Use of mini-temporalis transposition to improve free muscle outcomes for smile.

BACKGROUND: In managing late facial paralysis, association of cross-facial nerve grafting and free muscle transfer proves superior to any other method of mid-face reanimation. Nevertheless, predicting functional recovery remains a struggle, and in a minority of patients, results are deceiving. In this study, cases of fair or moderate outcomes from a free muscle transfer received a segmental temporalis transposition to upgrade the functional and aesthetic results. METHODS: From 1981 to 2007, 153 patients received a free muscle transfer for smile restoration in the authors' center. Of all patie

[Parapharyngeal and peritonsillar infection following mandibular third molar extraction].

Numerous complications following impacted third molar extractions have been described previously. Among these are swelling, infection, subdermal hematomas, nerve injuries, injuries to adjacent teeth and mandibular fractures. The parapharyngeal space is a funnel-shaped space with its base located at the base of the skull and its apex near the hyoid bone. It is bounded medially by the superior constrictor muscle and the tonsillar fossa and laterally by the medial pterygoid muscle, the mandibular ramus, the deep lobe of the parotid gland and the posterior belly of the digastric muscle. The juncti

MRI of local disease in nasopharyngeal carcinoma: tumour extent vs tumour stage.

The purpose was to use MRI to study in detail local tumour extension in patients presenting with nasopharyngeal carcinoma (NPC) and to compare the extent of local disease with the current T-stage classification. MR images of 150 patients with newly diagnosed nasopharyngeal carcinoma were obtained on a 1.5 T unit. 10 extranasopharyngeal sites were analysed for tumour involvement. The number of concurrently involved sites was determined. The extent of tumour invasion was compared with staging as defined by the fifth edition of the AJCC classification. The T-stage distribution was T1 21%, T2 16%,

[Glomus tumors of the temporal bone--surgical concept and results].

Between 1985 and 1991, 36 patients with glomus tumors of the temporal bone were operated. More than 60% of the patients presented with an advanced disease (Class C and D) according to the initially discret and slowly progressive clinical signs. Pulsatile tinnitus, hearing loss and paresis of the lower cranial nerves IV to XII were most often found. The management of the patients requires a complete surgical resection with different approaches depending on the extension of the tumor. High resolution CT of the temporal bone and selective angiography of the tumor feeding vessels proved as reliabl

A rare presentation of an oral cavity metachronous malignancy: Case Report.

INTRODUCTION OR BACKGROUND: Head and neck cancers, particularly those involving the oral cavity, are associated with high morbidity and mortality rates globally. Despite advancements in surgical techniques, chemotherapy, and radiotherapy, the development of second primary malignancies (SPMs) remains a critical concern for longterm survivors. The occurrence of metachronous double primary malignancies, where the second primary cancer arises at a different anatomical site and time, poses additional diagnostic and therapeutic challenges. CASE PRESENTATION: We present a rare case of a 42-year old

A rare cause of malignant otitis externa and skull base osteomyelitis.

An elderly male with type 2 diabetes presented with a 2-month history of otalgia and severe headaches. He was diagnosed with malignant otitis externa (MOE) and was commenced on empirical treatment with oral ciprofloxacin. Pseudomonas is the most common cause of MOE. A baseline CT scan was undertaken that demonstrated skull base osteomyelitis (SBO) due to findings of bone erosion at the mastoid tip and an infiltrating soft tissue mass eroding the clivus. Eight weeks later, he returned with worsening and bilateral symptoms of otitis externa, hearing loss, temporomandibular pain and dysfunction.

Evaluation of extensions of sinonasal mass lesions by computerized tomography scan.

INTRODUCTION: Cross-sectional imaging has had a significant impact on the management of the sinonasal malignancy. Staging of these lesions has been closely monitored by dependence on computerized tomography (CT) scan and now in small proportion with MRI. The objective of the study was to evaluate the reliability of CT scan in assessing the extensions of the sinonasal mass lesions and their impact on tumor staging. MATERIALS AND METHODS: All cases of sinonasal lesions were reviewed retrospectively from June 2001 to May 2006 (five years) at KMIO, Bangalore. Only those cases that had CT scan rep

[Report of a rare case of Langerhans-cell histiocytosis of the skull base].

INTRODUCTION: Langerhans cell histiocytosis, once called histiocytosis X, is a rare disease. Usually it can occur in children and is characterized by granulomas (eosinophilic granuloma and Hand-Schueller-Christian disease) or by a extensive involvement of various organs (Letterer-Siwe disease). The etiology remains uncertain and could be related to undefined immunologic disturbance. Lesions can involve bone marrow, skin, oral mucosa, retro-orbital tissue, central nervous system, lymph nodes, spleen, liver, lung, and gastroenteric tract. Surgery, radiotherapy and chemotherapy can be employed as

Vagal nerve monitoring: a comparison of techniques in a canine model.

HYPOTHESIS: An optimal technique exists for intraoperative, electrophysiologic vagal nerve monitoring. BACKGROUND: Analogous to facial nerve monitoring during lateral skull base surgery, vagal nerve monitoring may be used at surgery involving the jugular foramen, the posterior cranial fossa, the infratemporal fossa, the parapharyngeal space, and the thyroid gland to decrease the incidence of iatrogenic injuries. Laryngeal electromyography (EMG) is an accurate test of vagal nerve function: four applications have been described for use intraoperatively. The purpose of this study was to compare

Transmaxillary exploration of the intracranial portion of the maxillary nerve in malignant perineural disease. Technical note.

OBJECT: Achieving microscopically tumor-free margins during resection of skull base malignancies has consistently been identified as a positive prognostic factor for patient survival. When malignancies extend perineurally into the major nerves traversing the skull base and entering the cavernous sinus, achieving tumor-free margins can be challenging and typically necessitates performing a craniotomy to access the lateral wall of the cavernous sinus. This report describes a novel technique used to access and resect malignancy extending perineurally into the intracranial portion of V2 via the ma
