Epithelioid leiomyosarcoma of the skin and subcutaneous tissue. Clinicopathologic, immunohistochemical, and ultrastructural study of five cases.

This report describes five cases of an unusual variant of malignant smooth-muscle neoplasm involving the skin and subcutaneous tissue characterized by a proliferation of predominantly round to oval epithelioid cells with abundant eosinophilic cytoplasm. Of the five patients, four were men, one a woman; ages ranged from 69 to 85 years (mean, 76.4). The size of the lesions varied from 1.2 to 2.5 cm; two were located in the scalp, two in the face, and one in the back of the neck. Immunohistochemical evaluation of the tumor cells showed immunoreactivity for actin and vimentin in all cases and nega

Melioidosis with non-necrotising diffuse cellulitis and intramuscular abscesses of the trunk mimicking necrotising soft tissue infection: a case report.

BACKGROUND: Melioidosis is endemic in Southeast Asia and northern Australia and can resemble various conditions. Soft tissue involvement is usually limited to localised cellulitis or superficial abscesses. Our case presented with diffuse cellulitis and deep intramuscular abscesses of the thoracoabdominal wall—an exceedingly rare manifestation mimicking necrotising soft tissue infection, a surgical emergency. Reporting such cases is essential to prevent misdiagnosis and unnecessary surgery in endemic settings. CASE PRESENTATION: A man in his sixties from southern Vietnam with previously undiagn

One-stage dual latissimus dorsi muscle flap transfer with a pair of vascular anastomoses and double nerve suturing for long-standing facial paralysis.

OBJECTIVE: Various types of neurovascular free-muscle transfers have been reported as surgical treatments for long-standing facial paralysis. Among one-stage methods, two approaches, that is, latissimus dorsi transfer with nerve suturing to the contralateral facial nerve and gracilis transfer with nerve suturing to the ipsilateral masseteric nerve, have recently become popular. The former method has the advantage of making spontaneous smiling possible, but the contraction strength of the transferred muscle varies, whereas the latter approach has the advantage of guaranteeing voluntary contract

Superficial Temporal Artery to Middle Cerebral Artery Bypass for Moyamoya Disease: Surgical Nuances.

Moyamoya disease is a progressive pathology that generally presents with ischemic complications in the pediatric age group.1 Direct and indirect revascularization procedures have been shown to augment the cerebral blood flow and prevent disease progression. Some studies have reported better angiographic outcomes with direct revascularization, though its translation into clinical benefit is yet to be proven in prospective studies.1-4 In addition, direct revascularization is surgically challenging among the pediatric age group due to smaller vessel caliber. We present a case of a 10-year-old gir

Outcomes of combined anterior latissimus dorsi and teres major tendon transfer for irreparable anterosuperior rotator cuff tears.

BACKGROUND: Irreparable anterosuperior rotator cuff tears (IASRCTs) can result in loss of active elevation, loss of internal rotation, and pseudoparalysis. Joint-preserving treatment options for IASRCTs in young and high-demand elderly patients are limited. The purpose of this study was to evaluate the clinical and radiologic outcomes of combined anterior latissimus dorsi and teres major (aLDTM) tendon transfer in patients with IASRCTs without reconstruction of the superior rotator cuff and capsular portion. METHODS: This retrospective study was conducted between 2015 and 2018. We included pa

Combined bilobed trapezius myocutaneous and scapula osteomyocutaneous flaps for the restoration of through-and-through defects of the mandible following salvage surgery for recurrent head and neck tumours.

This study was performed to evaluate the outcomes of combined bilobed trapezius myocutaneous and scapula osteomyocutaneous flaps for the restoration of through-and-through defects of the mandible. The subjects were six patients with recurrent malignant tumours who underwent salvage surgeries and reconstruction with combined flaps based on the transverse cervical vessels (TCVs). All patients had combined bone (four type H, one type LCL, and one type C) and extensive soft tissue defects (through-and-through defects) following salvage surgery. All tumours were removed and the complex defects rest

Effectiveness of Hydro-Dissection of the Piriformis Muscle Plus Low-Dose Local Anesthetic Injection for Piriformis Syndrome: A Report of 2 Cases.

BACKGROUND Piriformis syndrome causes severe buttock and low back pain and numbness of the lower limbs, leading to difficulty in walking. The lack of unified diagnostic criteria for piriformis syndrome until 2018 made it difficult to diagnose; therefore, it is often underestimated, and some patients do not receive appropriate treatment. Treatment methods include local anesthetic injection, steroid injection, and local botulinum toxin injection. However, the most effective drug for the treatment of severe piriformis syndrome has not been established. This report aimed to propose a new and more

Reverse total shoulder arthroplasty with the Tornier Perform implant with lower trapezius transfer for symptomatic rotator cuff deficiency with external rotation lag and Hornblower's sign.

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) is a well-established treatment for cuff tear arthropathy and consistently restores active forward elevation. However, when there is a deficiency of the posterior rotator cuff (infraspinatus and teres minor), patients can exhibit lag signs at the side and a "Hornblower's sign." This loss of active external rotation (ER) in abduction can be functionally disabling, and rTSA alone may not reliably restore active ER ability in this clinical scenario. We present a series of rTSA with lower trapezius transfer (LTT) (utilizing tibialis anterior a

CT-Derived Pretreatment Thoracic Sarcopenia Is Associated with Late Mortality after Thoracic Endovascular Aortic Repair.

BACKGROUND: Frailty, characterized by physiologic depletion, predicts postoperative morbidity and mortality in vascular surgery patients. CT-derived sarcopenia is a valuable method for objectively staging frailty preoperatively. PURPOSE: With prior analyses primarily measuring psoas cross-sectional area on CT, we compared a method of measuring thoracic sarcopenia to existing techniques of lumbar sarcopenia and assessed the association with long-term survival and outcomes post-Thoracic Endovascular Aortic Repair (TEVAR). METHODS: Prospectively collected data of 217 patients undergoing TEVAR f

Non-invasive Quantitative Assessment of Layer-Specific Degeneration in the Lumbar Multifidus in Elite Male High School Soft Tennis Players Using Ultrasound Echo Intensity.

Introduction Soft tennis involves repetitive, high-intensity trunk rotation during strokes and serves, with high popularity among Japanese high school boys. The sport's biomechanical demands place cumulative stress on the lumbar spine, and a high prevalence of low back pain (LBP) has been reported, particularly in elite-level athletes. Previous studies identified factors such as hip rotation range of motion (ROM), trunk strength, and training load as LBP-related in rotational sports. However, foundational insights into the structural characteristics of the lumbar region in this population rema

Mechanisms of low back pain: a neurophysiologic and neuroanatomic study.

Idiopathic low back pain has confounded health care practitioners for decades. The cellular and neural mechanisms that lead to facet pain, discogenic pain, and sciatica are not well understood. To help elucidate these mechanisms, anesthetized New Zealand white rabbits were used in a series of neurophysiologic and neuroanatomic studies. These studies showed the following evidence in support of facet pain: an extensive distribution of small nerve fibers and endings in the lumbar facet joint, nerves containing substance P, high threshold mechanoreceptors in the facet joint capsule, and sensitizat

Postoperative Cancer Surveillance Following Oncoplastic Surgery with Latissimus Dorsi Flap: a Matched Case-Control Study.

BACKGROUND: The latissimus dorsi (LD) myocutaneous flap is a widely used local option in oncoplastic surgery for avoiding breast deformities; however, concerns exist regarding its influence in monitoring recurrence. In this study, we evaluated the impact of this flap on postoperative cancer surveillance. METHODS: Each patient receiving oncoplastic surgery with LD flap after partial mastectomy were matched in age, cancer stage, and body mass index with patients receiving partial mastectomy alone. Twenty-nine patients with the oncoplastic LD flap received 99 mammograms and 139 ultrasonograms, w
