Combined laparoscopic and endoscopic treatment of perforated gastroduodenal ulcer using the ligamentum teres hepatis (LTH).

We propose a novel technique for laparoscopic treatment of perforated gastroduodenal ulcers. The principle of this procedure involves the closure of the perforated ulcer using the ligamentum teres hepatis (LTH). The LTH is cut near its umbilical end and then dissected up to the site of its hepatic insertion. The umbilical extremity of LTH is grasped with a Dormia noose passed through the ulcerated perforation via a gastroscope. Using the noose, the LTH is pulled through the ulcerated perforation until its volume fits and completely closes the perforation. This laparoscopic technique was perfor

Laser lithotripsy of difficult biliary stones.

From June 1991 to September 1992, 16 patients (mean age, 72 +/- 5 years) were treated with intra-corporeal laser lithotripsy (ICL). Thirteen patients had choledocholithiasis with at least one stone larger than 20 mm; 3 patients had intra-hepatic lithiasis. All other methods, including mechanical lithotripsy, extra-corporeal lithotripsy (1 case), and intra-corporeal electrohydraulic lithotripsy (1 case), had failed to clear the bile ducts. Approaches for ICL were choledochoscopy with a "baby" endoscope via an existing sphincterotomy (8 cases), retrograde cholangioscopy with a gastroscope throug

Kinetics of piretanide absorption from the gastrointestinal tract.

In a recent report, it was shown that the loop diuretic piretanide is rapidly absorbed after placement of a piretanide solution in the duodenum, while the rate of absorption is definitely slower when the drug is instilled into the ascending colon (5). When piretanide is instilled into the stomach, the absorption process does not differ significantly from that after placement in the duodenum, neither with respect to amount nor rate (5). However, it was not clear from this study whether piretanide is directly absorbed from the stomach or rapidly released into the duodenum. In a study with five v

Video. Laparoscopic transgastric access to the common bile duct after Roux-en-Y gastric bypass.

INTRODUCTION: Rapid weight loss after Roux-en-Y gastric bypass (RYGBP) often is associated with gallstones formation, which can lead to cholecystitis and/or choledocholithiasis. Difficult access to the biliary tract is one of the disadvantages after RYGBP. We report a useful technique of laparoscopic transgastric access to the gastric remnant for an endoscopic retrograde cholangiopancreatography (ERCP). CASE REPORT: A 40-year-old woman with a BMI of 48 kg/m(2), was submitted to a laparoscopic RYGBP in December 2003. At that time the abdominal ultrasound was negative for gallbladder lithiasis.

[Dynamic imaging of gastric ulcer healing using the most modern Morph-Software].

The presentation of gastric ulcer healing taken from video endoscopy as a dynamic process could not be realized till now. The documentation of the dynamic healing process shattered either on the patient's compliance or on the inconstancy of the image cut due to wobbling. The replay should be performed as a time lapse whereby the picture disturbances would become an essential part.-Instead of presenting a continuous film, instant takes of ulcer healing were processed. A dynamic effect was produced by computer-assisted production of intermediate pictures. A video was created in which short video

A novel endoscopic visible full-thickness cryoablation device on stomach.

Cryoablation has been used for the treatment of various sorts of solid visceral tumors, but few are reported on gastric tumor via endoscope, in terms of accurate control of ablation site, freezing depth and effective temperature. Thus, we developed a novel device, which could perform accurate cryoablation on the stomach via endoscope. This study aimed to evaluate the efficacy and safety of the device on porcine stomach. Results showed that the novel device could provide direct view of the operation space, allowing accurate and safe ablation of the stomach. Three minutes cryoablation caused a t

Septotomy and Balloon Dilation to Treat Chronic Leak After Sleeve Gastrectomy: Technical Principles.

BACKGROUND: Chronic leaks after laparoscopic sleeve gastrectomy (LSG) are often difficult to treat by endoscopy metallic stent. Septotomy has been indicated as an effective procedure, but the technical aspects have not been detailed in previous publications (Campos JM, Siqueira LT, Ferraz AA, et al., J Am Coll Surg 204(4):711, 2007; Baretta G, Campos J, Correia S, et al., Surg Endosc 29(7):1714-20, 2015; Campos JM, Pereira EF, Evangelista LF, et al., Obes Surg 21(10):1520-9, 2011). Primary gastric tuberculosis mimicking gastric lymphoma: a case report and multimodality imaging findings.

Gastric tuberculosis represents a rare form of extrapulmonary tuberculosis. Owing to its non-specific clinical presentation, the condition is highly prone to misdiagnosis. It can mimic peptic ulcer disease or malignancy, typically manifesting as ulcerative lesions or gastric outlet obstruction, whereas less frequent presentations include massive gastrointestinal haemorrhage and gastric perforation. Consequently, establishing a definitive diagnosis remains challenging. This case report aims to augment the current understanding of this entity. Herein, we describe a 50-year-old female who present

PTCD-Assisted ERCP for Severe Stricture at the Biliary-Enteric Anastomosis following Pancreaticoduodenectomy: A Case Report.

INTRODUCTION: Post-pancreaticoduodenectomy (PD) biliary-enteric anastomosis stenosis (BEAS) poses substantial therapeutic challenges secondary to surgically altered anatomy. Conventional endoscopic interventions are frequently unsuccessful in patients with both altered gastrointestinal anatomy and severe fibrotic strictures. This study reports the utilization of a combined percutaneous transhepatic cholangiodrainage (PTCD) and endoscopic retrograde cholangiopancreatography (ERCP) rendezvous technique, incorporating an innovative mechanical dilation method using a stent retriever, for the manag

EBV-associated gastric carcinoma occurring one year after metabolic bariatric surgery: case report and literature review.

Metabolic bariatric surgery (MBS) does not appear to increase the overall risk of gastric cancer; therefore, there are few documented cases of Epstein-Barr virus-associated gastric carcinoma (EBVaGC) arising early after single anastomosis sleeve ileal bypass (SASI). Here, we report a 62-year-old man who underwent SASI in July 2024 and remained asymptomatic on follow-up. Surveillance endoscopy at 12 months disclosed a 1.2 × 1.0 cm deeply excavated ulcer in the posterior gastric body; biopsy revealed adenocarcinoma with Helicobacter pylori negativity and imaging showed focal wall thickening. Bridging the gap: intraoperative esophageal stent used as a scaffold for the treatment of complete postsurgical gastroesophageal anastomosis dehiscence.

BACKGROUND AND AIMS: Esophagogastric (EG) anastomotic leaks are serious adverse events after esophagectomy. Endoscopic stent placement has become an effective, minimally invasive option that promotes healing. While esophageal stents maintain luminal continuity, technical success rates are as high as 90.9%. A 62-year-old man with esophageal adenocarcinoma presented 12 days after McKeown esophagectomy with dyspnea and empyema. Imaging and chest tube output suggested an anastomotic leak. The aim of this study was to display the utility of esophageal stent placement even in cases of complete EG de

Risk of gastrojejunal anastomotic stricture following hand-sewn robotic Roux-en-Y gastric bypass: a retrospective cohort study.

Gastrojejunal (GJ) stricture is one of the most common complications following Roux-en-Y gastric bypass with a hand-sewn anastomosis. This study aimed to evaluate the risk of GJ strictures in a series of robotic Roux-en-Y gastric bypasses (RRYGBP) performed by a single surgeon. This retrospective observational study included 314 consecutive patients with severe obesity who underwent RRYGBP with a 3-layer continuous hand-sewn GJ anastomosis using absorbable sutures between September 2017 and September 2024. All patients were followed up at 6-month intervals after the first month post-surgery. T

Clinical experience sharing on gastric microneuroendocrine tumors: A case report.

BACKGROUND: The majority of gastric neuroendocrine tumors (G-NENs) are present in various lesions under endoscopy, and they can be polypoid uplifts, submucosal tumors or papules, erosions, and ulcers. The lesions are mostly confined to the mucosal or submucosal layer, usually less than 2 cm, and exclusively localized to the gastric body or fundus. In type 1 G-NENs, about 22% of cases have no visible lesions under an endoscope, and such lesions can only be detected via biopsies (microcarcinoids). CASE SUMMARY: A 67-year-old female patient with appetite loss for more than half a year and person

A unique tripartite collision tumor of the esophagus: a case report and literature review.

BACKGROUND: The coexistence of two or more distinct neoplasms within the same anatomical site characterizes collision tumors. While the presence of dual tumors is frequently observed in esophageal cases, the simultaneous occurrence of three distinct tumor types is extremely rare, posing significant challenges for pathological evaluation and diagnosis. Surgical resection remains the primary treatment, with generally favorable outcomes.
