Posterior fossa abnormalities in children with infantile spasms.

In order to explore possible pathophysiologic involvement of the brain stem in infantile spasms, we retrospectively compared clinical and electroencephalographic (EEG) features of 14 children with infantile spasms who had gross posterior fossa abnormalities on neuroimaging studies with 84 children with infantile spasms who had either normal neuroimaging (n = 19) or supratentorial abnormalities (n = 65). Children with posterior fossa abnormalities how lower mean initial and follow-up developmental quotients compared to those with normal imaging or supratentorial abnormalities alone. Age of onse

Difficulties in diagnosing congenital posterior fossa fluid collections after shunting procedures.

The differential diagnosis of retrocerebellar fluid collections in infants is important because of the prognostic implications. Usually the diagnoses are easy; however, shunting of the lateral ventricles or of the fluid collections may alter the appearance of the lesions, precluding accurate diagnosis. Under such circumstances a careful study of all the sequential radiologic studies is necessary.

The "keyhole": a sign of herniation of a trapped fourth ventricle and other posterior fossa cysts.

When a cystic structure in the posterior fossa increases in size, the accompanying increase in pressure may cause it to herniate upward through the tentorial hiatus. In our experience this happens most commonly with a dilated trapped fourth ventricle secondary to infection or intraventricular hemorrhage. However, herniation of an arachnoid cyst or a Dandy-Walker malformation through the tentorium may also occur. When herniation occurs, the cystic structure assumes a "keyhole" configuration, indicating that it is trapped and that surgical intervention is necessary. Five cases are presented that

Re-evaluation of the approach to an enlarging head in infancy.

Computerized axial tomography is a safe, non-invasive technique for measurement of ventricular size. A review of 44 children without myelodysplasia who were evaluated for inappropriately enlarging heads, documented a group of 20 children presenting at five months to two years of age with megalencephaly or mild communicating hydrocephalus. Of these, only two required shunts. At their most recent examination, the great majority of these children are developmentally normal. Serial neurological examination, developmental evaluation and head-circumference measurements, in association with computeri

Open fourth ventricle prior to 20 weeks' gestation: a benign finding?

OBJECTIVE: To evaluate the role of the brainstem-vermis (BV) angle in the diagnosis of fetal posterior fossa abnormalities at 15-18 weeks' gestation. METHODS: We examined retrospectively three-dimensional (3D) ultrasound volumes acquired at 15-18 gestational weeks in fetuses with normal posterior fossa (controls) and in those with cystic posterior fossa. Whether the fourth ventricle appeared open posteriorly in axial views was noted and the BV angle was measured. A detailed follow-up was obtained in all cases. RESULTS: Of the 139 controls, 46 cases were excluded because of inadequate quality

Unbalanced translocation 6p/16q (partial monosomy 6p and trisomy 16q): prenatal diagnosis and cytogenetics.

UNLABELLED: Unbalanced translocation 6p/16q in one fetus is a very rare event and the prenatal sonographic findings have never been published before. We will give a short overview of the literature along with a case report focussing on prenatal ultrasound features and molecular cytogenetic analysis. CASE DESCRIPTION: A 21-year-old primigravid woman presented with a singleton pregnancy at 19 weeks' gestation. The fetus revealed a mild hydrocephalus, a ventricular septal defect (VSD), a Dandy-Walker malformation as well as an intrauterine growth retardation (IUGR) and limb anomalities. MLPA ana

Endoscopic third ventriculostomy: 5 years' experience at the All India Institute of Medical Sciences.

Endoscopic third ventriculostomy (ETV) is one of the efficacious surgical options available for obstructive hydrocephalus, and is preferable to a ventriculoperitoneal shunt in those eligible. We retrospectively studied 115 cases, who underwent ETV at our institute over the last 5 years. Thirty-eight patients were infants. Major indications for ETV were aqueductal stenosis (n = 60/115, 52.2%), Dandy-Walker malformation (15/115, 13%), associated arachnoid cyst (n = 13/115, 11.3%) and post-meningitic hydrocephalus (n = 12/115, 10.4%). Average duration of the follow-up was 10.6 months. Ninety-nine

Revised classification of posterior fossa cysts and cystlike malformations based on the results of multiplanar MR imaging.

MR and clinical data on 31 patients with posterior fossa CSF collections were analyzed. A clear separation of these patients into classical categories was not possible because of new information obtained from the MR images. We present a new classification of these disorders. The Dandy-Walker malformation, Dandy-Walker variant, and mega-cisterna magna seem to represent a continuum of developmental anomalies of the posterior fossa. A possible embryologic basis for this continuum is suggested. Discrete posterior fossa CSF collections that are clearly separate from the fourth ventricle and vallecu

[Value of brain sonography in newborns and children (author's transl)].

Intracranial changes of newborns and young children can easily be discovered by modern sonography, especially as long as fontanelles are still open. Sonography of brain first should be done before computed tomography, encephalography and/or ventriculography are performed. Correspondence of most sonographic findings and computed tomography by other authors, is supporting reduction of CT-investigations in favour of sonography. We cannot agree with the idea that computed tomography should be the most important first investigation. Technical reasons as thick skull or closed fontanelles are making

Cerebellar malformations: some pathogenetic considerations.

1) Destructive processes are responsible for most cases of cerebellar microgyria of the trabecular pattern. Erosion and subsequent fusion of the folia produce the disorganized pattern in which the various cellular elements retain their noraml relationship and are capable of normal maturation. Intrauterine infection is responsible for most cases; the evidence is conclusive in some cases, presumptive in others. 2) Faulty genetic coding, as illustrated by the trisomies, may lead to formation of heterotopias. The primitive cells aggregating around the dentate nucleus should be interpreted as matri

NID1 variant associated with occipital cephaloceles in a family expressing a spectrum of phenotypes.

Autosomal dominant Dandy-Walker malformation and occipital cephalocele (ADDWOC) is a rare, congenital, and incompletely penetrant malformation that is considered to be part of the Dandy-Walker spectrum of disorders. Affected individuals often present with an occipital cephalocele with a bony skull defect, but typically have normal neurological development. Here, we report on a three-generation family in which individuals have variable phenotypes that are consistent with the ADDWOC spectrum: arachnoid cysts in the proband and his maternal grandfather, an occipital cephalocele in the proband and

Ultrasound diagnosis of central nervous system anomalies (bifid choroid plexus, ventriculomegaly, Dandy-Walker malformation) associated with multicystic dysplastic kidney disease in a trisomy 9 fetus: case report with literature review.

Trisomy 9 is a lethal chromosomal abnormality that rarely progresses beyond the second trimester of pregnancy. Multiple central nervous system anomalies, including bifid choroid plexus, ventriculomegaly, and Dandy-Walker malformation, associated with multicystic dysplastic kidney disease in a trisomy 9 fetus are reported. The prenatal ultrasound diagnosis has been aided by novel three-dimensional ultrasound software.

Homozygous mutations in the plasminogen gene of two unrelated girls with ligneous conjunctivitis.

Ligneous conjunctivitis is a rare and unusual form of chronic pseudomembranous conjunctivitis that usually starts in early infancy. The disease may be associated with pseudomembranous lesions of other mucous membranes in the mouth, nasopharynx, trachea, and female genital tract. We examined two unrelated Turkish girls both suffering from ligneous conjunctivitis and occlusive hydrocephalus. Both children exhibited a severe plasminogen deficiency. Genomic DNA from both patients as well as from clinically healthy family members were screened for mutations in the plasminogen gene by polymerase cha

Maternal Fish Intake in the Year Prior to Conception and Birth Defects, National Birth Defects Prevention Study, 1997-2011.

BACKGROUND: Epidemiologic data on the association between maternal fish intake and birth defects are sparse. Our objective was to assess associations between maternal fish intake and 52 different birth defects, most of which have not been assessed previously. METHODS: Using logistic regression, data was analyzed for 29,242 mothers of infants with birth defects and 10,973 mothers of control infants who participated in the National Birth Defects Prevention Study (NBDPS) and delivered between 1997 and 2011. We focused on associations between mothers who reported high fish intake of 2 or more ser

A rare mutant of OFD1 gene responsible for Joubert syndrome with significant phenotype variation.

Joubert syndrome (JBTS), a rare genetic disorder resulted from primary cilium defects or basal-body dysfunction, is characterized by agenesis of cerebellar vermis and abnormal brain stem. Both genotypes and phenotypes of JBTS are highly heterogeneous. The identification of pathogenic gene variation is essential for making a definite diagnosis on JBTS. Here, we found that hypoplasia of cerebellar vermis occurred in three male members in a Chinese family. Then, we performed whole exome sequencing to identify a novel missense mutation c.599T > C (p. L200P) in the OFD1 gene which is the candidate

Atypical case of Smith-Lemli-Opitz syndrome: implications for diagnosis.

Smith-Lemli-Opitz (SLO) syndrome is an autosomal recessive disorder comprised of recognizable facial abnormalities, growth retardation, and multiple congenital anomalies, commonly involving genitalia, second and third toe syndactyly, and cleft palate. The condition is associated with hypocholesterolemia and elevated levels of 7-dehydrocholesterol (7DHC) resulting from deficient activity of the enzyme 7-dehydrocholesterol reductase. The clinical spectrum of SLO ranges from individuals with mental retardation and minor anomalies to those with major structural defects and early or even prenatal l
