Indications for the Caldwell-Luc approach in the endoscopic era.

OBJECTIVE: Caldwell and Luc described the Caldwell-Luc operation more than 100 years ago as the surgical treatment for maxillary sinus disease. During the last decades less radical interventions using endoscopic approach have mainly replaced the classical procedures done for chronic and recurrent maxillary sinusitis. STUDY DESIGN AND SETTING: Between 1991 and 2002, 62 patients had the Caldwell-Luc approach for different indications. RESULTS: Twenty (32%) patients had chronic sinusitis, 16 (26%) patients had inverted papilloma, 9 (15%) patients had suffered from nasal polyposis, 4 patients (6

Dental impactions related to pseudoxanthoma elasticum.

Pseudoxanthoma elasticum is a rare genetic disorder causing degeneration and calcification of elastic fibers, leading to injuries in the skin, retina, and cardiovascular system. This report describes a case of a 30-year-old woman whose diagnosis of pseudoxanthoma elasticum was confirmed after a skin biopsy. The patient presented at the Clinical Management Unit of the Department of Oral and Maxillofacial Surgery, Virgen del Rocio Hospital (Seville, Spain) with pain related to a semi-erupted lower first molar. Radiologic examination disclosed multiple dental impactions. Cases of oligodontia, ame

How uncommon is the occurrence of orthokeratinized odontogenic cyst in the maxilla? A systematic review and a new case report.

BACKGROUND: The orthokeratinized odontogenic cyst (OOC) is a benign intraosseous lesion, typically found in the posterior mandible in male patients in the third and fourth decades of life. Although there has been a growing number of documented cases in the last decade, the prevalence and unique characteristics of maxillary OOC remain poorly understood. MATERIAL AND METHODS: To assess how uncommon is OOC occurrence in the maxilla, a systematic review of the literature was conducted using the major electronic databases and rigorous criteria, including a new case report involving a young male pa

Diagnostic Challenges in Differentiating Unicystic Ameloblastoma From Odontogenic Cysts: Report of Two Cases.

Ameloblastomas are benign odontogenic tumors characterized by local aggressiveness and a high potential for recurrence. Unicystic ameloblastoma (UA) is an uncommon variant that often mimics odontogenic cysts, making preoperative diagnosis challenging. Early and accurate differentiation is essential for guiding appropriate treatment planning and minimizing the risk of recurrence. This study is aimed at reporting two cases highlighting the diagnostic dilemma posed by UA. The first case involves a 65-year-old male patient with a well-defined unilocular radiolucency extending from Teeth 35 to 44.

Prevalence of Nonendodontic Diagnoses in Periapical Biopsies: A 6-year Institutional Experience.

INTRODUCTION: The purpose of this study was to identify nonendodontic periapical lesions (NPLs) mimicking endodontic pathosis, which are most frequently encountered by clinicians. METHODS: A retrospective study was conducted on biopsies obtained from 2015-2020 at Texas A&M College of Dentistry's oral pathology laboratory. The online database was screened for cases submitted as suspected endodontic pathology using specific key words. Histologic diagnoses were collected to determine the prevalence of NPLs that were originally thought to be of endodontic origin. The frequency and percentage of e

Cysts and cystic lesions of the mandible: clinical and radiologic-histopathologic review.

Many lesions that occur in the mandible have a cystlike radiographic appearance. These lesions are often difficult to differentiate on the basis of their radiographic features alone. Mandibular lesions may be odontogenic or nonodontogenic. Among odontogenic lesions without mineralization, ameloblastomas, odontogenic keratocysts, and dentigerous cysts can all appear as well-defined, unilocular, well-corticated, lucent lesions that are often associated with the crowns of impacted or unerupted teeth. Most radicular cysts appear as round or pear-shaped, unilocular, lucent lesions in the periapical

Diagnostic Pitfalls of Dental Follicles and Cyst-like Lesions in Juvenile Patients: An Early Odontogenic Myxoma Mimicking a Follicular Cyst.

The occurrence of cysts and tumors in pediatric patients varies across different age groups. Follicular and dentigerous cysts are among the most common lesions. However, typical odontogenic tumors in juvenile patients are not frequently observed. Early stages of cyst and odontogenic tumor development might exhibit some similar characteristics due to the presence of unerupted teeth or their relationship with various stages of tooth formation and eruption. Many small lesions are discovered accidentally on routine orthopantomography (OPG), while the bigger ones manifest themselves as bone swellin

Radiographic Features of Mandibular Second Molars with Eruption Disturbances: A Retrospective Study.

We aimed to establish the characteristics and potential etiological risk factors of eruption disturbances in mandibular second molars (MM2). We retrospectively enrolled patients with eruption disturbances in MM2. A total of 143 MM2 with eruption disturbances from 112 patients (mean age 17.45 ± 6.35) were included in this study. Panoramic radiographs were employed to determine the risk factor, angulation type, impaction depth, tooth development stage, and associated pathology. The novel classification method of MM2 was based on impaction depth and angulation. Of 143 MM2, 137 and 6 were diagnose

Odontogenic Keratocyst: The Dos and Don'ts in a Clinical Case Scenario.

BACKGROUND Odontogenic keratocysts are odontogenic cysts that increase in dimension based on growth factors and have a high recurrence rate. The radiological features of odontogenic keratocysts can be confusing owing to their similarity with other intraosseous cysts. The aim of treatment is to minimize patient morbidity and to reduce the risk of recurrence, along with complete surgical excision. CASE REPORT We report a case of a young man who presented to our hospital for a cystic lesion located in the posterior left mandible with clinical and radiological features of a dentigerous cyst. Radiographic features of large cystic lesions of the jaws in children.

BACKGROUND: The surgical approach to cystic lesions of the jaws is either marsupialisation or enucleation. The treatment of choice is dependent on the size of the lesion, the bony integrity of the cyst and its proximity to anatomical structures. OBJECTIVES: To assess large (>2.0 cm) cystic lesions of the jaws using plain film radiography (PFR), CT, multiplanar reconstruction program (MPR) and three-dimensional CT (3D-CT). PATIENTS AND METHODS: Twelve children aged 7-14 years. RESULTS: The classic radiological feature was a unilocular radiolucent area surrounded by a well-defined radio-opaqu

Odontogenic keratocyst: Review of 256 cases for recurrence and clinicopathologic parameters.

Odontogenic keratocyst (OKC) is of particular interest because of its high recurrence rate and aggressive behavior. Two hundred fifty-six cases of OKC were reviewed for the age of the patient at diagnosis, sex of the patient, OKC location, and radiographic findings, and 132 patients with OKC were observed to estimate recurrence, which was analyzed for age, sex, location, and several histopathologic findings. OKCs occurred more frequently in men (58.6%) than in women (41.4%), and they occurred in patients within a wide age range, most commonly in patients in the third decade of life (28.9%), fo

Under your nose: a rare finding during dissection provides insights into maxillary supernumerary teeth.

BACKGROUND: A supernumerary tooth was found during anatomical dissection. The position of this tooth, still impacted in the maxilla, and the associated pathology make this a rare case. METHODS: During dissection by dental students of the sagittally-sectioned head of a cadaver, a supernumerary tooth was identified in the mid-palatal area. Further dissection revealed a swelling with a thin bony covering related to the crown of the tooth. The maxilla was removed en bloc and radiographic examination, CT scanning, electron microscopy and histology were undertaken. RESULTS: The tooth had a crenula

Radiologic and pathologic characteristics of benign and malignant lesions of the mandible.

Mandibular lesions develop from both odontogenic and nonodontogenic origins and have varying degrees of destructive potential. Common benign cystic lesions include periapical (radicular) cysts, follicular (dentigerous) cysts, and odontogenic keratocysts. Benign solid tumors represent a broad spectrum of lesions such as ameloblastomas, odontomas, ossifying fibromas, and periapical cemental dysplasia. Malignant tumors that often involve the mandible include squamous cell carcinomas, osteosarcomas, and metastatic tumors. In addition, vascular lesions such as hemangiomas and arteriovenous malforma

Automatic segmentation of maxillofacial cysts in cone beam CT images.

Accurate segmentation of cysts and tumors is an essential step for diagnosis, monitoring and planning therapeutic intervention. This task is usually done manually, however manual identification and segmentation is tedious. In this paper, an automatic method based on asymmetry analysis is proposed which is general enough to segment various types of jaw cysts. The key observation underlying this approach is that normal head and face structure is roughly symmetric with respect to midsagittal plane: the left part and the right part can be divided equally by an axis of symmetry. Cysts and tumors ty
