Endotracheal balloon bursting: modified technique for percutaneous ultrasound-guided balloon puncture to reverse tracheal occlusion in congenital diaphragmatic hernia.

INTRODUCTION: Fetuses with congenital diaphragmatic hernia (CDH) treated with fetal endoscopic tracheal occlusion (FETO) require a second procedure to reverse tracheal occlusion before birth. In this study, we aimed to report the feasibility and perinatal outcomes of a modified technique for ultrasound-guided balloon bursting used as a primary attempt. METHODS: From January 2012 to December 2025 a prospective cohort of isolated CDH cases treated with FETO was included in a management protocol for balloon removal by a percutaneous balloon bursting using a 22-gauge needle in a transversal view

Fetal Brain Abnormalities Following Laser Surgery for Twin-to-Twin Transfusion Syndrome: Imaging Patterns and Evolution.

INTRODUCTION: Severe brain injury has been reported following fetoscopic laser ablation (FLA) for twin-to-twin transfusion syndrome (TTTS), but imaging patterns and temporal evolution remain poorly defined. This study aimed to characterize the progression of severe brain injury after FLA. METHODS: We conducted a retrospective case series of TTTS pregnancies with severe brain injury identified after FLA at the Ontario Fetal Center (June 2023-March 2025). FLA was performed for Quintero stage ≥ II and selected stage I cases. Severe abnormalities were defined as severe intraventricular haemorrhag

Diagnosis and Management of Spontaneous Twin Anemia Polycythemia Sequence during Early Second Trimester: A Case Report.

INTRODUCTION: Spontaneous twin anemia polycythemia sequence complicates about 3-5% of monochorionic pregnancies. This complication is associated with severe neurological morbidity and mortality in some cases. Prenatal diagnosis relies on Doppler assessment of the middle cerebral artery peak systolic velocity. However, there is ongoing debate regarding the appropriate gestational age to initiate surveillance. CASE PRESENTATION: We present a case of stage III twin anemia polycythemia sequence, first identified at 16 weeks of gestation, given discordant middle cerebral artery peak systolic veloc

Maternal propranolol as a safe alternative to invasive fetal surgery in chorioangioma management.

Chorioangiomas are associated with a significant risk of fetal demise due to high-output cardiac failure. Standard invasive treatment, including fetoscopic laser photocoagulation and intratumoral embolization, shows limited success and has high fetal mortality rates. The biological parallels between chorioangiomas and infantile hemangiomas suggest that these tumors may share common therapeutic targets. Propranolol is a proven therapy for infantile hemangiomas, therefore this established efficacy provides a strong rationale for its consideration in the management of symptomatic or rapidly growi

Maternal Dietary Choices Might Impact Intrauterine Healing Processes and Postnatal Phenotype and Function in Human Fetuses with Spina Bifida Aperta-Early Clinical Observations and Implications from a Retrospective Cohort Study.

Background: The severity of postnatal symptoms in patients with spina bifida aperta (SBA) is also determined by secondary factors that damage the exposed neural tissue throughout gestation. The purpose of this report is to present clinical cases, from 2010 to 2025, and a new hypothesis for a nonsurgical means of prenatal secondary prophylaxis. Patients: Eight fetuses underwent minimally invasive fetoscopic patch closure of SBA. After delivery, an unusual degree of prenatal patch healing was observed. Furthermore, time to complete postnatal skin closure was shorter (mean ± SD: 22.00 ± 6.53 days

Risk of Cord Entanglement After Iatrogenic Monoamnionicity, With Selective and Solomon Laser Treatment for Twin-To-Twin Transfusion Syndrome in Monochorionic Twin Pregnancies.

INTRODUCTION: Fetoscopic laser surgery (FLS) is the gold standard treatment for monochorionic (MC) twin pregnancies complicated by twin-twin transfusion syndrome (TTTS). The aim of our study was to evaluate the rate and risk factors for cord entanglement in the presence of iatrogenic monoamnioticity (iMA), a consequence of inadvertent septostomy during FLS. METHODS: This is a retrospective analysis of two consecutive cohorts of FLS performed either using the selective technique from January 2004 to January 2012, or with the Solomon technique, from that date onwards. Maternal and fetal charact

Management of twin-to-twin transfusion syndrome: update and current challenges.

Since a 2004 randomized trial established fetoscopy as the gold standard for the treatment for twin-to-twin transfusion syndrome (TTTS), advancements in surgical technique and expert training have considerably improved survival rates to 89% for at least one fetus and 65% for both fetuses. However, many challenges remain. This review provides an update on the current principles, issues, and debates. The Solomon technique has helped improve neonatal outcomes by reducing the risks of postoperative twin anemia-polycythemia sequence and TTTS recurrence. Nevertheless, fetoscopic laser photocoagulati

Anchoring device to prevent membrane detachment and preterm prelabor rupture of membranes after fetal intervention.

OBJECTIVE: To assess the feasibility of using a novel device designed for minimally invasive suturing to anchor fetal membranes to the uterine wall and to close surgical defects after fetoscopy. METHODS: We tested the WestStitch™ suturing device both ex vivo and in vivo. In the ex-vivo studies, 12-Fr trocar defects were created with a fetoscope in five specimens of human uterine tissue with fetal membranes attached. Specimens were examined for integrity of the anchoring stitch. For the in-vivo studies, trocar defects were created in the two uterine horns of three pregnant ewes, each carrying

The impact of a biocellulose-based repair of fetal open spina bifida on the need to untether the cord: is it time to unify techniques for prenatal repair?

OBJECTIVE: To report the need for cord untethering after prenatal repair of open spina bifida using a unique biocellulose-based technique performed at a later gestational age. METHODS: An observational cohort study was conducted to determine the incidence of tethered cord syndrome. Between May 2013 and May 2022, we performed 172 procedures using the percutaneous fetoscopic approach in fetuses at 26-28 weeks of gestation. After placode dissection, a biocellulose patch was placed to cover the placode, a myofascial flap (when possible) was dissected, and the skin was closed. Owing to death or lo

Fetal Surgery for Gastroschisis-A Review with Emphasis on Minimally Invasive Procedures.

(1) Background: The morbidity of gastroschisis is defined by exposure of unprotected intestines to the amniotic fluid leading to inflammatory damage and consecutive intestinal dysmotility, the viscero-abdominal disproportion which results in an abdomen too small to incorporate the herniated and often swollen intestine, and by associated pathologies, such as in complex gastroschisis. To prevent intestinal damage and to provide for growth of the abdominal cavity, fetal interventions such as amnio exchange, gastroschisis repair or covering have been evaluated in several animal models and human tr

Monochorionic diamniotic twin pregnancies.

About 1 in 5 twin pregnancies are monochorionic diamniotic. In these twins, transfusion imbalances may develop because of the vascular anastomoses that are invariably present. The most severe imbalance is twin-twin transfusion syndrome, which presents as a significant amniotic fluid difference diagnosed on ultrasound scan, usually before 26 weeks. Another transfusion imbalance is twin anemia polycythemia sequence, which is a severe hemoglobin difference occurring mainly after 26 weeks in previously uncomplicated pregnancies. Fetoscopic laser coagulation of the anastomoses addresses the cause o

Twin-twin transfusion syndrome - a University Hospital experience with intrauterine treatment.

INTRODUCTION: twin-to-twin transfusion syndrome (TTTS), defined by combination of polyhydramnios-oligohydramnios, is the most prevalent (5%-35%) of the abnormalities due to placental vascular anastomoses and the most lethal (80%-100% mortality) if untreated. Fetoscopic laser ablation of abnormal vasculature using the Solomon technique is the gold standard approach. It consists of interrupting the intertwin blood flow. OBJECTIVES: to present our initial experience at the Fetal Surgery Service of the Hospital de Clinicas of the Federal University of Parana (HC-UFPR) and to compare our results w

Survival Outcomes by Fetal Weight Discordance after Laser Surgery for Twin-Twin Transfusion Syndrome Complicated by Donor Fetal Growth Restriction.

INTRODUCTION: Management options for treatment of twin-twin transfusion syndrome (TTTS) with severe donor intrauterine growth restriction (IUGR) include fetoscopic laser surgery and umbilical cord occlusion (UCO). We studied perinatal survival outcomes in this select group after laser surgery, stratifying patients by preoperative estimated fetal weight (EFW) discordance. METHODS: In this retrospective study of monochorionic diamniotic twin gestations with TTTS and selective donor IUGR who underwent laser surgery (2006-2017), preoperative EFW discordance was calculated ([(larger twin - smaller

An injectable, degradable hydrogel plug for tracheal occlusion in congenital diaphragmatic hernia (CDH).

Congenital Diaphragmatic Hernia (CDH) is a birth defect characterized by failed closure of diaphragm, herniation of viscera in the thoracic cavity and impaired lung development. In most severe cases, fetoscopic endoluminal tracheal occlusion (FETO) is performed through a catheter-mounted balloon to contain pulmonary hypoplasia. To support the success rate of FETO and overcome some of the associated drawbacks, this work aimed at finding an injectable, degradable substitute for the balloon not requiring prenatal removal. Two different types of gels were evaluated as tracheal plugs: calcium-algin

Influence of maternal obesity and metabolic and vascular mediators in twin-twin transfusion syndrome.

Obesity is a risk factor for complications in singleton and twin pregnancies; however, there are limited data regarding maternal body mass index (BMI) in the setting of twin-twin transfusion syndrome (TTTS). We hypothesized that increased BMI in TTTS is associated with adverse perinatal outcomes and vascular pathology. A retrospective study of twin reversed arterial perfusion (n = 4), selective intrauterine growth restriction (n = 10) and TTTS (n = 33) was conducted. Treatment included fetoscopic laser photocoagulation (FLP) (n = 35) or Solomon technique (n = 12). Ex vivo placental intravascul
