Outbreak of Cryptosporidium parvum among children after a school excursion to an adventure farm, south west England.

We present the results of an outbreak investigation on a Cryptosporidium parvum outbreak among 35 people, (27 students and 8 teachers), who participated in a school excursion to an outdoor adventure farm in South West England, from 22 to 26 May 2006. A cohort study was implemented to investigate possible sources of infection during the farm visit. The most likely transmission route was contact with faecally contaminated surface water following heavy rainfall, or consumption of water from the private well. Disinfection of the water reservoir was by chlorination, to which cryptosporidium is resi

Influence of epidemic phase on the cost effectiveness of a prevention intervention for sexually transmitted infection: an exploratory analysis.

OBJECTIVES: To explore how the cost effectiveness of a behaviour-change prevention programme for sexually transmitted infection (STI) varies with the phase of an STI epidemic. METHODS: A model of STI transmission and standard methods of cost-effectiveness analysis was used to examine the cost effectiveness of a hypothetical, behaviour-change intervention initiated at various phases of an STI epidemic. RESULTS: The intervention was more cost effective when initiated in earlier phases of the epidemic rather than later phases, under a range of scenarios. However, the relative impact of the timi

Ultrasound evaluation of diaphragmatic mobility in different postures in healthy subjects.

OBJECTIVE: To assess, using ultrasound, the effects that changes in body position have on diaphragmatic mobility in healthy subjects during spontaneous breathing. METHODS: The study involved seven healthy female volunteers, all of whom were nonsmokers, well nourished, and free of any cardiopulmonary disease. They were submitted to pulmonary function testing and ultrasound evaluation of the mobility of the right diaphragm by the craniocaudal displacement of the left branch of the portal vein using an ultrasound device in mode B. The mobility of the right diaphragm was evaluated in right decubi

Dynamics of dot-like hemosiderin spots on T2*-weighted MRIs associated with stroke recurrence.

BACKGROUND AND PURPOSE: Dot-like low-intensity spots (dot-like hemosiderin spots: dotHSs) on gradient echo T2*-weighted (-w) brain magnetic resonance imaging (MRIs) are frequently associated with cerebral small vessel disease (SVD), including deep intracerebral hemorrhages and lacunar infarctions. This study investigated how numbers of newly appeared dotHSs contribute to recurrent SVD. METHODS: We prospectively analyzed numbers of newly appeared dotHSs in 12 patients with prior SVD (8 males, 4 females; mean 67.6 +/- 10.7 years old) readmitted with recurring SVD between October 2001 and March

[Study of a role of artificial mycobacterial particles in the realization of immunological processes in experimental animal tuberculosis].

The protective properties of artificial mycobacterial particles versus BCG vaccine were studied in laboratory animals with experimental tuberculosis. The findings of the decreased rate of a tuberculous process and on the increased mean life span in animals inoculated with M. bovis suggest that immunization of guinea-pigs with mycobacterial particles promotes the enhanced development of antituberculous immunity in the animals. The paper proposes a promising method for designing artificial immunogens, the high-polymer antigenic structures that imitates mycobacterial particles.

Performance assessment for radiologists interpreting screening mammography.

When interpreting screening mammograms radiologists decide whether suspicious abnormalities exist that warrant the recall of the patient for further testing. Previous work has found significant differences in interpretation among radiologists; their false-positive and false-negative rates have been shown to vary widely. Performance assessments of individual radiologists have been mandated by the U.S. government, but concern exists about the adequacy of current assessment techniques. We use hierarchical modelling techniques to infer about interpretive performance of individual radiologists in s

Subtypes of frontonasal dysplasia are useful in determining clinical prognosis.

Frontonasal dysplasia [FND; OMIM 136760] results from abnormal morphogenesis of the frontonasal process with disruption of the formation of the facial midline. Craniofacial anomalies in FND include anterior cranium bifidum, ocular hypertelorism, orofacial clefting and notching or clefting of the alae nasi. The majority of FND cases are sporadic and discordance has been demonstrated in monozygotic twin pairs, arguing against a strong inherited component in pathogenesis. However, pedigrees with Mendelian inheritance and non-recurrent chromosome aberrations in FND patients demonstrate that gene m

Dim light adaptation attenuates acute melatonin suppression in humans.

Abstract Studies in rodents with retinal degeneration indicated that neither the rod nor the cone photoreceptors obligatorily participate in circadian responses to light, including melatonin suppression and photoperiodic response. Yet there is a residual phase-shifting response in melanopsin knockout mice, which suggests an alternate or redundant means for light input to the SCN of the hypothalamus. The findings of Aggelopoulos and Meissl suggest a complex, dynamic interrelationship between the classic visual photoreceptors and SCN cell sensitivity to light stimuli, relative to various adaptiv

Important experimental parameters for determining infection rates in arthropod vectors using pool screening approaches.

Measuring transmission of a vector-borne infection is essential to understanding infection dynamics. When infection prevalence in the vector population is low, transmission is often measured by pool screening (also referred to as group testing). Several investigators have developed statistical methods to recover infection prevalence estimates from pool screen data. These are based on models that contain certain assumptions, and a pool screening approach must be designed to take these into account if accurate estimates of infection prevalence are to be obtained. Here we describe these assumptio

[Calculation of a risk for death due to tuberculosis in case of the Sverdlovsk Region].

Many factors influence the risk of death in patients with tuberculosis. The most significant factors are a clinical form of the disease, combinations, comorbidity, age, and the duration of death. The risk of death averaged 4.2% in patients with new-onset tuberculosis, 30.4% in those with recurrent tuberculosis, and 6.4% in the registered. In patients with new-onset tuberculosis, the highest risk of death was established in tuberculous meningitis, miliary tuberculosis, fibrocavernous tuberculosis, and complications of tuberculosis (such as hemorrhage, spontaneous pneumothorax) increased the ris

Local and systemic hemorheological effects of cerebral hyper- and hypoperfusion in a porcine model.

Using a well defined pig model, we investigated whether cerebral hypertension and hypotension influence hemorheological factors. After surgical preparation and stabilization, periods of hyperventilation, controlled periods of cerebral perfusion pressure increases and decreases were utilized. After each period, blood samples were collected from the cannulated femoral artery and vein, and from the superior sagittal sinus. Erythrocyte deformability, whole blood and plasma viscosity and hematological parameters were determined. Erythrocyte deformability significantly worsened in arterial samples a

Review of current actuator suitability for use in medical implants.

This paper presents an initial formal review of the suitability of currently available actuation technologies for use in fully implantable medical devices, with a focus on applications requiring linear motion. Examples of such applications are a mechatronic hydrocephalus shunt and implantable insulin pumps. Some general basic requirements for fully implantable devices are discussed, followed by an overview of potential actuators. Possible design concepts are presented for electromagnetic and shape memory technologies, including a comparison of their respective pros and cons. Methods of modelin

High tibial osteotomy for osteoarthritis of the knee with varus deformity utilizing the hemicallotasis method.

INTRODUCTION: A hemicallotasis method has been developed utilizing an external fixator as high tibial osteotomy (HTO), and satisfactory results of this method with the external fixator have been reported. This external fixator has a universal joint that moves in all directions. We have recently designed a hemicallotasis device for this operation. METHODS: HTO for the knee with varus deformity utilizing the hemicallotasis method was performed on 44 knees. The patients had a mean age at operation of 65 years (range 49-82 years), a mean follow-up period of 68 months (range 36-119 months), and a

Spread of equine West Nile virus encephalomyelitis during the 2002 Texas epidemic.

Using reports of clinical West Nile virus (WNV) encephalomyelitis in Texas equids during 2002, the distribution of disease was analyzed using cluster statistics and spatial modeling to develop hypotheses of disease spread during the first year of its detection. Significant (P < 0.05) clusters of cases reported early during the outbreak were identified in east, northcentral, and north Texas, and significant (P < 0.05) clusters late during the outbreak were detected in central, south, and west Texas. Two counties on the south Texas coast first reported disease significantly (P < 0.05) earlier th

Neurobiology and etiology of panic disorder.

Panic disorder entered the psychiatric nomenclature a quarter-century ago, and an explosion of studies followed. Defining the core phenomenology of panic disorder can be advanced by an understanding of its pathophysiology and exploration of its etiology. The lessons learned can guide the delivery of treatments to enhance the likelihood of achieving remission and the discovery of novel treatments for panic disorder.

Relevance of an evidence-based ejaculation time cutoff point for neurobiological research of premature ejaculation.

The Diagnostic and Statistical Manual (DSM)-IV definition of premature ejaculation is not based on evidence-based studies. In particular, the absence of a well-defined quantitative measure of the intravaginal ejaculation latency time (IELT) makes the DSM definition inadequate. Therefore, the DSM-IV definition should be replaced by a medical definition that incorporates both quantitative as qualitative parameters of premature ejaculation. An evidence-based medical definition should include a cutoff point of the IELT at the 0.5 and 2.5 percentiles of the IELT distribution in the general male pop

The effect of censoring on cancer risk estimates based on the Canadian National Dose Registry of occupational radiation exposure.

Cohort studies represent an important epidemiological tool for exploring the potential adverse health effects of low-dose exposure to ionizing radiation in the workplace. Analyses of data from the National Dose Registry of Canada have suggested that occupational radiation exposure leads to increased risk of several specific types of cancer, as well as increased overall risk of cancer. An important aspect of such studies is the censoring in recorded exposures induced by dosimetry detection limits. Such a censoring effect can lead to significant underestimation of cumulative doses which, in turn
