The midterm effectiveness of extracorporeal shockwave therapy in the management of chronic calcific shoulder tendinitis.

BACKGROUND: This systematic review explored the midterm effectiveness of extracorporeal shockwave therapy (ESWT) in reducing pain and improving shoulder function. Calcified rotator cuff tendinitis is a common cause of chronic shoulder pain that leads to significant pain and functional limitations. ESWT is an alternative to surgery when conservative treatments such as nonsteroidal antiinflammatory drugs, steroidal injections, and physiotherapy fail to relieve symptoms. It is hypothesised that ESWT is effective in the midterm for reducing pain and improving function for patients with chronic cal

Treatment of persistent shoulder pain with sodium hyaluronate: a randomized, controlled trial. A multicenter study.

BACKGROUND: Presently, there are no approved nonoperative therapies for the ongoing treatment of persistent shoulder pain. Preliminary data suggest that intra-articular sodium hyaluronate injections may be beneficial for the treatment of persistent shoulder pain resulting from various etiologies. The present study evaluated the efficacy and safety of sodium hyaluronate (Hyalgan; molecular weight, 500 to 730 kDa) for these patients. METHODS: Six hundred and sixty patients with persistent shoulder pain and limitation resulting from glenohumeral joint osteoarthritis, rotator cuff tear, and/or ad

The surgical treatment of the rheumatoid shoulder.

Rheumatoid disease of the shoulder may affect not only the gleno-humeral joint but also the other joints of the shoulder girdle complex. Excision arthroplasty of the acromioclavicular and much less commonly of the sternoclavicular joints may be necessary to relieve persistent symptoms. Bursectomyin conjunction with anterior acromioplasty should provide a definitive solution to rheumatoid involvement of the subacromial bursa when this fails to respond to simple aspiration and hydrocortisone injection. Synovectomy has been little employed and perhaps deserves re-appraisal with regard to its role

Open Gluteus Reconstruction with Inlay Enthesis Allograft and Onlay Acellular Dermal Allograft.

BACKGROUND: Gluteus medius/minimus tears are common, especially in older women. These can occur acutely due to trauma or chronically from degeneration. Treatment options vary depending on the extent of the tear. In cases with poor tendon quality or retracted tears, allograft augmentation is a promising surgical option that may enhance repair strength and healing potential. INDICATIONS: Open gluteus medius/minimus repair with allograft augmentation is indicated for patients with large, chronic tears that are unlikely to heal due to factors such as age, tissue degeneration, or prior failed repa

Estimating the Minimal Detectable Change (MDC) of the Disability of the Arm, Shoulder and Hand (DASH) scale in idiopathic frozen shoulder.

BACKGROUND: Idiopathic frozen shoulder is a disabling condition characterized by pain and restricted range of motion, often evaluated using patient-reported outcome measures such as the Disabilities of the Arm, Shoulder and Hand (DASH) score. However, the clinical interpretation of DASH changes remains unclear without defined thresholds for meaningful improvement. While the minimal detectable change (MDC) has been established for various musculoskeletal conditions, poor data exist for patients with frozen shoulder treated with focal extracorporeal shock wave therapy (f-ESWT). This study aims t

Active vs. sham pulsed electromagnetic field therapy added to standardized conservative care for partial-thickness supraspinatus tears: study protocol for a randomized, assessor-blinded, sham-controlled trial.

BACKGROUND: Partial-thickness supraspinatus tears commonly cause shoulder pain and functional limitation. Although conservative treatment can improve symptoms, whether pulsed electromagnetic field (PEMF) therapy provides additional clinically meaningful benefit remains uncertain. Existing PEMF evidence is mainly derived from rotator cuff tendinopathy or subacromial impingement populations, whereas sham-controlled evidence in MRI-confirmed partial-thickness supraspinatus tears remains sparse. METHODS: This is a single-center, parallel-group, randomized, assessor-blinded, sham-controlled trial.

Therapeutic effect of sustained release of aloe emodin via novel grafted polymer in electrospun nanofiber membranes on peritendinous adhesions.

INTRODUCTION: Tendon injuries are prevalent musculoskeletal conditions in clinical settings, affecting various regions such as the rotator cuff tendons, forearm flexor/extensor tendons, finger flexor tendons, and Achilles tendon. While surgery is an effective treatment, it often fails to balance intrinsic and extrinsic healing processes, leading to abnormal cell activation, proliferation, and migration, resulting in collagen deposition at the injury site. This pathological fibrosis causes severe peritendinous adhesions, posing a therapeutic challenge. Traditional nonsteroidal anti-inflammatory

Hybrid fixation with a double-row suture and low-profile plate for comminuted or avulsed greater tuberosity fractures.

BACKGROUND: Isolated greater tuberosity (GT) fractures account for approximately 20% of proximal humeral fractures and are frequently associated with glenohumeral dislocation and rotator cuff injuries. Displacement exceeding 5 mm is recommended for surgery to prevent subacromial impingement and functional impairment. Current fixation methods, such as cannulated screws, suture anchors, and locking plates, each have limitations, including inadequate stability for comminuted fractures, risk of impingement, and suboptimal anatomical adaptation. This underscores the necessity of a hybrid technique

Dark side of the shoulder: suprascapular and axillary nerve compressions.

BACKGROUND: The suprascapular and axillary nerves can be subject to entrapment due to both their anatomical courses and their anatomical relationships with surrounding anatomical structures around shoulder. These entrapments were previously considered as a diagnosis of exclusion. However, today these pathologies can be diagnosed as primary. The most common complaints of patients are pain and sometimes weakness. The clinician's suspicion is very important in making diagnosis. The patient's history, duration of symptoms, and information such as the movements in which the complaints increase shou

Radiological and electrodiagnostic insights into suprascapular nerve dysfunction: a key predictor of poor functional outcomes in shoulder hemiarthroplasty.

BACKGROUND: Suprascapular nerve (SSN) dysfunction has emerged as an underexplored factor influencing functional outcomes after shoulder hemiarthroplasty (SHA) for proximal humerus fractures. Despite achieving optimal tuberosity healing and prosthesis alignment, some patients continue to experience poor functional recovery. This study investigates the role of SSN dysfunction as a key determinant of unfavorable outcomes following SHA. METHODS: This retrospective study analyzed 38 patients who underwent SHA for Neer type III or IV proximal humerus fractures. Inclusion criteria included radiograp

The impact of blood flow restriction training on tendon adaptation and tendon rehabilitation - a scoping review.

BACKGROUND: Tendon injuries are common in athletes and in the general population and require extensive rehabilitation. Current conservative treatment often includes different high-load resistance training (HLRT) modalities. However, certain populations may not tolerate HLRT well. Blood flow restriction training (BFRT) incorporates low load while achieving hypertrophy and strength adaptations comparable to HLRT. However, the effects of BFRT on healthy and pathological tendons are unknown. The aims of this scoping review were therefore to summarize the reported impact of BRFT: (1) on tendon adap

Opioid consumption following isolated and recurrent shoulder dislocation and reduction.

BACKGROUND: Recurrent shoulder dislocations often lead to multiple encounters for reduction and eventual surgical stabilization, both of which involve exposure to opioids and potentially increase the risk of chronic opioid exposure. The purpose of our study was to characterize shoulder instability and compare pre- and postreduction opioid usage in singular dislocators (SDs) and recurrent dislocators (RDs). METHODS: This retrospective study was performed at a single academic institution using a prospective database. Patients were included if they were (1) age 18 years or older and (2) sustaine

Si-ITGA6-Loaded Liposomes Inhibit Capsule Fibrosis via the FAK/PI3K/Akt Signaling Pathway in Adhesive Capsulitis of Shoulder.

BACKGROUND: Adhesive capsulitis of shoulder (ACS) is a common shoulder disease with pain, joint stiffness, and capsule fibrosis. The pathogenesis of ACS remains unclear, and long-term efficacy of current treatment for ACS is not satisfactory. Given the absence of targeted therapy, proteomic analysis is conducted on the capsules of ACS patients, in an effort to identify the potential target for ACS treatment. The liposome-based siRNA delivery system was utilized for in vivo experiments due to its non-immunogenicity, stability, and low off-target effects compared to other siRNA delivery methods.

Increased postoperative deltoid signal seen after suprapectoral biceps tenodesis: potential risk to the anterior branch of the axillary nerve.

BACKGROUND: Arthroscopic suprapectoral biceps tenodesis is a common procedure for lesions of the long head of the biceps in the setting of anterior shoulder pain. However, the distal portal poses a theoretical risk to the terminal branches of the axillary nerve as the nerve travels from posterior to anterior to innervate the anterior deltoid. The purpose of this retrospective cohort study was to assess for axillary nerve branch injury, identified by deltoid signal change in postoperative magnetic resonance imaging (MRI) in patients who underwent an arthroscopic suprapectoral biceps tenodesis.

Proximal Humerus Fractures: A Review of Anatomy, Classification, Management Strategies, and Complications.

Proximal humerus fractures are prevalent in older adults, particularly women, primarily due to osteoporosis and increased fall risk. These fractures often result from low-energy falls in elderly patients, while in younger individuals, they are more likely to occur with high-energy trauma, which may involve additional injuries to soft tissue and neurovascular structures. Proper anatomical understanding, including key structures and blood supply, is crucial for effective management and to prevent complications. Several classification systems assist in guiding treatment for proximal humerus fract

Arthroscopic suprapectoral biceps tenodesis provided earlier shoulder function restoration compared with open subpectoral biceps tenodesis during the recovery phase.

BACKGROUND: This study compared the clinical outcomes of open subpectoral biceps tenodesis and arthroscopic suprapectoral biceps tenodesis for symptomatic biceps tenosynovitis. Although both techniques have pros and cons, no studies have compared clinical and functional outcomes during the recovery phase. Previous studies show that suprapectoral tenodesis has a higher probability of Popeye deformity and postoperative bicipital pain and stiffness, whereas subpectoral tenodesis has a higher risk of nerve complications and wound infections. This study aimed for clinical comparison between arthros

Correlation of Glenoid Concavity With Surgical Failure After Arthroscopic Stabilization for Recurrent Anterior Shoulder Instability.

BACKGROUND: Glenoid concavity compression by rotator cuff muscle contraction is one of the key mechanisms in the stability of the glenohumeral joint. PURPOSE/HYPOTHESIS: The purpose of this study was to evaluate the effects of glenoid concavity, as represented by the bony shoulder stability ratio (BSSR) and other factors, including glenoid bone defect size, on the surgical failure of arthroscopic stabilization procedures for recurrent anterior shoulder instability. The authors also aimed to determine the critical value of BSSR. It was hypothesized that both glenoid concavity and glenoid bone

Chronic Low Back Pain Forced Me to Search for and Find Pain Solutions: An Autobiographical Case Report.

Lifelong, pregnancy-induced low back pain forced me to search for solutions to the problem of pain. Currently, low back pain is often diagnosed as "nonspecific" and, as a result, a multitude of tests and poorly effective, at times side effect-laden or habit-forming treatments are recommended. My quest for relief took me to first diagnose my pain as coming from the sacroiliac joints, then to prolotherapy, the first treatment which brought me prolonged relief. I then learned how to perform prolotherapy. In 2009, when I undertook a randomized controlled study of dextrose prolotherapy for rotator
