Excision and Primary Anastomosis for Short Bulbar Strictures: Is It Safe to Change from the Transecting towards the Nontransecting Technique?

OBJECTIVE: To explore whether it is safe to change from transecting excision and primary anastomosis (tEPA) towards nontransecting excision and primary anastomosis (ntEPA) in the treatment of short bulbar urethral strictures and to evaluate whether surgical outcomes are not negatively affected after introduction of ntEPA. MATERIALS AND METHODS: Two-hundred patients with short bulbar strictures were treated by tEPA (n=112) or ntEPA (n=88) between 2001 and 2017 in a single institution. Failure rate and other surgical outcomes (complications, operation time, hospital stay, catheterization time,

Intravesical formalin for hemorrhagic cystitis: A contemporary cohort.

INTRODUCTION: Hemorrhagic cystitis presents a difficult clinical challenge, yet data regarding treatment options is sparse. Here, we sought to evaluate outcomes of a contemporary cohort of patients treated with intravesical formalin for hemorrhagic cystitis. METHODS: We identified a retrospective cohort of eight patients managed with formalin for hemorrhagic cystitis from 2000-2014. All patients failed prior measures, including bladder irrigation, clot evacuation, and other intravesical agents. Treatment success was defined as hematuria resolution during the given hospitalization without use

Ureteral reimplantation before bladder neck reconstruction in modern staged repair of exstrophy patients: indications and outcomes.

OBJECTIVE: To evaluate a single referral center's experience with vesicoureteral reflux (VUR) and its role in management before bladder neck reconstruction (BNR) in exstrophy patients after modern staged repair of exstrophy (MSRE). VUR after primary closure of bladder exstrophy can usually be managed conservatively until ureteral reimplantation (UR) at the time of BNR. However, VUR in these patients is occasionally associated with febrile urinary tract infection and worsening renal function necessitating UR before BNR. MATERIALS AND METHODS: A total of 199 patients who underwent MSRE were ide

Bladder neck sling suspension during robot-assisted radical prostatectomy to improve early return of urinary continence: a comparative analysis.

OBJECTIVE: To present our experience of a novel technique, bladder neck sling suspension technique, in patients undergoing robot-assisted radical prostatectomy (RARP) and evaluate its efficacy for improving early continence outcomes after RARP in comparison with that of the standard technique. METHODS: Fifty-seven consecutive patients underwent RARP between August 2011 and April 2012. We performed RARP with and without bladder neck sling suspension technique for 27 (sling group) and 30 (nonsling group) patients, respectively, and compared the urinary outcome between the groups. Both subjectiv

Primary, nonsyndromic vesicoureteric reflux and nephropathy in sibling pairs: a United Kingdom cohort for a DNA bank.

BACKGROUND AND OBJECTIVES: Primary vesicoureteric reflux (VUR) can coexist with reflux nephropathy (RN) and impaired renal function. VUR appears to be an inherited condition and is reported in approximately one third of siblings of index cases. The objective was to establish a DNA collection and clinical database from U.K. families containing affected sibling pairs for future VUR genetics studies. The cohort's clinical characteristics have been described. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: Most patients were identified from tertiary pediatric nephrology centers; each family had an

Horseshoe-shaped marlex mesh for the treatment of pelvic floor prolapse.

OBJECTIVE: Pelvic prolapse results from weakness or damage to the normal pelvic-support systems. The main support for the pelvic viscera is provided by the pelvic fascia, which is naturally reinforced by urethra-pelvic ligaments, cardinal ligaments and uterosacral ligaments. A polypropylene mesh (Marlex-Bard) was used as a genitourinary and rectal support in order to substitute the damaged pelvic fascia. METHODS: Sixteen consecutive females suffering from severe genitourinary prolapse entered the study. Using the HWS (Baden-Walker) classification 10 patients presented a grade-IV and 6 patient

Safety and efficacy of concomitant holmium laser enucleation of the prostate with transurethral endoscopic management of symptomatic large bladder diverticulum: revisiting a historical technique in the modern era with literature review.

INTRODUCTION: Acquired bladder diverticulum (BD) is typically managed using open, laparoscopic, or robotic approaches. Although transurethral techniques demonstrated favorable outcomes in the 1970s and 1980s, they have largely fallen out of favor. This study revisits transurethral endoscopic management of large, symptomatic BD, combined with Holmium laser enucleation of the prostate (HoLEP) for patients with benign prostatic obstruction (BPO) and coexisting BD. METHODS: We retrospectively reviewed patients who underwent combined HoLEP with transurethral endoscopic management of bladder divert

Ureteral reimplantation after deflux failure for vesicoureteral reflux in renal transplant.

PURPOSE: To evaluate the outcomes of ureteral reimplantation (UR) after failure of endoscopic treatment for symptomatic vesicoureteral reflux (VUR) in renal transplant recipients. METHODS: We conducted a monocentric retrospective study that included all renal transplant recipients with failure of Deflux™ as first-line treatment of VUR from January 2007 to December 2020. Failure of Deflux™ was defined by: VUR on retrograde cystography and at least one acute pyelonephritis of the renal graft. The preferred surgical treatment was native ureteropyelostomy (NPUS) in the recent years. If the native

Intra-Abdominal Hypertension-Induced Gastroesophageal Intussusception: A Rare Complication of Transurethral Resection of a Bladder Tumor.

BACKGROUND Gastroesophageal intussusception (GEI) generally develops in patients with risk factors. However, intra-abdominal hypertension (IAH) rarely causes sudden GEI in patients without known risk factors. Endoscopic or surgical intervention is generally performed to reduce GEI. However, when GEI is induced by IAH, intra-abdominal pressure (IAP) decompression can contribute to GEI reduction. CASE REPORT An 81-year-old man who underwent transurethral resection of bladder tumor (TURBT) for hematuria from a bladder tumor located at the left lateral wall had a deteriorated general status and bl

Laparoscopic Boari flap for treatment of benign midureter stricture.

INTRODUCTION:: Laparoscopic ureteral reconstructive surgery represents a real challenge for most of the urologists as it requires advanced skills. Impacted stones (>2 months) and endoscopic procedures are known major risk factors for ureteral strictures. Boari flap is a good alternative, due to the high recurrence of kidney stone disease, as it preserves the feasibility of ureteroscopy. MATERIAL AND METHODS:: We present a case of a 21-year-old female patient complaining of dull pain in the left flank, associated with vomiting and high-grade fever (39 degrees Celsius), for three days. Unorthodox cause of urinary leak post radical prostatectomy: Catheter balloon within a bladder diverticulum - Case report and highlights on various methods to overcome leaks.

INTRODUCTION: Radical prostatectomy for prostate cancer has been increasingly adopted, especially for localized disease. Nevertheless, this has not been without any morbidities. Complications, such as urine leak, related to malpositioned or malfunctional indwelling catheters, are rarely reported. PRESENTATION OF CASE: A 75-year-old male underwent an open radical prostatectomy for prostate cancer. Soon after surgery, he witnessed urine leak and extravasation. Imaging including a plain CT scan with a retrograde cystography, showed the tip of the indwelling catheter within a diverticular pouch o
