Gradual closed correction of equinus contracture using the Ilizarov apparatus.

We treated 16 patients with equinus contracture using the Ilizarov method without open soft tissue release. No hinge was used in seven patients; instead, their ankle joint was used as the hinge (natural hinge; unconstrained construct). In the remaining nine patients, medial and lateral hinges connecting the tibial and foot rings were placed at the center of the talar dome (rotating hinge; constrained construction). We assessed the results in terms of the extent of dorsiflexion, its improvement, and complications. There was little difference between the results in the natural hinge group and th

Bridle procedure combined with a subtalar implant: a case series and review of the literature.

BACKGROUND: The bridle procedure is a tritendon anastomosis between the tibialis posterior, peroneus longus, and tibialis anterior, utilized in the treatment of drop foot. Concerned about the potential for acquired flatfoot deformity following the bridle procedure, the authors hypothesized that placing a sinus tarsi implant would prevent this potential sequela. MATERIALS AND METHODS: Over a 10-year period, 15 patients (16 feet) were treated with a bridle procedure for drop foot, and 9 of these feet also had a subtalar arthroereisis implanted; 8 patients (9 feet) positively responded to reques

Posterior tibial tendon transfer: results of fixation to the dorsiflexors proximal to the ankle joint.

BACKGROUND: Most techniques for posterior tibial tendon transfer attach the posterior tibial tendon somewhere at the dorsum of the foot. Inadequate tendon length and difficulties in securing the tendon to bone or tendon can complicate these procedures. Performing the tenodesis proximal to the ankle joint can prevent these problems. The present study is the first to describe the results of posterior tibial tendon transfer through the interosseous membrane attaching only the posterior tibial tendon to the extensor tendons proximal to the ankle joint. METHODS: We examined 13 feet in 12 patients

Tibiotalar joint arthrodesis for the treatment of severe ankle joint degeneration secondary to rheumatoid arthritis.

The technical aspects of fusion of the rheumatoid ankle do not deviate from those in the post-traumatic or osteoarthritic ankle. Screw fixation can usually be achieved, and rarely is fixation failure a problem in rheumatoid ankle arthrodesis. If fixation is difficult because of deformity or bone quality, external fixation or locking intramedullary nails should be used. The placement of cannulated screws and adequacy of screw fixation has not been a problem (Fig. 13). Screw fixation provides compression and prevents rotation. The surgeon, however, needs to be assured that no screws invade the s

[Clubfoot associated with tibial and fibular hemimelia].

AIM: Clubfoot is rarely associated with tibial or fibular hemimelia. Treatment is complex and in most of the cases extensive surgery is required. At present experience with Ponseti casting is limited. We describe casting and surgical treatment of 10 clubfeet associated with tibial and fibular hemimelia. MATERIAL AND METHOD: Between 1.1.2004 and 31.12.2009 398 clubfeet were treated with casting in our institution. In the same period 10 clubfeet were associated with fibular or tibial hemimelia. Treatment started in 9 clubfeet with Ponseti manipulation and casting. We used the classification of

Plantaris tendon resection increases ankle dorsiflexion independently of knee position, with greater effect in tendons inserting close to the Achilles.

BACKGROUND: Equinus deformity, defined as limited ankle dorsiflexion, is a frequent clinical finding with implications in multiple foot and ankle disorders. While gastrocnemius tightness is considered the main dynamic cause, the plantaris tendon (PT) has been proposed as an additional restrictor of ankle mobility. Its anatomical variability and proximity to the Achilles tendon may influence ankle dorsiflexion. METHODS: Eighteen cadaveric feet from nine donors were examined to confirm the presence of a plantaris tendon (PT) and classify its insertion type. Ankle dorsiflexion and plantarflexion

Leg lengthening using intramedullay skeletal kinetic distractor: results of 57 consecutive applications.

BACKGROUND: Mechanically activated intramedullary lengthening nails are advantageous over external fixator. However, difficulties with the control of the distraction rate are the main drawbacks, which may in turn cause insufficient bone regenerate. PATIENTS AND METHODS: A total of 57 lengthening procedures were performed using intramedullary skeletal kinetic distractor (ISKD) nail in 53 patients (femoral = 45 and tibial = 12). Average length gain was 4.3 ± 1.6 cm. The cause of shortening was post-traumatic (n = 33), congenital (n = 20), post-tumour resection (n = 1), cosmetic femoral lengthen

Temporary Circular External Fixation for Spanning the Traumatized Ankle Joint.

BACKGROUND: Temporary ankle-spanning circular fixation aims to provide osseous stability while (1) allowing access to and recovery of the traumatized soft-tissue envelope and (2) facilitating safe, comfortable, and clinically relevant cross-sectional imaging for surgical planning. It is most commonly utilized in a "span-scan-plan" treatment strategy in cases of peri-articular fractures around the ankle2. Conventional monolateral fixators are prone to morbidity at the half-pin sites in the foot and variation in construct stability. Temporary ankle-spanning circular external fixation of the trau

Complex Clubfoot Treatment With Ponseti Method: A Latin American Multicentric Study.

BACKGROUND: Complex clubfoot is a term used to describe those feet that present after treatment with a short first metatarsal, severe plantar flexion of all metatarsals, rigid equinus, and deep folds through the sole of the foot and above the heel. Ponseti has described a modification of his original technique for the treatment of the deformity. Few series have reported the treatment outcomes of this group of patients. The purpose of this study is to analyze mid-term results and complications of a large multicenter cohort. METHODS: Patients with complex clubfoot treated at 6 tertiary-care ins
