Development of Evidence-based Guidelines on Assessment and Management of Internal Contamination with Transuranic Actinides.

OBJECTIVES: To describe the process of the development of evidence-based guidelines on the assessment and clinical management of internal contamination with transuranic actinides (specifically plutonium, americium, and curium) in incidents where workers, emergency responders, and the public might uptake these radionuclides internally through inhalation, ingestion, or wound contamination. METHODS: The World Health Organization (WHO) set up a guidelines development group (GDG) that follows the protocol required for producing evidence-based recommendations as described elsewhere. Evaluating the Effectiveness of Dry Decontamination Methods for Hazmat Incidents: A Scoping Review.

OBJECTIVES: CBRN incidents require specialized hazmat decontamination protocols to prevent secondary contamination and systemic toxicity. While wet decontamination is standard, it can present challenges in cold weather or when resources are limited. Dry decontamination offers an alternative and supportive approach, though its effectiveness across different contaminants remains unclear. This scoping review evaluates the effectiveness, advantages, and limitations of dry decontamination in hazmat incidents. METHODS: A scoping review was conducted using MEDLINE, CINAHL, and other databases. Follo

Adult Medical Countermeasures: Antidotes and Cytokines for Radiological and Nuclear Incidents and Terrorism.

The war in Ukraine raises concerns for potential hazards of radiological incidents and their impact on humans, especially families. Preparedness and response to radiological and nuclear incidents necessitates familiarity with pharmaceutical countermeasures, including antidotes and cytokines. Searches found no published study comparing adult indications and dosing among standard references. This study addresses this gap by collecting, tabulating, and disseminating information to health care professionals. Expert consensus chose the following references to compare adult indications and dosing of

Long-Term Mental Health Findings and Concerns After Potential Exposure to Petroleum in Drinking Water on Joint Base Pearl Harbor-Hickam, November 20, 2021-February 24, 2023.

OBJECTIVE: An estimated 93,000 persons were potentially exposed to drinking water contaminated with petroleum jet propellant (JP)-5 fuel after a November 20, 2021, leak at the Red Hill Bulk Fuel Storage Facility on Oahu, Hawaii. Previous investigations identified the need to evaluate long-term mental health effects associated with JP-5 exposure. METHODS: We identified adults potentially exposed to jet fuel-contaminated water during November 20, 2021-March 18, 2022, who sought care within the military health system through February 24, 2023. We abstracted a sample of electronic medical records

EURADOS/REMPAN Review on Monitoring and Dosimetry for Radionuclide-contaminated Wounds.

The European Radiation Dosimetry Group (EURADOS) and the WHO's Radiation Emergency Medical Preparedness and Assistance Network (REMPAN) have collaborated to review best practices for managing radionuclide intakes through wounds. Rapid response and decisions on wound decontamination, tissue excision, and chelation therapy are based on measurements of the exposed individual and preliminary dose assessments using reasonable default assumptions. The goal is to minimize exposure, prevent tissue reactions, and reduce the risk of stochastic effects. The management of a contaminated wound is always cas

Evacuation Decision-Making Post-COVID-19 Vaccine Availability: Implications of Compound Hazards in Puerto Rico and the US Virgin Islands.

OBJECTIVE: The threat of novel pathogens and natural hazards is increasing as global temperatures warm, leading to more frequent and severe occurrences of infectious disease outbreaks and major hurricanes. The COVID-19 pandemic amplified the need to examine how risk perceptions related to hurricane evacuations shift when vaccines become available. This study explores individuals' expected evacuation plans during the early stages of COVID-19 vaccine availability. METHODS: In March 2021, an online survey was disseminated in Puerto Rico and the US Virgin Islands. RESULTS: An overwhelming majori

Sociodemographic and housing factors for flood and mold damage in the Greater New Orleans Area after Hurricane Ida.

OBJECTIVE: As extreme weather events become more pervasive due to climate change, identifying populations with lower access to resources becomes critical for timely mitigation efforts. Here we analyzed data from a survey conducted in Louisiana after Hurricane Ida to investigate demographic and housing characteristics of those vulnerable to home damage, flooding, and mold exposure after an extreme weather event. METHODS: The survey participants comprised a convenience sample of 167 total respondents, most of whom resided in the Greater New Orleans Area. Sociodemographic, housing, and geographi

Protocol and Progress of a Thyroid Study in the Epidemiological Study of Health Effects in Fukushima Emergency Workers.

OBJECTIVE: To describe the protocol and progress of a thyroid study using thyroid ultrasonography in emergency workers who responded to the Fukushima nuclear accident. METHODS: Thyroid ultrasonography was performed on Fukushima emergency workers at over 60 health examination institutions. The accuracy of ultrasonography is controlled by standard procedural protocols, examiner training, and a central review system. Thyroid findings are classified into 4 categories: Category A1 (no nodule or cyst), Category A2 (nodules ≤ 5.0 mm and/or cysts ≤ 20.0 mm), Category B (nodules ≥ 5.1 mm and/or cysts

Early Surveillance and Public Health Emergency Response Measures to SARS, H1N1, and COVID-19: A Case-Comparative Study.

OBJECTIVE: At present, COVID-19 has already spread rapidly as a global pandemic, just like SARS in 2003 and H1N1 swine influenza in 2009. This study analyzes surveillance and emergency responses to these three epidemics to identify gaps in public health emergency management. METHODS: This case-comparative study uses 6 critical time points to evaluate and compare the responses. RESULTS: Results indicate that China has demonstrated improvements in pathogen identification and governmental coordination since the SARS outbreak, though its overall responsiveness during COVID-19 remained slower tha
