Inappropriate sinus tachycardia.

Inappropriate sinus tachycardia (IST) is a syndrome in which the sinus heart rate is inexplicably faster than expected and associated symptoms are present. The heart rate at rest, even in a supine position, can exceed 100 beats/min; minimal activity accelerates the rate rapidly and substantially. Patients with IST may require restriction from physical activity. Mechanisms responsible for IST are understood incompletely. It is important to distinguish IST from so-called appropriate sinus tachycardia and from postural orthostatic tachycardia syndrome, with which overlap may occur. Because the lo

Parasympathetic autonomic dysfunction is more often evidenced than sympathetic autonomic dysfunction in fluctuating and polymorphic symptoms of "long-COVID" patients.

Several disabling symptoms potentially related to dysautonomia have been reported in "long-COVID" patients. Unfortunately, these symptoms are often nonspecific, and autonomic nervous system explorations are rarely performed in these patients. This study aimed to evaluate prospectively a cohort of long-COVID patients presenting severe disabling and non-relapsing symptoms of potential dysautonomia and to identify sensitive tests. Autonomic function was assessed by clinical examination, the Schirmer test; sudomotor evaluation, orthostatic blood pressure (BP) variation, 24-h ambulatory BP monitori

Reduced cerebral blood flow with orthostasis precedes hypocapnic hyperpnea, sympathetic activation, and postural tachycardia syndrome.

Hyperventilation and reduced cerebral blood flow velocity can occur in postural tachycardia syndrome (POTS). We studied orthostatically intolerant patients, with suspected POTS, with a chief complaint of upright dyspnea. On the basis of our observations of an immediate reduction of cerebral blood flow velocity with orthostasis, we hypothesize that the resulting ischemic hypoxia of the carotid body causes chemoreflex activation, hypocapnic hyperpnea, sympathetic activation, and increased heart rate and blood pressure in this subset of POTS. We compared 11 dyspneic POTS subjects with 10 healthy

Median arcuate ligament syndrome: challenges, comorbidities, and controversies.

PURPOSE OF REVIEW: Median arcuate ligament syndrome (MALS) is understood to be a condition where compression of the celiac artery by the median arcuate ligament (MAL) may lead to symptoms of postprandial or exercise-induced abdominal pain, nausea, vomiting, diarrhea, oral aversion, and weight loss. This review summarizes recent literature on pediatric MALS while highlighting the challenges, comorbidities, and controversies encountered in this condition. RECENT FINDINGS: The pathophysiologic mechanism by which MALS leads to pain is currently unknown. Several overlapping conditions are describe

Sex differences between postmenopausal women and similar-age men in response to orthostatic stress following 2 wk of bed rest.

Reduced orthostatic tolerance is common following periods of bed rest that are associated with illness or surgery, putting individuals at higher risk for syncope and falls following hospitalization. Following menopause, mechanisms of female cardiovascular regulation change, which may be associated with sex-specific responses to orthostatic stress following bed rest. The purpose of our experiment was to investigate sex differences between healthy postmenopausal women and similar-age men (age: 55-65 yr) for their orthostatic tolerance and cerebrovascular responses to standing following bed rest.

Inspiratory muscle training improves cerebrovascular and postural control responses during orthostatic stress in older women.

PURPOSE: We aimed to investigate the effect of inspiratory muscle training (IMT) on, hemodynamic, cerebrovascular and postural balance responses during orthostatic stress, in older women. METHODS: Fourteen elderly women were assigned to perform IMT at 50% of maximal inspiratory pressure (MIP) (IMT group, n = 8) or placebo training at 5% MIP (Sham group, n = 6), in a counter-balanced order, using an inspiratory threshold device for 4 weeks. During the protocol, MIP was tested weekly once. In a second visit, blood pressure, heart rate, stroke volume, cardiac output, middle cerebral artery blood

Cardiovascular response of individuals with spinal cord injury to dynamic functional electrical stimulation under orthostatic stress.

In this pilot study, we examined how effectively functional electrical stimulation (FES) and passive stepping mitigated orthostatic hypotension in participants with chronic spinal cord injury (SCI). While being tilted head-up to 70 (°) from the supine position, the participants underwent four 10-min conditions in a random sequence: 1) no intervention, 2) passive stepping, 3) isometric FES of leg muscles, and 4) FES of leg muscles combined with passive stepping. We found that FES and passive stepping independently mitigated a decrease in stroke volume and helped to maintain the mean blood press

The chief seat of mischief: soldier's heart in the First World War.

Soldier's heart was a medico-psychiatric condition that was first documented during the American Civil War. This condition affected British and American soldiers during the late-nineteenth and early-twentieth centuries; doctors recorded patients experiencing palpitations, breathlessness, headaches, and praecordial pain among other symptoms. While the number of cases of this disorder reached its peak in the First World War, it disappeared shortly afterwards. Based on an analysis of experimental results published in generalist and specialized medical journals as well as the correspondence betwee

Age-related symptom clustering in pediatric hypermobility spectrum disorders: a scoping review.

BACKGROUND: Pediatric generalized hypermobility spectrum disorders (pgHSD), are a group of multisystemic heritable connective tissue disorders frequently under-recognized in pediatric populations. The variable and often vague early symptomatology, coupled with the absence of definitive genetic markers, present significant diagnostic challenges. Early manifestations are commonly misattributed to other conditions, leading to substantial diagnostic delays and prolonged morbidity. The purpose of this study is to provide clarity on the presentation of pgHSD symptoms that may ultimately reduce diagn

Exercise Intolerance and Response to Training in Patients With Postacute Sequelae of SARS-CoV2 (Long COVID): A Scientific Statement From the American Heart Association.

The postacute sequelae of SARS-CoV-2, also known as Long COVID, may affect 10% to 25% of individuals diagnosed with SARS-CoV-2. More than 100 symptoms have been reported among patients with Long COVID, but almost all patients report severe fatigue, orthostatic intolerance, shortness of breath, and reductions in exercise tolerance. Emerging data suggest that cardiovascular deconditioning plays a major role in the development of this syndrome and that reductions in functional capacity among patients with Long COVID are comparable to reductions seen among individuals with cardiovascular deconditi

Evaluation of abdominal compression band for pediatric postural tachycardia syndrome: a crossover study.

BACKGROUND: Abdominal compression is an effective nonpharmacological treatment for orthostatic intolerance. Although the efficacy of abdominal compression bands has been studied in adults, studies involving children are limited. This study investigates the efficacy of abdominal compression bands in children diagnosed with postural tachycardia syndrome (POTS), using a larger cohort and incorporating autonomic nervous function assessment via frequency analysis. METHODS: A crossover study was conducted with 23 patients with POTS (mean age 13.4 ± 0.8 years), with and without abdominal compression

Cyproheptadine Treatment in Children and Adolescents with Migraine: A Retrospective Study in Japan.

OBJECTIVE: Evidence on prophylactic drugs for pediatric migraine is limited, especially when comorbid conditions contribute to treatment resistance. This study evaluated the efficacy of cyproheptadine in children with migraine and explored the impact of comorbid neurodevelopmental disorders and orthostatic intolerance (OI). METHODS: We retrospectively analyzed pediatric migraine patients treated with cyproheptadine. Efficacy was assessed based on the reduction in headache frequency, with responders defined as patients experiencing at least a 50% reduction in headache episodes. Fisher's exact

ME/CFS and Long COVID share similar symptoms and biological abnormalities: road map to the literature.

Some patients remain unwell for months after "recovering" from acute COVID-19. They develop persistent fatigue, cognitive problems, headaches, disrupted sleep, myalgias and arthralgias, post-exertional malaise, orthostatic intolerance and other symptoms that greatly interfere with their ability to function and that can leave some people housebound and disabled. The illness (Long COVID) is similar to myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) as well as to persisting illnesses that can follow a wide variety of other infectious agents and following major traumatic injury. Eight Orthostatic Haemodynamic Patterns in The Irish Longitudinal Study on Ageing (TILDA): Stability and Clinical Associations after 4 Years.

Previous research cross-sectionally characterised eight morphological systolic blood pressure (SBP) active stand (AS) patterns using a clinical clustering approach at Wave 1 (W1) of the Irish Longitudinal Study on Ageing. We explored the longitudinal stability and clinical associations of these groupings at Wave 3 (W3), four years later. Eight AS groups had their clinical characteristics and AS patterns at W3 compared to W1. We explored longitudinal associations (new cognitive decline, falls, syncope, disability, and mortality) using multivariate logistic regression models. In total, 2938 part

A physiologic-based approach to the evaluation of a patient with hypokalemia.

Hypokalemia is a common electrolyte disorder. Transient causes of hypokalemia are due to cell shift, whereas sustained hypokalemia is caused by either inadequate intake or excessive potassium loss. Evaluation of the intake, distribution, and excretion of potassium should include the following: (1) a careful history, including use of drugs, medications, and the presence of vomiting or diarrhea; (2) physical examination, including orthostatic changes in blood pressure and heart rate; and (3) measurement of urine and plasma electrolytes. Urinary potassium wasting is caused by pathophysiologic con

The importance of exercising in space.

As a direct consequence of exposure to microgravity, astronauts experience a set of physiological changes which can have serious medical implications when they return to earth. Most immediate and significant are the headward shift of body fluids and the removal of gravitational loading from bone and muscles, which lead to progressive changes in the cardiovascular and musculoskeletal systems. Cardiovascular adaptations result in an increased incidence of orthostatic intolerance (fainting) following flight, decreased cardiac output, and reduced capacity for exercise. Changes in the musculoskelet

Digital twin framework for postural tachycardia syndrome and autonomic disorders.

Autonomic disorders, especially those characterized by orthostatic intolerance such as Postural Tachycardia Syndrome (POTS), remain diagnostically and therapeutically challenging due to their complex pathophysiology and limited access to specialized care. This paper proposes a conceptual framework for applying digital twin technology to POTS and other autonomic disorders. A digital autonomic twin-a dynamic, virtual replica of a patient's autonomic system-offers a transformative approach to understanding, predicting, and managing these conditions. A dynamic digital twin framework integrates mec
