Shoulder Injuries in the Overhead-Throwing Athlete: Epidemiology, Mechanisms of Injury, and Imaging Findings.

The unparalleled velocity achieved by overhead throwers subjects the shoulder to extreme forces, resulting in both adaptive changes and pathologic findings that can be detected at imaging. A key biomechanical principle of throwing is achieving maximum external rotation, which initially leads to adaptive changes that may result in a pathologic cascade of injuries. In addition to the well-established concepts of glenohumeral internal rotation deficit and internal impingement, osseous and soft-tissue injuries of the shoulder unique to overhead athletes are illustrated. The epidemiology and biomec

Functional results of hemi- and total shoulder arthroplasty according to diagnosis and patient age at surgery.

Background and purpose - There is a lack of information on any associations between the functional outcome and age and diagnosis in patients who have undergone shoulder arthroplasty. We therefore evaluated the functional outcome in "young" and "old" patients treated with either hemiarthroplasty (HA) or total shoulder arthroplasty (TSA) with diverse diagnoses. Patients and methods - The functional results of 496 primary shoulder arthroplasties were analyzed using the Constant score (age- and sex-adjusted) and subjective satisfaction. Patients ≤55 years of age at surgery were defined as "young.

Imaging of sports injuries of the upper extremity in children.

Competitive athletics in school-aged children has become the norm rather than the exception. The increased repetitive stresses placed on the upper extremity in a wide variety of sports result in a host of injuries unique to the skeletally immature athlete. This article focuses on a discussion of the more common upper extremity injuries encountered in the child athlete and the role of radiography and MRI in diagnosis and management.

Clinical significance of the arthroscopic drive-through sign in shoulder surgery.

PURPOSE: During arthroscopy of the shoulder, the ability to pass the arthroscope easily between the humeral head and the glenoid at the level of the anterior band of the inferior glenohumeral ligament is considered a positive drive-through sign. The drive-through sign has been considered diagnostic of shoulder instability and has been associated with shoulder laxity and with SLAP lesions. The goal of this study was to examine the prevalence of the drive-through sign in patients undergoing shoulder arthroscopy and to determine its relationship to shoulder instability, shoulder laxity, and to SL

Intramedullary Nailing Technique for Proximal Humeral Fractures Using a Straight Antegrade Nail with Locking Tuberosity Fixation.

BACKGROUND: Intramedullary straight nail fixation of proximal humeral fractures using a locking mechanism provides advantages compared with plating, including (1) less soft-tissue dissection, which preserves periosteal blood supply and soft-tissue attachments; (2) improved construct stability for comminuted fractures or osteopenic bone; and (3) shorter operative time for simpler fractures. DESCRIPTION: The patient is placed in the beach-chair position with the head of the bed elevated approximately 45°. The fracture is reduced with use of closed or percutaneous methods, ideally, or with an op

Platelet-rich plasma in the management of musculoskeletal disorders: a narrative review of clinical evidence and practical considerations.

BACKGROUND: Platelet-rich plasma (PRP) has emerged as a widely applied autologous biologic therapy in musculoskeletal medicine. Its concentrated growth factor content is postulated to promote tissue regeneration, modulate the inflammatory cascade, and provide clinically meaningful and durable symptom relief in both degenerative joint disease and chronic tendinopathy. Despite its widespread clinical adoption, the evidence base remains heterogeneous, and its rational application requires a critical synthesis of current findings. OBJECTIVE: This narrative review aimed to critically examine the e

Revision risk of cross-linked versus non-crosslinked polyethylene in reverse shoulder arthroplasty: an analysis of 63,502 cases from the Australian Orthopaedic Association National Joint Replacement Registry.

BACKGROUND: The effect of increased cross linking of modular polyethylene components in primary total stemmed reverse shoulder replacement (rTSA) upon revision rates is unknown outside biomechanical testing. We compared the rate of revision of primary stemmed reverse shoulder replacement with either a highly crosslinked (XLPE) or non-highly crosslinked polyethylene (non-XLPE) glenosphere or humeral cup. MATERIALS AND METHODS: Using data from the Australian Orthopaedic Association National Joint Replacement Registry, all primary rTSA with either polyethylene humeral cups or glenospheres were i

Systematic review of shoulder arthroplasty outcomes: what sample size is meaningful?

BACKGROUND: Shoulder arthroplasty is increasingly performed for shoulder conditions such as arthritis, rotator cuff arthropathy, and traumatic injuries. Registries and other compilations of patient data provide the opportunity to detect meaningful differences in outcomes between alternative techniques and implants. A wide range of outcome measurements are reported after shoulder arthroplasty, but the sample sizes needed to identify meaningful differences have not been studied systematically. This review systematically analyzes common clinical outcomes reported for shoulder arthroplasty and rep

A standardized construct of blocked threaded wires for treating complex three-part proximal humerus fractures. A long-term follow-up of a previously published series.

BACKGROUND: Percutaneous pinning is classically considered an option for treating proximal humerus fractures (PHFs) in elderly low-demanding patients; recently, promising clinical and radiographic medium-term outcomes have been documented after the treatment of displaced PHF using different configurations of blocked threaded wires. However, long-term follow-up (FU) is still lacking. The aim of the present study was to evaluate the clinical and radiographic outcomes of a previously published midterm FU cohort after a minimum of 8 years. METHODS: In this observational study, all 52 patients fro

Association of post-stroke hemiplegic shoulder pain with sleep quality, mood, and quality of life.

BACKGROUND: Hemiplegic Shoulder Pain (HSP) is a prevalent post-stroke complication, characterized by paralysis, spasticity, altered sensation, neuropathic pain, shoulder subluxation, and soft tissue injuries such as rotator cuff tears and bicipital tendonitis. Muscle imbalances, weakness, and altered scapular positioning further contribute to the exacerbation of HSP. These factors lead to poorer functional outcomes, reduced hand function, and difficulties in performing daily activities. HSP is often associated with substantial mental and physical health burdens. The disorder significantly impa

How low can we go? A randomized controlled trial of low-quantity initial opioid prescriptions for shoulder surgery.

BACKGROUND: Orthopedic surgeons are the third most frequent prescribers of opioid medications. Given the current opioid addiction crisis, it is critical to limit opioid prescriptions to the lowest effective dose. In this study, we investigated how the initial opioid prescription after shoulder surgery affects maximum possible opioid consumption. We hypothesized that fewer pills in the initial opioid prescription would lead to less opioid consumption, a lower refill request rate, and fewer post-surgery office contacts for pain. METHODS: In this single-center, prospective, randomized controlled

The Global Track Concept for Assessment of Engaging Hill-Sachs Defects in Anterior Shoulder Instability.

BACKGROUND: The glenoid track concept is used to determine preoperatively whether a Hill-Sachs defect is engaging or not. Currently, the glenoid track concept relies on measurements of bony structures as well as on the confines and elasticity of the rotator cuff as a reference point, which varies extensively among individuals and therefore limits the reliability and accuracy of this concept. PURPOSE: To evaluate the reliability of the global track concept, which determines the angular distance of the Hill-Sachs defect from the center of the articular surface of the humeral head as a new refer

Does the stemless reverse arthroplasty compare to a conventional stemmed implant? Clinical and radiographic evaluation at 2 years' minimum follow-up.

BACKGROUND: Reverse shoulder arthroplasty (RSA) is a well-recognized treatment for many shoulder conditions, including rotator cuff arthropathy, primary glenohumeral joint arthritis, and rheumatoid arthritis, and can be used in both trauma and revision settings. Over the past 10 years, its popularity in New Zealand has been increasing, with a 6%-7% annual growth rate during this period. Stemless RSA designs have the following proposed advantages: They can preserve humeral bone stock, they can limit periprosthetic fractures, and they can be indicated in patients with abnormal diaphyseal humeral

Prolotherapy in the Treatment of Sports-Related Tendinopathies: A Systematic Review of Randomized Controlled Trials.

BACKGROUND: As sports have increased in popularity, the incidence of tendinopathy has also grown dramatically. Nonoperative techniques and treatments used to address these pathologies continue to evolve and improve. One such treatment, prolotherapy (PrT), has become increasingly popular and may provide patients with an alternative nonoperative treatment option. PURPOSE: To review high-quality randomized controlled trials (RCTs) that analyzed PrT treatments for the most common tendinopathies. Specifically, this review aims to provide meaningful data regarding methods and outcomes for each cond

CCL4L2 participates in tendinopathy progression by promoting macrophage inflammatory responses: a single-cell analysis.

BACKGROUND: Tendinopathy is very common in clinical practice, which is highly prevalent in athletes, sports enthusiasts and other people involved in high-load weight-bearing activities. Common types of tendinopathy include rotator cuff injury, Achilles tendinitis, tennis elbow and so on. Macrophages (Macs) are key immune cells in the pathogenesis of tendinopathy. In this study, CCL4L2+ M1-related signaling pathways were screened by combining single-cell RNA sequencing (scRNA-seq) to explore their significance in tendinopathy treatment. METHODS: Immune cell populations were screened by Uniform

Quantifying success after first revision reverse total shoulder arthroplasty: the minimal and substantial clinically important percentage of maximal possible improvement.

BACKGROUND: When patients require reoperation after primary shoulder arthroplasty, revision reverse total shoulder arthroplasty (rTSA) is most commonly performed. However, defining clinically important improvement in these patients is challenging because benchmarks have not been previously defined. Furthermore, although the minimal clinically important difference and substantial clinical benefit are commonly used to assess clinically relevant success, these metrics are limited by ceiling effects that may cause inaccurate estimates of patient success. Our purpose was to define the minimal and s

Abnormal preoperative leukocyte counts and postoperative complications following total shoulder arthroplasty.

BACKGROUND: Total shoulder arthroplasty (TSA) has become the mainstay of treatment for degenerative glenohumeral arthritis, proximal humerus fracture, and rotator cuff arthropathy. The expanding indications for reverse TSA have increased the overall demand for TSA. This necessitates higher quality preoperative testing and risk stratification. White blood cell counts can be obtained from routine preoperative complete blood count testing. The association between abnormal preoperative white blood cell counts and postoperative complications has not been extensively studied. The purpose of this stu

Clinical Relevance of Persistent Off-Track Hill-Sachs Lesion After Arthroscopic Latarjet Procedure.

BACKGROUND: The Latarjet procedure is often used to address off-track Hill-Sachs lesions (OFF-HS) in shoulders with anterior instability. There are concerns as to whether the Latarjet procedure is able to convert all OFF-HS into on-track Hill-Sachs lesions (ON-HS) and whether this limitation could explain the cases of recurrent postoperative instability. HYPOTHESIS: Latarjet surgery converts many preoperative OFF-HS lesions, but not all of them, and there is a difference in the failure rate between shoulders with converted lesions and those with persistent OFF-HS lesions. STUDY DESIGN: Case
