Systemic Inflammatory Response During Coronary Artery Bypass Grafting with Cardiopulmonary Bypass: Cellular and Molecular Mechanisms.

Coronary artery bypass grafting (CABG) utilizing cardiopulmonary bypass (CPB) remains a cornerstone of treatment for advanced multivessel coronary artery disease. During the ischemic period, myocardial cells become injured and increasingly depend on anaerobic metabolism. After reperfusion, exposure of blood components to artificial surfaces, combined with ischemia-reperfusion injury, surgical trauma, and gut translocation of endotoxins, triggers a complex systemic inflammatory response syndrome (SIRS). This inflammatory cascade involves a tightly regulated network of humoral cascades (compleme

Case report: Atypical SJS/TEN-like fulminant pemphigus vulgaris: diagnostic challenges and the potential impact of anti-Dsg1 titers and CMV reactivation.

Pemphigus vulgaris (PV) is a chronic autoimmune blistering disease characterized by intraepidermal flaccid blisters of the skin and mucous membranes. Although typically chronic and relapsing, rare fulminant variants have been described. We report a life-threatening case of PV in a previously healthy 55-year-old woman who developed a rapidly progressive, Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN)-like phenotype with extensive epidermal detachment and mucocutaneous involvement. Histopathological analysis demonstrated suprabasal acantholysis, and direct immunofluorescence showe

Bulk transcriptomics of peripheral blood mononuclear cells delineates systems-level immune dysregulation in pediatric dengue infection.

Children are disproportionately at a greater risk of developing severe dengue disease, yet the molecular mechanisms driving the heightened susceptibility remains poorly understood. To elucidate the immune determinants of pediatric dengue pathogenesis, we profiled the transcriptomic landscape by performing RNA sequencing (RNA-Seq) on peripheral blood mononuclear cells (PBMCs) from laboratory-confirmed cases of primary and secondary pediatric dengue, as well as pediatric healthy controls. Differential gene expression (DEG) and pathway analyses were performed to delineate the transcriptional alte

The P2X7 receptor: insights from the murine monoclonal antibody clone L4.

The P2X7 receptor (or P2X7) is an ATP-gated ion channel with important roles in the immune and other body systems. Much has been garnered about P2X7 using the first published anti-human P2X7 monoclonal antibody (mAb) (clone L4). This mAb can be used to impair P2X7 channel and pore activity and events downstream of P2X7 activation in vitro and in vivo. Immunolabelling with this mAb has been used in flow and mass cytometry, confocal microscopy and immunohistochemistry to help establish the presence of P2X7 on human leukocytes and other primary cell types, as well as various malignant cell lines.

Functionalized nanoparticles for the precise treatment and prevention of bacterial infections.

Multidrug-resistant (MDR) bacterial infections are an escalating health issue, and their rise poses a major threat worldwide. Inappropriate antibiotic use, including unnecessary prophylaxis, and nonpathogen-directed therapy, accelerates resistance, diminishing the effectiveness of conventional antibiotics. Consequently, novel antimicrobial strategies are urgently required to achieve precise prevention and treatment of bacterial infections while minimizing selective pressure for resistance. Nanotechnology-based antimicrobials are promising candidates because of their unique advantages: payload

Longitudinal Plasma Proteomic Profiling Reveals Divergent Immune Responses in Durably Cured and Relapsed Pulmonary Tuberculosis.

Background: Although clinical biomarkers of tuberculosis (TB) relapse are well characterized, the biological mechanisms underlying different treatment outcomes remain poorly understood. Elucidating these mechanisms may reveal improved biomarkers of long-term treatment outcomes. Methods: We conducted a longitudinal, global proteomic study on 60 participants with active pulmonary TB, half who were durably cured and half who relapsed. Plasma was collected at seven time-points: at treatment initiation (baseline), during therapy, and 52 weeks postbaseline. Samples were analyzed by high-resolution L

Therapeutic updates in NMOSD and MOGAD: From present practice to future promise.

Aquaporin-4-IgG-positive neuromyelitis optica spectrum disorder (AQP4+NMOSD) and myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) are rare, antibody-mediated inflammatory disorders of the central nervous system (CNS). Although historically grouped under the umbrella of multiple sclerosis, both entities differ markedly in pathophysiology, clinical course, prognosis and therapeutic requirements. The identification of disease-specific autoantibodies has not only refined diagnostic accuracy but has also highlighted fundamental differences in therapeutic strategy, particularl

Proinflammatory Epstein-Barr Virus Antibody Functions Track with Disease Activity in Multiple Sclerosis.

OBJECTIVE: Epstein-Barr virus (EBV) has been implicated in the pathogenesis of multiple sclerosis (MS). Elevated immunoglobulin G (IgG) titers against EBV nuclear antigen 1 (EBNA1) represent the most consistent serological marker of MS risk, with levels remaining persistently elevated following disease onset. METHODS: We conducted high-dimensional profiling of virus-specific antibody-associated immune features in 60 individuals with relapsing-onset MS followed over time and in 54 age- and sex-matched EBV-seropositive healthy controls. RESULTS: EBNA1-specific IgG in MS patients exhibited a di

Targeting C3aR/STAT3 signaling attenuates neuroinflammation and reverses neurological impairment in Neuromyelitis optica.

Abundant evidence indicates that the interaction between astrocytes and microglia through the C3-C3a receptor (C3aR) pathway is a crucial contributor to the pathogenesis of neurodegenerative and autoimmune disorders, such as neuromyelitis optica (NMO). However, the role of C3aR in microglia in modulating inflammation related to local lesions and neurological dysfunctions in NMO remains unclear. In this study, we reveal for the first time that the expression of C3aR on microglia is positively correlated with microglial activation, proinflammatory cytokine secretion, and complement deposition, w

Research Progress on the Effect and Mechanism of Gene Transfection in Reducing the Inflammatory Response of Atherosclerosis.

INTRODUCTION: Gene transfection techniques have potential therapeutic value in reducing the inflammatory response in atherosclerosis. Atherosclerosis is a chronic inflammatory disease. Its pathological process involves multiple types of cells and signaling pathways. METHODS: In recent years, researchers have used gene transfection techniques to introduce specific genes into vascular or immune cells in order to inhibit inflammatory responses, stabilize plaques, and slow down the process of atherosclerosis. Research progress has shown that gene transfection can exert anti-inflammatory effects t

XON9-A Glyco-Humanized Polyclonal Antibody Effective Against Hepatocellular Carcinoma.

Hepatocellular carcinoma (HCC) is the main leading cause of cancer-related deaths. Treatments for advanced HCC include multikinase inhibitors (Sorafenib or Lenvatinib), with limited response rates and serious side effects, or immunotherapy applicable to a small fraction of patients. Thus, new strategies are needed to improve the management of HCC. We evaluate here the efficacy and safety of XON9, a first-in-class glyco-humanized polyclonal antibody (GH-pAb). Cytotoxic activity of XON9 against Hep3B, Huh7, HepG2 or primary hepatocytes was investigated. Apoptosis, caspase activity, production of

Peripheral blood mononuclear cell transcriptomic trajectories reveal dynamic regulation of inflammatory actors in delirium.

Delirium is a neurologic syndrome characterized by inattention and cognitive impairment frequently encountered in the medically ill. Peripheral inflammation is a key trigger of delirium, but the patient-specific immune responses associated with delirium development and resolution are unknown. This retrospective cohort study of prospectively collected biospecimens examines RNA sequencing from peripheral blood mononuclear cells of adults hospitalized for COVID-19 to better understand patient-specific factors associated with delirium (n = 64). Longitudinal transcriptomic analyses highlight persis

Xenosiderophore transporter gene expression and clade-specific filamentation in Candida auris killifish (Aphanius dispar) infection.

Candida auris is a critical priority fungal pathogen (World Health Organization). Clinical management is challenging, with high associated mortality, rapidly increasing antifungal resistance, and frequent nosocomial outbreaks. A critical bottleneck in understanding virulence is the lack of gene expression profiling models during infection. We developed a fish embryo yolk-sac microinjection model using Aphanius dispar (Arabian killifish; AK) at human body temperature. This enabled interrogation of infection dynamics via dual host-pathogen RNA-seq across five major clades of C. auris (I-V). Host

Proteomic profiling identifies serpin G1, ApoA-II, and LBP as potential biomarkers of dementia in an Egyptian cohort.

BACKGROUND: Dementia, including Alzheimer's disease (AD), is a growing concern in Egypt, yet biomarker research in this population is scarce. Identifying serum biomarkers is essential for early diagnosis and understanding disease mechanisms in underrepresented groups. METHODS: We performed serum proteomic profiling on 20 Egyptian dementia patients and 10 cognitively unimpaired controls from the Egyptian Dementia Registry using mass spectrometry. Differential protein expression and pathway enrichment analyses were conducted. RESULTS: Of 260 quantified proteins, 21 were significantly different

Investigation into the Pancreatic Pathogenesis of SFTSV across Multiple Levels.

Severe Fever with Thrombocytopenia Syndrome virus (SFTSV) infection induces hepatitis, myocarditis, and even multi-organ dysfunction with a high mortality rate, while the impact on the pancreas remains unknown. In a retrospective analysis of clinical parameters in a cohort of 290 patients with severe fever with thrombocytopenia syndrome (SFTS), it is observed that pancreatic injury biomarkers in 19.4% (elevated serum amylase ≥3 × upper limit of normal (ULN)) and 25.8% (elevated serum lipase ≥3 × ULN). Notably, 17.6% of patients met the diagnostic criteria for clinically confirmed pancreatitis.

Pathogenesis of Chronic Spontaneous Urticaria With or Without Angioedema.

Chronic spontaneous urticaria (CSU) is a complex, immune-mediated skin disorder characterized by the appearance of wheals and/or angioedema. Histopathologic analysis reveals skin mast cell degranulation and a perivascular infiltrate composed of lymphocytes, eosinophils, neutrophils, and basophils. Mast cells and basophils are key effector cells in the pathogenesis and severity of CSU, and both cells can be activated by IgE-dependent and independent mechanisms. The pathogenesis of CSU remains incompletely understood, though recent studies have highlighted autoimmune mechanisms involving both Ig

Single-cell immune profiling at time of M. tuberculosis exposure reveals antigen-reactive programs that predict progression to active disease.

Early delineation of host immune responses at the moment of Mycobacterium tuberculosis (Mtb) exposure and infection is critical to identify individuals at risk of progressing to active tuberculosis (TB). We performed single-cell transcriptional profiling of over 500,000 peripheral blood mononuclear cells from 57 HIV-negative close contacts of TB cases in Brazil, including 25 individuals who developed active disease within two years (progressors) and 32 matched controls who remained disease-free (non-progressors). Cells were stimulated separately with the MTB300 peptide pool or irradiated Mtb (

Thiazole orange: a complement to established methods for diagnosing inherited dense granule defects.

Inherited delta storage pool deficiencies are likely underdiagnosed due to limitations in available diagnostic tools. Achieving accurate diagnosis is thus challenging for clinical laboratories highlighting the need for improved methods. Our aim was to explore thiazole orange (TO) as a flow cytometric marker for dense granules and compare it with mepacrine, reported to have low positive predictive value, after optimization of both protocols. TO, typically used for reticulocyte marking, binds dense granules at high concentrations producing a strong signal in resting platelets. Upon activation, t
