Summit program to taper long-term opioid therapy in Veterans: A mixed-methods feasibility study.

Due to the harms associated with long-term opioid therapy and, often, the lack of benefit, it is guideline concordant to offer patients the opportunity to engage in a supportive, patient-centered taper. However, tapering is challenging for both clinicians and patients, with barriers including lack of resources and tools to support tapering and fear of worsening pain. To address these barriers, we created Summit - a web-based app that uses principles of behavioral therapy to offer patients support during voluntary opioid tapers. The aim of this study was to conduct a nine-month randomized pilot

Spatially Confined Co-N4 Sites on N-Doped Carbon Nanotube for Efficient Salt-Free Neutral H2O2 Electrosynthesis.

Two-electron oxygen reduction reaction (2e--ORR) represents a sustainable and energy-efficient approach for decentralized hydrogen peroxide (H2O2) production compared with the conventional anthraquinone process. Among various electrocatalysts, metal-nitrogen-carbon (M-N-C) materials have attracted extensive attention owing to their tunable electronic structures and favorable *OOH adsorption behavior. However, the uncontrolled pyrolysis process generally leads to structurally heterogeneous and ill-defined coordination environments, making it difficult to precisely regulate active sites and unde

Study protocol: Feasibility and acceptability of culturally adapted CBT for anxiety, depression, and stigma in Chinese IBD patients.

BACKGROUND: Inflammatory bowel disease (IBD) is associated with high anxiety, depression, and stigma in Chinese populations, where Western-centric cognitive behavioral therapy (CBT) often fails to align with Confucian values of familial interdependence. This protocol for a pilot randomized controlled trial (RCT) aims to evaluate the feasibility, acceptability, and preliminary efficacy of a Confucianism-adapted CBT (CA-CBT) for Chinese IBD patients. METHODS/DESIGN: Forty IBD patients from a single tertiary center in China via consecutive inpatient wards and outpatient clinics recruitment will

Conscious analgesia with oxycodone improves the efficacy and safety in patients at high risk of aspiration during gastroscopy: a randomized double-blind trial.

BACKGROUND: For the patients at high-risk of aspiration, general anesthesia may increase the risk of aspiration, while local pharyngeal anesthesia may cause extreme discomfort and even failure of gastroscopy. In this study, we tested the efficacy and safety of conscious analgesia with oxycodone in these patients. METHODS: In this prospective, randomized, double-blind, parallel-controlled trial, the patients were randomly assigned to either the oxycodone group (group O) or the 0.9% saline group (group C). Before the gastroscope insertion, group O received oxycodone (0.05 mg/kg), while group C r

Differences in psychological well-being, physical activity, and perception of the value of physical education among Italian middle and high school teachers: A cross-sectional study.

BACKGROUND: Research shows that teachers with high self-efficacy and emotional stability are better able to manage lessons, respond to student behavior, and apply proactive, student-centered approaches. In addition, teachers' emotional well-being enhances their teaching effectiveness and has a positive impact on students' perceptions of their competence, with positive emotions increasing student satisfaction and engagement. In this study, we want to examine the perception that middle and high school physical education teachers have of the importance of physical education, as well as their leve

Analysis of frailty status and its influencing factors in maintenance hemodialysis patients based on the health ecological model.

BACKGROUND: Frailty is a significant public health concern in maintenance hemodialysis (MHD) patients.Previous studies have predominantly focused on isolated risk factors, lacking a comprehensive framework to address its multifactorial nature. This study aims to systematically investigate the prevalence and determinants of frailty in MHD patients using the Health Ecological Model (HEM), which integrates five ecological tiers—individual traits, behavioral characteristics, interpersonal networks, living and working conditions, and policy environment—and to develop a backpropagation neural networ

Heart Rate Variability Measurement Can Be a Point-of-Care Sensing Tool for Screening Postpartum Depression: Differentiation from Adjustment Disorder.

Postpartum depression (PPD) is a serious mental health issue among women after childbirth, and screening systems that incorporate questionnaires have been utilized to screen for PPD. These questionnaires are sensitive but less specific, and the additional use of objective measures could be helpful. The present study aimed to verify the usefulness of a measure of autonomic function, heart rate variability (HRV), which has been reported to be dysregulated in people with depression. Among 935 women who had experienced childbirth and completed the Edinburgh Postnatal Depression Scale (EPDS), HRV w

Decision-making under ambiguity after frontal lobe resection for epilepsy.

PURPOSE: Decision-making is crucial to daily life and can impact our society as well as economic conditions. Although the frontal lobes have been identified as important for decision-making, this capacity has only been studied to a limited extent in frontal lobe epilepsy and not at all after frontal lobe resection (FLR) for epilepsy. This study aimed to explore decision-making under ambiguity after FLR for epilepsy. METHODS: Fourteen patients having undergone FLR for epilepsy completed the Iowa Gambling Task (IGT) which is a widely used tool to measure decision-making under ambiguity. Iowa Ga

Evaluation of a smartphone application for self-help for patients with social anxiety disorder: a randomized controlled study-SMASH.

BACKGROUND: There is growing evidence that Internet-based cognitive behavioral therapy (ICBT) is as effective as a stand-alone treatment and helps facilitating access to treatment. Given the complexity of the treatment, we argue that the effect of ICBT could be even greater if guided by a therapist, as this could increase treatment adherence. We modified an established and well-evaluated treatment approach and developed a mobile application for treating social anxiety disorder (SAD). In the present study, we compare the efficacy of app use alone (APP) with video-based, therapist-guided app use

Emotional self-regulation, impulsivity, 5-HTTLPR and tobacco use behavior among psychiatric inpatients.

BACKGROUND: While the serotonin transporter (SLC6A4) gene, 5-HTTLPR, interacts with the social environment to influence both emotional self-regulation and smoking behavior, less is known about interactions between emotional self-regulation and 5-HTTLPR or their joint influence on tobacco use. Here, we examined such interactions among psychiatric inpatients, the population with the highest rates of smoking. METHODS: Participants (506 adults) were psychiatric inpatients at The Menninger Clinic in Houston TX between 2012 and 16. Most were white (89%), male (55%), with a mean age of 32.3 years. P

The efficacy of 90-min versus 60-min sessions of prolonged exposure for PTSD: A randomized controlled trial in active-duty military personnel.

OBJECTIVE: Prolonged exposure (PE) therapy is a first-line posttraumatic stress disorder (PTSD) treatment, but the manualized 90-min session format constitutes a barrier to adopting PE in most settings because they use 60-min sessions for scheduling and billing. We examined whether 60-min PE sessions were as effective and efficient as 90-min PE sessions. METHOD: In total, 160 active-duty military personnel with PTSD were randomized to 8-15 sessions of 60- or 90-min PE sessions and assessed pre- and posttreatment, and 3- and 6-month posttreatment, using the Clinician Administered PTSD Scale fo

Incorporating Pharmacological Target-Mediated Drug Disposition (TMDD) in a Whole-Body Physiologically Based Pharmacokinetic (PBPK) Model of Linagliptin in Rat and Scale-up to Human.

Linagliptin demonstrates substantial nonlinear pharmacokinetics due to its saturable binding to its pharmacological target dipeptidyl peptide 4 (DPP-4), a phenomenon known as target-mediated drug disposition (TMDD). In the current study, we established a novel whole-body physiologically-based pharmacokinetic (PBPK)-TMDD model for linagliptin. This comprehensive model contains plasma and 14 tissue compartments, among which TMDD binding process was incorporated in 9 of them, namely the plasma, kidney, liver, spleen, lung, skin, salivary gland, thymus, and reproductive organs. Our final model ade

Association Between Cardiovascular Risk and Perceived Fatigability in Mid-to-Late Life.

Background Cardiovascular disease (CVD) and fatigue commonly co-occur in older adults, yet the subjective nature of fatigue and its situational dependence leave the true magnitude of this association undefined. Methods and Results Six-hundred and twenty-five participants with no history of CVD (aged 68.1+12.0 years), from the Baltimore Longitudinal Study of Aging who underwent ≥2 clinic visits between 2007 and 2015 were classified according to sex-specific predicted 10-year CVD risk scores using the Framingham CVD risk score (Framingham) and the Pooled Cohort Equation at baseline.
