Effect of manganese dipyridoxal diphosphate on liver magnetic resonance imaging and serum bilirubin in rats with removable biliary obstruction.

RATIONALE AND OBJECTIVES: It is known that manganese dipyridoxal diphosphate (Mn-DPDP) causes persisting liver enhancement in cholestatic rats, that free Mn++ plus bilirubin induces intrahepatic cholestasis, and that free Mn++ is released in vivo after Mn-DPDP injection. Hence, there is a concern about potential secondary intrahepatic cholestasis in patients who have biliary obstruction. In this study, we further investigated this issue. METHODS: Removable total biliary obstruction (RTBO) was induced in 12 rats. Six of them (group A) received Mn-DPDP (25 mumol/kg). The others (group B) served

Use of risk projection models for the comparison of mortality from radiation-induced breast cancer in various populations.

The aim of our study is to compare lifetime risk estimates for radiation-induced breast cancer between various populations. Some epidemiological studies, mainly on Japanese A-bomb survivors, but also on North American populations irradiated for medical reasons, provide risk coefficients for excess mortality from breast cancer after exposure to ionizing radiation. For this comparative study, these risk coefficients have been transferred into the Japanese, American, and French populations, using demographic data, risk projection models, and assuming an individual acute exposure. The lifetime ris

Notulae seminologicae. 5. Mathematical evaluation of interdependent submicroscopic sperm alterations.

This paper concerns the mathematical evaluation of sperm quality as examined by electron microscopy. Proceeding with a Bayesian technique, we have developed a formula considering all statistical possibilities for defects of the examined sperm to be present in a sperm cell, the total number of affected spermatozoa, and, as consequence, that of sperm devoid of defects, also considering the probability of association characteristics of some of them. The formula has been studied in three applications. The first concerns the number of healthy spermatozoa present in ejaculates of fertile men. We hav

An optimal rule for screening subjects for clinical trials in the presence of within-person variability.

In screening subjects for a continuous variable, such as blood pressure (BP), for eligibility for clinical trials, multistage rules are often used to select subjects within a target range. Multiple readings may be taken over successive visits, and the subject must meet eligibility criteria at each visit. We present an optimal rule that minimizes the number of clinic visits needed per eligible subject or the overall number of screening visits needed to achieve a given sample size of eligible subjects. We illustrate these methods with the design of phase I of the Trials of Hypertension Preventio

Birthplace as a risk factor in motor neurone disease and Parkinson's disease.

A proportional mortality study was conducted to determine if state of birth is a risk factor associated with motor neurone disease (MND) and Parkinson's disease (PD) using US death certificate information for 1981. State of birth was used as a surrogate variable for location of early childhood environment. A gradient of risk by geographical area in the US was found for MND and PD. Cerebral vascular accident (CVA) deaths served as a comparison group. Multiple sclerosis (MS) deaths were analysed to validate the methods used. A geographical relationship between latitude proportional mortality rat

Impact of a breast cancer screening community intervention.

BACKGROUND: Efforts to detect breast cancer in its early stages are necessary to reduce breast cancer-associated mortality. This study evaluated the impact of a multicomponent intervention implemented between 1987 and 1990 to increase a community's utilization of breast cancer screening by women over 50 years of age. METHODS: The study used a pretest/post-test two-community design, with one community assigned as the intervention community and the other as the comparison. The intervention consisted of a comprehensive physician involvement component and a community education effort. To assess t

The effect of marital disruption on children's health.

This study uses retrospective illness histories to investigate whether children's health deteriorates after parental separation. Separation is associated with illness in a multivariate cross-sectional analysis as well as in an analysis of a sample of disrupted children only, in which illness rates before and after separation are compared. Three explanations are hypothesized: (1) divorce reduces the resources available to children, (2) the stress of divorce depletes children's health, and (3) frailer children are selected into divorce. The first hypothesis has stronger support than the second,

Genetic etiology of gastric carcinoma: II. Segregation analysis of gastric pH, nitrate, and nitrite.

A study of gastric pH, nitrate, and nitrite in 110 families collected as part of a cohort from the Narino region of Colombia is presented. All three traits are familial and have a significant linearly increasing age trend. Gastric pH has a clear bimodal distribution but does not show Mendelian segregation. The nitrate distribution is slightly skewed, but generational heterogeneity explains the data best. Gastric nitrite is also biomodal with a clear break at concentration 1.08 micrograms/ml, and 74% of the observations at zero concentration; it shows a recessive Mendelian segregation with sign

Spontaneous mutation and parental age in humans.

A statistical analysis of parental age and the incidence of new mutation has been performed. Some new data on Apert, Crouzon, and Pfeiffer syndromes is presented and combined with all available data from the literature on parental age and new mutation. Significant heterogeneity among syndromes for the rate of increase in incidence with parental age was found. A parsimonious conclusion is that mutations fall into two groups, one with a high rate of increase with age and the other with a low rate of increase with age. For the high-rate-of-increase group, a linear model relating incidence to age

Model for the study of the preeruptive effect of NaF tablets on caries in permanent teeth.

The purpose of this study was to elaborate a model for estimating the caries-reducing effect of a preventive measure with due consideration of background factors. The material comprised 50 children, who had consumed NaF tablets regularly during preschool age, and 76 comparable children who had never consumed NaF tablets. They were all examined at 12 and 13 yr of age. Information on caries and, as confounding factors, dietary habits, oral hygiene and microbiologic variables, was obtained on both occasions. Correlations between the parameters and caries, their discriminating effects and interact

Insurance effects on employer group dental expenditures.

As the number of dental insureds and the amount of insured expenditures have increased over the past decade, so has interest in determining insurance effects on dental demand among employer groups and their insureds. Using Pennsylvania Blue Shield (PBS) employer group data representing 50-65% of all PBS insureds, the authors estimate the own-effect of basic insurance rates (diagnostic, preventive, restorative, endodontic, extraction services) and the cross-effects of riders A (crowns and oral surgery), B (prosthodontics), C (periodontics), and D (orthodontics) insurance rates on annual basic e

Gravity Modeling to Predict Patient Choice of Hospital for Pancreaticoduodenectomy.

BACKGROUND: Prior studies simulating how regionalization of complex surgeries such as pancreatoduodenectomy (PD) would affect access to care assumed that patients travel to their closest hospital. The validity of this assumption had not been evaluated. Our objectives were to assess the accuracy of a distance-only model in predicting where a patient received PD and determine whether incorporating hospital characteristics improved accuracy. METHODS: Data on patients undergoing PD from 2016 to 2017 in New York or Florida were obtained from the Statewide Inpatient Databases. We assessed three mod

Bayesian network meta-regression models for multivariate aggregate responses with partially observed or completely missing within-treatment sample covariance matrices.

In this paper, we propose a Bayesian multivariate network meta-regression model to compare multiple treatments used to treat cardiovascular and diabetes diseases, where the multivariate aggregate outcomes include low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, and triglycerides. We assume a log-linear regression model for the standard deviation of the treatment random effects to overcome the difficulty that some treatments may present only in a single study. As the within-study sample covariance matrix $\boldsymbol {S}$ is partially observed or completely missing and

A Bayesian Prevalence-Incidence Mixture Model for Screening Outcomes With Misclassification.

Screening and surveillance programs for cancer, such as colorectal cancer (CRC), often yield electronic health records (EHR) of screening time, test results, and covariates. We consider EHR from CRC surveillance of individuals who have a high cancer risk due to their family history. These individuals, therefore, receive regular colonoscopies with the goal of finding and removing adenomas, precursor lesions to CRC. Our objective is to estimate time to adenoma incidence and explore associations with covariates. However, in doing so, several challenges of the CRC surveillance EHR have to be addre

Practical considerations when using the covariate-adjusted log-rank test for the analysis of time-to-event endpoints in oncology trials.

Adjusting for prognostic baseline covariates in the design and analysis of randomized controlled trials can increase statistical power and improve the precision of treatment effect estimates by reducing outcome variability. In this paper, we describe how the covariate-adjusted log-rank test can be used to enhance the power of late-phase oncology studies with primary time-to-event endpoints. Using a case study of a metastatic castration-resistant prostate cancer trial with overall survival as the primary endpoint, we demonstrate how to estimate variance reductions achievable through different c

A Prediction Model for 9-Year Depression Risk Among Patients With COPD in China.

OBJECTIVES: To establish a prediction model for the 9-year risk of depression in individuals with chronic obstructive pulmonary disease (COPD) using survey data from a random, nationally representative population of Chinese adults with COPD. METHODS: Data from 851 middle-aged and older adults with COPD participating in the China Health and Retirement Longitudinal Study (CHARLS) were analyzed. Depression risk was described as the development of depression during follow-up among participants who were free of depression risk at baseline (CHARLS 2011) and met the depression risk criteria at the 9

Developing predictive models for COVID-19 positive tests based on the XGBoost and random forest algorithms with internet search data.

BACKGROUND: Although strategies for COVID-19 have shifted towards normalized measures globally, establishing predictive models based on Internet search data remains crucial for swiftly controlling and preventing future outbreaks. This study aims to utilize Internet search data for early epidemic surveillance and warning. METHODS: We collected the daily number of COVID-19 positive tests and the daily Baidu Search Index (BSI) of COVID-19 related keywords. First, we screened keywords with a maximum correlation coefficient exceeding 0.9 by time-lagged correlation analysis. Then, we used the origi

Application of Causal Forest Model to Examine Treatment Effect Heterogeneity in Substance Use Disorder Psychosocial Treatments.

OBJECTIVES: Heterogeneity of treatment effect (HTE) is a concern in substance use disorder (SUD) treatments but has not been rigorously examined. This exploratory study applied a causal forest approach to examine HTE in psychosocial SUD treatments, considering multiple covariates simultaneously. METHODS: Data from 12 randomized controlled trials of nine psychosocial treatments were obtained from the National Institute on Drug Abuse Clinical Trials Network. Using causal forests, we estimated the conditional average treatment effect (CATE) on drug abstinence. To assess HTE, we compared CATE var
