Pain medication use in patients with X-linked hypophosphatemia treated with burosumab: a retrospective real-world study.

X-linked hypophosphatemia (XLH) is a rare, progressive phosphate-wasting disorder that leads to significant skeletal morbidity and pain. The current study used real-world administrative claims data to assess the demographic and clinical characteristics of patients with XLH related to claims for prescription pain medication (PPM) during the 12 mo before starting burosumab treatment (baseline) and explores changes in PPM claims after starting burosumab treatment. Claims for XLH-related musculoskeletal and deformity-related conditions, obesity, and osteoarthritis were common among the adult cohor

Multi-Trajectories of Sleep Disturbance and Depressive Symptoms and Incident Osteoarthritis: Evidence From the English Longitudinal Study of Ageing.

BACKGROUND: Osteoarthritis (OA) is a leading cause of pain and disability in older adults. Sleep disturbance and depressive symptoms are common and have been linked to multiple chronic conditions, yet how their co-developing longitudinal patterns relate to OA onset remains unclear. We aimed to identify joint trajectories of sleep disturbance and depressive symptoms and examine their associations with incident OA. METHODS: Data were drawn from the English Longitudinal Study of Ageing (ELSA), a nationally representative prospective cohort of community-dwelling adults aged ≥ 50 years. Sleep dist

Concurrent pulmonary infection with Nocardia spp. and Mycobacterium tuberculosis in an adult with cerebral palsy: a case report.

BACKGROUND: Nocardiosis is a rare opportunistic infection typically affecting immunocompromised individuals and those with chronic lung disease, prolonged corticosteroid therapy, malignancy, or other conditions impairing cell-mediated immunity, while tuberculosis (TB) remains a leading global health concern. Dual infections with Nocardia and TB are rare but clinically important, as overlapping clinical and radiologic features make diagnosis difficult. Reports have also documented co-infections with Nocardia and other pathogens, including Aspergillus and nontuberculous mycobacteria, underscorin

Recruitment of the multiple sclerosis cohort within the European Mobilise-D clinical validation study-lessons learnt, baseline demographics and clinical characteristics.

BACKGROUND: Multiple sclerosis (MS) is a common cause of disability in working age adults. Current clinical assessments are inadequate at disability assessment or predicting clinically relevant outcomes. Loss of mobility is an important functional disability to people with MS. Mobilise-D aims to develop, validate, and implement a digital mobility solution which measures unsupervised mobility performance across several chronic conditions, including MS, using a single wearable device. METHODS: Six hundred two adults with MS, an Expanded Disability Status Scale (EDSS) score of 3.0-6.5, documente

Quantitative Analysis of Smooth Pursuit and Saccadic Eye Movements in Multiple Sclerosis.

Introduction: Multiple sclerosis (MS) is a chronic inflammatory and neurodegenerative disease of the central nervous system, frequently associated with visual and oculomotor disturbances. Quantitative analysis of eye movements represents a non-invasive method for assessing central nervous system dysfunction beyond conventional imaging; however, the diagnostic and predictive value of oculomotor metrics remains insufficiently defined. Objectives: The aims of this study were to compare smooth pursuit gain and reflexive saccade parameters (latency, velocity, and precision) between individuals with

Graded hypergravity is associated with large-scale reorganization of resting-state cortical networks: EEG evidence from a neurological inactivity phenotype.

BACKGROUND: Chronic physical inactivity and reduced sensorimotor engagement are associated with alterations in large-scale brain network organization, often reflected as changes in resting-state functional connectivity. Interventions capable of modulating multisensory input without requiring volitional movement may therefore provide a useful experimental framework for investigating inactivity-related cortical plasticity. Short-arm human centrifugation (SAHC) delivers controlled, intermittent graded hypergravity, providing a reproducible vestibular-somatosensory stimulus with potential neuromod

Exploring the Correlation between Pulmonary Hypertension and Pectoralis Muscle Area and Density.

HIGHLIGHTS: Markers of sarcopenia showed a weak correlation with the prognosis of PH.Some indices including right atrial pressure and systemic arterial oxygen saturation were considered as predictors of PH prognosis. BACKGROUND: Sarcopenia is a predictor of mortality in multiple conditions, but the potential prognostic value of sarcopenia indices in pulmonary hypertension (PH) has not been clarified. This study aimed to determine whether there is an association between computed tomography (CT) scan-measured pectoralis muscle area (PMA) and density (PMD) and adverse clinical outcomes in PH pat

Potential miRNAs as Diagnostic Biomarkers for Differentiating Disease States in Ulcerative Colitis: A Systematic Review.

Ulcerative colitis (UC) is a chronic inflammatory disease that affects the colon, triggering persistent inflammation and ulceration, resulting in a severe impact on patients' quality of life. Currently, the standard diagnostic methods for UC include invasive procedures such as colonoscopy and the use of non-specific inflammatory markers like C-reactive protein, which can be inconvenient or painful and lack specificity. This underscores the need for non-invasive and highly specific biomarkers for UC. MicroRNAs (miRNAs) are small non-coding RNAs, typically 22 nucleotides in length, which are wel

Altered cortical alpha modulations and connectivity in complex regional pain syndrome.

Chronic pain alters resting-state cerebral alpha rhythms (8-12 Hz), including whole-brain slowing of peak alpha frequency (PAF). Nevertheless, how the power of brain activity at PAF (PAF power) or across the entire alpha frequency range (alpha power) is decreased or increased compared to healthy controls (HC) varies greatly across pain phenotypes. The brain regions exhibiting such alterations and their association with the pain severity experienced by patients are also unclear. The study addressed this question in participants experiencing complex regional pain syndrome (CRPS; 19 females and 1

Validity of the Leading Causes of Death Classification for Premature Mortality in Inflammatory Bowel Disease: A Population-Based Comparison with ICD-10 Coding in Ontario, Canada.

INTRODUCTION: Studying patterns of death, particularly premature deaths (<75 years), provides insights to address health inequities among those living. Multiple coding systems for cause of death (COD) exist. The Leading Causes of Death (LCD) scheme is designed for identifying priority COD for interventions in global populations. The extent to which such classification is effective for identifying priority causes of premature mortality among subpopulations with chronic health conditions, such as inflammatory bowel disease (IBD), is unknown. OBJECTIVE: To evaluate the usability of the LCD for c

Pleiotropic regulatory mechanisms and targeted therapeutic prospects of Galectin-3 in aging-related diseases.

Aging is a major risk factor for numerous chronic diseases, including Alzheimer's disease (AD), Parkinson's disease (PD), atherosclerosis (AS), type 2 diabetes mellitus (T2DM), osteoarthritis (OA) and age-related macular degeneration (AMD). Galectin-3 (Gal-3), a unique β-galactoside-binding lectin, has emerged as a critical mediator in the pathogenesis of AD and other age-related disorders by modulating key mechanisms such as inflammation, oxidative stress, and apoptosis. While emphasizing neurological disorders (AD, PD), this review also examines Gal-3's role in systemic age-related condition

Nezavist (DCUK-OEt) and aminoquinoline analogues interact with the etomidate site and display a complex mode of modulatory action of the GABAA receptor.

The aminoquinoline compound Nezavist (DCUK-OEt; ethyl 5,7-dichloro-4-(3,3-diphenylureido)quinoline-2-carboxylate) is under preclinical and early clinical investigation for chronic pain, alcohol use disorder (AUD), and relapse prevention in nicotine dependence. It and its metabolite DCUKA (5,7-dichloro-4-(3,3-diphenylureido)quinoline-2-carboxylic acid) have been shown previously to be allosteric positive modulators of GABAA receptors. Here, we investigated the two new derivatives BCUKA (5,7-dichloro-4-(3,3-dibutylureido)quinoline-2-carboxylic acid) and BCUK-OEt (ethyl 5,7-dichloro-4-(3,3-dibuty

Migraine with and without aura-two distinct entities? A narrative review.

Migraine is a primary headache disorder, with a prevalence estimated at approximately 15% globally. According to the International Classification of Headache Disorders, 3rd edition (ICHD3), there are three significant types of migraine: migraine without aura (MO), migraine with aura (MA), and chronic migraine (CM), the former being the most common. Migraine diagnosis is based on official criteria specific to each type. Although a lot is already known about the origin of migraine aura, its pathophysiology is still an object of research.Long-term discussions have been held about MO and MA, with
