic surgery has mitigated many perioperative complications such as mispositioning, vascular damage, and nerve structure injuries, leading to enhanced postoperative outcomes, reduced blood loss, and decreased intraoperative radiation exposure. Our study proposes a multicenter comparison between robotic surgery and neuronavigation for treating degenerative diseases of the lumbo-sacral

Development and Validation of a Prediction Model for Pain and Functional Outcomes After Lumbar Spine Surgery.

IMPORTANCE: Functional impairment and pain are common indications for the initiation of lumbar spine surgery, but information about expected improvement in these patient-reported outcome (PRO) domains is not readily available to most patients and clinicians considering this type of surgery. OBJECTIVE: To assess population-level PRO response after lumbar spine surgery, and develop/validate a prediction tool for PRO improvement. Influence of racial disparities on patient-reported satisfaction and short- and long-term perception of health status after elective lumbar spine surgery.

OBJECTIVE In spine surgery, racial disparities have been shown to impact various aspects of surgical care. Previous studies have associated racial disparities with inferior surgical outcomes, including increased complication and 30-day readmission rates after spine surgery. Recently, patient-reported outcomes (PROs) and satisfaction measures have been proxies for overall quality of care and hospital reimbursements. However, the influence that racial disparities have on short- and long-term PROs and patient satisfaction after spine surgery is relatively unknown. The aim of this study was to inv
