Predicting prolonged length of stay after endoscopic transsphenoidal surgery for pituitary adenoma.

BACKGROUND: Endoscopic transsphenoidal surgery (ETS) for the resection of pituitary adenoma has become more common throughout the past decade. Although most patients have a short postoperative hospitalization, others require a more prolonged stay. We aimed to identify predictors for prolonged hospitalization in the setting of ETS for pituitary adenomas. METHODS: A retrospective chart review as performed on 658 patients undergoing ETS for pituitary adenoma at a single tertiary care academic center from 2005 to 2019. Length of stay (LoS) was defined as date of surgery to date of discharge. Pati

Awake Microsurgical Resection for Optochiasmatic Cavernous Malformation.

Cavernous malformations (CM) affect approximately 0.5% of the population, with only a limited portion being located in the optic nerve and chiasma. The clinical presentation is determined by their locations. In the optochiasmatic CM, the acute visual disturbance is the most common presentation. Chronically, many show a progressive visual loss, chronic headache, and pituitary disturbances. The differential diagnosis includes optic glioma, arteriovenous malformations, aneurysm, craniopharyngioma, pituitary apoplexy, and inflammatory conditions. In Video 1, we present the case of a 39-year-old wo

Cabergoline-Resistant Recurrent Macroprolactinoma During Pregnancy: A Case Report and Review of the Literature.

We report a rare case of cabergoline-resistant recurrent macroprolactinoma during pregnancy. A 34-year-old woman, 17 weeks pregnant, presented with a four-week history of headache and a one-day history of visual disturbance. The medical history was significant for a macroprolactinoma with pituitary apoplexy, which was treated with an emergent pituitary resection approximately two years and four months before this presentation. At that time, postoperative prolactin levels improved but remained elevated despite gradually escalating cabergoline doses up to 6 mg per week. Our patient met the estab

Neurological Complications in Pregnancy and the Puerperium: Methodology for a Clinical Diagnosis.

Neurological complications in pregnancy and the puerperium deserve particular attention from specialists due to the worsening of the clinical picture for both the mother and the fetus. This narrative review of existing data in the literature aims to analyze the most common "red flag symptoms" attributable to neurological complications such as pre-eclampsia (PE), eclampsia, HELLP syndrome, posterior reversible encephalopathy syndrome (PRES), cerebral vasoconstriction syndrome (RCVS), stroke, CVS thrombosis, pituitary apoplexy, amniotic fluid embolism and cerebral aneurysm rupture, with the aim

Pericardial Effusion as an Initial Presentation of Panhypopituitarism.

UNLABELLED: Pericardial effusion has a broad spectrum of clinical presentation, ranging from an incidental finding on imaging to a potentially fatal emergency such as pericardial tamponade, the most severe presentation. The authors present a case of a middle-aged male hospitalized due to shortness of breath. Initial work-up was positive for massive pericardial effusion with haemodynamic compromise. Additional study revealed panhypopituitarism. The acromegalic phenotype was suggestive of acromegaly secondary to pituitary adenoma, which had probably evolved to apoplexy. Hormone replacement was s

Spontaneous retroclival hematoma presenting as a thunderclap headache. Case report.

An excruciating headache of instantaneous onset, or thunderclap headache, may be caused by a variety of serious disorders, including aneurysmal subarachnoid hemorrhage, cerebral venous sinus thrombosis, pituitary apoplexy, and carotid artery or vertebral artery dissection. The authors describe a patient with this type of headache who was found to have a spontaneous retroclival hematoma. A 49-year-old woman experienced an instantaneous excruciating headache. Results of computerized tomography (CT) scans of the head were normal, but on examination of the cerebrospinal fluid xanthochromia was fou

[Neurologic diseases and pregnancy].

Neurological diseases during and following pregnancy represent a small subgroup of all neurological diseases. They can be divided into three groups. 1) Diseases which existed already before pregnancy or which appear just by chance during this phase like migraine, multiple sclerosis, myasthenia gravis, epilepsy, brain tumors or Guillain-Barre syndrome. 2) Diseases that can appear without pregnancy but which display a higher incidence in connection with pregnancy. Cerebral ischemia, intracerebral hemorrhage, subarachnoidal hemorrhage, intracranial venous thrombosis and compression neuropathies b

Clinical Outcomes After Endoscopic Endonasal Resection of Giant Pituitary Adenomas.

BACKGROUND: Giant pituitary adenomas represent a surgical challenge. We present the results of the endoscopic endonasal approach (EEA) for giant pituitary adenomas. METHODS: We retrospectively reviewed the medical records of 55 patients with giant pituitary adenomas (>4 cm in maximum diameter) who underwent surgery with an EEA between 2008 and 2016. Factors affecting the extent of resection were evaluated. RESULTS: The mean patient age was 55.5 years. Tumors were nonfunctional in all but 4 patients, including 2 with growth hormone-secreting tumors, 1 with an adrenocorticotropic hormone-secre

Prolactinoma course and management in transgender women: case report and systematic review.

OBJECTIVE: Prolactinomas are the most common functioning pituitary neuroendocrine tumors (PitNETs). In transgender women, gender-affirming hormone therapy (GAHT) usually combines high doses of estrogen and anti-androgen therapy, both of which can elevate serum prolactin levels. Whether GAHT influences tumor behavior in patients with pre-existing prolactinomas remains unclear. DESIGN AND METHODS: Case report illustrating the clinical challenges in managing a prolactinoma in the context of GAHT initiation, combined with a systematic review of all published cases and available guidelines of GAHT

Craniofacial and Radiological Features as Diagnostic Clues to Unmask Acromegaly: A Case Report.

A 70-year-old female with a past medical history of type 2 diabetes mellitus (treated with insulin), hypertension, and bipolar disorder was admitted to the Intensive Care Unit after suffering cardiac arrest post-seizure. During the initial workup, CT of the head was notable for marked, diffuse calvarial thickening (~17 mm), a large sella turcica, and no evidence of pituitary apoplexy. MRI of the head showed a 1.9 cm pituitary macroadenoma. The physical examination was notable for thick facial features such as macroglossia, acrochordon, and fleshy hands - consistent with acromegaly. Insulin-lik

Hemorrhagic Atypical Planum Sphenoidale Meningioma with Intermittent Vision Loss-Rare Presentation of Apoplexy.

BACKGROUND: Symptoms that mimic pituitary apoplexy may be encountered with other neoplastic or infectious lesions. CASE DESCRIPTION: This 38-year-old man presented with severe sudden-onset headache and relapsing and remitting vision loss. Radiographic imaging studies demonstrated radiographic features of a hyperdense, hemorrhagic mass in the sellar region. Magnetic resonance imaging (MRI) revealed a 4-cm mass abutting the optic chiasm and compressing the pituitary. After 4-week follow-up, surveillance MRI demonstrated near-complete resolution of the previously identified planum sphenoidale an

Drivers of Intraoperative Costs for Transsphenoidal Endoscopic Surgery for Sellar Lesions: A Time-Driven Activity-Based Cost Analysis.

BACKGROUND: Neurosurgeons lack precise insights into the true costs of transsphenoidal endoscopic surgery for sellar and suprasellar lesions (TESS), including pituitary adenomas, craniopharyngiomas, and apoplexy. To address this critical knowledge gap, we employ time-driven activity-based costing (TDABC) for TESS. METHODS: We analyzed 221 TESS procedures performed between 2017 and 2022 at a large academic medical center. Costs were calculated using TDABC. Software was developed to extract information regarding all resources utilized intraoperatively. Supply cost was calculated as the aggregat

Pregnancy-associated diabetes insipidus in Japan-a review based on quoting from the literatures reported during the period from 1982 to 2019.

This Review Article overviews the literature on diabetes insipidus (DI) associated with pregnancy and labor in Japan published from 1982 to 2019. The total number of patients collected was 361, however, only one-third of these cases had detailed pathophysiologic information enabling us to identify the respective etiology and subtype. Pregnancy-associated DI can be divided into 3 etiologies, central (neurogenic) DI, nephrogenic DI, and excess vasopressinase-associated DI. Neurogenic DI has various causes: for example, DI associated with tumoral lesions in the pituitary and neighboring area, DI

Panhypopituitarism in acute myocardial infarction.

While hypopituitarism is known to be associated with increased cardiovascular morbidity and mortality, panhypopituitarism as a complication of myocardial infarction (MI) is very rare. Here, we report a case of rapidly developing empty sella syndrome with florid manifestations of panhypopituitarism after MI (due to critical stenosis in the left anterior descending artery) complicated by cardiogenic shock in a 65-year-old man. The patient was initially stabilized with conservative management of non-ST-elevated MI and cardiogenic shock, but after initial improvement, he again deteriorated with re
