Mucoepidermoid carcinoma of the parotid gland with invasion of the jugular foramen region: A case report.

RATIONALE: Mucoepidermoid carcinoma of the parotid gland, being the most prevalent malignant neoplasm among salivary gland tumors, represents the leading incidence rate among primary parotid malignancies. This pathological entity predominantly affects young and middle-aged females, typically presenting as a painless parotid mass. Due to its variable disease progression and nonspecific clinical manifestations, preoperative misdiagnosis as a benign tumor is frequently encountered. However, when parotid mucoepidermoid carcinoma progresses to involve the jugular foramen region, a complex anatomica

[Classification of ethmoid malignancies].

The UICC and AJCC never classified ethmoid malignancies prior to the latest edition (1997). Most classifications in the literature refer to a single histological type (estensioneuroblastoma or carcinoma) while others basically consider the intracranial extension, without distinguishing between intra or extradural. Still others consider invasion of the orbit. There is as yet no classification which considers all the prognostic factors associated with the extension of this neoplasm. The authors reviewed 84 patients with ethmoid malignancy who had undergone anterior cranio-facial resection betwee

Long-term oncological results in 47 cases of jugular paraganglioma surgery with special emphasis on the facial nerve issue.

Oncological and functional results were assessed in 47 type C and/or D jugular paraganglioma operated on between 1984 and 1998 using the classical infratemporal fossa type A approach (mean follow-up = 66 months). In 24 instances, however, the facial nerve was not re-routed. Total resection was achieved in 33 cases (70 per cent). In 25 patients available for follow-up this resulted in a 92 per cent cure rate while two patients (eight per cent) developed recurrences that are being followed-up clinically and radiologically. Sub-total resection, leaving infracentimetric tumour remnants after being

Rhabdomyosarcoma of Parameningeal Sites.

Rhabdomyosarcoma arising in the skull base has a feature of its natural history that is unique in oncology: by virtue of parameningeal location and invasive behavior, it can extend intracranially and produce neoplastic meningitis. The four anatomic sites with this potential are the nasopharynx/nasal cavity, the middle ear, the paranasal sinuses, and the infratemporal fossa/pterygopalatine space. Patients with these primary sites comprise 41% of patients with head and neck sites and 15% of all patients with rhabdomyosarcoma. Most are under 10 years old at diagnosis (72%) and are Intergroup Rhab

Operative exposure and management of the petrous and upper cervical internal carotid artery.

The exposure and operative management of the petrous and upper cervical internal carotid artery (ICA) in 29 patients is detailed. Twenty-seven of these patients had extensive cranial base neoplasms (benign or malignant), 1 had an inflammatory cholesteatoma, and 1 had an aneurysm of the upper cervical ICA immediately proximal to the carotid canal. Preoperative studies useful in the evaluation of these patients included computed tomography, magnetic resonance imaging, cerebral and cervical angiography, and a balloon occlusion test of the ICA with evaluation of neurological status and of cerebral

Preservation of continence in radical prostatectomy patients: a laparoscopic surgeon's perspective.

BACKGROUND: The knowledge of sinonasal vasculature is inevitable in transnasal neurosurgery. We performed an anatomical study on the sphenopalatine artery from the perspective of skull base procedures. METHODOLOGY: To analyse the anatomical landmarks of the sphenopalatine artery, arterial skull corrosion casts (26 head halves) underwent endoscopic transnasal phantom surgery. Furthermore, we performed microsurgical dissection on formaldehyde-fixated cadavers with arterial perfusion (14 head halves) as well as studied Cone Beam CT-scans of anonymised patients and cadavers (115 head sides). Technique for reconstruction of large clival defects through an endoscopic-assisted tunneled retropharyngeal approach.

BACKGROUND: Reconstruction of the clivus and posterior cranial base defects following endoscopic skull-base surgery can be particularly challenging. Commonly, defects in this region are repaired with pedicled mucoperichondrial flaps from the sinonasal cavity. Complex and large defects often require regional or free flaps, particularly when intranasal flaps have been exhausted. While there are no primary barriers to routing flaps into the clivus or nasopharynx in large open approaches, secondary surgical corridors are necessary during endoscopic cases for routing of free flaps. Routing of free-

First bite syndrome: incidence, risk factors, treatment, and outcomes.

OBJECTIVES/HYPOTHESIS: First bite syndrome (FBS) refers to facial pain characterized by a severe cramping or spasm in the parotid region with the first bite of each meal that diminishes over the next several bites.1, 2 It is a potential sequela of surgery involving the infratemporal fossa (ITF), parapharyngeal space (PPS), and/or deep lobe of the parotid gland. The incidence, risk factors, treatment options, and outcomes of FBS are poorly understood. We hypothesized that certain clinical and tumor variables independently predict the development of FBS. STUDY DESIGN: Retrospective cohort study

Craniofacial resection for paranasal sinus cancers.

BACKGROUND: Combined anterior craniofacial resection (CFR) has been in use for more than 25 years. The advent of the free revascularized tissue transfer flap in l980 permitted safe resection of tumors that had spread beyond the confines of the paranasal sinuses with immediate reconstruction of the sino-orbital cranial defect. The purpose of this study was to examine the outcomes and morbidity of a management policy of primary CFR and postoperative radiotherapy for paranasal sinus cancers infiltrating the skull base over a 21-year period. METHODS: Seventy-three patients with paranasal sinus ca
