Salvage After Patient-Driven Delayed Conversion From External Fixation to Intramedullary Nailing in an Open Tibial Shaft Fracture: A Case Report.

BACKGROUND: Open tibial shaft fractures require timely management of both skeletal instability and soft-tissue injury. Temporary external fixation is useful for early stabilization, but prolonged retention without progression to definitive fixation may increase the risk of delayed union and other complications. This case illustrates a patient-driven delay from the preferred staged pathway and subsequent salvage after restoration of mechanical stability and biological support. CASE PRESENTATION: A 40-year-old man sustained a Gustilo-Anderson Type II open fracture of the right tibial shaft with

Open Reduction and Internal Fixation Versus Suprapatellar Nailing and Condylar Bolts for the Management of Complex Bicondylar Tibial Plateau Fractures: A Clinical Study.

Purpose The surgical fixation of bicondylar tibial plateau fractures is a challenging process. Open reduction and internal fixation (ORIF) is considered the "gold standard" treatment, while recently, a minimally invasive surgical technique that combines Suprapatellar Nailing and condylar Bolts (SNB) has been proposed for the fixation of these injuries. The present study compares ORIF and SNB techniques with the aim of providing information about the effectiveness of the SNB technique. Methods This clinical study examined 48 patients with complex tibial plateau fractures who were managed at the

Long-term implant retention after impacted elastic stable intramedullary nailing in pediatric diaphyseal forearm fractures: a retrospective cohort study.

BACKGROUND AND PURPOSE: In elastic stable intramedullary nailing (ESIN) for forearm fractures, nail protrusion may cause irritation-related implant removal. The primary aim was to evaluate implant retention after nail impaction, and secondarily to assess secondary procedures, complications, and protrusion as a predictor of removal. METHODS: We analyzed a retrospective cohort of children with diaphyseal forearm fractures treated with impacted ESIN between 2017 and 2024. Implant retention was defined as no nail removal at record review. Secondary procedures were unplanned operations, while comp

320 mm InterTan nail optimizes biomechanics in AO/OTA 31A2.3 fractures: superior stress distribution, micromotion, and strain for enhanced healing.

PURPOSE: This study compares the biomechanical performance of InterTan nails of three lengths (180 mm, 240 mm, and 320 mm) in treating AO/OTA 31A2.3 comminuted intertrochanteric fractures, which are highly unstable and prone to fixation failure. The research question focuses on identifying the nail length that optimizes stress distribution, displacement, and strain to enhance fracture healing and reduce failure risk, thereby providing a theoretical foundation for clinical decision-making. METHODS: Femoral CT images from a healthy 24-year-old male were used to reconstruct cortical and cancello

Prospective Comparative Study of Functional Outcomes of Distal Extra-articular Tibia Fracture Fixed With Intramedullary Nailing Versus Locking Compression Plating.

Introduction Distal tibial fractures are prevalent long bone injuries that pose difficulties owing to insufficient soft tissue coverage and their closeness to the ankle joint. The selection of the fixing method continues to be contentious, especially for extra-articular fractures. This study sought to investigate extra-articular distal tibial fractures, compare intramedullary nailing (IMN) with minimally invasive plate osteosynthesis (MIPO), evaluate their clinical, radiological, and functional outcomes, and identify the problems linked to each procedure. Methodology This prospective comparati

Epidemiological and clinical features of nail disorders in patients presenting to a pediatric dermatology outpatient clinic: A cross-sectional study in a tertiary referral center.

Nail disorders in the pediatric population encompass a broad spectrum of conditions with distinct epidemiological and clinical profiles compared with those observed in adults. Despite their clinical relevance, comprehensive data on the prevalence, distribution, and associated factors of pediatric nail disorders remain scarce. This study was designed to test the hypothesis that the prevalence and etiological distribution of nail disorders vary significantly across pediatric age groups and are associated with identifiable clinical and demographic factors. This cross-sectional observational study

A Rare Case of Titanium Elastic Nail Removal 7 Years after Pediatric Femoral Shaft Fracture Using a Screw Retriever Technique.

INTRODUCTION: Elastic stable intramedullary nailing is a commonly used modality for the surgical stabilization of long-bone fractures in children, with routine implant removal recommended within 6-12 months. Delayed removal beyond this timeframe is uncommon and presents unique surgical challenges. We report a rare case of titanium elastic nails (TENs) removal performed 7 years after initial fixation, highlighting the technical considerations and successful outcome. CASE REPORT: A 14-year-old female presented with discomfort in the left thigh. The patient had a history of TENs nails inserted a

Novel anchor-type proximal femoral nail for the improvement of bone-fixation integrity in treating intertrochanteric fractures: an experimental and computational characterization study.

This study introduces a novel anchor-type proximal femoral nail (AT-PFN) to improve the bone-fixation integrity over the standard screw-type nail (SST-PFN). Quasi-static incremental cyclic load test was performed to investigate load-displacement, cumulative deformation energy, time-strain, and backbone curves. The finite element analysis (FEA) was implemented to identify the stress and strain distributions. Additionally, non-destructive dynamic tests were conducted, and the measurements were processed using statistical pattern recognition, based on vector autoregression and principal component

Removal of bent intramedullary nail in diaphyseal segmental femoral fracture with intra-articular distal femoral fracture after below-knee amputation.

Refracture accompanied by nail bending in healed or unhealed femoral fractures is a rare complication, and its removal is challenging intraoperatively. A man in his mid-20s presented to the emergency department with a deformed thigh with a puncture wound in a below-knee amputee limb following a road traffic accident, and radiographs revealed a diaphyseal segmental femur fracture with an intra-articular distal femur fracture with a bent femur nail in situ. Attempts to remove a bent femoral nail frequently fail non-invasively and may cause iatrogenic damage. Cutting the bent femur nail using a c

Onychomycosis prevalence etiology and associated factors in women using nail cosmetics attending Mbarara regional referral hospital dermatology clinic Uganda.

Onychomycosis is a fungal infection of the nails caused primarily by dermatophytes, Non-Dermatophyte Moulds (NDMs) and yeast species. Fungal transmission occurs through direct contact with contaminated objects. Nail cosmetic treatments involve using tools that may be contaminated with fungus and can traumatize the nail, potentially increasing the risk of infection. However, little is known about its prevalence, etiology and associated factors among women using nail cosmetics in Uganda. A cross-sectional study at Mbarara Regional Referral Hospital (MRRH) skin clinic (January-March 2025) enrolle

How accurately do finite element models predict the fall impact response of ex vivo specimens augmented by prophylactic intramedullary nailing?

Hip fracture prevention approaches like prophylactic augmentation devices have been proposed to strengthen the femur and prevent hip fracture in a fall scenario. The aim of this study was to validate the finite element model (FEM) of specimens augmented by prophylactic intramedullary nailing in a simulated sideways fall impact against ex vivo experimental data. A dynamic inertia-driven sideways fall simulator was used to test six cadaveric specimens (3 females, 3 males, age 63-83 years) prophylactically implanted with an intramedullary nailing system used to augment the femur. Impact force mea
