The Economic Value of Hybrid Single-photon Emission Computed Tomography With Computed Tomography Imaging in Pulmonary Embolism Diagnosis.

ObjectiveThe objective was to quantify the potential economic value of single-photon emission computed tomography (SPECT) with computed tomography (CT; SPECT/CT) versus CT pulmonary angiography (CTPA), ventilation-perfusion (V/Q) planar scintigraphy, and V/Q SPECT imaging modalities for diagnosing suspected pulmonary embolism (PE) patients in an emergency setting.MethodsAn Excel-based simulation model was developed to compare SPECT/CT versus the alternate scanning technologies from a payer's perspective. Clinical endpoints (diagnosis, treatment, complications, and mortality) and their correspo

Potential Role of Global Longitudinal Strain in Cardiac and Oncological Patients Undergoing Cardio-Oncology Rehabilitation (CORE).

Although shown to be effective in improving survival and quality of life in patients with cancer, some treatments are well-known causes of cardiotoxicity, such as anthracyclines, monoclonal antibodies against human epidermal growth factor receptor 2 (HER2) and radiotherapy. To prevent cardiovascular disease (CVD) in patients living with cancer, cardiologists and oncologists promoted the development of cardio-oncology, an interdisciplinary field which aims to further improving life expectancy in these patients. Cardio-oncology rehabilitation (CORE), through correction of risk factors, prescript

Weekend admission and outcomes in cancer-associated pulmonary embolism: a cross-sectional national inpatient sample study, 2016-2022.

The "weekend effect" has been variably associated with outcomes in acute pulmonary embolism (PE), but its relevance in cancer-associated PE remains uncertain. Among U.S. hospitalizations for cancer-associated PE, is weekend admission independently associated with in-hospital mortality, and do PE-related interventions differ by day of admission? Retrospective cross-sectional study using the HCUP National Inpatient Sample (2016-2022). Adults (≥ 18 years) hospitalized with a principal diagnosis of PE and concomitant malignancy were included. Weekend admission was Saturday-Sunday. Survey-weighted

Concurrent evolution of cancer cachexia and heart failure: bilateral effects exist.

Cancer cachexia is defined as a multifactorial syndrome of involuntary weight loss characterized by an ongoing loss of skeletal muscle mass and progressive functional impairment. It is postulated that cardiac dysfunction/atrophy parallels skeletal muscle atrophy in cancer cachexia. Cardiotoxic chemotherapy may additionally result in cardiac dysfunction and heart failure in some cancer patients. Heart failure thus may be a consequence of either ongoing cachexia or chemotherapy-induced cardiotoxicity; at the same time, heart failure can result in cachexia, especially muscle wasting. Therefore, t

Burden of non-communicable diseases among adolescents aged 10-24 years in the EU, 1990-2019: a systematic analysis of the Global Burden of Diseases Study 2019.

BackgroundDisability and mortality burden of non-communicable diseases (NCDs) have risen worldwide; however, the NCD burden among adolescents remains poorly described in the EU.MethodsEstimates were retrieved from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019. Causes of NCDs were analysed at three different levels of the GBD 2019 hierarchy, for which mortality, years of life lost (YLLs), years lived with disability (YLDs), and disability-adjusted life-years (DALYs) were extracted. Estimates, with the 95% uncertainty intervals (UI), were retrieved for EU Member Stat

Machine learning and multi-omics technologies for precision cardiovascular medicine: advancing diagnosis, risk prediction, and therapeutic guidance.

Cardiovascular disease remains a major global health burden. Owing to its complex pathogenesis and marked clinical heterogeneity, conventional one-size-fits-all strategies often yield limited benefit for a substantial proportion of patients. Precision medicine advocates individualized management based on patients' clinical and molecular characteristics to improve outcomes. In this context, multi-omics and machine learning provide critical technical support for precision medicine: multi-omics can capture the full spectrum of cardiovascular disease from molecular alterations to phenotypic manife

Wearables research for continuous monitoring of patient outcomes: A scoping review.

BackgroundThe use of wearable devices for remote health monitoring is a rapidly expanding field. These devices might benefit patients and providers; however, they are not yet widely used in healthcare. This scoping review assesses the current state of the literature on wearable devices for remote health monitoring in non-hospital settings.MethodsCINAHL, Scopus, Embase and MEDLINE were searched until August 5, 2024. We performed citation searching and searched Google Scholar. Studies on wearable devices in an outpatient setting with a clinically relevant, measurable outcome were included and we

Burden and temporal trends of non-communicable diseases from 1990 to 2021 and prediction to 2035 in the group of twenty countries: a systematic analysis of the Global Burden of Disease Study 2021.

BackgroundNon-communicable diseases (NCDs) represent a major global public health threat, closely linked to disruptions in host-microbe symbiosis and metabolic homeostasis. The Group of Twenty (G20) comprises the world's major economic powers with diverse developmental stages, yet the NCD burden and its potential implications for host and ecosystem resilience remain poorly characterized across these nations. This study aimed to systematically estimate the burden, temporal trends, and future projections of NCDs from 1990 to 2021 and up to 2035 in G20 countries.MethodsUsing data from the Global

Contemporary treatment patterns of overweight and obesity: insights from the Mass General Brigham health care system.

ObjectiveThe objective of this study was to describe the prevalence of obesity, obesity-related conditions (ORCs), and antiobesity medication (AOM) eligibility and prescribing practice among eligible patients in a large health care system.MethodsIn this cross-sectional analysis of the multicenter Mass General Brigham health care system (Boston, Massachusetts) spanning 2018 to 2022, adults eligible for AOMs (BMI ≥ 30 kg/m2 or BMI 27-29.9 kg/m2 with ≥1 ORC) were identified. Among those AOM-eligible, the prevalence of prescriptions for AOMs approved for long-term weight management was evaluated.R

Current approach to the diagnosis and treatment of femoral-popliteal arterial disease. A systematic review.

Peripheral arterial disease (PAD) is a common manifestation of atherosclerosis affecting 5 million adults in the United States, with an age-adjusted prevalence of 4% to 15% and increasing up to 30% with age and the presence of cardiovascular risk factors. In this article we focus on lower extremity PAD and specifically on the superficial femoral and proximal popliteal artery (SFPA), which are the most common anatomic locations of lower extremity atherosclerosis. We summarize current evidence and perform a systematic review on the diagnostic evaluation as well as the medical, endovascular and s

Case series discussion of cardiac and vascular events following carfilzomib treatment: possible mechanism, screening, and monitoring.

BackgroundCarfilzomib is a selective proteasome inhibitor approved in the United States in 2012 for the treatment of relapsed and refractory multiple myeloma. Although cardiopulmonary and vascular events have been reported infrequently, they can be potentially serious complications, and their incidence and pathophysiology following carfilzomib treatment remain poorly characterized in a real-world patient population.MethodsWe retrospectively reviewed the records of 67 patients with relapsed and/or refractory multiple myeloma treated at our institution.ResultsWe describe 12 patients who experien

Machine Learning Meets Cancer.

The role of machine learning (a part of artificial intelligence-AI) in the diagnosis and treatment of various types of oncology is steadily increasing. It is expected that the use of AI in oncology will speed up both diagnostic and treatment planning processes. This review describes recent applications of machine learning in oncology, including medical image analysis, treatment planning, patient survival prognosis, and the synthesis of drugs at the point of care. The fast and reliable analysis of medical images is of great importance in the case of fast-flowing forms of cancer. The introductio

Thrombotic risk in hepatitis C: Interplay between hepatic dysfunction, viral-driven inflammation, and cardiovascular consequences.

Hepatitis C virus (HCV) infection, traditionally regarded as a hepatotropic disease, is increasingly recognized as a systemic condition with significant thrombotic implications. Chronic HCV induces a persistent proinflammatory and prothrombotic state that substantially elevates the risk of both venous and arterial events. Mechanistically, HCV drives endothelial dysfunction, enhances platelet activation, disrupts coagulation and fibrinolytic balance, and promotes immune-mediated vascular injury through cryoglobulinemia and chronic systemic inflammation. Clinical manifestations range from portal

Unlocking Novel Therapeutic Potential of Angiotensin II Receptor Blockers.

Pharmaceutical companies keep producing novel drugs and drug treatments for improving the life of every sick individual, most often following a pattern; a specific drug for a specific condition. Evidence suggests that different medications can have a positive effect on different pathological conditions. The full potential of existing therapies can be revealed through drug repurposing-also referred to as drug repositioning, reprofiling, or re-tasking-which involves identifying new therapeutic uses for approved or investigational drugs beyond their original indications. One significant target in
