Anterior femoral epiphysiodesis for the treatment of fixed knee flexion deformity in spina bifida patients.

BACKGROUND: Fixed knee flexion deformity is a common problem in spina bifida patients. Owing to interference with ambulation, this deformity may lead to anterior knee pain and progressive crouch gait. If conservative treatment fails, surgical procedures including supracondylar femoral extension osteotomy, joint distraction, and posterior capsulotomy have to be considered. Potential complications of these procedures are fractures, neurovascular lesions, knee instability, and recurrent deformity with continued growth. As fixed knee flexion deformity in spina bifida patients frequently occurs bil

Image Contrast and Spectral Transmission in Intraocular Lenses with Nd:YAG Pits.

PURPOSE: Neodymium yttrium aluminum garnet (Nd:YAG) laser capsulotomy is considered as safe and effective method in the treatment of posterior capsule opacification. Nevertheless, side effects are described. The incorrectly adjusted focus of the laser beam during the procedure can lead to so-called YAG-pits or YAG-shots. In this experimental study, we measured spectral transmission to evaluate the image contrast and analyze the impact of YAG-pits in intraocular lenses (IOL). METHODS: Acrylic, foldable, one-piece IOLs with 6.0 mm optic and different material properties were studied. These incl

Anatomical retro-apical technique of synchronous (posterior and anterior) urethral transection: a novel approach for ameliorating apical margin positivity during robotic radical prostatectomy.

OBJECTIVE: To describe a novel synchronous approach to apical dissection during robotic-assisted radical prostatectomy (RARP) which augments circumferential visual appreciation of the prostatic apex and membranous urethra anatomy, and assess its effect on apical margin positivity. PATIENTS AND METHODS: Positive surgical margins (PSM) during RP predispose to earlier biochemical recurrence, and occur most frequently at the prostatic apex. Conventional apical transection after early ligation of the dorsal venous complex (DVC) remains suboptimal, as this approach obscures visualization of the int

Results following secondary intraocular lens implantation in children.

BACKGROUND: Placement of a secondary intraocular lens (IOL) in a child may be considered in children with congenital monocular cataracts who have had complete opacities removed early in life and who later become contact lens intolerant, in eyes that have received trauma which precluded placement of a primary IOL, and in young adults who have bilateral aphakia who become resistant to use of contact lenses or spectacles. METHODS: Clinical records of all children in our practice who received a secondary IOL between January 1988 and December 1994 were reviewed. Indications, biometry, type of proc

Global challenges for lens and cataract research.

The lens is sadly too often dismissed as a less important tissue within the eye, but this neglects to appreciate the role it plays in the wellbeing and quality of life of individuals, the economic burden lens pathologies place on society and the true biological wonder it is! A normal healthy lens in isolation, looks like a jewel, formed from biological glass. The lens is made of cells enveloped in a soft collagen shell. No blood supply, no nerves. There is slow cell growth that occurs progressively over a lifetime and little cell death takes place. Furthermore, the lens is not a static system

Open Capsulorrhaphy With Capsular Shift and Open Coracohumeral Ligament Reconstruction for Chronic Recurrent Multidirectional Shoulder Instability.

BACKGROUND: This case features the surgical reconstruction of the coracohumeral ligament (CHL) in conjunction with open capsulorrhaphy and capsular shift for the management of chronic, recurrent, multidirectional shoulder instability (MDI). While there is a paucity of existing literature examining the clinical outcomes of this technique, biomechanical studies have implied its potential to restore stability. This procedure may be of particular benefit to patients who have failed previous conservative management and/or attempts at surgical stabilization, as well as those with severe joint hyperl

Quadriceps Coxae-Sparing Modified Posterior Approach to the Hip Joint for Hemiarthroplasty.

BACKGROUND: Various approaches have been described for hip arthroplasty1-3. The posterior approach to the hip remains a popular choice for hemiarthroplasty1. In the classic description, it involves detachment of the short external rotators, which include the quadriceps coxae (QC) (i.e., piriformis, superior gemellus, obturator internus, and inferior gemellus) along with the obturator externus and quadratus femoris (as needed). Since the QC are important for joint stability, detachment during the approach is associated with higher rates of postoperative dislocations4, even following subsequent

Open Bankart Repair with Subscapularis Split.

BACKGROUND: Anterior shoulder dislocations are a common injury, especially in the young, active, male population1. Soft-tissue treatment options for shoulder instability include arthroscopic or open Bankart repair, with open Bankart repair historically having lower rates of recurrence and reoperation, faster return to activity2-4, and a similar quality of life compared with arthroscopic repair5. More recent literature has suggested similar recurrence rates between arthroscopic and open procedures6. However, open Bankart repair may be indicated in cases of recurrent instability, especially if t

Pseudophakic Dysphotopsia: Review of Incidence, Cause, and Treatment of Positive and Negative Dysphotopsia.

We reviewed the literature concerning positive dysphotopsia (PD) and negative dysphotopsia (ND) regarding cause, incidence, and clinical and surgical management. In addition, we summarized our surgical experience in managing dysphotopsia. A PubMed review, limited to English language articles, yielded 149 citations; multifocal (diffractive optic) and phakic intraocular lens (IOL) dysphotopsia were excluded. Overall, 39 articles were determined to be relevant for the objectives of this investigation. Regarding PD, 7 articles corroborated that the cause of PD is related primarily to internal refl
