Hemangioblastomas of central nervous system: molecular genetic analysis and clinical management.

OBJECTIVE: Hemangioblastomas of the central nervous system (CNS) are benign neoplasms that may occur sporadically or in association with von Hippel-Lindau (VHL) disease. The proportion of primary symptomatic hemangioblastomas associated with VHL disease is estimated to be from 10 to 40%, but it seems to be underestimated. We investigated the frequency of VHL germline mutation in patients with symptomatic CNS hemangioblastoma without evidence of VHL disease to define the role of molecular genetic analysis in the management of such patients and their relatives. METHODS: We analyzed 14 patients

Cisapride or cimetidine in the treatment of functional dyspepsia. Results of a double-blind, randomized, Swiss multicentre study.

BACKGROUND: Functional dyspepsia is a major diagnostic and therapeutic challenge for the clinician. Several systems for the identification of 'high-risk' patients and classifications of dyspepsia subtypes and treatment schemes have been proposed in the past with limited experimental evidence to support the claims made. The present trial was designed to compare two different treatment modalities in a group of functional dyspepsia patients selected on the basis of a standardized diagnostic procedure as 'non-risk' for organic disease and to assess the result in the major symptom sub-groups of fun

Multicenter Standardized Mortality Review for Quality-of-Care Improvement: Implementing an Automated Workflow.

BACKGROUND: Mortality review is a key mechanism for identifying preventable harm and improving hospital-based quality and safety. Traditional review processes are manual, often delayed, and inconsistent across services, resulting in variable completion rates and limited ability to generate improvement strategies. Structured, multidisciplinary review models have shown benefits, but many institutions continue to rely on decentralized, retrospective approaches that lack electronic integration or automation. Traditional approaches often fail to identify systemwide patterns, yielding insights with

Early diagnostic strategy for central nervous system bacterial infections after neurosurgery: A retrospective study.

Accurate diagnosis of post-neurosurgical bacterial infection of central nervous system is challenging due to the nonspecific nature of clinical signs and cerebrospinal fluid (CSF) parameters, which often overlap with sterile postoperative inflammation. This study aimed to develop and validate a stepwise diagnostic strategy integrating readily available clinical and basic laboratory indicators to improve the early identification of post-neurosurgical bacterial infection of the central nervous system. A retrospective cohort study was conducted at Tianjin Huanhu Hospital, a tertiary neurosurgical

Laser desorption rapid evaporative ionization mass spectrometry (LD-REIMS) demonstrates a direct impact of hypochlorous acid stress on PQS-mediated quorum sensing in Pseudomonas aeruginosa.

UNLABELLED: To establish infections in human hosts, Pseudomonas aeruginosa must overcome innate immune-generated oxidative stress, such as the hypochlorous acid (HOCl) produced by neutrophils. We set out to find specific biomarkers of oxidative stress through the development of a protocol for the metabolic profiling of P. aeruginosa cultures grown in the presence of different oxidants using a novel ionization technique for mass spectrometry, laser desorption rapid evaporative ionization mass spectrometry (LD-REIMS). We demonstrated the ability of LD-REIMS to classify samples as untreated or tr

CRISPR-Cas Diversity in Clinical Salmonella enterica Serovar Typhi Isolates from South Asian Countries.

Typhoid fever, caused by Salmonella enterica serovar Typhi (S. Typhi), is a global health concern and its treatment is problematic due to the rise in antimicrobial resistance (AMR). Rapid detection of patients infected with AMR positive S. Typhi is, therefore, crucial to prevent further spreading. Clustered Regularly Interspaced Short Palindromic Repeats and CRISPR-associated genes (CRISPR-Cas), is an adaptive immune system that initially was used for typing purposes. Later, it was discovered to play a role in defense against phages and plasmids, including ones that carry AMR genes, and, at pr

Development and validation of interpretable machine learning models to predict intensive care unit outcomes in patients on hemodialysis: a multicenter study.

BACKGROUND: Hemodialysis patients are at high risk for ICU admission due to elevated mortality, cardiovascular disease, and infection rates. Traditional ICU scoring systems (e.g., APACHE-II, SOFA) demonstrate limited accuracy in this population. This study aimed to identify key risk factors and develop interpretable machine learning (ML) models for predicting ICU outcomes to enable early intervention. METHODS: This multicenter study analyzed data from three cohorts: The First Affiliated Hospital of Sun Yat-sen University (n = 248), MIMIC-IV (n = 769), and eICU-CRD (n = 1,878). Primary outcome

Chronic Obstructive Pulmonary Disease and Obstructive Sleep Apnea Coexistence: From Pathophysiology to Clinical Complications.

BACKGROUND: Obstructive sleep apnea (OSA) and chronic obstructive pulmonary disease (COPD) are two common chronic respiratory diseases. Their co-existence forms overlap syndrome (OVS), a distinct clinical entity that imposes a dual burden on patients due to its unique pathophysiological mechanisms and significant clinical implications. PURPOSE: This narrative review describes the consequences of OVS by tracing its pathophysiological from molecular mechanisms to clinical phenotypes, including non-cardiovascular complications. The primary aim is to synthesize this knowledge into a conceptual fr

A non-invasive AI-based system for precise grading of anosmia in COVID-19 using neuroimaging.

COVID-19 (Coronavirus), an acute respiratory disorder, is caused by SARS-CoV-2 (coronavirus severe acute respiratory syndrome). The high prevalence of COVID-19 infection has drawn attention to a frequent illness symptom: olfactory and gustatory dysfunction. The primary purpose of this manuscript is to create a Computer-Assisted Diagnostic (CAD) system to determine whether a COVID-19 patient has normal, mild, or severe anosmia. To achieve this goal, we used fluid-attenuated inversion recovery (FLAIR) Magnetic Resonance Imaging (FLAIR-MRI) and Diffusion Tensor Imaging (DTI) to extract the appear

A systematic review of the research progress of non-coding RNA in neuroinflammation and immune regulation in cerebral infarction/ischemia-reperfusion injury.

Cerebral infarction/ischemia-reperfusion injury is currently the disease with the highest mortality and disability rate of cardiovascular disease. Current studies have shown that nerve cells die of ischemia several hours after ischemic stroke, which activates the innate immune response in the brain, promotes the production of neurotoxic substances such as inflammatory cytokines, chemokines, reactive oxygen species and - nitrogen oxide, and mediates the destruction of blood-brain barrier and the occurrence of a series of inflammatory cascade reactions. Meanwhile, the expression of adhesion mole

A Machine Learning Tool Using Digital Microscopy (Morphogo) for the Identification of Abnormal Lymphocytes in the Bone Marrow.

Morphological analysis of the bone marrow is an essential step in the diagnosis of hematological disease. The conventional analysis of bone marrow smears is performed under a manual microscope, which is labor-intensive and subject to interobserver variability. The morphological differential diagnosis of abnormal lymphocytes from normal lymphocytes is still challenging. The digital pathology methods integrated with advances in machine learning enable new diagnostic features/algorithms from digital bone marrow cell images in order to optimize classification, thus providing a robust and faster sc

A rare case of multiple phosphaturic mesenchymal tumors along a tendon sheath inducing osteomalacia.

BACKGROUND: Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome characterized by renal phosphate wasting, hypophosphatemia, reduction of 1,25-dihydroxyl vitamin D, and bone calcification disorders. Tumors associated with TIO are typically phosphaturic mesenchymal tumors that are bone and soft tissue origin and often present as a solitary tumor. The high production of fibroblast growth factor 23 (FGF23) by the tumor is believed to be the causative factor responsible for the impaired renal tubular phosphate reabsorption, hypophosphatemia and osteomalacia. Complete removal of the t

Comparison of Harmless Acute Pancreatitis Score (HAPS) and Bedside Index of Severity in Acute Pancreatitis (BISAP) Scoring Systems in Predicting Severity and Outcomes in Acute Pancreatitis: A Prospective Study.

Introduction Acute pancreatitis (AP) is a common yet severe digestive disorder characterized by sudden inflammation of the pancreas, resulting in substantial morbidity and mortality. Although the underlying causes of AP are multifactorial, gallstones and alcohol abuse remain the most prevalent triggers. The first 24 hours are critical in determining a patient's risk of complications and outcomes. Various scoring systems, including the Bedside Index of Severity in Acute Pancreatitis (BISAP) and the Harmless Acute Pancreatitis Score (HAPS), have been developed to predict disease severity and gui

Ocular involvement, fever, and holocranial headache predict intracranial infection in immunocompetent patients with cranial herpes zoster: a multicenter propensity score-matched study.

BACKGROUND: Varicella-zoster virus (VZV) reactivation in the head and facial region can cause severe intracranial complications. Early identification of central nervous system (CNS) involvement is critical, yet specific bedside predictors to guide prompt antiviral escalation are lacking, frequently leading to diagnostic delays and poor neurological prognosis. OBJECTIVES: To develop and internally validate a clinical risk-stratification scoring system based on ocular involvement, systemic inflammation, and headache patterns to identify intracranial infection in head and facial herpes zoster (H
