Neurovascular Coupling in Acutely Concussed Adolescent Patients.

Neurovascular coupling (NVC) uniquely describes cerebrovascular response to neural activation and has demonstrated impairments following concussion in adult patients. It is currently unclear how adolescent patients experience impaired NVC acutely following concussion during this dynamic phase of physiological development. The purpose of this study was to investigate NVC in acutely concussed adolescent patients relative to controls. We recruited patients presenting to a sports medicine practice within 28 days of a concussion or a musculoskeletal injury (controls). Transcranial Doppler ultrasoun

Remote Longitudinal Tracking of Sleep, Depression, and Cognition Across Early Adolescence: Protocol for a Longitudinal Observational Study.

BACKGROUND: Depression in adolescents is a growing public health concern, with 3.5 million youth in the United States experiencing a depressive episode each year. When symptoms emerge in early adolescence, they can have lasting psychological, physical, and cognitive consequences, highlighting the urgent need for proactive intervention. Insufficient sleep-characterized by short duration, irregular timing, and poor quality-is bidirectionally linked to depression and associated with impaired cognitive performance. OBJECTIVE: This protocol provides the map for a study tracking sleep patterns and

PhiCube: a reconfigurable bilateral robotic device for neurorehabilitation.

INTRODUCTION: Neuromotor disorders affecting upper-limb function represent a substantial clinical challenge in pediatric populations, with conditions such as hemiplegic cerebral palsy imposing lasting limitations on functional independence and quality of life. Bilateral motor training has emerged as a neurophysiologically grounded paradigm, offering functional advantages over purely unilateral approaches by actively exploiting interlimb coordination mechanisms. Despite the growing evidence base for bilateral training, existing robotic devices present critical limitations in kinematic reconfigu

The Clinical Characteristics and Neurophysiological Assessments of the Occipital Cortex in Visual Snow Syndrome With or Without Migraine.

OBJECTIVE: Visual snow syndrome (VS) is mainly characterized by flickering, little dots in both visual fields. The recognition of the clinical entity of VS has been increasing recently. Diagnosis is based on patient reports and not better accounted for by another diagnosis. BACKGROUND: The exact pathophysiology of this syndrome is still unknown. In this study, our aim was to investigate the role of neurophysiological assessments of the occipital cortex in VS patients with (VSm ) or without migraine (VSwom ) and the healthy control (HC). METHODS: To assess the occipital cortex hyperexcitabili

10 Hz Amplitude Modulated Sounds Induce Short-Term Tinnitus Suppression.

Objectives: Acoustic stimulation or sound therapy is proposed as a main treatment option for chronic subjective tinnitus. To further probe the field of acoustic stimulations for tinnitus therapy, this exploratory study compared 10 Hz amplitude modulated (AM) sounds (two pure tones, noise, music, and frequency modulated (FM) sounds) and unmodulated sounds (pure tone, noise) regarding their temporary suppression of tinnitus loudness. First, it was hypothesized that modulated sounds elicit larger temporary loudness suppression (residual inhibition) than unmodulated sounds. Second, with manipulati

Positive Emotion Dysregulation in Opioid Use Disorder and Normalization by Mindfulness-Oriented Recovery Enhancement: A Secondary Analysis of a Randomized Clinical Trial.

IMPORTANCE: It remains unknown whether difficulties in positive emotion regulation (ER) drive opioid use disorder (OUD) and whether these difficulties may be remediated. OBJECTIVE: To test whether OUD is associated with attenuated positive ER and whether such positive emotion dysregulation may be treated by mindfulness-based intervention. DESIGN, SETTING, AND PARTICIPANTS: In this mechanistic substudy of a randomized clinical trial, patients with chronic pain who did and did not meet criteria for OUD (OUD+ and OUD-, respectively) were recruited from primary care and pain clinics in Utah from

Effectiveness of Reflex Integration Exercises on Sensory-Motor Outcomes in Children With Autism Spectrum Disorder: A Quasi-experimental Study.

Background Autism spectrum disorder (ASD) is a complex neurodevelopmental condition associated with impairments in sensory processing, motor coordination, and postural control. A prominent yet underappreciated contributor to these deficits is the persistence of primitive reflexes beyond the expected developmental timeline. Reflexes such as the asymmetrical tonic neck reflex (ATNR), symmetrical tonic neck reflex (STNR), and tonic labyrinthine reflex (TLR) are brainstem-mediated automatisms that, under typical neurodevelopment, integrate within the first year of life. In children with ASD, their

Locus-Coeruleus Norepinephrine Functioning as a Predictor of Childhood Mental Health (LOCUS-MENTAL): Protocol for a Longitudinal Study.

BACKGROUND: Mental health disorders (MHDs) remain a leading cause of the global burden of diseases. Early identification of neurobiological mechanisms mediating a risk for MHDs is key to reducing a lifetime burden. Recent findings emphasize the locus coeruleus-norepinephrine (LC-NE) system as a neuromodulator of arousal translating acute stress responses into neuronal excitability. We propose that individual differences in LC-NE functioning can explain a differential susceptibility to psychological adversity, which mediates the development of transdiagnostic psychopathology in early childhood.

Discographic, MRI and psychosocial determinants of low back pain disability and remission: a prospective study in subjects with benign persistent back pain.

BACKGROUND CONTEXT: A range of morphologic and psychosocial variables has been suggested as risk factors for serious low back pain (LBP) illness. Although the relative contributions of structural and psychosocial variables are intensely debated, the validity of differing hypotheses has proven difficult to test because the incidence of serious disabling LBP illness is low in healthy subjects. These factors dictate the requirement for large sample sizes, extensive structural imaging and extended longitudinal study. Previous studies included either small cohorts with intensive imaging testing or

Smartphone App-Delivered Mindfulness-Based Intervention for Concussion in Adolescents (MBI-4-mTBI): Feasibility Randomized Controlled Trial.

BACKGROUND: Persisting symptoms affect about one-third of youth following concussion. Mental health history, distress, and coping style are key predictors of prolonged recovery. Early and scalable psychological interventions, such as mindfulness-based intervention (MBI) delivered via smartphones, may improve patients' ability to regulate their emotions and neurophysiologically recover, reducing overall symptom burden. However, no digital therapeutic (DTx) trials in adolescents experiencing concussion exist. OBJECTIVE: This study primarily aimed to assess the feasibility of conducting a larger

Additional arm-hand boost therapy (AHA-BOOST) in an inpatient rehabilitation setting during the subacute phase after stroke: protocol for a randomised controlled trial including a clinical, process and health economic evaluation.

INTRODUCTION: A considerable number of patients present with an upper limb impairment after stroke, which has a significant impact on daily life activities and quality of life. Currently, based on a Cochrane review, there is no high-quality evidence for any of the upper limb interventions used in routine practice. Therefore, the Jessa Hospital developed an additional arm-hand boost programme (AHA-BOOST), a novel, stratified intervention that integrates neurophysiological knowledge with an early start to optimise poststroke upper limb recovery. METHODS AND ANALYSIS: This multicentre randomised

Association of Repetitive Transcranial Magnetic Stimulation Treatment With Subgenual Cingulate Hyperactivity in Patients With Major Depressive Disorder: A Secondary Analysis of a Randomized Clinical Trial.

IMPORTANCE: Hyperactivity in the subgenual cingulate cortex (SGC) is associated with major depressive disorder (MDD) and anticorrelated with activity in the dorsolateral prefrontal cortex (DLPFC). This association was found to be predictive of responsiveness to repetitive transcranial magnetic stimulation (rTMS) treatment. Such findings suggest that DLPFC-SGC connectivity is important for understanding both the therapeutic mechanism of rTMS in patients with MDD and the underlying pathophysiology of MDD. OBJECTIVE: To evaluate SGC hyperactivity in patients with MDD before and after rTMS treatm

From Biology to Treatment of Monoclonal Gammopathies of Neurological Significance.

Monoclonal gammopathy and peripheral neuropathy are common diseases of elderly patients, and almost 10% of patients with neuropathy of unknown cause have paraprotein. However, growing evidence suggests that several hematological malignancies synthesize and release monoclonal proteins that damage the peripheral nervous system through different mechanisms. The spectrum of the disease varies from mild to rapidly progressive symptoms, sometimes affecting not only sensory nerve fibers, but also motor and autonomic fibers. Therefore, a multidisciplinary approach, mainly between hematologists and neu
