Baseline and Early On-treatment Circulating Tumour DNA Fraction Are a Key Prognostic Biomarker in Metastatic Castration-resistant Prostate Cancer Treated with [177Lu]Lu-PSMA-617.

BACKGROUND AND OBJECTIVE: The prostate-specific membrane antigen (PSMA)-targeted radioligand [177Lu]Lu-PSMA-617 is an approved treatment for metastatic castration-resistant prostate cancer (mCRPC). However, identification of genomic biomarkers associated with treatment efficacy remains a critical unmet need. METHODS: We used a customized 78-gene panel to analyse circulating tumour DNA (ctDNA) from 150 patients with mCRPC included in a prospective [177Lu]Lu-PSMA-617 registry. KEY FINDINGS AND LIMITATIONS: Undetectable ctDNA was a significant and positive prognostic biomarker at both baseline

Outcome Prediction After [177Lu]Lu-PSMA I&T in Metastatic Castration-Resistant Prostate Cancer: External Validation of a [68Ga]Ga-PSMA-11 PET-Based Model with [18F]F-Flotufolastat.

177Lu-prostate-specific membrane antigen (PSMA)-targeted radiopharmaceutical therapy is an established treatment for metastatic castration-resistant prostate cancer, though responses vary. A prognostic model using [68Ga]Ga-PSMA-11 PET-derived parameters was developed to predict therapy outcomes. This retrospective analysis validates the model in an independent cohort imaged with [18F]F-flotufolastat ([18F]rhPSMA-7.3 PET) before [177Lu]Lu-PSMA I&T. Methods: In total, 174 consecutive patients treated with [177Lu]Lu-PSMA I&T at a single center were included in this analysis. The Cox proportional

Anti-EGFRvIII monoclonal antibody armed with 177Lu: in vivo comparison of macrocyclic and acyclic ligands.

INTRODUCTION: Monoclonal antibody (mAb) L8A4 binds specifically to the epidermal growth factor receptor variant III (EGFRvIII) that is present on gliomas but not on normal tissues, and is internalized rapidly after receptor binding. Because of the short range of its β-emissions, labeling this mAb with (177)Lu would be an attractive approach for the treatment of residual tumor margins remaining after surgical debulking of brain tumors. MATERIALS AND METHODS: L8A4 mAb was labeled with (177)Lu using the acyclic ligands [(R)-2-amino-3-(4-isothiocyanatophenyl)propyl]-trans-(S,S)-cyclohexane-1,2-di

Discovery of a highly specific radiolabeled antibody targeting B-cell maturation antigen: Applications in PET imaging of multiple myeloma.

PURPOSE: Multiple myeloma (MM) is characterized by the uncontrolled proliferation of monoclonal plasma cells (PC) in the bone marrow (BM). B-cell maturation antigen (BCMA) is predominantly expressed in malignant plasma cells, and associated with the proliferation, survival, and progression of various myeloma cells. Given these important roles, BCMA emerges as an ideal target antigen for MM therapy. However, effective stratification of patients who may benefit from targeted BCMA therapy and real-time monitoring the therapeutic efficacy poses significant clinical challenge. This study aims to de

The RECIST criteria compared to conventional response evaluation after peptide receptor radionuclide therapy in patients with neuroendocrine neoplasms.

OBJECTIVE: The Response Evaluation Criteria In Solid Tumors (RECIST) is the most used radiological method for evaluating response after peptide receptor radionuclide therapy (PRRT) in patients with neuroendocrine tumors. This method may give too positive estimates of response in slow growing tumors as it allows a substantial increase in tumor size before patients are classified as having progressive disease. We wanted to compare RECIST with a conventional method in routine use for estimating treatment effect based on defining any unequivocal increase in size of tumor load as progressive diseas

Feasibility of a portable PET system for interactive point-of-care (PoC) PET and ultrasound imaging.

BACKGROUND: Positron emission tomography (PET) is a molecular imaging technology that has been used for detection and staging of diseases, and patient stratification to identify candidates for molecularly targeted drugs. Clinical PET/CT scanners are optimized for whole-body diagnostic imaging instead of real-time guidance of interventional procedures or point-of-care (PoC) imaging applications. With rapid advances in novel radio-theranostics, there is growing interest in versatile, accessible molecular imaging tools. PURPOSE: This study evaluates the feasibility of a compact PET device integr

Preclinical Development and Comparison of [99mTc]Tc/[177Lu]Lu-Labeled Anti-Trop2 Fc-Fused Nanobodies as Pancreatic Cancer Theranostics.

Antibody-based radiotheranostics hold significant promise for tumor diagnosis and treatment; however, the clinical translation of radiotherapeutic agents is significantly hindered by suboptimal pharmacokinetics, particularly undesirable retention in healthy tissues. In this study, we developed novel high-affinity anti-Trop2 nanobodies using phage display technology and then generated two VHH-Fc constructs, B9 and C10, which are recombinant single-chain antibodies fused with IgG1-Fc. These were radiolabeled with [99mTc]Tc and [177Lu]Lu. A systematic comparison was then conducted against the cli

64Cu/177Lu-DOTA-diZD, a Small-Molecule-Based Theranostic Pair for Triple-Negative Breast Cancer.

Despite advances in targeted therapies, the prognosis for patients with triple-negative breast cancer (TNBC) is poor because there are few actionable molecular targets. The dependence of solid tumor growth on angiogenesis prompted our development of angiogenic-receptor-targeted radionuclide therapy (TRT) to treat TNBC by targeted delivery of therapeutic doses of ionizing radiation to tumors. A high-affinity vascular endothelial growth factor receptor (VEGFR)-targeted agent, diZD, was synthesized and labeled with 177Lu and 64Cu by 1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid (DOTA)

Preclinical efficacy of hK2 targeted [177Lu]hu11B6 for prostate cancer theranostics.

Androgen ablating drugs increase life expectancy in men with metastatic prostate cancer, but resistance inevitably develops. In a majority of these recurrent tumors, the androgen axis is reactivated in the form of increased androgen receptor (AR) expression. Targeting proteins that are expressed as a down-stream effect of AR activity is a promising rationale for management of this disease. The humanized IgG1 antibody hu11B6 internalizes into prostate and prostate cancer (PCa) cells by binding to the catalytic cleft of human kallikrein 2 (hK2), a prostate specific enzyme governed by the AR-path

Radiometallic Complexes of DO3A-Benzothiazole Aniline for Nuclear Medicine Theranostics.

To develop a radioactive metal complex platform for tumor theranostics, we introduced three radiopharmaceutical derivatives of 1,4,7,10-tetraazacyclododecane-1,4,7-trisacetic acid-benzothiazole aniline (DO3A-BTA, L1) labeled with medical radioisotopes for diagnosis (68Ga/64Cu) and therapy (177Lu). The tumor-targeting ability of these complexes was demonstrated in a cellular uptake experiment, in which 177Lu-L1 exhibited markedly higher uptake in HeLa cells than the 177Lu-1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid complex. scFv-Fcs as a Radiotheranostic Platform for Targeting TRA-1-60 in Pancreatic Cancers.

PURPOSE: TRA-1-60 (TRA) is an established cancer stemness marker implicated in tumor initiation, therapeutic resistance, and disease relapse. It is minimally expressed in normal differentiated tissues, making it an appealing biomarker for immunopositron emission tomography (immunoPET) imaging and radiopharmaceutical therapy (RPT). Full-length monoclonal antibodies (mAbs) often exhibit prolonged circulation, which can lead to increased background signal and potential off-target radiation exposure to normal tissues, thereby raising concerns of blood-associated toxicity.
