BACKGROUND: Cholera is a large public health issue, especially in countries with limited resources. The aim of the study was to determine trends in global cholera mortality from 1990-2019.
METHODS: This research is an observational, descriptive epidemiological study. The age-standardized rates (ASRs, per 100,000 population) of cholera mortality from 1990 to 2019 were evaluated through joinpoint regression analysis (by calculating Odds Ratio-OR, with corresponding 95% Confidence Interval-95% CI).
RESULTS: From 1990-2019 in the world, the number of deaths due to cholera in both sexes together

BACKGROUND: Mathematical models predict that community-wide access to HIV testing-and-treatment can rapidly and substantially reduce new HIV infections. Yet several large universal test-and-treat HIV prevention trials in high-prevalence epidemics demonstrated variable reduction in population-level incidence.
METHODS: To elucidate patterns of HIV spread in universal test-and-treat trials, we quantified the contribution of geographic-location, gender, age, and randomized-HIV-intervention to HIV transmissions in the 30-community Ya Tsie trial in Botswana. We sequenced HIV viral whole genomes fro

SETTING: A high proportion of individuals with multidrug-resistant tuberculosis (MDR-TB) develop permanent hearing loss due to ototoxicity caused by injectable aminoglycosides (AGs). The prevalence of AG-induced hearing loss is greatest in tuberculosis (TB) and human immunodeficiency virus (HIV) endemic countries in sub-Saharan Africa. However, whether HIV coinfection is associated with a higher incidence of AG-induced hearing loss during MDR-TB treatment is controversial.
OBJECTIVE: To evaluate the impact of HIV coinfection on AG-induced hearing loss among individuals with MDR-TB in sub-Saha

A 26 year-old female patient presented to the Tropical Medicine outpatient unit of the Ludwig Maximilians-University in Munich with febrile illness after returning from Southern Africa, where she contracted a bite by a large mite-like arthropod, most likely a soft-tick. Spirochetes were detected in Giemsa stained blood smears and treatment was started with doxycycline for suspected tick-borne relapsing fever. The patient eventually recovered after developing a slight Jarisch-Herxheimer reaction during therapy. PCR reactions performed from EDTA-blood revealed a 16S rRNA sequence with 99.4% simi

BACKGROUND: Isoniazid preventive therapy (IPT) prevents tuberculosis (TB) in people living with HIV (human immunodeficiency virus, PLWH). Symptom screening without chest radiographs (CXRs) was established as the strategy for excluding TB disease among PLWH seeking IPT in Botswana's 2001 pilot project. This strategy was evaluated in 2004-2006 among candidates screened for an IPT clinical trial.
METHODS: PLWH referred from clinics and HIV testing centers were screened for TB symptoms. All asymptomatic candidates received CXRs; those with abnormal CXRs were investigated further.
RESULTS: Among

OBJECTIVE: To assess hematologic and hepatic toxicities associated with in utero and breastfeeding exposure to maternal highly active antiretroviral therapy (HAART) among infants in Botswana.
DESIGN: A nested cohort study within a randomized clinical trial (the Mashi Study). Laboratory toxicities among infants born to women who initiated HAART before delivery were compared with toxicities among those born to women who received zidovudine and a single dose of nevirapine or placebo in labor. Infants were randomized to breastfeed with extended zidovudine or to formula-feed.
METHODS: Hemoglobin

Breastfeeding is one of the most cost-effective health interventions in existence, improving child survival through improved health and nutritional status and providing numerous benefits to the mother, including lowering the risk of breast, ovarian, and endometrial cancer. Current WHO guidelines recommend exclusive breastfeeding (EBF) for the first 6 months of an infant's life, which Ministries of Health across Southern Africa support. However, rates of EBF in this region, particularly Sub-Saharan Africa, vary widely and fail to reach global targets. Botswana and South Africa offer specific ex

Family physicians (FPs), as educators, capacity builders and clinical governance leaders of primary care (PC) teams, work to make them more effective and responsive to the needs of their patients. Various strategies are required to raise the profile of Family Medicine (FM) to ensure stronger representation in health sector planning and policy development for advocacy on behalf of the communities they serve. An illustration is given of the need for FP leaders to become equal partners in the National Surgical, Obstetric and Anaesthesia Planning process to ensure safer surgery at district hospita

Sweet sorghum variety (Sorghum bicolour (L)) commonly known as sweet reeds, Ntšhe, in Setswana, is a valuable cash crop mostly for small scale farmers in Botswana and other southern African countries. These reeds are widely consumed as a delicacy and contribute significantly to food security, employment, and rural incomes. However, infestations by the larval stages of Chilo partellus (stem borer moths) lead to substantial economic losses, as consumers reject worm-infested reeds. To mitigate these losses, valorisation of condemned sweet reeds is attractive. Here, we took advantage of our unders

A 40-year-old woman with a history of poorly controlled HIV presented to a district referral hospital in rural Botswana for a generalized skin rash of several months duration. The highly pruritic rash predominantly involved her hands and feet and was associated with bullae that were present for days at a time before rupturing without drainage or discharge. The patient endorsed night sweats, periodic fevers, occasional cough productive of blood-tinged sputum, fatigue, and weight loss. On admission, CD4 count was 46 cells/mm3 and viral load was >750000 copies/mL. Pulmonary tuberculosis testing v

As COVID-19 spreads rapidly across Africa, causing havoc to economies and disruption to already fragile healthcare systems, it is becoming clear that despite standardised global health strategies, national and local government responses must be tailored to their individual settings. Some African countries have adopted stringent measures such as national lockdown, quarantine or isolation, in combination with good hand hygiene, mandatory wearing of masks and physical distancing, to prevent an impending healthcare crisis. The impact of stringent measures in low- to middle-income African countries

Dermatophytosis is currently a disease of global importance and a public health burden. It is caused by dermatophytes, which attack and grow on dead animal keratin. Dermatophytes belong to three genera, namely, Epidermophyton, Microsporum, and Trichophyton. The predominant clinical forms and causative agents vary from one region of the world to another. Poor socioeconomic status, high population densities, and poor sanitary conditions are some of the factors responsible for the high prevalence of dermatophytosis in many developing countries, which include countries in southern and eastern Afri

OBJECTIVE: To measure the prevalence of Trichomonas vaginalis (TV) infection and bacterial vaginosis (BV) among pregnant women in Botswana, and to evaluate the syndromic approach and alternative management strategies for these conditions in pregnancy.
METHODS: In a cross-sectional study, 703 antenatal care attendees were interviewed and examined, and specimens were collected to identify TV, BV, Candida species, Chlamydia trachomatis and Neisseria gonorrhoeae. Information on reproductive tract infections earlier in pregnancy was obtained from a structured interview and the antenatal record.

The human serum resistance associated (SRA) gene isolated from a Ugandan strain of Trypanosoma brucei rhodesiense has been shown to be capable by itself of conferring the trait of human infectivity on T.b. brucei by transfection. This gene has also been identified in several other isolates of T.b. rhodesiense, but not in the other human pathogenic trypanosome in Africa, T.b. gambiense, casting doubt on its ubiquity and function. Here, we show that this gene occurs in T.b. rhodesiense from sleeping sickness foci throughout East Africa (Ethiopia, Uganda, Kenya, Tanzania, Zambia, Botswana), but i

The recommended role of ultraviolet germicidal irradiation (UVGI) is to reduce the risk of tuberculosis (TB) transmission in health care facilities. However, excess exposure may result in dermatosis and photokeratitis. In one hospital setting in Botswana, two nurses and one housekeeper complained of eye discomfort, 'like sand in the eyes', after working in an administrative office. The following day, one employee noted facial skin peeling. All symptoms resolved over 2-4 days without sequelae. Six weeks later, the syndrome recurred for all three employees. A workplace investigation revealed tha

Increasing attention is being directed toward the role of retail meat in introducing pathogens and antibiotic-resistant bacteria into local food supplies. This study characterized the antibiotic resistance (AR) and virulence of E. coli isolates from chicken and beef (n = 109) imported for retail sale in Kasane, Botswana. In this relatively pristine environment, commercial beef and chicken production is absent, resulting in reliance on imports, creating concerns that multidrug-resistant (MDR) and pathogenic E. coli may be introduced through the food supply sourced from distant regions. E. coli

Infections with strains of the Cryptococcus neoformans species complex are responsible for over 100,000 deaths per year, predominantly due to meningitis in immunocompromised individuals. Despite much research, there are no licensed fungal vaccines available. Most experimental cryptococcal vaccine formulations have been tested in preclinical models using laboratory strains of C. neoformans, particularly H99 and KN99. However, to be effective, vaccines need to protect against the wide variety of cryptococcal isolates found worldwide, particularly in regions that have the highest burden of infect
