Effect of an Individually Tailored and Home-Based Intervention in the Chronic Phase of Traumatic Brain Injury: A Randomized Clinical Trial.

IMPORTANCE: Traumatic brain injury (TBI) can cause long-lasting and heterogeneous difficulties that require an individually tailored approach to rehabilitation. However, high-quality studies of treatment options in the chronic phase of TBI are lacking. OBJECTIVE: To evaluate the effect of a home-based, individualized, and goal-oriented rehabilitation intervention in the chronic phase of TBI. DESIGN, SETTING, AND PARTICIPANTS: This study was an intention-to-treat parallel-group assessor-blinded randomized clinical trial with 1:1 randomization to an intervention or control group. Participants

Opportunity to reduce transfer of patients with mild traumatic brain injury and intracranial hemorrhage to a Level 1 trauma center.

OBJECTIVE: Current guidelines do not address the disposition of patients with mild traumatic brain injury (TBI) and resultant intracranial hemorrhage (ICH). Emergency medicine clinicians working in hospitals without neurosurgery coverage typically transfer patients with both to a trauma center with neurosurgery capability. Evidence is accruing which demonstrates that the risk of neurologic decompensation depends on the type of ICH and as a result, not every patient may need to be transferred. The purpose of this study was to identify risk factors for admission among patients with mild TBI and

Anti-Inflammatory Potential of Stearidonic Acid Ethanolamide in a Model of Mild Traumatic Brain Injury in Mice.

Chronic neuroinflammation is recognized as a pivotal mechanism responsible for secondary damage following mild traumatic brain injury (mTBI), underscoring the critical need for therapeutic strategies capable of mitigating this pathological process. This study evaluated the anti-inflammatory properties of stearidonic acid ethanolamide (SDEA, C20H33NO2). The findings indicate that mTBI triggers persistent neuroinflammation, which is correlated with cognitive deficits. A ten-day treatment regimen with SDEA at 10 mg/kg/day facilitated the restoration of cognitive abilities and suppressed the neuro

A Technology-Enabled Concussion Care Pathway Reduces Costs and Enhances Care.

BACKGROUND: The standardization of care along disease lines is recommended to improve outcomes and reduce health care costs. The multiple disciplines involved in concussion management often result in fragmented and disparate care. A fundamental gap exists in the effective utilization of rehabilitation services for individuals with concussion. PURPOSE: The purpose of this project was to (1) characterize changes in health care utilization following implementation of a concussion carepath, and (2) present an economic evaluation of patient charges following carepath implementation. DESIGN: This

Symptomatology following traumatic brain injury in a multidisciplinary clinic: experiences from a tertiary centre.

BACKGROUND: Physical, cognitive and emotional sequelae in patients with traumatic brain injury (TBI) have been identified; some as late as two years post-injury. To aid in the specialist management of such patients, a multidisciplinary neurotrauma clinic was initiated at a tertiary centre.  Aim: This study sought to describe the clinical features of patients who attended the clinic. METHODS: Patient data was collected under several categories: basic demographics, mechanism and severity of injury, initial CT findings and management, hospital stay and discharge details, symptoms in clinic and a

Complicating factors associated with mild traumatic brain injury: impact on pain and posttraumatic stress disorder treatment.

The nature of combat in Iraq and Afghanistan has resulted in high rates of comorbidity among chronic pain, posttraumatic stress disorder (PTSD), and mild traumatic brain injury (mTBI) in Veterans of Operations Enduring Freedom and Iraqi Freedom (OEF/OIF). Although separate evidence-based psychological treatments have been developed for chronic pain and PTSD, far less is known about how to approach treatment when these conditions co-occur, and especially when they co-occur with mTBI. To provide the best care possible for OEF/OIF Veterans, clinicians need to have a clearer understanding of how t

Central auditory processing disorders after mild traumatic brain injury.

Auditory processing disorders are common following mild traumatic brain injury (mTBI), but the neurocircuitry involved is not well understood. The present study used functional MRI to examine auditory cortex activation patterns during a passive listening task in a normative population and mTBI patients with and without clinical central auditory processing deficits (APD) as defined by the SCAN-3:A clinical battery. Patients with mTBI had overall patterns of lower auditory cortex activation during the listening tasks as compared to normative controls. A significant lateralization pattern (pairwi

Deployment risk factors and postdeployment health profiles associated with traumatic brain injury in heavy drinking veterans.

Along with post-traumatic stress disorder (PTSD), mild traumatic brain injury (mTBI) is considered one of the "signature wounds" of combat operations in Iraq (Operation Iraqi Freedom [OIF]) and Afghanistan (Operation Enduring Freedom [OEF]), but the role of mTBI in the clinical profiles of Veterans with other comorbid forms of post-deployment psychopathology is poorly understood. The current study explored the deployment risk and postdeployment health profiles of heavy drinking OIF and OEF Veterans as a function of mTBI. Sixty-nine heavy-drinking OIF/OEF Veterans were recruited through a Veter

Longitudinal MAP-MRI-based Assessment of Tissue Microstructural Alterations in Acute mTBI.

BACKGROUND: Mild traumatic brain injury (mTBI) is a signature injury in civilian and military populations that remains invisible to detection by conventional radiological methods. Diffusion MRI has been identified as a potential clinical tool for revealing subtle microstructural alterations associated with mTBI. OBJECTIVE: This study evaluates whether a comprehensive and powerful diffusion MRI (dMRI) technique called mean apparent propagator (MAP) MRI can detect sequelae of mTBI. METHODS: We analyzed data from 417 participants of the GE/NFL prospective mTBI study which included 143 matched c

Educational and employment outcomes associated with childhood traumatic brain injury in Scotland: A population-based record-linkage cohort study.

BACKGROUND: Traumatic brain injury (TBI) is a leading cause of death and disability among young children and adolescents and the effects can be lifelong and wide-reaching. Although there have been numerous studies to evaluate the impact of childhood head injury on educational outcomes, few large-scale studies have been conducted, and previous research has been limited by issues of attrition, methodological inconsistencies, and selection bias. We aim to compare the educational and employment outcomes of Scottish schoolchildren previously hospitalised for TBI with their peers. METHODS AND FINDI

Nationwide incidence and risk factors for posttraumatic seizures in children with traumatic brain injury.

OBJECTIVEPosttraumatic seizures (PTSs) are the most common complication following a traumatic brain injury (TBI) and may lead to posttraumatic epilepsy. PTS is well described in the adult literature but has not been studied extensively in children. Here, the authors utilized the largest nationwide registry of pediatric hospitalizations to report the national incidence, risk factors, and outcomes associated with PTS in pediatric TBI.METHODSThe authors queried the Kids' Inpatient Database (KID) using ICD-9-CM codes to identify all patients (age < 21 years) who had a primary diagnosis of TBI (850
