Oxidative DNA damage and somatic mutations: a link to the molecular pathogenesis of chronic inflammatory airway diseases.

BACKGROUND: Acquired somatic mutations induced by oxidative stress may contribute to the molecular pathogenesis of chronic inflammatory airway diseases. The objective of this study was to assess the intensity of oxidative DNA damage and the presence of microsatellite DNA instability (MSI), a marker of acquired somatic mutations, in patients with COPD, patients with noncystic fibrosis bronchiectasis, and control subjects. METHODS: Induced sputum and peripheral blood from 97 subjects were analyzed; 36 patients with COPD, 36 patients with bronchiectasis, 15 smokers without COPD, and 10 healthy c

Blinded 12-week comparison of once-daily indacaterol and tiotropium in COPD.

Two, once daily (q.d.) inhaled bronchodilators are available for the treatment of chronic obstructive pulmonary disease (COPD): the β(2)-agonist indacaterol and the anticholinergic tiotropium. This blinded study compared the efficacy of these two agents and assessed their safety and tolerability. Patients with moderate-to-severe COPD were randomised to treatment with indacaterol 150 μg q.d. (n=797) or tiotropium 18 μg q.d. (n=801) for 12 weeks. After 12 weeks, the two treatments had similar overall effects on "trough" (24 h post-dose) forced expiratory volume in 1 s. Indacaterol-treated patien

Bisoprolol in patients with heart failure and moderate to severe chronic obstructive pulmonary disease: a randomized controlled trial.

AIMS: Heart failure (HF) and chronic obstructive pulmonary disease (COPD) frequently coexist. No study has prospectively examined the effects of beta-blockade in those with both conditions. METHODS AND RESULTS: We randomized 27 patients with HF and coexistent moderate or severe COPD to receive bisoprolol or placebo, titrated to maximum tolerated dose over 4 months. The primary outcome was forced expiratory volume in 1 s (FEV(1)). The study is registered with ClinicalTrials.gov, number: NCT00702156. Patients were elderly and predominantly male. Cardiovascular comorbidity, smoking history, and

Effect of home mechanical ventilation on inspiratory muscle strength in COPD.

BACKGROUND: The mechanism responsible for chronic hypercapnic respiratory failure (HRF) in patients with COPD remains unclear. In this study, we tested the hypothesis that chronic HRF in patients with COPD is associated with low-frequency fatigue (LFF) of the diaphragm. METHODS: To test this hypothesis, we measured the twitch transdiaphragmatic pressure (Tw Pdi) elicited by stimulation of the phrenic nerves in 25 patients with chronic HRF (mean [+/- SD] Paco(2), 55.2 +/- 5.2 mm Hg) due to COPD before and 2 months after the initiation of noninvasive mechanical ventilation (NIV) [pressure-cycle

Does chronic cervical myelopathy affect respiratory function?

OBJECT: Although respiratory function is often impaired by acute cervical spinal cord injury, changes in respiratory function in patients with chronic cervical myelopathy (CCM) are not well documented. The purpose of this study was to evaluate the respiratory function of patients with CCM. METHODS: Spirometric parameters were measured in 94 patients with CCM before they underwent expansive laminoplasty. These measurements were compared with those obtained in age- and sex-matched control group patients without myelopathy. The study patients were also subdivided into two groups: those with spin

The role of rib infarcts in the acute chest syndrome of sickle cell diseases.

The acute chest syndrome is a generic term for pulmonary complications of sickle cell diseases with heterogeneous etiologies that include pneumonia, vaso-occlusion of pulmonary arterioles, rib infarction, and fat embolism syndrome. My review summarizes these etiologies, the evidence, and pathophysiology supporting the hypothesis that infarction of segments of ribs by the same vaso-occlusive process responsible for the acute episodes of pain (characteristic of the sickle cell diseases) is often involved in the acute chest structure. Inflammation associated with the infarct then causes splinting

A Comparative Study of Erector Spinae Plane Block and Thoracic Epidural Block on Respiratory, Analgesic, and Hemodynamic Outcomes in Patients With Traumatic Rib Fractures.

Background Rib fractures are associated with significant morbidity and mortality. Thoracic epidural analgesia (TEA) provides excellent pain relief in the management of rib fractures. However, side effects, such as hypotension, and the technical challenges associated with its insertion can limit the use of this technique. Erector spinae plane block (ESPB) is a more superficial ultrasound-guided block, which is easier to perform and does not pose the same risk factors. The primary objective of this study was to compare the analgesic efficacy of continuous TEA and continuous ESPB in patients with

Dynamic Digital Radiography (DDR) in the Diagnosis of a Diaphragm Dysfunction.

Dynamic digital radiography (DDR) is a recent imaging technique that allows for real-time visualization of thoracic and pulmonary movement in synchronization with the breathing cycle, providing useful clinical information. A 46-year-old male, a former smoker, was evaluated for unexplained dyspnea and reduced exercise tolerance. His medical history included a SARS-CoV-2 infection in 2021. On physical examination, decreased breath sounds were noted at the right-lung base. Spirometry showed results below predicted values. A standard chest radiograph revealed an elevated right hemidiaphragm, a fin

Clinical Characteristics of Obstructive Sleep Apnea in Psychiatric Disease.

Patients with serious psychiatric diseases (major depressive disorder (MDD), bipolar disorder (BD), and schizophrenia and psychotic disorder) often complain about sleepiness during the day, fatigue, low energy, concentration problems, and insomnia; unfortunately, many of these symptoms are also frequent in patients with Obstructive Sleep Apnea (OSA). However, existing data about the clinical appearance of OSA in Psychiatric Disease are generally missing. The aim of our study was a detailed and focused evaluation of OSA in Psychiatric Disease, in terms of symptoms, comorbidities, clinical chara

A Cross-Sectional Observational Study of the Correlation Between Asthma-Related Quality of Life (AQLQ) and Asthma Control Measures.

Introduction The Asthma Control Questionnaire (ACQ) and Asthma Control Test (ACT) are widely used to assess asthma control but are more time-consuming than the Global Initiative for Asthma (GINA) level of control (LoC), a simpler tool that is commonly used in busy clinical settings. Despite the substantial impact of asthma on quality of life, quality-of-life assessment is performed less frequently than assessment of asthma control. This study aimed to evaluate the correlation between the Asthma Quality of Life Questionnaire (AQLQ) and measures of asthma control, including the ACQ and GINA LoC.

Immediate and Sustained Effects of Intensive Equine-Assisted Physiotherapy Based on Neuroproprioceptive "Facilitation and Inhibition" on Psychomotor Development, Clinical Functions, Quality of Life, and Molecular Biological Indicators in Children With Spinal Muscular Atrophy: Protocol for a Crossover Randomized Controlled Trial.

BACKGROUND: Spinal muscular atrophy (SMA) is a rare neuromuscular disease and the most common genetic cause of infant death. Although pharmacological treatment improves survival rates and functional capacity, physiotherapy remains a key component of care. A newly developed innovative equine-assisted physiotherapy method based on neuroproprioceptive "facilitation and inhibition" principles (NEUROEQUIP-SMA) is hypothesized to improve the quality and extent of motor development in children with SMA. OBJECTIVE: The aim of this study is to assess the efficacy of NEUROEQUIP-SMA compared with standa

Different Interdialytic Intervals and Cardiorespiratory Fitness in Patients Undergoing Hemodialysis.

KEY POINTS: This is the first study exploring differences in cardiorespiratory fitness assessed with cardiopulmonary exercise testing between the 2-day and the 3-day interdialytic interval. The 3-day interdialytic interval was associated with further impaired cardiorespiratory fitness. This effect was predominantly driven by excess fluid accumulation during the extra interdialytic day. BACKGROUND: Long interdialytic interval in thrice-weekly hemodialysis is associated with excess cardiovascular and all-cause mortality risk. Impaired cardiorespiratory fitness is a strong predictor of mortality

Pulmonary sarcoidosis: A comprehensive review: Past to present.

Sarcoidosis is a sterile non-necrotizing granulomatous disease without known causes that can involve multiple organs with a predilection for the lung and thoracic lymph nodes. Worldwide it is estimated to affect 2-160/100,000 people and has a mortality rate over 5 years of approximately 7%. For sarcoidosis patients, the cause of death is due to sarcoid in 60% of the cases, of which up to 80% are from advanced cardiopulmonary failure (pulmonary hypertension and respiratory microbial infections) in all races except in Japan were greater than 70% of the sarcoidosis deaths are due to cardiac sarco

The Effect of Aerosolized Chlorpheniramine Maleate on Exercise Induced Bronchospasm and Gas Exchange in Asthmatics.

INTRODUCTION: Exercise induced asthma (EIB) is an acute, reversible, usually selflimiting airways obstruction which sets in after exercise in patients with asthma. One popular mechanism of EIA is the increase in histamine and its metabolites in circulation after exercise, which leads to bronchoconstriction via histamine receptors in bronchi. Chlorpheniramine Maleate is potent, less sedative antihistaminic drug, which acts by inhibiting histamine release from mast cells. It is also said to have anticholinergic properties. The aerosol route of administration of a drug has the advantages of a fas
