Diagnosis and management of pulmonary hypertension in systemic sclerosis.

Patients with systemic sclerosis (SSc) can develop pulmonary hypertension (PH; mean pulmonary artery pressure >/= 25 mm Hg) caused by pulmonary arterial hypertension (PAH), left ventricular disease, or pulmonary fibrosis. PAH is a pulmonary vascular disease, the diagnosis of which requires pulmonary capillary wedge pressure less than 15 mm Hg, pulmonary vascular resistance greater than 3 Wood Units, and exclusion of thromboembolism and parenchymal lung disease. Molecular mechanisms underlying PAH-SSc include activation of inflammatory and fibrogenic pathways in the vasculature and right ventri

Ergometry for estimation of mechanical power output in sprinting in humans using a newly developed self-driven treadmill.

An evaluation of mechanical power during walking and running in humans was undertaken after developing a specially designed running ergometer (RE) in which the subjects gripped the handlebar in front of them keeping both arms straight and in a horizontal position. Ten subjects participated in comparisons of the mean horizontal pushing force (MFam) on the handlebar with the mean horizontal ground reaction force (MFfp) recorded by force platform under the RE during five different constant speeds of walking or running and sprint running with maximal effort. Mechanical power developed during sprin

[The dynamics of functional heart volumes in the descents of divers to depths of up to 15 m].

In an underwater environment at a depth up to 15 m the cardiodynamic parameters of resting and exercising man were recorded echocardiographically. Increased functional volumes of the heart and decreased ejection fraction (EF) have been noted. Underwater reactions of the cardiac functional volumes in response to exercise were characterized by: a maximum rise in systolic volume on exposure to low level loading; decreases of end diastolic and end systolic blood volumes; an elevation of EF lower than on the surface. With an increase in diving depth the functional reserves of the heart of exercisin

[On the role of potassium as a limiting factor for exercise hyperpnea].

In view of the common findings that patients with pulmonary disease exhibit larger maximal exercise hyperpnea (Vmax) than their maximal voluntary ventilation (MVV), the limiting factor for the exercise loading in these patients has been considered to be excessive hyperventilation. Recently, plasma potassium released from exercising muscle has been found to effectively stimulate arterial chemoreceptors. This effect is further potentiated by the presence of hypoxia. Since patients often become hypoxemic by exercise loading, it was suspected that potassium may play a role to limit exercising capa

The effect of sodium citrate ingestion on the metabolic response to intense exercise following diet manipulation in man.

Feeding a high-carbohydrate (CHO) diet and administration of alkalinizing agents have both been shown to improve performance in high-intensity exercise. The effect of these treatments in combination was investigated in the present study. Six healthy male subjects exercised to exhaustion on an electrically braked cycle ergometer at a power output equivalent to 100% of their maximum oxygen uptake (VO2,max) on four separate occasions. Each subject consumed either a diet with the same composition as his normal diet (termed the experimental normal (N) diet; 54 +/- 7% CHO, 13 +/- 2% protein, 33 +/-

Ischemic preconditioning with active reperfusion does not improve high-intensity intermittent exercise in youth team sport players.

PURPOSE: We investigated whether an active protocol of ischemic preconditioning (IPC-A) would improve high-intensity intermittent exercise performance in youth team sport players. METHODS: Fifteen male amateur team sport players (15.5 ± 0.5 yrs) attended four different preconditioning sessions before the YoYo Intermittent Recovery Test level 1 (YYIR1) interspersed by seven days in a counterbalanced randomized cross-over design. IPC protocol consisted of three cycles of 5 min occlusion (220 mmHg) and 5 min reperfusion (0 mmHg) in both thighs. SHAM was similar to the IPC protocol, but 'occlusio

Effects of an ICT-Based Wearable Intervention on Physical Function in Arteriosclerosis Obliterans: A 12-Week Study.

Arteriosclerosis obliterans (ASO) is associated with impaired walking function and claudication. However, the effects of information and communication technology (ICT)-based wearable interventions on objectively measured gait outcomes in this population have not been determined. In this 12-week intervention, 52 patients with ASO were randomly assigned to an ICT-based wearable-assisted exercise intervention (n = 30) or a control (n = 22) group. All participants wore a triaxial accelerometer-based device on the non-dominant wrist to monitor moderate-to-vigorous physical activity (MVPA), expresse

Thoracoabdominal breathing pattern across exercise intensity domains: performance level insights in cyclists.

Thoracoabdominal breathing pattern (THA) can be influenced by factors such as exercise intensity, postural demands, and individual variability, which could be associated with performance level. The present study aims to investigate the THA of cyclists with different performance levels during increasing exercise intensity. Fourteen male professional cyclists performed resting breathing manoeuvres and a step-incremental test on a cycle ergometer. Physiological responses were monitored using a metabolic gas analyser and were then classified into two groups: Higher and Lower VO2max. THA was record

Using the 3-Minute All-Out Test to Explore the Durability of the Speed-Duration Relationship in Endurance Running.

BACKGROUND: Critical power and critical speed (CS) are key markers in exercise physiology for distinguishing between heavy- and severe-intensity domains and are useful for training prescription and performance prediction. Durability, defined as the rate and extent of physiological decline during prolonged exercise, has become a focus in endurance sports. This study aimed to investigate whether an hour of heavy-intensity running induces a decline in CS and other performance parameters using GPS-based assessments. METHODS: Seven trained runners completed a set of 3-minute all-out tests to measu

Pharmacological prolyl hydroxylase inhibition enhances high-intensity exercise capacity in mice.

Increased blood hemoglobin concentration theoretically promotes oxygen delivery to the periphery. Clinically, in addition to recombinant human erythropoietin analogs, prolyl hydroxylase (PH) inhibitors to stabilize the expression of hypoxia-inducible factor (HIF) protein were recently approved for the treatment of anemia in chronic kidney disease. However, whether the new agent helps enhance physical exercise capacity remains a matter of controversy. Treatment of C57BL/6J mice via oral gavage with roxadustat of 30 mg/kg three times per week for 5 weeks elevated the blood hemoglobin concentrati
