Interleukin-35 exhibits protective effects in a rat model of hypoxic-ischemic encephalopathy through the inhibition of microglia-mediated inflammation.

BACKGROUND: Hypoxic-ischemic encephalopathy (HIE) brain damage is related to inflammatory responses and oxidative stress. Interleukin (IL)-35 is an antioxidant and anti-inflammatory cytokine. Thus, the effect of IL-35 treatment on neonatal rats with hypoxic-ischemic brain injury was investigated. METHODS: A total of 96 7-day-old Sprague Dawley rats were randomly divided into three groups: sham group, HIE group, and IL-35 group. After left common carotid occlusion and 2.5 h hypoxia (HI injury), IL-35 (20 µg/g) was intraperitoneally (i.p.) administered to the pups. In vitro, BV2 cells were trea

Periventricular White Matter Hyperintensities and Functional Decline.

BACKGROUND/OBJECTIVES: We previously showed that global brain white matter hyperintensity volume (WMHV) was associated with accelerated long-term functional decline. The objective of the current study was to determine whether WMHV in particular brain regions is more predictive of functional decline. DESIGN: Prospective population-based study. SETTING: Northern Manhattan magnetic resonance imaging (MRI) study. PARTICIPANTS: Individuals free of stroke at baseline (N = 1,195; mean age 71 ± 9; n = 460 (39%) male). MEASUREMENTS: Participants had brain MRI with axial T1, T2, and fluid attenuated

Osler and the "medico-chirurgical neurologists": Horsley, Cushing, and Penfield.

Sir Victor Horsley's lecture "On the Technique of Operations on the Central Nervous System," delivered in Toronto in 1906, set the stage for an appraisal of Sir William Osler as a protagonist for the emerging specialty of neurosurgery. During his time at McGill University from 1871 to 1884, Osler performed more than 1000 autopsies. Hispathological reports covered the topics of cerebral aneurysm, apoplectic hemorrhage, vascular infarction, subdural hematoma, meningitis, multiple sclerosis, cerebral abscess, and brain tumor. He wrote about cerebral localization and anatomy and the relationships

Successful Treatment with Rituximab in a Patient with Clinically Suspected Small-vessel Primary Central Nervous System Vasculitis Refractory to Cyclophosphamide Pulse Therapy.

A 58-year-old man developed a progressive cognitive impairment and recurrent cerebral infarctions that were refractory to antithrombotic therapy. No abnormalities were detected in the major cerebral arteries, and a cerebrospinal fluid analysis revealed mild inflammatory findings. Although a brain biopsy was not feasible, the patient was clinically diagnosed with small-vessel primary central nervous system vasculitis based on comprehensive evaluations. Treatment with glucocorticoids and cyclophosphamide failed to prevent a relapse. The presence of cerebrospinal fluid oligoclonal bands and eleva

Transient third nerve palsy after percutaneous coronary intervention - A case report.

Oculomotor nerve palsy is a rare complication of interventional cardiac procedures. We report a case of a 64-year-old male who developed diplopia after he underwent a percutaneous transluminal coronary angioplasty (PTCA) for double-vessel disease. Ocular examination revealed partial third nerve palsy in his right eye. His anterior and posterior segment examinations were within normal limits. Brain MRI revealed a hyperacute infarct in the right paramedian region of the superior colliculus. MR angiography displayed stenosis in his right internal carotid artery in the cervical region. He was mana

Moyamoya Disease in an Eight-Year-Old Female Child-A Case Report.

An eight-year-old girl presented with a first episode of focal seizure and transient loss of consciousness. Brain MRI revealed a large subacute infarct in the left fronto-parieto-temporal region, and MR angiography showed bilateral supraclinoid internal carotid and proximal posterior cerebral artery narrowing with extensive basal collateral formation, consistent with Moyamoya disease. The absence of aneurysm or arteriovenous malformation excluded other vascular anomalies. She was managed with hypertonic saline, intravenous antibiotics, and antiplatelet therapy, followed by stabilization and re

[Cerebral amyloid angiopathy].

Cerebral amyloid angiopathy (CAA) is a progressive disease characterized by the deposition of β-amyloid in the walls of blood vessels in the brain, which leads to their damage and disruption of normal blood flow. Morphologically, CAA is characterized by both isolated lesions (microhemorrhages with the appearance of cortical superficial siderosis, lacunar infarctions) and widespread changes (hyperintensity of the deep and periventricular white matter, expansion of the perivascular spaces) of cortical and subcortical localization. CAA is considered a major cause of cognitive impairment and intra

Transient Kluver-Bucy Syndrome as a Manifestation of Post-temporal Lobe Seizure: A Rare Case Entity.

Following epileptic seizures, patients can subsequently experience a post-ictal state characterized by disorienting symptoms, such as confusion, drowsiness, hypertension, headache, and nausea, rather than neurobehavioral sequelae. We report the case of a 64-year-old male with unilateral left temporal lobe injury, who presented with post-ictal transient Kluver-Bucy Syndrome symptomatology following a complex partial seizure. Brain magnetic resonance imaging revealed encephalomalacia of the left temporo-parieto-occipital region from a previous infarct, and his symptoms resolved following the adm

Case Report: Diffuse Cerebral Microbleeds in Cerebral Autosomal Recessive Arteriopathy With Subcortical Infarcts and Leukoencephalopathy.

Cerebral autosomal recessive arteriopathy with subcortical infarcts and leukoencephalopathy (CARASIL) is a hereditary cerebral small vascular disease caused by a homozygous mutation in the high-temperature requirement A serine peptidase 1 (HTRA1) gene. Cerebral microbleeds (CMBs) are increasingly being recognized as neuroimaging findings occurring with cerebrovascular disease and have different etiologies. Mild to moderate CMBs are not unusual in CARASIL, and they are observed to affect cortical and subcortical structures; in contrast, diffuse CMBs, especially in the cerebellum, are rare. In t

Lethal Neurotoxicity in Lambda-Cyhalothrin Poisoning: A Rare Case Report.

Agricultural poisons (insecticides and pesticides) are the most common types of poison implicated in the morbidity and mortality associated with acute poisoning. Suicidal ingestion is more frequent than accidental or homicidal poisonings. Pyrethroids are considered relatively safer than other insecticides. Lambda-cyhalothrin (LCH) belongs to the fourth-generation, type II synthetic pyrethroid. To the best of our knowledge, fatalities after LCH exposure have not yet been reported in the literature. Here, we describe a case of LCH poisoning in a 54-year-old male farmer after an accidental pipe b

The Association Between Vascular Inflammation and Depressive Disorder. Causality, Biomarkers and Targeted Treatment.

Diabetes, obesity, atherosclerosis, and myocardial infarction are frequently co-morbid with major depressive disorder. In the current review, it is argued that vascular inflammation is a factor that is common to all disorders and that an endothelial dysfunction of the blood-brain barrier could be involved in the induction of depression symptoms. Biomarkers for vascular inflammation include a high plasma level of C-reactive protein, soluble cell-adhesion molecules, von Willebrand factor, aldosterone, and proinflammatory cytokines like interleukin-6 or tumor necrosis factor α. A further possible

Managing psychotic depression and diagnostic uncertainty in liaison psychiatry.

A middle-aged woman presented with a history of symptoms of depression with psychotic features severely affecting her physical health. Neuroimaging of her brain suggested pathological changes out of keeping with her age, leading to further investigations including genetic testing for cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL). Anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis was also considered as a differential diagnosis as an initial serum test for anti-NMDAR autoantibodies was positive. Her symptoms resolved following treatmen

Neuro-regeneration Therapeutic for Alzheimer's Dementia: Perspectives on Neurotrophic Activity.

Alzheimer's disease (AD), the leading disorder of memory impairment in our aging population, is increasing at an alarming rate. AD is currently identified by three 'gold standard criteria': (i) dementia in life, (ii) amyloid plaques at autopsy, and (iii) neurofibrillary tangles at autopsy. Several autopsy studies have indicated that dementia in life is a consequence of lost synaptic networks in the brain, while many clinical trials targeting neurotoxic amyloid beta (Aβ) have consistently failed to produce therapeutic effects on memory function in AD patients. Restoring cognitive function(s) by

A Young Patient with Stroke and Primary Tuberculosis.

A 21-year-old woman presented with left hemiparesis, fever, dyspnea, tachycardia, and pericardial rub on examination. She was provisionally diagnosed with infective endocarditis and received the final diagnosis of the primary pulmonary tuberculosis (PTB) and extra PTB (EPTB) with pericardial effusion and thoracic lymphadenitis. Left hemiparesis due to a pontine infarct was attributed to TB with neurovasculitis. The diagnosis was supported by findings on imaging studies such as echocardiography, computed tomography of the thorax, and magnetic resonance imaging of the brain. She improved with an

Varicella-zoster virus (VZV) multifocal vasculopathy in a patient with systemic lupus erythematosus - a diagnostic and treatment dilemma.

Cerebral vasculopathy due to varicella-zoster virus (VZV) infection is well-documented. We report a fatal case of VZV multifocal vasculopathy in a patient with systemic lupus erythematosus (SLE) who presented with subacute changes in mental status and had multiple areas of hemorrhagic infarcts on brain imaging. However, the correct diagnosis was delayed by several confounding factors including the absence of zoster rash, normal cerebral angiography, persistently low cerebrospinal fluid (CSF) glucose and negative initial polymerase chain reaction (PCR) for VZV DNA in the CSF. Our case and liter

Non-Bacterial Thrombotic Endocarditis in a Patient with Rheumatoid Arthritis.

Rheumatoid arthritis (RA) is frequently associated with various extra-joint complications. Although rare, thromboembolic complications are associated with high morbidity and mortality. We experienced a very rare case of nonbacterial thrombotic endocarditis (NBTE) and subsequent embolic stroke in a patient with RA. A 72-year-old male with a 15-year history of RA suddenly developed neurologic symptoms of vomiting and dizziness. Brain magnetic resonance imaging revealed recently developed multiple cerebellar and cerebral lacunar infarctions. Echocardiography showed a pulsating mitral valve vegeta

Severe hypernatremia in an extremely low birth weight infant with subsequent normal neurological development.

Severe preterm hypernatremia is a life-threatening electrolyte disorder which mainly causes neurological complications such as brain edema, intracranial hemorrhages, hemorrhagic infarctions and thromboses. We report the case of an extremely low birth weight infant (520 g, 24+6 weeks of gestation) with severe hypernatremia of 199 mmol/l due to a failure in the preparation of an infusion on the 7th day of life. The infant subsequently showed no signs of central nervous system complications and no neurological symptoms due to this mishap so far. MRI scan performed after 4 months showed no evidenc

[Anterior ischemic optic neuropathy: Etiology, pathogenetic mechanisms and therapy].

Ischemic optic neuropathy is caused by ischemia of the optic nerve head in the region of the lamina cribrosa. Differentiation is made between arteritic (AION) and nonarteritic (NAION) forms. AION is the most common ophthalmological manifestation of giant cell arteritis and is usually well controlled with systemic steroid therapy. Temporal artery biopsy for confirmation of the diagnosis is mandatory. NAION is not a disease entity but rather the common pathogenetic pathway of a large variety of diseases and conditions and is often the result of several interacting factors. For this reason, there

Headache and cerebral vein and sinus thrombosis.

Headache is the most frequent and often the earliest symptom of cerebral vein and sinus thrombosis (CVT). Intracranial hypertension, vein distension and venous infarction alone or in combination are thought to be the principal mechanisms in the pathophysiology of CVT headache. The absence of specific features and the possibility of presentation without associated neurological signs can make its recognition difficult. The early diagnosis of an association of headache with CVT is crucial for patient well being. Physicians must always consider the possibility of CVT in patients with recent headac
