Outcomes of an anatomic total shoulder arthroplasty with a contralateral reverse total shoulder arthroplasty.

BACKGROUND: It is common for patients to require staged bilateral shoulder arthroplasties. There is a unique cohort of patients who require an anatomic total shoulder arthroplasty (TSA) and a contralateral reverse shoulder arthroplasty (RSA). This study compared the outcomes of patients with a TSA in 1 shoulder and an RSA in the contralateral shoulder. METHODS: Our institutional database was queried to identify all patients with a TSA and a contralateral RSA. Data collection included patient demographics, preoperative and latest follow-up shoulder range of motion, radiographic analysis, and p

[Health surveillance and suitability: the role of the company physician in maintaining work ability].

The ageing workforce is one of the major issues in Europe. Ageing has an important effect on work ability, especially in manual jobs where physical demand is high. Musculoskeletal disorders are the most common health problems among European workers and are able to affect work ability. In order to increase the employment rate of people aged 45-65, measures to sustain work ability and proper age management should be promoted: return to work strategies include adjustment of the job to the worker's health conditions and abilities. The contribution of health surveillance.programs in the management

Attachments of muscles as landmarks for implantation of shoulder hemiarthoplasty in fractures.

BACKGROUND: The attachments of muscles and the position of the humeral head are important for a good functional outcome of shoulder hemiarthroplasties after displaced fractures of the proximal humerus. Deviations in the attachments and changes in their spatial position with respect to the humeral head during surgical reconstruction change the biomechanics and reduce the range of motion of the should joint postoperatively. METHODS AND RESULTS: We used 198 humerus preparations and using 3-dimensional analysis measured the angular relationships between the humeral head axis and medial margin of

[Parsonage-Turner Syndrome - Case Report].

Neuralgic amyotrophy of the shoulder (Parsonage-Turner syndrome) is a rare condition of unknown aetiology which manifests as acute neuropathy of the brachial plexus. Diagnosis is based on typical symptoms and physical examination. In addition, magnetic resonance imaging of the affected shoulder and the cervical spine is advisable, in order to distinguish this syndrome from other conditions with similar symptoms. There is no specific treatment for neuralgic amyotrophy, but in about 50 - 67% of cases complete recovery occurs within two to three years, depending on the severity of the symptoms wh

Combined SLAP repair and biceps tenodesis for superior labral anterior-posterior tears.

PURPOSE: Long-head biceps tenodesis has been suggested as an alternative to superior labral anterior-posterior (SLAP) repair. However, an unrepaired superior labral tear may increase glenohumeral translation, and thus, labral repair may be considered in the setting of biceps tenodesis. METHODS: Patients who underwent tenodesis, SLAP repair, or combined tenodesis and labral repair for SLAP tears were included. The indication for combined tenodesis and labral repair was biceps tendonitis in the setting of a SLAP lesion with labral instability. Demographics, range of motion, return to work, retu

Comprehensive Evaluation and Arthroscopic Management of Circumferential Labral Tears Following Traumatic First Time Shoulder Dislocation: A Case Report and Review.

INTRODUCTION: Pan-labral tears, commonly associated with recurrent shoulder dislocations, are a well-documented pathology. However, circumferential pan-labral tears following a first-time shoulder dislocation represent a rare and scarcely reported entity in the literature. Accurate diagnosis requires a comprehensive clinical history, physical examination, and further evaluation, often involving MRI. Even with advanced imaging, the acute nature of the injury can lead to the oversight of pan-labral tears, necessitating arthroscopic assessment for definitive diagnosis. Repairing such extensive gl

A Novel Ultrasound-Guided Injection Strategy for Rapid Functional Recovery in Frozen Shoulder: A Multicenter Randomized Trial.

BACKGROUND: Current frozen shoulder (FS) therapies present a clinical dilemma: although intra-articular corticosteroids with home exercise provide proven medium-term (3-month) benefits, they lack short-term (<4-week) efficacy for rapid recovery; conversely, manipulative treatments achieve faster outcomes but carry invasiveness risks. These limitations highlight the urgent need for developing and validating novel noninvasive approaches with prompt therapeutic effects. PURPOSE: To compare the efficacy and safety of ultrasound-guided tendon surface (UGTS) injection versus manipulation under loca

Long-term (minimum 10 years) survival and outcomes of pyrocarbon interposition shoulder arthroplasty.

BACKGROUND: There are some major controversies surrounding the use and longevity of pyrocarbon interposition shoulder arthroplasty (PISA). The objective of this study was to investigate the long-term survival and outcomes (minimum 10-year) following PISA for osteoarthritis (OA) in young and active patients. METHODS: This was a retrospective review of prospectively collected data of patients who underwent PISA (InSpyre; Tornier-Stryker) for OA between 2009 and 2012. Arthroplasty survival was known for 71 patients followed longitudinally for a minimum of 10 years. The clinical and radiologic ou

No difference in outcomes for posterior shoulder instability surgery in patients with a normal vs. pathological radiologist reported magnetic resonance arthrogram study.

BACKGROUND: Despite improvement in diagnostic imaging techniques, there remain patients with clinical posterior shoulder instability (PSI) with so called normal or nonpathologic findings on magnetic resonance arthrogram (MRA). This diagnostic dilemma creates challenges for clinicians and patients who fail conservative treatment when determining treatment options due to the incongruent findings on physical exam and MRA. We hypothesized that patients undergoing capsulolabral repair with a clinical exam consistent with PSI but normal radiologist reported MRA findings would experience similar clin

Biomechanical and clinical comparison of a novel dual-wing locking plate versus the PHILOS plate for neer three- and four-part proximal humeral fractures: a finite element analysis combined with a retrospective cohort study.

BACKGROUND: Neer three- and four-part proximal humeral fractures treated with conventional PHILOS plating are associated with high complication rates, primarily due to tuberosity re-displacement and calcar screw cut-out. We developed a novel dual-wing locking plate featuring bilateral tuberosity-capturing wing extensions and an omnidirectional ball-head calcar screw. This study compared its biomechanical performance and clinical outcomes with those of the PHILOS plate. METHODS: A three-dimensional finite element model was reconstructed from CT data using Mimics 21.0, Geomagic 2021, and Rhino

Sam2Rad: A segmentation model for medical images with learnable prompts.

The Segment Anything Model and its variants, such as MedSAM, have demonstrated potential for medical image segmentation. However, they heavily rely on high-quality manual prompts, which are both time-consuming and require domain expertise. Even when using manual prompts, including sparse prompts like boxes, points, or text, and dense prompts such as masks, SAM and its variants like MedSAM (fine-tuned on medical images) fail to segment bones in ultrasound images due to significant domain shift. To address these limitations, we propose Sam2 for Radiology (Sam2Rad), a framework that extends SAM a
