OBJECTIVES: Crystal deposits in ligaments of the wrist are typical findings in patients with calcium pyrophosphate deposition (CPPD) disease. CPPD crystals trigger inflammation and ultimately result in ligament tears with scapholunate (SL) advanced collapse (SLAC). This study aimed to investigate carpal instabilities in patients with CPPD using four-dimensional computed tomography (4D-CT) of the wrist.
METHODS: This IRB-approved prospective feasibility study investigated patients with CPPD of the hand. All patients underwent a static 3D-CT and two dynamic 4D-CT in ulnar- and radial abduction

Malunited distal radius fractures (DRFs) occasionally restrict forearm rotation, but the underlying pathology remains unclear. We aimed to elucidate the mechanism of rotational restriction by retrospective analysis of 23 patients with unilateral malunited DRFs who presented restricted forearm rotation. We conducted computed tomography during forearm rotation on both sides. Three-dimensional (3D) bone surface models of the forearm were created, and 3D deformity of the distal radius, translation of the distal radius relative to the ulna, distal radioulnar joint (DRUJ) contact area, and estimated

Background Frequently, patients undergo repeated wrist arthroscopies for single wrist problems. Purpose The purposes of this study were to assess the indications for repeat wrist arthroscopies and to identify potentially preventable procedures. Methods For this retrospective, two-center study, the electronic patient records were examined for patients, who underwent repeat wrist arthroscopy in a 5-year period. The cases were sorted by the underlying pathologies and the causes that necessitated repeat arthroscopies. Results Ulnar-sided wrist pain accounted for 100 (77%) of all 133 revision arthr

PURPOSE: To determine the accuracy of computed tomography after wrist arthrogram (CT arthrogram) compared with findings during wrist arthroscopy as interpreted by a musculoskeletal radiologist and a board-certified hand surgeon.
METHODS: Seventy-six consecutive patients who had a wrist CT arthrogram and subsequent wrist arthroscopy were identified in the arthroscopic database of four hand surgeons working at a single institution between 2001 and 2006. Operative findings were obtained from the medical record. A single, blinded, musculoskeletal fellowship-trained radiologist prospectively read

BACKGROUND: TDetection of distal radioulnar joint (DRUJ) instability has proven inconsistent despite numerous examination maneuvers. Computed tomography (CT) has been suggested as a modality for evaluating DRUJ instability; however, without stress across the DRUJ, it fails to reliably identify this. No study has simultaneously assessed stress CT with clinical stress maneuvers. As such, the purpose of this study was to compare both methods in stable and unstable wrists.
METHODS: An arthrometer was developed to evaluate a clinical stress test of the DRUJ in various degrees of forearm rotation.

BACKGROUND Hauck type II ulnar styloid fractures (USFs) occur at the base of the ulnar styloid process, where the triangular fibrocartilage complex attaches. Nonunion or disruption at this level may compromise distal radioulnar joint (DRUJ) stability, although the clinical significance of associated USFs in distal radius fractures (DRFs) remains debated. This retrospective cohort study compared outcomes after mini-anchor fixation vs no ulnar styloid fixation in patients with DRFs and concomitant Hauck type II USFs. MATERIAL AND METHODS Forty-five patients treated between 2021 and 2023 were inc

BACKGROUND: Triangular fibrocartilage complex (TFCC) injuries can cause significant patient dysfunction. Platelet-rich plasma (PRP) has emerged as a potential adjunctive treatment for arthroscopic TFCC repair, with some studies suggesting improved outcomes. This study aims to evaluate and compare PRP as an adjunctive treatment in arthroscopic TFCC tear repair.
METHODS: This retrospective cohort study examined patients undergoing arthroscopic TFCC repair at Institut de la Main, Paris, France (December 2021-2022). Patients were split into two groups: arthroscopic repair alone (1) and repair wit

PURPOSE: To identify factors influencing overall radiologic changes in the distal radioulnar joint after ulnar shortening osteotomy (USO) in patients with idiopathic ulnar impaction syndrome.
MATERIALS AND METHODS: A total of 310 patients who had undergone USO between May 2007 and December 2013 were included in this retrospective case-control study. The patients were classified into two groups (with or without radiologic findings of new arthritic changes), after which the following factors were analyzed: 1) demographic factors; 2) radiologic aspects, including ulnar variance (UV), morphologic

PURPOSE: The management of distal radius fractures differs based on the nature of the fracture and the experience of the surgeon. We hypothesized that patients requiring surgical intervention would undergo different procedures when in the care of a surgeon with subspecialty training in hand surgery as compared with surgeons with no subspecialty training in hand surgery.
METHODS: We queried the ABOS database for case log information submitted for part II of the ABOS examination. Queries for all codes involved with distal radius fracture management were combined with associated codes for the ma

PURPOSE: To present a new arthroscopic method for treating supination losses.
METHODS: Six patients (15-71 y) were eligible for this study. All had a history of trauma to the wrist more than 6 months previously. Five of them had sustained a distal radius fracture: 3 had been treated with a volar plate (1 of them for an extra-articular malunion), 1 with an external fixator and K-wires, and 1 had been treated in a cast. One of these patients underwent a further operation for correcting an intra-articular malunion. The last patient underwent an open reduction of a transscaphoid perilunate disloc

BACKGROUND: Stabilization of the distal radioulnar joint (DRUJ) with reconstruction of distal radioulnar ligaments as outlined by Adams is indicated if a refixation of the triangular fibrocartilage complex (TFCC) is no longer possible. There is little information given on the results of these procedures in literature.
PURPOSE: The purpose of this retrospective study was to analyze with use of objective and subjective criteria, whether the reconstruction of the radioulnar ligaments according to Adams allows a sufficient re-stabilization of the DRUJ.
PATIENTS AND METHODS: 14 patients (9 female

Purpose Management of acute distal radioulnar joint (DRUJ) instability is complex and controversial. Common treatment options include prolonged immobilization, stabilization with wires, and acute triangular fibrocartilage complex repair. However, none of these permits an early range of motion. The purpose of this study is to investigate the feasibility of a suture-based stabilization (SBS) technique for acute DRUJ instability to permit early active motion. Materials and Methods A biomechanical study utilizing eight cadaveric arms was performed. All specimens were tested in the intact state p

Background Triangular fibrocartilage complex (TFCC) injury often results in distal radioulnar joint (DRUJ) instability. However, not all patients with a ruptured TFCC have an unstable DRUJ as in these patients a distal oblique bundle (DOB) may be present. We assumed that augmentation of the DOB leads to a more stable situation following reinsertion of the TFCC. We present the clinical results of a new surgical technique using the TightRope system as a DOB augmentation. Description of Technique All cases were treated under regional anesthesia with the TightRope implant for which a tunnel was
